Background: Elderly (>60 years old) population is growing in Indonesia. It is important to prevent degradation of cognitive capacity by risk factor identification and treatment.Objective: To identify the relationship between anthropometric status and cognitive capacity on elderly population.Method: This is an analysis of The Fifth Wave of the Indonesia Family Life Survey (IFLS5) data with cross-sectional design. Anthropometric status is consisted of: body weight, body height, body mass index (BMI), knee height, upper arm length, waist circumference, hip circumference, and waist-hip ratio (WHR). Cognitive capacity is measured by modified telephone survey of cognitive status (TICS). Chi-Square and Mann-Whitney test are used for bivariate analysis, logistic regression is used for multivariate analysis.Results: Variables with significant relationship to cognitive capacity are body weight (p=0.0002), body height (p=0.0001), knee height (p=0.0387), upper arm length (p=0.0114), age (p=0.011), sex (p=0.014), and history of hypercholesterolemia (p=0.003). Logistic regression shows that body height, age, and history of hypercholesterolemia are simultaneously affecting cognitive capacity.Conclusion: There is significant relationship between body height, body weight, upper arm length, knee height, and cognitive capacity on elderly population with obesity.

Introduction: When both parents are obesity, thus the children are 80% potentially become overweight or obesity. If one of the parent is overweight or obesity, thus the children are 40% potentially to become overweight or obesity. The purpose of the study is to analyze is there any significant relationship between parents’ body shape with overweight among students at Singkawang Adventist School.  Method: The method of the study is descriptive with quantitative method and the cross sectional design. Sampling technique is total sampling among the overweight students. Number of respondens are 35 persons which have been measure body height, body weight and determined Body Mass Index (BMI). Data processing using SPSS Program.  Result: The results of the study show that the status of overweight students is obesity and parents’ body shape are overweight (there is no significant relationship between obesity in adolescents with the body shape of the father with a significance of (0.086) with a ρ value> 0.05 as well as no significant relationship between obesity in adolescents with the body shape of their mothers, with a significance of  (0.411) with ρ value> 0.005). There is no significant relationship between overweight students with parents’ body shape. There is not enough evidence to refuse null hypothesis.  Disscussion: Recommendation are given to Singkawang Adventist School Students in order to maintain a healthy lifestyle and give effort to reduce body weight. The relationship between reduced disc height and disc bulging and/or protrusion has been controversial. The purposes of this study were to examine the relationship between disc morphology and disc bulging and protrusion and to establish a model for predicting disc bulging and protrusion. This is a retrospective study. A total of 452 MRI scans from a spine study were analysed, 210 (46.5%) were men. Logistic regression analysis was applied to identify the association between anthropometric factors, disc morphology factors, and outcome. Model 1 was constructed using anthropometric variables to investigate the capacity for predicting outcomes. Model 2 was constructed using anthropometric and disc morphology variables. Age, body weight, body height, disc height, and disc depth were significantly associated with outcome. The area under the curve (AUC) statistics of Model 2 were significantly better than those of Model 1 at the L3-L4 and L4-L5 levels but not at the L5-S1 level. The results showed an association between disc morphology and disc bulging and/or protrusion at the L3-L4, L4-L5, and L5-S1 levels. The model utilizing both anthropometric factors and disc morphology factors had a better capacity to predict disc bulging and/or protrusion compared with the model using only anthropometric factors.

OBJECTIVEThe purpose of this study was to determine the rate of decompression for Chiari malformation type II in individuals with myelomeningocele in the National Spina Bifida Patient Registry (NSBPR). In addition, the authors explored the variation in rates of Chiari II decompression across NSBPR institutions, examined the relationship between Chiari II decompression and functional lesion level of the myelomeningocele, age, and need for tracheostomy, and they evaluated for temporal trends in rates of Chiari II decompression.METHODSThe authors queried the NSBPR to identify all individuals with myelomeningocele between 2009 and 2015. Among these patients, they identified individuals who had undergone at least 1 Chiari II decompression as well as those who had undergone tracheostomy. For each participating NSBPR institution, the authors calculated the proportion of patients enrolled at that site who underwent Chiari II decompression. Logistic regression was performed to analyze the relationship between Chiari II decompression, functional lesion level, age at decompression, and history of tracheostomy.RESULTSOf 4448 individuals with myelomeningocele identified from 26 institutions, 407 (9.15%) had undergone at least 1 Chiari II decompression. Fifty-one patients had undergone tracheostomy. Logistic regression demonstrated a statistically significant relationship between Chiari II decompression and functional lesion level of the myelomeningocele, with a more rostral lesion level associated with a higher likelihood of posterior fossa decompression. Similarly, children born before 2005 and those with history of tracheostomy had a significantly higher likelihood of Chiari II decompression. There was no association between functional lesion level and need for tracheostomy. However, among those children who underwent Chiari II decompression, the likelihood of also undergoing tracheostomy increased significantly with younger age at decompression.CONCLUSIONSThe rate of Chiari II decompression in patients with myelomeningocele in the NSBPR is consistent with that in previously published literature. There is a significant relationship between Chiari II decompression and functional lesion level of the myelomeningocele, which has not previously been reported. Younger children who undergo Chiari II decompression are more likely to have undergone tracheostomy. There appears to be a shift away from Chiari II decompression, as children born before 2005 were more likely to undergo Chiari II decompression than those born in 2005 or later.

