Degenerative diseases especially diabetes mellitus in indonesia has increased in line with the progress work in social economic and the changing lifestyle .International diabetic federation ( idf ) reckon incidence diabetes mellitus in the world 2012 as many as 371 million people , 2013 increased to 382 million people and diperirakan in the 2035 increased to 592 million people . In indonesia, according to the basic health research ( riskesdas ) years 2013 mentioned an increasing prevalence of diabetes mellitus based on the doctor diagnosis in at age ≥15 year in 2013 as much as 1,5 % and in 2018 2,00 %. as much as. This community service aims to reduce the level of stress in patients with Diabetes Mellitus. Using training methods, conduct stress level measurements before and after training, with a total target of 20 people. Data analysis using Paired Sample T-Test. The results show there is the influence of training visualize on the level of stress patients diabetes mellitus with p value much as 0.021 ( lt; 0.05 ) .Suggested in research locations to consider the use of visualize as one of the act of to reduce the level of stress patients diabetes mellitus and to respondents to conduct visualize continuously in any given period of time .Key Words: Diabetes Mellitus, Stress, Visualisasi

Diabetes mellitus is one of the communicable diseases that have become a public health problem, not only in Indonesia but also the world. Currently morbidity of diabetes mellitus is increasing every year, where in 2006 there were 14 million people in 2011 and ranks fourth with 773 cases. DM is also a cause of disease mortality by 5.8%. And Samarinda own particular Islamic Hospital years 2009, there were 449 patients with DM, and 2011 an increase in the 1931 patients with diabetes mellitus. To increase patients’ knowledge about diabetes and diabetic patients be directing attitudes that support or positive attitude towards keeping blood glucose levels to remain normal. Methode to use pra eksperiment with one group pretest posttest. individual conseling withAudiovisual media. There is increased knowledge about diabetes diabetic patients before and after intervention with increasing value of 3.77 (p value = 0.000) and increase in attitudes regarding diabetes mellitus diabetic patients with an increase in the value of 5,35 (p value = 0.003). There was an increase in knowledge and attitudes of patients hospitalized with diabetes mellitus hospital after islam samarinda given nutritional counseling using audio-visual media

Patients with diabetes mellitus routinely require management in the adult intensive care unit (ICU). These patients have increased morbidity, mortality, hospital length of stay, cost of care, and frequency of hospital and ICU admission. Glucose control in these patients presents challenges for the clinician. In the critically ill, hyperglycemia does not occur exclusively in patients with diabetes or prediabetes but may be related to stress-induced hyperglycemia or iatrogenic causes. Hyperglycemia can contribute to decreased wound healing and immune function and a host of cellular and molecular dysfunctions and has been linked to increased hospital mortality. Hypoglycemia in the ICU is associated with patients with preexisting diabetes, those receiving insulin and other medications, and septic individuals, among others. Insulin therapy is the mainstay of glucose management in the critically ill. ICU practitioners should be aware that point-of-care glucose meters are not as accurate as core laboratory results. Finally, both hypoglycemia and wide fluctuations in blood glucose during critical illness are also associated with increased mortality, although clear cause-and-effect relationships have not been established. Diabetes mellitus is a major cause of morbidity and mortality.  Nearly 30 million Americans have diabetes, more than 25% of which are undiagnosed. Patients with diabetes have multiple problems that should be addressed prior to surgery. They often have uncontrolled glucose levels that should be treated preoperatively. Current studies suggest that outcomes may be improved if perioperative glycemic control is optimized. Patients with diabetes develop end-organ dysfunction that can complicate perioperative management. Preoperative assessment of cardiac, neurologic, vascular, and renal function is necessary for all patients with diabetes that undergo major surgery. Optimization of cardiac disease in particular can reduce major adverse cardiac events for patients with risk factors such as diabetes. Diabetic