Hyperglicemia in DM Type will induce macrofag to produce cytokin proinflamation TNF-α. High levels of TNF-α in DM Type 2 can lead to more severe insulin resistance and become involved in endothelial dysfunction which lead to complications of the disease. This research aims to know differences between TNF-α level in DM type 2 controlled and uncontrolled. This research used obervational analizing method and cross sectional study comparative approach. Blood samples are grouped to 35 patient DM type 2 controlled and uncontrolled in the medicinal treatment in Padang State University polyclinic. The level of serum TNF-α will be examined in Biomedical Laboratory FKUA using ELISA method. TNF-α level in the diabetes type 2 controlled on average 7,55 ± 0,43 pg/ml whereas uncontrolled on average 149,28 ± 26,82 pg/ml. The Mann-Whitney test showed a p value < 0,001. There are significant differences in the levels of TNF-α in patients with DM type 2 controlled and uncontrolled. Key Words : Diabetes Mellitus type 2 controlled and uncontrolled, TNF-α

Diabetes Melitus (DM) merupakan salah satu kelompok penyakit metabolik dan kronis dengan karakteristik hiperglikemia yang terjadi karena kelainan sekresi insulin, kerja insulin atau keduaduanya yang membutuhkan perawatan medis dan pendidikan pengelolaan mandiri untuk mencegah komplikasi akut jangka panjang (Nian, 2017). Tujuan penelitian ini adalah Mendeskripsikan hubungan kualitas tidur dengan kadar glukosa darah Puasa pada pasien DM tipe II di PKM Kassi-Kassikota Makassar. Manfaat : Meningkatkan pengetahuan pada Penderita DM Tipe II yang mengalami gangguan Kwalitas dan Pola Tidur shari-hari Meningkatkan pengetahuan pada Penderita DM Tipe II yang mengalami gangguan Kwalitas dan Pola Tidur shari-hari Metode : Pada penelitian ini menggunakan desain cross sectional, jenis penelitian ini menggunakan metode analitik yaitu metode penelitian yang dilakukan dengan tujuan untuk mengetahui hubungan antara Kualitas tidur dengan kadar glukosa darah puasa pada pasien DM Tipe II. Sampel menggunakan purposive sampling dengan menggunakan rumus Slovin dengan jumlah sampel  55  orang  yaitu  seluruh pasien DM tipe 2 yang menjalani rawat jalan di PKM Kassi-Kassi Kota Makassar. Hasil Uji Statistik Chi Square diperoleh p value 0,000 < 0,05.sehingga peneliti berasumsi bahwa  ada hubungan antara kualitas tidur dengan kadar glukosa darah pada pasien DM Type 2 di Puskesmas Kassi-Kassi Makassar.  Kesimpulan yaitu terdapat hubungan kualitas tidur dengan kadar glukosa darah pada pasien diabetes melitus tipe 2 di Puskesmas Kassi-Kassi Makassar. Saran dapat dijadikan sebagai salah satu acuhan bagi pasien diabetes melitus tipe 2 untuk meningkatkan kualitas tidur dan menjaga kadar glukosa darah puasa

Type 2 diabetes mellitus (DM2) is known to be associated with the accelerated development of atherosclerosis. The current concepts of atherosclerosis take into consideration the possible role of inflammation in its pathogenesis which theoretically implies the modification of macrophages in accordance with the proinflammatory phenotype and the production of the proinflammatory cytokine tumour necrosis factor-alpha (TNF-α) by these cells. In connection with this, we undertook a study of spontaneous and induced secretion of proinflammatory cytokine TNF-α by monocytes-macrophages from the blood of 20 patients presenting with newly diagnosed type 2 diabetes mellitus (HbA1c - 8,9%) and the healthy volunteers showing up no disturbances of carbohydrate metabolism. It was shown that blood monocytes-macrophages of the patients presenting with DM2 are characterized by the enhanced ability (compared with the cells from the healthy subjects) to synthesize TNF-α in both native and interferon-y stimulated states up to 750 versus 270 pg/ml culture medium and to 1653 pg/ml versus 378 pg/ml culture respectively. This difference was statistically significant (p <0.05). It is concluded that the results of the study help to explain the elevated blood TNF-α level in the patients with type 2 diabetes mellitus and provide an insight into the mechanism underlying the development of the systemic inflammatory reaction accelerating the progression of atherosclerosis associated with diabetes mellitus.

