Objective: To study the effect of Stereoscopic Comprehensive of Therapy Zhuang Medicine on IL-6 in serum of patients with rheumatoid arthritis. Methods: Sixty rheumatoid arthritis patients who met the inclusion criteria were selected and randomly divided into a control group and a treatment group using a random number table. Among them, 30 cases in the control group were treated with Western medicine, and 30 cases in the treatment group were treated with Western medical and Stereoscopic Comprehensive Therapy of Zhuang Medicine. The observation period was 8 weeks. Results: After 8 weeks of treatment, the level of IL-6 in the treatment group and the control group decreased, and the treatment group was better than the control group (P<0.05). Conclusion: The Stereoscopic Comprehensive of Zhuang Medicine can reduce the level of IL-6 in the serum of patients with rheumatoid arthritis, and has a good regulating effect on the inflammation of rheumatoid arthritis.

Objective: To investigate the clinical effect of modified closed negative pressure suction technique combined with flap transplantation on the treatment of deep chronic refractory wounds.Methods: During March of 2015 to April of 2018, 52 cases of patients with deep chronic refractory wounds were selected as research objects. They were divided into the control group and the treatment group by use of the random number table method, with 26 cases in each group. Among them, the control group was given conventional debridement combined with flap reconstruction, and the treatment group was treated with modified closed negative pressure suction technique combined with flap transplantation to observe the clinical effect.Results: (1) According to the analysis on the effect of flap transplantation, the excellent and good rate of the treatment group was 92.3%, and in the control group, it was 76.9% (p < .05). (2) According to the statistics, the incidence of complications in the treatment group was lower than that in the control group (p < .05).Conclusions: Modified closed negative pressure suction technique combined with flap transplantation has a good effect on the treatment of deep chronic refractory wounds with fewer complications.

Coronavirus disease 2019 (COVID-19) has become a global pandemic. Community and close contact exposures continue to drive the COVID-19 pandemic. There is no confirmed effective treatment for suspected cases and close contacts. Lianhuaqingwen (LH) capsules, a repurposed Chinese herbal product that is currently on the market, have proven effective for influenza and COVID-19. To determine the safety and efficacy of LH capsules for the prevention of COVID-19, we conducted a prospective open-label controlled trial of LH capsules on subjects who had close contact with people infected with COVID-19. Subjects received LH capsules (4 capsules, three times daily) or the usual medical observation for 14 days. The primary endpoint was the rate of positive nucleic acid tests of nasal and pharyngeal swabs during the quarantine medical observation period. We included 1976 patients, including 1101 in the treatment group and 875 in the control group. The rate of positive nucleic acid tests in the treatment group was significantly lower than that in the control group (0.27% vs. 1.14%, respectively; mean difference: −0.87%; 95% CI: −1.83 to −0.13; p = 0.0174 ) during the quarantine medical observation period (14 days). Among subjects with different close contact states, there was no significant difference in the rate of positive nucleic acid test results among close contacts in the treatment group and the control group (6.45% vs. 11.43%, respectively; p = 0.6762 ). Among secondary close contacts, the rate of positive nucleic acid tests in the treatment group was significantly lower than that in the control group (0.09% vs. 0.71%, respectively; p = 0.0485 ). No serious adverse events were reported. Taken together, and in light of the safety and effectiveness profiles, these results show that LH capsules can be considered to prevent the progression of COVID-19 after close contact with an infected person. Objective. To examine the efficacy of vitamin E in methotrexate- (MTX-) induced transaminitis in patients with rheumatoid arthritis (RA). Methods. A case-control study was conducted at a tertiary rheumatology center for 12 months. Patients with RA on MTX and deranged aminotransferases were included. Patients with previous liver diseases, baseline transaminitis before methotrexate initiation, alcohol intake, muscle diseases, under hepatotoxic drugs, and aminotransferases>3 times the upper normal limit were excluded. The patients were divided into treatment (vitamin E 400 mg bid for 3 months) and control groups (no vitamin E) using a random number table. The dose of MTX was unaltered. Follow-up was done after 3 and 6 months. Independent t-test was done to compare means of two groups. Paired t-test was done to compare differences in mean. Results. Among 230 patients, 86.5% were female with a mean BMI of 25.9±4.5 kg/m2. In the treatment group, SGPT and SGOT at baseline were 73.1±20.4 and 60.2±24.5 IU/L, respectively; at 3-month follow-up 44.6±34.2 and 38.3±20.8 IU/L, respectively; and at 6-month follow-up 40.4±35.7 and 34.2±21.9 IU/L, respectively. In the control group, SGPT and SGOT at baseline were 63.4±15.1 and 46.8±13.7 IU/L, respectively, and at 3-month follow-up 55.8±45.9 and 45.5±30.9 IU/L, respectively. Significant decrease in the level of aminotransferases was seen in the treatment group (p value < 0.001) and not in the control group (p values 0.161 and 0.728, respectively). The change in levels of SGPT and SGOT from baseline to 3 months of follow-up was statistically significant in between two study groups (p values 0.007 and <0.001, respectively). From the control group, 29 patients were crossed over to vitamin E for the next 3 months. SGPT and SGOT decreased from 97.6±44.1 to 46.1±40.9 and 69.3±34.9 to 29.1±11.6 IU/L, respectively (p values 0.031 and 0.017, respectively). Conclusion. Vitamin E significantly attenuates MTX-induced transaminitis.