patients can occasionally present for major surgery with hyperglycemic emergencies such as diabetic ketoacidosis and hyperglycemic hyperosmolar state. These conditions require urgent treatment to prevent mortality regardless of the need for surgery. This article reviews the preoperative assessment and management of these issues. Heart Failure (HF) and Type 2 Diabetes Mellitus (T2D) are important clinical conditions that often coexist, mutually influencing intra- and extra-hospital morbidity and mortality. In order to characterize the diagnostic and therapeutic management of patients with HF and T2D, a retrospective observational study was conducted on database of the last 12 months in 8 Campania Diabetology Centers. 164 patients were affected by HF and T2D. Among them, 123 patients had a medical record with reproducible data and were recruited for the study. Diabetic patients were divided into three groups: group A included patients with preserved ejection fraction (EF) (> 50 %), group B patients with midrange EF (40-49%) and the group C patients with reduced EF (< 40%). All patients had performed ECG and echocardiography, repeated every 6 months; 41 patients (33% of the sample) also performed a cardiac Holter. The most frequent causes of HF were ischemic heart disease and hypertension. After a 12 months follow up, the clinical and laboratory parameters and the treatments adopted were re-evaluated. The antidiabetic drugs resulted modified with a reduction in the use of metformin, sulfonylureas, glinids and pioglitazone; at the same time a greater use of gliptins, gliflozines and GLP1 AR and a lower use of insulin therapy was observed. Cardiological therapy was modified with a greater use of diuretics and nitrates and a reduction of ACEI and ARB, probably due to the use of sacubitril/valsartan association in the group of patients with reduced EF. According to the recent guidelines, antidiabetic therapy must be tailored to the characteristics of the patients with diabetes and heart failure, preferring the use of molecules that have shown a cardiovascular protection effect or, secondarily, those with cardiovascular neutrality. Similarly, cardiological therapies have to consider the type of antidiabetic agents used and benefit from molecules that impact clinical symptoms and natural history. Finally, the approach to the patients with both the pathologies must necessarily take place in the healthcare districts and absolutely be multidisciplinary and integrated, involving firstly Diabetologists and Cardiologists, but also other professional roles (nurses, psychologists, physiotherapists, caregivers) operating in the territorial healthcare services. Diabetes mellitus is associated with an increased incidence of some infections and a greater severity and/or frequency of complications related to these diseases. Influenza is characterized by an increased severity of clinical course and risk of complications, especially in diabetic patients who are more susceptible to influenza infection. For these reasons, the Italian Vaccine Prevention Plan 2017-19 provides an active and free offer of influenza vaccine for the diabetic subjects. In addition, among the vaccinations recommended by the Italian Prevention Plan in adults, including the diabetes ones, there is the diphtheria-tetanus-pertussis vaccine and the decennial booster. To investigate what is the perception of Italian diabetologists on the role and importance of the influenza and the diphtheria-tetanus-pertussis vaccines, AMD has promoted an online survey. Participants claimed to be aware of the importance of carrying out and recommending influenza vaccination, while awareness of the usefulness of performing and suggesting the decennial booster for diphtheria-tetanus-pertussis was lower. Diabetologists attribute to patients’ resistance and lack of interest in such vaccinations the main motivation for which they are not used to recommend them, even if they acknowledge that they have little knowledge about the decennial booster of diphtheria-tetanus-pertussis vaccine. The survey shows that the percentage of patients with diabetes who seek advice on these vaccinations is inadequate and the diabetologists’ knowledge of the National Vaccination Prevention recommendations and the need to actively promote vaccinations is inappropriate. This survey has allowed to highlight the opinion, of a significant percentage of Italian diabetologists, on some key aspects of the vaccination therapy indicated in diabetic patient, allowing to gather important information to open a