Abtract Diabetes mellitus (DM) is a serious chronic disease that occurs because the pancreas does not produce enough insulin or when insulin is not use effectively in the body. This study aims to analyze the incidence of Drug Related Problems (DRPs) and find out the category of DRPs in outpatients with a diagnosis of type 2 diabetes mellitus at UPT Puskesmas Gedong Air Inpatient for the Bandar Lampung. This research is a nonexperimental research with a descriptive design taken retrospectively. The sampling technique used in this study is total sampling based on the patient's medical record data. Data were analyzed using the classification of DRPs according to Cipolle 2004, then using Medscape Tools as a tool to check potential drug interactions. The results of this study were 61 patients including 29 patients (46%) with DRPs and 33 patients (54%) without DRPs. The number of male patients is 25 patients and the number of female patients is 36 patients with age range 25-45 and 46-65. A total 52 patien Type 2 Diabetes Mellitus have cormobid and 9 patients without comorbid. DRP categories that occur include drugs without indication as much (8%), indications without drugs as much as (15%), and potential drug interactions (48%). The conclusion in this study is that Drug Related Problems (DRPs) have occurred in outpatients diagnosed with type 2 diabetes mellitus in UPT Puskesmas Gedong Air Inpatient with the highest DRPs category is the potential for drug interactions (48%). With the outbreak of the COVID-19 pandemic and the resulting restrictions, telemedicine has enabled healthcare to be provided to patients with chronic diseases, although with some restrictions. During the lockdown The Campania region was the first to order (03/27/2020), to protect the health of citizens suffering from diabetes mellitus, the use of telemedicine and a tele-health service was activated in our clinics. At the end of the consultation, a survey was submitted to our diabetic patients in order to evaluate their opinion about the telemedicine experience in comparison with the traditional clinic visit. 100% of both type 1 and type 2 patients consider the telemedicine visit adequate, 89.2% of types 1 and 75.9% of types 2 consider the two forms of visit absolutely overlapping from a professional point of view, and even 6.9% of type 2, but none of type 1, considers the tele-visit more effective than the traditional visit. However, only 44.8% (type 2 diabetic patients) and 13,5%% (type 1 diabetic patients) expressed a preference for telemedicine visits over traditional visits. Telemedicine has allowed continuity of care for diabetic patients despite the difficulties of this emergency time. We believe that the opinion of patients and health care professionals on the telemedicine experience can be useful to assess the limitations and advantages in order to improve and enhance the use of this method. Type 2 diabetes accounts for up to 90% of all diabetes cases. It is known that people with type 2 diabetes are often obese. Long-term complications also include problems with the cardiovascular, nervous and urinary systems. The model of type 2 diabetes mellitus based on a diet rich of fat and carbohydrates was used in the work. We observed a significant increase in the body weight of mice and sugar levels in their blood. Morphometric analysis of the pancreas showed an increase of its mass coefficient. Tactile sensitivity measurement showed an increase in its threshold. The developed model is quite suitable for analyzing the diabetes progression and its side effects on the body. Temporary remission of type 1 diabetes mellitus (T1DM) occurs following initiation of insulin therapy. This period of temporary remission without insulin therapy is called ‘honeymoon period'. Degenerative diseases especially diabetes mellitus in indonesia has increased in line with the progress work in social economic and the changing lifestyle .International diabetic federation ( idf ) reckon incidence diabetes mellitus in the world 2012 as many as 371 million people , 2013 increased to 382 million people and diperirakan in the 2035 increased to 592 million people . In indonesia, according to the basic health research ( riskesdas ) years 2013 mentioned an increasing prevalence of diabetes mellitus based on the doctor diagnosis in at age ≥15 year in 2013 as much as 1,5 % and in 2018 2,00 %. as much as. This community service aims to reduce the level of stress in patients with Diabetes Mellitus. Using training methods, conduct stress level measurements before and after training, with a total target of 20 people. Data analysis using Paired Sample T-Test. The results show there is the influence of training visualize on the level of stress patients diabetes mellitus with p value much as 0.021 ( lt; 0.05 ) .Suggested in research locations to consider the use of visualize as one of the act of to reduce the level of stress patients diabetes mellitus and to respondents to conduct visualize continuously in any given period of time .Key Words: Diabetes Mellitus, Stress, Visualisasi