Objective. This study aims to evaluate the efficacy of moxibustion on joint swelling and pain and the levels of C-X-C motif chemokine ligand 1 (CXCL1), β-endorphin (β-EP) in serum of rheumatoid arthritis (RA) patients and to investigate the anti-inflammatory and analgesic mechanism of moxibustion on improving RA. Methods. Sixty-eight patients with RA were randomly and equally classified into the control and treatment groups. The control group was treated with routine drug therapy, while the treatment group received routine drug therapy and moxibustion. Both groups were treated for eight weeks. The symptoms and laboratory indicators of RA patients were compared in the two groups before and after intervention. Results. Sixty-one patients completed the study: four patients dropped out from the treatment group and three from the control group. Trial endpoints were change (∆) in symptoms, measured by Ritchie’s articular index (RAI), swollen joint count (SJC), and laboratory indicators, measured by the level of CXCL1, β-EP, tumor necrosis factor-a (TNF-α), and interleukin-1β (IL-1β). ∆RAI, ∆SJC, ∆CXCL1, ∆β-EP, ∆TNF-α, and ∆IL-1β in the treatment group were superior to the control group (13.50 [14.50] versus 6.00 [13.00] in ∆RAI, 4.00 [3.00] versus 2.00 [4.00] in ∆SJC, 0.04 ± 0.79 ng/mL versus -0.01 ± 0.86 ng/mL in ∆CXCL1, -2.43 [5.52] pg/mg versus -0.04 [4.09] pg/mg in ∆β-EP, 3.45 [5.90] pg/mL versus 1.55 [8.29] pg/mL in ∆TNF-α, and 6.15 ± 8.65 pg/mL versus 1.28 ± 8.51 pg/mL in ∆IL-1β; all P  < 0.05). Conclusion. Moxibustion can improve the joint swelling and pain symptoms in patients with RA, which may be related to the fact that moxibustion can reduce the release of inflammatory factors in patients with RA and downregulate the level of CXCL1 and increase the level of β-EP at the same time. Objective. To investigate the effects of dexmedetomidine intervention on serum inflammatory factor concentration and postoperative cognitive malfunction in elderly patients with general anesthesia. Methodology. 174 patients with general anesthesia were selected, who were categorized into a control group (HC) and a dexmedetomidine group (HS) using the random number table method, with 87 patients in individual groups. The dexmedetomidine group was pumped intravenously with dexmedetomidine at a loading dose of 1 μg/kg before induction of anesthesia for 15 min, followed by continuous intravenous pumping at a rate of 0.4 μg/kg/h, and the dosing was stopped at 30 min before concluding the surgery. The control group was administered the identical dose of saline in the same manner. Interleukin 6 (IL-6) and tumor necrosis factor α (TNF-α) levels and MMES scores were tested at 1 h before and 24 h after anesthesia. Results. Comparing to HC group, patients in the HS group had lower TNF-α and IL-6 levels at both scheduled points ( P  < 0.05). Conclusion. Dexmedetomidine reduced the expression of inflammatory factors in elderly patients with general anesthesia and effectively reduced the incidence of postoperative cognitive dysfunction after general anesthesia surgery.

Objective: To study and analyze the clinical effect of cluster intervention strategy in patients with stroke associated pneumonia. Methods: The time span of the study was from April 2018 to March 2019. 70 stroke patients were selected and divided into the study group and the control group according to the random number table model. The control group was treated with routine nursing, and the study group was treated with cluster intervention. The indicators of the two groups were compared and analyzed. Results: Compared with the incidence of aspiration and stroke associated pneumonia, the study group had more advantages (P< 0.05). Conclusion: Cluster intervention can significantly reduce the incidence of associated pneumonia in stroke patients, which is worthy of comprehensive promotion.

