Abstract We report a case of aseptic abscess in the cavernous body at the base of the penis. In our clinical observation, the patient underwent puncture and drainage of the corpus cavernosum abscess, followed by surgical resection of the abscess wall, with the incisions closed layer by layer with primary suture. In addition, we paid attention to strengthening the postoperative management by using elastic bandages to wrap the penis intermittently to prevent edema; the incision would not be covered with dressings from the third day after the operation, so as to keep the incision site dry in an open way. During the period of indwelling of the catheter after the operation, we noticed the care of the external orifice of the urethra to reduce the occurrence of catheter-related infections. Finally, the patient was diagnosed with a penile aseptic abscess in the cavernous body at the base of the penis. The patient recovered well after surgery and was discharged 1 week later. At 1.5 years after the operation, the shape of the penis returned to normal, and the erectile function was normal. It was seen that good nursing concept is of great help for prognosis, which could avoid infection and edema, and is conducive to wound healing.

Having made comparative observations on a whole row, in equally favorable conditions, the puerperas, from whom one did not receive anything, the other ergotin, and the third, finally, cornutin-Kbert (8 mg. Pro dosi 2 cg. Pro die), K. could be convinced of the undoubted action of this the last means for the earliest possible reverse development of the uterus. Thus, it has been confirmed, already established by other authors, that many of this preparation are included as the only unmistakable component of the uterine horns.

Abstract In bioprinting, biomaterials are deposited layer-by-layer to fabricate structures. Bioprinting has many potential applications in drug screening, tissue engineering, and regenerative medicine. Both animal cells and plant cells can be used to synthesize bioinks. Green bioprinting uses bioinks that have been synthesized using plant cells. Constructs fabricated via green bioprinting contain immobilized plant cells, with these cells arranged at desired locations. The constructs provide scaffolds for cell growth. Printing parameters affecting the growth of cells in green bioprinted constructs include print speed, needle diameter, extrusion temperature, and extrusion pressure. This paper reports a study to examine effects of extrusion pressure on cell growth (measured by cell count) in bioprinted constructs, using bioink containing Chlamydomonas reinhardtii algae cells. Three levels of extrusion pressure were used: 3, 5, and 7 bar. Cell counts in the bioprinted constructs were measured on the third and sixth days after bioprinting. It was found that, as extrusion pressure increased, cell count decreased on both the third and sixth days after bioprinting. Furthermore, the difference in cell counts between the third and the sixth days decreased as extrusion pressure increased. These trends suggest that increasing extrusion pressure during green bioprinting negatively affects cell growth. A possible reason for these trends is physical damage to or death of cells in the bioprinted constructs when extrusion pressure became higher.

The focus of this article is a symbolic image often found in world mythology - a giant snake or a dragon biting its own tail. This image is usually denoted by the Greek word “ouroboros” ( οὐροβόρος ), which means literally “eating its own tail.” This essay is devoted to an interpretation of this symbol, which the author sees as leading to the much broader topic of human unfreedom and the forms that this unfreedom takes. The first section deals with the unique features of Gnosticism which have made it appealing in extremely varied times and situations. Theauthor’s reflections start from understanding the Gnostic worldview as an expression of apprehensiveness about the radical otherworldliness of the human spirit and its alienation from the universe. The second section deals with the symbolism of the ouroboros and its place in Gnostic conceptual schemes as a reference to the closed cycle of nature that enslaves the human spirit. The third section attempts to decipher layer by layer the Gnostic conceptions associated with the ouroboros. Various levels of interpretation are identified: literal, mythological-magical, psychological-ascetic and socio-political. In the fourth section, the author connects Gnostic ideas with Christianity by interpreting St. Paul’s Epistles, particularly his ideas concerning rulers and authorities. The place occupied by the ouroboros in the Christian universe is analyzed. The last section relies on the ideas of René Girard, Jacques Lacan and Alain Badiou to illustrate the manifestations of the ouroboros in different dimensions of human existence, both individual and collective, with special emphasis on human desire and its futile circlings.

This study examines the variations in incisions and postoperative protocol of open carpal tunnel release. A questionnaire was distributed to 65 hand surgeons. Respondents were asked to draw their preferred incision on original photocopies of the same palm. The results were measured against standard anatomical landmarks (thenar crease, vertical axis of the third web space, proximal palmar crease and the distal wrist crease). The participants were also asked to answer questions concerning their postoperative protocols. Demographics of the cohort, as well as the variations in incisions and postoperative management, were analyzed. Significant variations existed in the length and location of the incision in the palm. The differences in postoperative care in this cohort of surgeons were less significant.