debate, to know strengths and weaknesses on this topic and implement training activities. The success of changing the damage to the integrity of the skin in patients with Diabetes Mellitus Type II depends not only on early detection, but also on understanding the causative mechanism. One condition that can cause damage to the integrity of the skin was the xerosis and pruritus on skin . The purpose of this study to determine the effect of Oil Olive to damage the integrity of the skin in patients with DM type II in the District of Pagar Merbau. This type of research uses the Quasy Experiment Method using the One-Group Pretest-Posttest Design research design. Determination of the sample was done by purposive sampling, with a total sample of 22 people. Data analysis was performed using Paired Sample T Test with 95% confidence interval and α= 0.05. The results of this study indicate the change in damage to the integrity of the skin before and after given a basting oil olives with a p-value = 0.000 (α> 0.05). Conclusion, there was an influence of olive oil on damage to skin integrity in patients with type II DM in Pagar Merbau District which was carried out in accordance with standards. For Health Institutions the need for supervision and counseling to the community that the importance of maintaining skin health especially in patients with diabetes mellitus in Pagar Merbau District

Hyperglicemia in DM Type will induce macrofag to produce cytokin proinflamation TNF-α. High levels of TNF-α in DM Type 2 can lead to more severe insulin resistance and become involved in endothelial dysfunction which lead to complications of the disease. This research aims to know differences between TNF-α level in DM type 2 controlled and uncontrolled. This research used obervational analizing method and cross sectional study comparative approach. Blood samples are grouped to 35 patient DM type 2 controlled and uncontrolled in the medicinal treatment in Padang State University polyclinic. The level of serum TNF-α will be examined in Biomedical Laboratory FKUA using ELISA method. TNF-α level in the diabetes type 2 controlled on average 7,55 ± 0,43 pg/ml whereas uncontrolled on average 149,28 ± 26,82 pg/ml. The Mann-Whitney test showed a p value < 0,001. There are significant differences in the levels of TNF-α in patients with DM type 2 controlled and uncontrolled. Key Words : Diabetes Mellitus type 2 controlled and uncontrolled, TNF-α

Depresi pada diabetes mellitus tipe 2 lebih umum pada: wanita, ras minoritas, seseorang yang tidak menikah di usia paruh baya, status sosial, ekonomi rendah dan tidak bekerja. Kurangnya perhatian pada kondisi ini seharusnya tidak terjadi, untuk itu kita memerlukan metode psikoterapi untuk membantu pasien diabetes mellitus dalam menangani depresi.Banyak metode psikoterapi yang bisa digunakan, salah satunya adalah teknik kebebasan emosional spiritual. Tujuan dari penelitian ini adalah untuk mengidentifikasi pemberian terapi Spiritual Emotional Fredoom Technique (SEFT) untuk mengurangi tingkat depresi pada pasien diabetes militus di Wilayah Kerja Pusat Kesehatan Singkawang Barat pada tahun 2018. Desain penelitian ini adalah eksperimen semu dengan pre dan post test tanpa kontrol. Hipotesis yang digunakan adalah Ha yaitu Intervensi Teknik Spiritual Emotional Freedom (SEFT) untuk mengurangi tingkat depresi pada pasien Diabetes Millitus, instrument pengumpulan data menggunakan dengan Beck Depression Inventory yang sudah dimodifikasi. Hasil penelitian ini adalah karakteristik responden sebagian besar responden berusia 40-60 tahun 17 responden (68%), mayoritas adalah responden perempuan 17 responden (68%), bekerja sebagai ibu rumah tangga 13 responden (52%), pendidikan Responden SMA 9 (36%) dan sebagian besar responden menderita DM selama <5 tahun sebanyak 15 responden (60%). Hasil penelitian menunjukkan perbedaan nilai antara sebelum dan sesudah melakukan terapi Spiritual Emotion Freedom Technique (SEFT) untuk responden yang dilakukan dengan Wilcoxon Test menghasilkan nilai probabilitas 0,000 (nilai p> 0,05), yang menunjukkan perbedaan yang signifikan antara sebelum dan sesudah perawatan pada pasien diabetes mellitus. Penelitian ini menunjukkan bahwa pemberian terapi SEFT dapat mengurangi depresi pada pasien dengan diabetes mellitus di wilayah Puskesmas Singkawang Barat. Kata kunci: depresi, diabetes millitus, teknik kebebasan emosional spiritual FREDOOM THERAPY SPRITUAL THERAPY THERAPY REDUCES DEPRESSION IN DIABETES MILLITUS PATIENTS ABSTRACTIt turns out that depression in type 2 diabetes mellitus is more