Identification of Public Health Development Index (PHDI) Indicators to Increase the Value of Sub-Index of Communicable DiseaseAbstractSince 2009, Ministry of Health through Indonesia Agency fro Health Research and Development has formulated Public Health Dvelopment Index (PHDI) to describe public health status in Indonesia. In 2013, PHDI grouped into seven sub indices, namely health status of children under five, reproductive health, health service, health beaviour, non-communicable disease, communicable disease, and environmental health. Diarrhea and acute respiratory infection (ARI) on children under five and pneumonia are infectious diseases used in the calculation of subindex of communicable diseases in PHDI 2013. The aim of this research was to identify indicator in PHDI that could improve the subindex of communicable disease. PHDI is a composite of 30 major health indicators calculated from Basic Health Research (Riskesdas) 2013. The Riskesdas 2013 design was cross sectional survey with total sample of all households in 33 provinces and 497 district/cities in Indonesia. The data was analyzed by linier regression test. The result showed that indicator of access to sanitation (p=0.000), access to clean water (p=0.027) and people’s behavior in proper hand washing (p=0.001) had an important role in imporving the score of sub-index of communicable disease. It is expected that with the increase of the three indicators above, the value of three subindices (communicable disease, environmental health, and health behaviour) will also increase. Therefore, if the district/city wants to increase the value of sub index of communicable disease, the intervention to the environment and human behavior is needed. Thus, the PHDI score in district/city will also increase. Keywords: Behavior, communicable disease, environment.

Background. The increased morbidity and mortality rates in children under five in developing countries are mostly attributed to poor availability and failure of prescribing lifesaving medicines. This study was aimed at evaluating the availability and utilization of the WHO-recommended priority lifesaving medicines for children under five in public health facilities. Method. A cross-sectional survey complemented with a qualitative method was conducted in 14 health centers and four hospitals in the Jimma Zone, Ethiopia. In the facilities, we assessed the availability within the last half year and on the day of the visit. Utilization of the medicines was assessed through a review of patient records of the last one year. Twelve in-depth interviews were carried out to collect the qualitative data, and the analysis was executed using thematic analysis. Results. For treatment of pneumonia, amoxicillin dispersible tablets and gentamycin injection were available in 94.4% of the facilities. For treatment of malaria, artemether/lumefantrine was available in 61.1% of the facilities. For pain management, paracetamol tablets were available in 94.4% of the facilities. AZT+3TC+NEV for HIV/AIDS management was available in all facilities. At least one essential medicine was out of stock in the past six months with the average duration of 33.6 days in health centers and 28.25 days in hospitals. Oral rehydration salt and zinc (84.7%) and AZT+3TC+NEV (100%) had better utilization. However, for almost all cases, other nonpriority medicines were highly prescribed. Lack of administrative commitment, supply of near expiry products, complexity of diseases, and lack of customized child formulations were among the challenges of availability and utilization of those medicines. Conclusions. The overall availability of lifesaving medicines on the day of the visit was fairly good but with poor utilization in almost all facilities. Some products were not available for considerable length of time in the past six months.

Background Despite remarkable reduction in the prevalence of dental caries in developed countries, dental caries is still a highly prevalent disease among children in developing countries. Objectives To estimate the oral health status of children under five years of age by assessing decayed, missing and filled teeth (dmft) status of primary teeth along with their tooth cleaning and food habits. Materials and Methods This cross-sectional study on oral health status of children under five years of age was carried out among 107 children attending the Outpatient Department of Sapporo Dental College and Hospital, situated at Uttara, Dhaka. Data was collected both in English and Bengali with a pre-tested structured questionnaire and a checklist. Results Dental caries (d) was found in 69.2% of the children while missing (m) and filled (f) teeth were recorded in 14% and 21.5% of the children respectively. Almost three quarter children (72%) demonstrated an overall dmft score 1 or higher; whereas one third children (28%) demonstrated dmft score 0 or free from any form of dental decay, missing teeth and dental filling. Increased age of the children was directly associated (p<0.05) with the number of decayed, missing or filled teeth. The study also revealed that correct method of brushing was practiced by only 44% of the children and recommended brushing time (1-3 minutes) was done by 37.3% only. Children who were assisted by their parents during brushing experienced less caries compared to the children who brushed their teeth by themselves and this finding was statistically significant (p<0.05). Taking snacks between meals was preferred by 106 children and among them 97.2% did not clean their mouth after having snacks. DOI: http://dx.doi.org/10.3329/updcj.v1i1.13945 Update Dent. The objectives of the current research are to examine Jamaicans’ health status, pulse rate, body mass index (BMI), hypertension, and factors influencing 1) hypertension, 2) blood pressure, and 3) pulse rate, during COVID-19. A cross-sectional survey using associational research design by way of convenience sampling was used to conduct this research. This study examined correlation of age, gender, BMI, hypertension, and blood pressure among Jamaicans age ranging from 15 to 85 and above. Cross–sectional study and associational research design were used to collect data for the period of June to August 2020. The study demonstrated that hypertension is significantly predicted by gender, age, and pulse rate one─Omnibus tests of model (χ2(4) =138.947, P < 0.0001, -2Ll=2098.67) and Wald statistic being 90.161, P < 0.0001, and the variance in model 10.8% (Nagelkerke R2). Furthermore, ordinary least square (OLS) regression can be used to model pulse rate (bpm) for Jamaicans (F [4, 1676] =17.236, P < 0.0001), with the model explaining 4.0% of the variance in pulse rate of Jamaicans. Gender, BMI and Hypertensive emerged as the factors that determine the pulse rate of Jamaicans. Diastolic & systolic blood pressure were determined by age, weight, and pulse rate, with age being the most significant predictor. The risk of hypertension was higher among population groups who were overweight and obese. Gender was found to correlate with blood pressure and hypertension; female participants were more likely to be hypertensive than the male participants. BMI measurement should be recommended as a simple and effective predictor of hypertension in public health strategies. There is empirical evidence that can be used to establish that Jamaicans health status has worsen since March 10, 2020, and that Covid-19 has brought with it unhealthy lifestyle practices, which are pending public health challenges come 2021 and beyond.

Background:  Anemia is a severe public health problem affecting more than half of children under five years of age in low-, middle- and high-income countries. We aimed to determine the prevalence and factors associated with anemia among children under five years of age in northern Tanzania. Methods: This was a community-based cross-sectional study conducted in Rombo district, Kilimanjaro region, northern Tanzania in April 2016. Multistage sampling technique was used to select a total of 602 consenting mothers and their children aged 6-59 months and interviewed using a questionnaire. Data were analyzed using Stata version 15.1. We used generalized linear models (binomial family and logit link function) with robust variance estimator to determine factors associated with anemia. Results: Prevalence of anemia was 37.9%, and it was significantly higher among children aged 6-23 months (48.3%) compared to those aged 24-59 months (28.5%). There were no significant differences in anemia prevalence by sex of the child. Adjusted for other factors, children aged 6-23 months had over two times higher odds of being anemic (OR=2.44, 95% CI 1.71, 3.49, p<0.001) compared to those aged 24-59 months. No significant association was found between maternal and nutritional characteristics with anemia among children in this study. Conclusion: Prevalence of anemia was lower than the national and regional prevalence but it still constitutes a significant public health problem, especially among children aged 6-23 months. Interventions such as iron supplementation, food fortification and dietary diversification and management of childhood illnesses in this setting should be targeted towards mothers and children less than two years.