The prevalence of diabetes and its complication is increasing all over the world particularly in developing countries. The latest estimates shows a global prevalence of 382 million people with diabetes in 2013 ,expected to rise 592 million by 2035. Diabetes mellitus is a multi system involving disease whose effects are seen by means of severe complication, commonest is musculoskeletal disorder. Diabeties is know to affect the shoulder in many ways commonest among them is Avabuhuka (frozen shoulder). Avabahuka is a disease of amsa sandhi with the clinical presentation such was Stambha, Shula, Shosha which can be correlated with the symptom of frozen shoulder, with no radiographic change only characterized by pain and restricted movement of shoulder joint. Prameha is a babhudoshaja vyadi mainly the meda and kapha are involved at the first it is necessary to bring rookshana effect before Shodhana that is Virechana karma to remove the kapha from kostha. Nasya is a main Chikitsa sutra mentioned in Avabahuka. Hence in the present study Rookshanapoorva Shodhana in the form of Udwartana followed by Virechana karma as kostha shodhanaartha followed by Nasya karma has been adopted. Statistical analysis showed highly significant results p value (less than 0.0001) in almost all subjective and objective parameters of Avabahuka.

Diabetes adalah penyakit kronis serius yang terjadi karena pankreas tidak menghasilkan cukup insulin (hormon yang mengatur gula darah atau glukosa), atau ketika tubuh tidak dapat secara efektif menggunakan insulin yang dihasilkannya. Melitus merupakan sindrom metabolis kronis yang menjadi salah satu penyebab kematian terbesar di dunia. Desain Penelitian ini adalah penelitian jenis Observasional, dengan menggunakan pendekatan cross sectional, dimana subyek penelitian hanya diobservasi. Penelitian ini mengambil subyek sebanyak 109 pasien. Pengambilan data secara prosepektif dan dilakukan dengan cara mengumpulkan informasi dari pasien melalui wawancara tatap muka (face-to-face interview) menggunakan kuesioner. Kuesioner yang digunakan pada penelitian ini berjumlah dua kuesioner yaitu, data demografi pasien, kuesioner kepuasan (TSQM), kuesioner Kualitas Hidup (EQ5D).           Analisis data  menggunakan Uji Statistik dengan bantuan SPSS versi 23. Data sosio-demografi pasien meliputi jenis kelamin, usia, pernikahan, pekerjaan, pendidikan dan penyakit penyerta pasien pendeita DM type 2 disajiakan dalam tabel distribusi frekuensi. Uji Chi Squaredilakukan untuk mengetahui hubungan antara sosio-demografi dengan kepuasan terapi dan hubungan sosio-demografi dengan kualitas hidup.           Hasil hitung dengan bantuan SPSS versi 23 menunjukkan bahwa nilai p value >0,05 sehingga dapat disimpulkan bahwa tidak ada hubungan antara sosio-demografi dengan kepuasan terapi dan tidak ada hubungan antara sosio-demografi dengan kualitas hidup. Hubungan antara kepuasan terapi dengan kualitas hidup dilakukan uji korelasi Spearman’s rho. Hasilnya diperoleh nilai p value (0,006) < 0,05. dengan demikian dapat disimpukan bahwa terdapat hubungan antara kepuasan terapi dengan kualitas hidup pasien penderita DM type-2.   Kata Kunci: Diabetes melitus tipe 2, Kepuasan Terapi dan Kualitas Hidup

The results of prospective international research for risk factors determining of depression and assessing their impact on life in patients with type 2 Diabetes Mellitus (DM) were described in this article. Aims: To study risk factors of depression in patients with type 2 DM, based on results of conduc ted research. Materials and me thods: accor ding to study requi rements of international research INTERPRET-DD (International Prevalence and Treat ment of Diabetes and Depres sion), 190 pa tients diff erent sex were rando mized. Adapted scales PHQ-9, PAID, WHO, MINI-6, Hamilton Score were used as materials for depression diagnostic; processed data from patients Medical History for the last 15 years, anamnesis, physical examination, consulting by psychiatrist, endocrinologist, podiatrist, HbA1C and Lipid profi le measu ring. The work consisted of tandem collaboration between a psychiat rist and an endocrinologist, who monitored randomized patients for one year, and also determined the tactics of diagnosis, treatment, and the development of preventive measures. According to received results, the follow risk factors of depression in patients with type 2 DM were frequent episodes of hypohyperglycemia, high level of concomitant diseases in anamnesis, patient’s age, low education level, job absence, unstable family income, family status, presence of depression in family anamnesis.