common in: women, racial minorities, someone who is not married in middle age, social status, low economy and not work. Lack of attention to this condition should not occur, for that we need a method of psychotherapy to help patients with diabetes mellitus in dealing with depression. Many psychotherapy methods that can be used, one of which is the spiritual emotional freedom technique. The purpose of this study was to identify the provision of Spiritual Emotional Fredoom Technique (SEFT) therapy to reduce levels of depression in diabetes millitus patients in the Working Area of Singkawang Barat Health Center in 2018. The design of this study is a quasi experiment with pre and post test without control. The hypothesis used is Ha namely the Intervention of Spiritual Emotional Freedom Technique (SEFT) to reduce the level of depression in Diabetes Millitus patients. The results of this study are the characteristics of the respondents most of the respondents aged 40-60 years 17 respondents (68%), the majority are female respondents 17 respondents (68%), work as housewives 13 respondents (52%), education SMA 9 respondents (36%) and most of the respondents suffered from DM for <5 years as many as 15 respondents (60%). The results showed differences in values between before and after conducting Spiritual Emotion Freedom Technique (SEFT) therapy for respondents conducted with the Wilcoxon Test resulting in a probability value of 0,000 (p value> 0.05), which indicates a significant difference between before and after treatment in diabetes mellitus patients. The conclusion from the above research shows that the administration of SEFT therapy can reduce depression in patients with diabetes mellitus in the area of Singkawang Barat Health Center in 2018 and it is recommended that psychotherapy is needed by patients experiencing psychosocial problems due to physical illness, one of which is depression. Keywords: depresi, diabetes millitus, spiritual emaotional freedom technique

Diabetes mellitus dapat menyebabkan cukup banyak komplikasi seperti kelainan mata, kelainan ginjal, kelainan pembuluh darah dan kelainan pada kaki. Penderita diabetes mellitus yang mengalami komplikasi kronis perlu diberikan upaya preventif untuk mencegah komplikasi, salah satunya adalah kemampuan perawatan kaki. Penelitian ini dilakukan untuk menguji efektivitas supportive educative system dalam meningkatkan kemandirian perawatan kaki pada pasien dengan diabetes mellitus Tipe II di Puskesmas Permata Sukarame, Bandar Lampung. Penelitian ini menggunakan metode quasy eksperimen dengan desain pretest-posttest with control group dengan total masing-masing kelompok sebanyak 18 responden. Pengumpulan data dilakukan dengan menyebarkan kuesioner kepada responden yang memenuhi kriteria inklusi penelitian. Penelitian ini diuji menggunakan analisis univariat, bivariat dan uji T-test. Hasil penelitian menunjukkan bahwa terdapat perbedaan rata-rata dalam kemandirian perawatan kaki pada kelompok intervensi dan kelompok kontrol dengan p-value 0,000. Pendidikan dan praktik perawatan kaki harus diberikan sejak dini sebagai upaya pencegahan untuk komplikasi.   Kata kunci : Supportive educative system, kemandirian perawatan kaki   SUPPORTIVE EDUCATIVE SYSTEM IN IMPROVING INDEPENDENCE OF FOOT CARE IN PATIENTS WITH DIABETES MELLITUS TYPE II   ABSTRACT Diabetes mellitus can cause quite a lot of complications such as eye disorders, kidney disorders, vascular disorders and abnormalities in the legs. Patients with diabetes mellitus who have chronic complications need to be given a preventive effort to prevent complications, one of which is foot care ability. This research was conducted to examine the effectiveness of supportive educative systems in increasing the independence of foot care in patients with Type II diabetes mellitus in the Permata Sukarame Health Center Bandar Lampung Working Area. This study used a quasi-experimental method with pretest-posttest with control group design with a total of 18 respondents each. Data collection is done by distributing questionnaires to respondents who meet the research inclusion criteria. This study was tested using univariate, bivariate, T-Test analysis. The results showed that the mean differences in the independence of foot care in the intervention group and the control group in the Permata Sukarame Community Health Center work area with a p-value of 0,000. Education and practice of foot care should be given early as a preventative effort for complications.   Keywords: Supportive educative system, independence of foot care