Abstract OBJECTIVE: To explore whether opening the external urethral orifice in the coronal sulcus can reduce the incidence of epididymitis after operating on hypospadias with a prostatic cyst connected to the vas deferens.MATERIALS AND METHODS: Three patients (group A) (average age = 3.3 years old) with severe hypospadias and prostatic cyst underwent cystostomy, hypospadias correction and urethroplasty, along with relocation of the external orifice of urethra. Group B consisted of 4 patients (average age = 6.9 years old) presenting with epididymitis after hypospadias surgery and Unsuccessful conservative treatment. Patients underwent urethral dilatation along with urethral catheterization, cutting of the original corpus cavernosum that encapsulated the urethra, and extension of the position of the external urethral orifice to the coronal sulcus. RESULTS: In group A, 3 children underwent bladder fistula removal 2 weeks after operation. All patients were followed up for 5-7 years. The vulva developed normally without any complications. Four children in group B underwent stent removal 12 weeks after operation, and one patient was still stenosed and dilated again. All patients were followed up for 2-12 years without epididymitis recurrence. Penile erection and ejaculation were normal in adulthood. CONCLUSION: For hypospadias patients experiencing complications due to the presence of a prostatic sac, especially those with prostatic sac connected to the vas deferens, the positioning of the external urethral orifice in the coronary sulcus would be helpful to reduce the occurrence of epididymitis.

The author compares the two manuscript versions of Beyond the Pleasure Principle, composed in April/May 1919 and by August 1920 respectively, with each other and with the printed editions. In the first version the idea that all drives are urging to death is already present; however the term ‘death drive’ is not. Here, Freud attempted to take account of the clinical observation that in traumatic neurosis unpleasurable experiences are repeated and that there is a kind of compulsive repetition that is inaccessible to analysis. On the other hand he extended his perspective far into biology. Already in the first version Freud withdrew the notion that all drives urge to death, restricting this urge to self-preservative drives. The second manuscript version of BPP represents a further stage of reflection – primarily in the sixth chapter that was inserted into the older text. The author highlights that Freud now gave up the idea of the first version that self-preservative drives are urging to death and ascribed this urge to a special group of drives: the ‘death drives’. As their counterpart only at this point did he introduce Eros, which encompassed both the sexual and the self-preservative drives. The concept of Eros enabled him to come to terms with phenomena of union, whether at the level of cells, organisms or human beings, including sexual reproduction, which had resisted accommodation to his notion of autoerotic infantile sexuality. It is a main finding of this paper that the process of theoretical reflection, leading to several changes of concept, is reflected in the printed text as we know it.

Three clinical observations dealing with sexual problems in women as a result of influence of unfavourable psychogenic factors are described. The first case (a false sexual disorder) is a report about the woman, whose psychogenic anorgasmia was caused by the situation (a response to nonverbal stimuli during the coitus on the part of her husband). Her husband had regular sexual relations with another woman. For him the coitus, which he had with his wife after 10 months of a break in their intimacy, was extremely undesirable. It was almost by force that she made him have it with her, but she failed to feel orgasm. The second clinical observation deals with a female patient having a pathological sexual stereotype, whose formation resulted from many years of masturbation. She had been masturbating since the age of 11. From the age of 11 to 14 she simply had pleasant sensations. Since the age of 14 her masturbations have been causing orgasm. She reaches it after pressing her legs together (the myocompression variant). Different kinds of genital stimulation on the part of men (their fingers, tongue) fail to cause orgasm. During the coitus, orgasm appears only in case of its combination with pressing of her legs (myocompression masturbation). Virtually this is a pathological variant of coital anorgasmia. The third case reports about a woman, who was subjected to significant psychosexual traumatization on the part of her sexual partner. By all means he degraded and outraged her, accusing of an early start of her sexual life. It resulted in the formation of a sexual disorder, which manifested itself with different sexual dysfunctions, as well as in a long-lasting “slipping” to homosexual relations. But due to their futility with respect to formation of the family as well as because of the absence of the formed homosexual desire the patient changed over to heterosexual relations.
