Based on currently available literature, clinical examination remains the major method when handling cases of suspected malignancy. However, this method does not allow diagnosing cancer, due to which a large group of patients with possible oral mucosa cancer are referred to an oncologist. The search and use of affordable non-invasive methods for early diagnosis of oral mucosa tumors is an urgent issue facing the health system. The study involved analyzing 134 records of outpatients examined at the Samara Regional Oncological Clinic who were referred by dentists within 2014-2019 from the local polyclinic in Samara due to detection of tumors in oral mucosa and who underwent a biopsy. The patients were divided into two groups according to the examination methods. The inclusion criteria were: detection of various superficial oral mucosa neoplasms; referral from the dentist. The exclusion criteria were as follows: patients with submucosal oral cavity neoplasms referred to the oncologist by other medical specialists or self-referred patients. The control group included 63 patients who, after a conventional examination (including interview, examination, palpation), underwent an incisional biopsy followed by morphological examination at the oncologist’s office. In the major group, in 71 patients at their respective initial dental appointments a special examination algorithm was applied. This algorithm entailed an assessment of the identified risk factors. Indications for biopsy were identified using the histological verification index (HVI). Apart from the conventional examination methods (interview, examination, palpation), autofluorescence stomatoscopy was used, this being done for the purpose of differential diagnostics of inflammation, precancerous and malignant issues, depending on the glow type. In the main group, the initial stages of oral mucosa cancer were detected in 17 patients after biopsy; in the control group – in 4 patients (p=0.004). The developed algorithm used for scoring the patient’s clinical examination data combined with autofluorescence stomatoscopy allowed diagnosing accurately (90% of reliability) precancerous and cancerous diseases, as well as to use invasive research methods (biopsy) strictly following the indications. Aim of study: to improve diagnosis of oral mucosa neoplasms through improvement of the examination algorithm.

Introduction: Currently, the methods of integrated assessment of patient’s condition based on scoring of separate multidirectional changes in subjective clinical and objective laboratory parameters are used in medical science more and more often. The aim: Based on the identified differences in clinical manifestations and immunological indicators of patients of different age with chronic tonsillitis and patients with no signs of ENT pathology, to develop an integral scoring scale as a basis for an objective assessment of the state of patients with CT and the effectiveness of their treatment. Materials and methods: The clinical signs and immunological factors were evaluated. Patient’s general information, such as age, the presence of allergies, was also taken into account. Based on the analysis of the data array, a scoring system for assessing the clinical and immunological state of patients was proposed. Results: The abnormalities in clinical and immunological assessment in adult population with no signs of ENT pathology (control group) are 2.75 times higher than in healthy children, while in case of chronic tonsillitis the differences between children and adults in terms of integral abnormalities are 1.66 (p<0.05). Conclusions: Integral scoring assessment of clinical and immunological tests makes it possible to provide more objective evaluation of the state of lymphoid tissue of tonsils and the immunity in patients with chronic tonsillitis, which may become an objective basis for choosing patient management strategy, as well as for estimating treatment efficacy.

Material and methods. The study included patients (n=164) in peri- and postmenopause, having a laboratory-confirmed picture of the genitourinary menopausal syndrome - atrophic vulvovaginitis and suffering from recurrent lower urinary tract infections (UTI) - cystitis and urethritis. All patients underwent standard antibacterial therapy (ABT) UTI in accordance with the results of bacteriological urine culture according to Russian clinical guidelines on urology. Against the background of ABT, 164 patients received additional therapy with Trioginal® (the study group - SG) containing estriol, micronized progesterone and Lactobacillus casei rhamnosus Doderleini 35 (LCR 35) lactobacillus strain. Trioginal® was administered intravaginally in two stages: for 20 days, 2 capsules per day, then for 10 days, 1 capsule per day. In the comparison group - CG (n=67) with ABT, Ovipol Klio® (estriol monopreparation) was additionally intravaginally used in two stages: for 14 days, 1 suppository per day, then for 2 weeks, 1 suppository 2 times a week. The control group consisted of 30 patients receiving only standard ABT UTI, local hormone therapy for vulvovaginal atrophy (VVA) was not performed. The observation period was 12 months after the end of the course of therapy. At all stages of treatment, a subjective and objective assessment of the severity of urination disorders and urogenital atrophy (UDI-6 questionnaire), their impact on the quality of life of patients (questionnaire IIQ-7), indicators of bacteriuria, bacterial urine culture and vaginal biocenosis, frequency of undesirable phenomena were carried out. According to the initial characteristics of the group of patients were comparable. Results. At all stages of the study, in the group of patients who received therapy with the use of the Trioginal® drug, there was a significant improvement in the clinical picture of urination disorders compared to the CG and the control (main) group - MG (in SG - 18% at the end of therapy versus 100% before the start of therapy; CG - 26 % versus 100% respectively; Ledger - 58% versus 100% respectively; p

Breast cancer (BC) is the most common diagnosed cancer and the leading cause of cancer mortality in women worldwide. The epidemiological picture of BC among the population of the Crimea Republic is analyzed with a comparative evaluation with nation-wide rates. According to the presented data the incidence of BC in the Crimea Republic has a clear upward trend. Incidence growth is mainly due to the female population among the «employable» contingent that requires regular clinical examination and screening these patients. In prognostic estimates by 2020 the incidence rates of BC in female population will increase by 10 % compared to 2015. The increase of the prevalence, contingent accumulation index, the proportion of patients experiencing more than 5 years, and reduction in mortality of the observed patient cohort shows better results of BC treatment in the Crimea Republic as compared to the All-Russian data.