<p class="MsoNormal" style="margin: 0cm 12.6pt .0001pt 18pt; text-align: justify; text-indent: 27pt;"><span style="font-size: 10pt;" lang="en-us" xml:lang="en-us">Nutritional status of pregnant women can be measured by anthropometry which is simple and non expensive. Waist circumference of pregnant women (WCP) may reflects uterus and fetus  growth. The objective of this study is to develop an estimation model of birth weight based on WCP during the third trimester. For this purpose 250 pregnant women were selected from six health centers and five midwife clinics of Tanah Sareal, Bogor.  Body height and weight, waist and hip circumferences, mid-upper arm circumference, fundal height were measured on 28<sup>th</sup>, 32<sup>nd</sup> , and 36<sup>th</sup>  week of gestation. Body weight prior to pregnancy, history of pregnancy and birth, and food consumption  was collected through an interview. The results show the two most appropriate e</span><span style="font-size: 10pt;" lang="en-us" xml:lang="en-us">stimation models of birth weight are based on WCP (R<sup>2</sup>=</span><span style="font-size: 10pt;" lang="en-us" xml:lang="en-us">28.58%) </span><span style="font-size: 10pt;" lang="en-us" xml:lang="en-us">and body weight (R<sup>2</sup>=</span><span style="font-size: 10pt;" lang="en-us" xml:lang="en-us">28.27%) </span><span style="font-size: 10pt;" lang="en-us" xml:lang="en-us">on 28 week of gestation, with the equations are : birth weight =  </span><span style="font-size: 10pt;" lang="en-us" xml:lang="en-us">166.4(WCP)<sup>3</sup> - 977.6(WCP)<sup>2</sup> + 1914.7(WCP) - 1246.6 and birth weight = 14.7(body weight)<sup>3</sup> - 79.4(body weight)<sup>2</sup> + 143.1(body weight) - 82.5. </span><span style="font-size: 10pt;" lang="en-us" xml:lang="en-us">Further prospective study with larger sample size are required to develop an estimation model of birthweight.</span></p>

Background :. Stunting among children is one of the major health problems in developing countries. Globally, 45% of deaths among children are caused by malnutrition, meanwhile stunting, as one of the factors, plays an important role. Children who experience stunting can not achieve optimal growth and cognitive potential. Hence, that will have an impact on future productivity. Muara Enim district is included in one district with stunting prevalence, which is above 20%, i.e. 26.9%. This study was to identify the risk factors of stunting children in Pulau Panggung district, Muara Enim, South Sumatera. Methods: This study applied observational analytic research using cross sectional design. It was conducted on August to October 2017 at public health center in Pulau Panggung district, Muara Enim. There were 83 children who fullfilled the inclusion criteria. The relationship between risk factors and stunting was analyzed using  Chi square test and Logistic Regression test was applied to identify the risk factor that play the most role to stunting. Results: 49,4% out of 83 children suffer from stunting, with a mean age of 29,37±13,69 months where the number of children stunting with male gender is 53.7% while female gender is 46.3%. Chi square analysis showed that there was a significant relationship among birth weight, birth length, mother's occupation, history of infection, exclussive breastfeeding and stunting  (p< 0.05) but no significant relationship was found among sex, mother's education, maternal height and income with stunting (p> 0.05). Multivariate Logistic Regression test showed that mother's occupation and history of infection are the risk factors that contibute the most to stunting among children aged 6-59 months in Pulau Panggung district. The risk of stunting among working mothers is higher 5 times than unworking  mothers and children with infection history have risk of stunting 3 times compared to children without infection history. Conclusion: Mother's occupation and infection history are risk factors that contribute the most to stunting among children at Pulau Panggung district, Muara Enim, South Sumatera.

Background and Purpose: Recurrent ischemic strokes increase the risk of disability and mortality. The role of conventional risk factors in recurrent strokes may change due to increased awareness of prevention strategies. The aim of this study was to explore the potential risk factors besides conventional ones which may help to affect the advances in future preventive concepts associated with one-year stroke recurrence (OSR). Methods: We analyzed 6,632 adult patients with ischemic stroke. Differences in clinical characteristics between patients with and without OSR were analyzed using multivariate logistic regression and classification and regression tree (CART) analyses. Results: Among the study population, 525 patients (7.9%) had OSR. Multivariate logistic regression analysis revealed that male sex (OR 1.243, 95% CI 1.025 – 1.506), age (OR 1.015, 95% CI 1.007 - 1.023), and a prior history of ischemic stroke (OR 1.331, 95% CI 1.096 – 1.615) were major factors associated with OSR. CART analysis further identified age and a prior history of ischemic stroke were important factors for OSR when classified the patients into three subgroups (with risks of OSR of 8.8%, 3.8%, and 12.5% for patients aged > 57.5 years, ≤ 57.5 years/with no prior history of ischemic stroke, and ≤ 57.5 years/with a prior history of ischemic stroke, respectively). Conclusions: Male sex, age, and a prior history of ischemic stroke could increase the risk of OSR by multivariate logistic regression analysis, and CART analysis further demonstrated that patients with a younger age (≤ 57.5 years) and a prior history of ischemic stroke had the highest risk of OSR.