Objective: To observe the efficacy of Yin-tonifying Ophiopogonis Decoction combined with Rehabilitation New Liquid in the treatment of reflux esophagitis (RE). Methods: Seventy-four patients with RE were included in the study and they were divided into an observation group and a control group using the random number table method with 37 cases in each group. The patients in the control group were given oral Yin-tonifying Ophiopogonis Decoction whereas the patients in the observation group were additionally treated with the Rehabilitation New Liquid. The clinical efficacy, traditional Chinese medicine (TCM) symptoms, and the efficiency of gastroscopy were compared between the two groups. Results: The total effective rate of clinical symptoms, TCM symptoms, and gastroscopy results between the observation group and the control group were significantly different (P < 0.05) and were statistically significant. Conclusion: The clinical effect of Yin-tonifying Ophiopogonis Decoction combined with the Rehabilitation New Liquid in the treatment of RE is significant.

<B>Objective:</B> To evaluate the effect of traditional Chinese medicine (TCM)-characteristic rehabilitation program on levels of pain and swelling after total knee arthroplasty. <B>Methods:</B> A total of 72 in-patients who recovered from total knee arthroplasty in our department were selected as the study subjects. They were divided into two groups according to a random number table: the control group (n = 36) with routine nursing and the treatment group (n = 36) with TCM rehabilitation nursing. The pain scores and joint swelling were statistically analyzed. <B>Results:</B> The pain scores in the treatment group were lower than those in the control group, and the difference was statistically significant (P < 0.05).<B>Conclusion:</B> The implementation of a TCM rehabilitation program for patients after total knee arthroplasty can effectively and quickly reduce the pain and swelling of the affected limb, promote the local blood circulation, and promote the quality of life of patients after rehabilitation.

Objective: The aim of this study was to compare the functional and aesthetic outcomes of endonasal septoplasty (ES) and extracorporeal septal reconstruction (ESR) in anterocaudal septal deviation. Methods: In this study, patients (n = 46) who underwent nasal septoplasty surgery due to anterocaudal septal deviation during February 2015 to August 2017 were analyzed; 23 patients underwent ES and the others (n = 23) underwent ESR. The decision of whether to use the ES or ESR was randomized by random number table method. Nasal obstruction symptoms evaluation (NOSE) scores, total nasal resistance (TNR), aesthetic visual analog scale (VAS), nasal anatomical angles, and incidence of complications were used to assess the patients in 2 groups. Results: The NOSE scores, TNR, aesthetic VAS, tip deviation angle (TDA), nasolabial angle (NLA), nasofrontal angle (NFA) in the ESR group were significantly improved from preoperative to postoperative 1 year, whereas in the ES group, except aesthetic VAS, NLA, NFA, all other postoperative outcomes were improved from preoperative values. The objective and subjective postoperative results of ESR group were better than the ES group except TDA. The incidence of complications was not significantly different between the 2 groups. Conclusion: Our study have compared the nasal functional and aesthetic outcomes of 2 septoplasty techniques in a randomized controlled trial. The ESR technique is more effective than ES technique in correcting functional and aesthetic disorders caused by anterocaudal septal deviation.

Objective To determine the effectiveness of a cognitive behaviour language therapy (CBLT) programme to reduce speech anxiety among stuttering school adolescents. Methods This was a group randomized clinical trial that enrolled stuttering school adolescents who had severe speech anxiety. The participants were randomized to either the treatment group or the control group. The Speech Anxiety Thoughts Inventory (SATI) score was recorded before and after a 12-week CBLT programme was delivered in 24 group sessions to the treatment group. The control group did not receive any therapy. Results A total of 92 stuttering school adolescents who met the inclusion criteria were randomized to the treatment group ( n = 46; 22 males, 24 females; mean ± SD age, 16.36 ± 2.20 years) or the control group ( n = 46; 28 males, 18 females; mean ± SD age, 15.45 ± 2.10 years). Results showed that the CBLT intervention significantly reduced speech anxiety among stuttering school adolescents compared with the control group (post-test SATI assessment, mean ± SD 26.52 ± 1.67 versus 89.92 ± 3.17, respectively). Conclusion These findings suggest that speech educators and therapists in educational institutions and hospitals should follow the principles of CBLT when treating speech anxiety.