In conditions of limited financial resources of the state, the task of assessing the appropriateness and choosing rational options for the joint use of traditional and new types of high-tech products is topical. The paper proposes a method for substantiating rational options for the joint use of traditional and new products of high-tech products, based on the criteria of their comparative technical and economic assessment, namely, comparing the achieved efficiency and the required cost of performing a fixed set of tasks. The dialectical foundations of the method are presented, in particular, it is established that the law of development of high-tech products fully corresponds to the well-known classical law of «denial of denial». The structure of the method, the order of formation of the set of Pareto-optimal options for the joint use of traditional and new products, as well as the algorithm for choosing a rational option are considered.

Considering the particular danger of remote skin reactions to ultraviolet irradiation (UVI), it is advisable to use ointments with antioxidant activity to reduce its negative effect on the skin. The rationale for the choice of ointments with antioxidant activity was the fact that they reduce the damaging effect of ultraviolet radiation in the erythemal and early post-erythemal period. The presence of a regular connection between the development of the early and late periods has given reason to assume the protective effect of ointments on the remote skin reactions. Objective: to study the effect of thiotriazoline ointment and thiotriazoline ointment with silver nanoparticles on the state of the morphological structures of the skin of guinea pigs after local UVI. Material and methods of research. The study involved 132 albino guinea pigs weighing 400-500 g, divided into 4 groups: 1 - intact, 2 - control (guinea pigs subjected to local UVI), 3 and 4 main ones. The third main group included guinea pigs that after UVI were administered thiotriazoline ointment in the treatment and prophylactic regime, the fourth main group included guinea pigs that after UVI were administered thiotriazoline ointment with silver nanoparticles in the same mode as Group 3. Ointments were applied 1 hour before irradiation and daily until erythema disappeared. Ultraviolet erythema was caused by irradiation in 1 minimum erythemal dose. After 2, 4 hours, on the 3rd, 8th, 15th, 21st, 28th day, the fragments of irradiated skin were investigated using histochemical and morphometric methods (fibroblast density and epidermis thickness). Results. Morphological changes in the skin after applying ointments with antioxidant activity were unidirectional. It was revealed that in the early periods after irradiation, thiotrazoline ointment and thiotrazoline ointment with silver nanoparticles do not affect changes in the thickness of the epidermis, but statistically significantly reduce the density of fibroblasts in the dermis on the 3rd day of the experiment compared to the control group. In the later periods, under the influence of thiotriazoline ointment, a gradual decrease in the thickness of the epidermis, which reached the norm by the end of the experiment, was observed. On the 8th day, the maximum density of fibroblasts was recorded, in the subsequent periods of the experiment, the index gradually decreased, which was accompanied by collagenization of the papillary layer in the loci of damage to collagen and elastic fibers detected in 50% of cases. In later times, under the influence of thiotriazoline ointment with silver nanoparticles, the processes of restoring the morphological structures of the skin occurred faster. In parallel with the decrease in the density of fibroblasts in the loci of the previous damage to the collagen and elastic fibers of the papillary layer, thickening of collagen fibers was observed, replacing them with segments of destruction of elastic fibers. In this group, at the end of the experiment, the collagenization locus was small, single, occurring in 16.7% of cases. Conclusions Ointments with antioxidant activity exert a positive effect on the state of morphological structures of the skin, damaged as a result of local UVI, in erythemal and post-erythemic periods. In the early periods after the local UVI, there was a general tendency for the effect of both ointments, as they reduced the density of fibroblasts on the 3rd day, but did not result in complete normalization. In the late period after local UVI , under the influence of thiotriazoline ointment and thiotriazoline ointment with silver nanoparticles, thickness of the epidermis (by 21st and 15th day, respectively) and density of fibroblasts (by the 28th day) decreased to normal while without treatment both indicators exceeded the norm by several times for 28 days of the experiment.

Abstract Background Electronic cigarettes have been a popular alternative to tobacco smoking. The effect of tobacco smoking on nasal airway resistance has been investigated before; however, the effect of the aerosol generated by electronic cigarettes is still unknown. This study aimed to evaluate the short-term effects of e-cigarettes on nasal airway resistance. Methods Sixty-one participants were recruited into a vapers group and a control group. The vapers group was instructed to smoke for 5 minutes, and their nasal resistance was measured pre-procedure and at 1 and 5 minutes post-procedure. The results were compared between both groups. Results Repeated measures analysis of variance demonstrated that vaping has no statistically significant effect on total nasal airway resistance. Conclusion Although the differences between both groups were not statistically significant overall, the vapers group showed a reduction in nasal airway resistance in the short term.

Abstract Introduction To improve the quality of life in nursing homes, meaningful activities and social contact are indispensable. Exergames can play a role addressing these needs. Methods In a randomized single blinded controlled intervention study, we investigated the effect of playing exergames on general wellbeing, fun and on social interaction. Results Thirty-five residents participated: 18 residents took part in the intervention group and 17 in the control group. The median mental wellbeing score of the intervention group increased from 42/50 to 45. The median sleep score of the intervention increased from 23/30 to 28. The median pain score of the intervention group improved from 18/20 to 20. The median score on subjective cognition increased from 24/30 to 26 while the mean scores on the objective assessment decreased from 1.8/2 to 1.7. Coaches gave an average fun score of 8.9/10 and an average intensity of exercise score of 11.6/20. Residents and coaches appreciated the social contact. Coaches reported a high feasibility (average of 4.1/5) but a low accessibility and a high intensity of supervision. Conclusions Exergaming is a feasible and pleasant complement to the usual activities with a positive impact on wellbeing, sleep, pain, and perceived cognition. Future research should focus on vulnerable groups and aim to develop a study in an implementation design.