Abstract Background For most women who have had a previous cesarean section, vaginal birth after cesarean section (VBAC) is a reasonable and safe choice, but which will increase the risk of adverse outcomes such as uterine rupture. In order to reduce the risk, we evaluated the factors that may affect VBAC and and established a model for predicting the success rate of trial of the labor after cesarean section (TOLAC). Methods All patients who gave birth at Northwest Women’s and Children’s Hospital from January 2016 to December 2018, had a history of cesarean section and voluntarily chose the TOLAC were recruited. Among them, 80% of the population was randomly assigned to the training set, while the remaining 20% were assigned to the external validation set. In the training set, univariate and multivariate logistic regression models were used to identify indicators related to successful TOLAC. A nomogram was constructed based on the results of multiple logistic regression analysis, and the selected variables included in the nomogram were used to predict the probability of successfully obtaining TOLAC. The area under the receiver operating characteristic curve was used to judge the predictive ability of the model. Results A total of 778 pregnant women were included in this study. Among them, 595 (76.48%) successfully underwent TOLAC, whereas 183 (23.52%) failed and switched to cesarean section. In multi-factor logistic regression, parity = 1, pre-pregnancy BMI < 24 kg/m2, cervical score ≥ 5, a history of previous vaginal delivery and neonatal birthweight < 3300 g were associated with the success of TOLAC. The area under the receiver operating characteristic curve in the prediction and validation models was 0.815 (95% CI: 0.762–0.854) and 0.730 (95% CI: 0.652–0.808), respectively, indicating that the nomogram prediction model had medium discriminative power. Conclusion The TOLAC was useful to reducing the cesarean section rate. Being primiparous, not overweight or obese, having a cervical score ≥ 5, a history of previous vaginal delivery or neonatal birthweight < 3300 g were protective indicators. In this study, the validated model had an approving predictive ability.

Background: We aimed to describe red blood cell (RBC) transfusions in the emergency department (ED) with a particular focus on the hemoglobin (Hb) level thresholds that are used in this setting. Methods: This was a cross-sectional study of 12 EDs including all adult patients that received RBC transfusion in January and February 2018. Descriptive statistics were reported. Logistic regression was performed to assess variables that were independently associated with a pre-transfusion Hb level ≥ 8 g/dL. Results: During the study period, 529 patients received RBC transfusion. The median age was 74 (59–85) years. The patients had a history of cancer or hematological disease in 185 (35.2%) cases. Acute bleeding was observed in the ED for 242 (44.7%) patients, among which 145 (59.9%) were gastrointestinal. Anemia was chronic in 191 (40.2%) cases, mostly due to vitamin or iron deficiency or to malignancy with transfusion support. Pre-transfusion Hb level was 6.9 (6.0–7.8) g/dL. The transfusion motive was not notified in the medical chart in 206 (38.9%) cases. In the multivariable logistic regression, variables that were associated with a higher pre-transfusion Hb level (≥8 g/dL) were a history of coronary artery disease (OR: 2.09; 95% CI: 1.29–3.41), the presence of acute bleeding (OR: 2.44; 95% CI: 1.53–3.94), and older age (OR: 1.02/year; 95% CI: 1.01–1.04). Conclusion: RBC transfusion in the ED was an everyday concern and involved patients with heterogeneous medical situations and severity. Pre-transfusion Hb level was rather restrictive. Almost half of transfusions were provided because of acute bleeding which was associated with a higher Hb threshold.

AbstractCongenital Heart Defects (CHDs) are associated with different patterns of malnutrition and growth retardation, which may vary worldwide and need to be evaluated according to local conditions. Although tetralogy of Fallot (TOF) is one of the first described CHDs, the etiology outcomes in growth and development of TOF in early age child is still unclear in most cases. This study was designed to investigate the growth retardation status of Chinese pediatric TOF patients under 5 years old. The body height, body weight and body mass index (BMI) of 262 pediatric patients (138 boys and 124 girls) who underwent corrective surgery for TOF between 2014 and 2018 were measured using conventional methods. The average body height, body weight and BMI of the patients were significantly lower than WHO Child Growth Standards, while the most affected was body height. Meanwhile, higher stunting frequency and greater deterioration of both the body height and weight happened in elder age (aged 13–60 months) rather than in infant stage (aged 0–12 months) among these patients. Our results confirmed that intervention should be given at early age to prevent the growth retardation of TOF patients getting severer.
