Background: Indonesia is a developing country with a high maternal mortality rate (MMR) and perinatal mortality, the third highest in ASEAN and the second highest in the South East Asian Nation Regional Organization. In pregnancy hypertension has been proven that oxidants, especially if increased fat peroxide will damage endothelial cells called endothelial dysfunction. Vasospasm that occurs also induces platelet integration and endothelial damage which adds to the contribution in maintaining platelet dysfunction and triggering the use of platelets. Thrombocytopenia is the most important sign of the severity of preeclampsia.Objective: to determine the comparison of platelet levels in patients with preeclampsia with eclampsia.Methode: This study was observational cross sectional method. The sample in this study was a total sampling, using purposive sampling of patients with preeclampsia and eclampsia at the Wiyung Sejahtera Hospital in January-December 2019. The instrument used was using secondary data of medical records of patients.Results: Patients with preeclampsia have a minimum platelet level of 301,000/mm3, a maximum of 415,000/mm3, and an average of 351,733.33/mm3 with a standard deviation of 33,552.66/mm3. Patients with eclampsia have a minimum platelet level of 122,000/mm3, a maximum of 281,000/mm3, and an average of 209,200/mm3 with a standard deviation of 42,465.45/mm3. There are significant differences in platelet levels of patients with preeclampsia with eclampsia(p <0.05)..Conclusion: there is a significant difference between the platelet levels of patients with preeclampsia and eclampsia.

Background: The criteria set of assessing hospital quality in Vietnam in 2016 was revied from the criteria set in 2013 by the Ministry of Health in order to help hospitals to self-assess towards improvinge quality of hospitals in the international integration context. The study aimed to assess the quality of public hospitals in Can Tho City according to the revised criteria set of the Ministry of Health in 2016 and compare the quality among three hospital ranks (including grade I, grade II, and grade III) via to 5 groups of quality criteria. Methods: A cross-sectional study, using secondary data analysis was applied to assess the service quality of 7 general public hospitals in Can Tho City. Results: The average total score of 7 hospitals is 245 and the average for the criteria of 7 hospitals is 2.99, which is just satisfactory. In the criterion of quality, criterion D and E had the lowest scores compared to the other three groups. There was no statistically significant difference (p = 0.076) among the mean scores for the three hospital categories. Conclusion: The quality of public hospitals in Can Tho city in 2016 only reached moderately good level (2.99). Interventions should be developed to improve the quality of hospitals, with particular emphasis on improving the quality of criteria groups D and E. Aim: Atherosclerosis involves vascular endothelial damage and lipid metabolism disorder, which is closely related to the occurrence and development of diabetic kidney disease (DKD). However, studies on non-high albuminuria DKD (NHADKD) with an albumin to creatinine ratio (ACR) <30 mg/g are rare. This study is to investigate the relationship between atherogenic factors and the occurrence of NHADKD. Methods: Serum lipid indicators, lipoprotein-associated phospholipase A2 (Lip-PLA2) and homocysteine levels were measured in 1116 subjects to analyze their relationship with NHADKD. Results: Among all subjects, Lip-PLA2 had the closest but relatively weak correlation with ACR ( r = 0.297, p < 0.001) and only homocysteine was moderately correlated with eGFR ( r = −0.465, p < 0.001). However, in patients with NHADKD, these atherosclerotic factors were weakly correlated or uncorrelated with eGFR (max. | r| = 0.247). Stratified risk analysis showed that when ACR was <10 mg/g, homocysteine [OR = 6.97(4.07–11.95)], total cholesterol (total-Chol) [OR = 6.04(3.03–12.04)], and high-density lipoprotein cholesterol (HDL-Chol) [OR = 5.09(2.99–8.64)] were risk factors for NHADKD. There was no significant difference of OR between these three factors ( Z = 0.430–1.044, all p > 0.05). When ACR was ⩾10mg/g, homocysteine [OR = 17.26(9.67–30.82)] and total-Chol [OR = 5.63(2.95–10.76)] were risk factors for NHADKD, and ORhomocysteine was significantly higher than ORtotal-Chol ( Z = 3.023, p < 0.05). Conclusions: The occurrence of NHADKD may be related to the levels of homocysteine, total-Chol, HDL-Chol, and Lip-PLA2 in blood. Among them, homocysteine may be most closely related to NHADKD.

Abstract Background: Formerly, preeclampsia was only diagnosed if high blood pressure and proteinuria were present in a gravid woman at or past 20 gestation weeks. Although it is possible to have preeclampsia yet never have proteinuria, the literature on how common and factors associated with non-proteinuric preeclampsia as well as whether the frequency of end-organ dysfunction among women with non-proteinuric differs from those with proteinuric pre-eclampsia is scant.Objectives: To determine the prevalence of, factors associated with non-proteinuric preeclampsia, and compare the frequency of end-organ dysfunction between women with non-proteinuric and proteinuric preeclampsia.Methods: Between November 2019 and May 2020, women with pre-eclampsia were consecutively enrolled in a cross-sectional study at Mbarara Regional Referral Hospital. We interviewed all pregnant women ≥20 gestation weeks presenting with hypertension ≥140/90mmHg and recorded their socio-demographic, medical, and obstetric characteristics. We excluded women with chronic hypertension and pregnancy hypertension. We measured bedside proteinuria on clean-catch urine, complete blood count, and renal function and liver enzyme tests. Pre-eclampsia was defined as hypertension plus any feature of severity including <100,000 platelets/ul, creatinine >1.1g/dl, and liver transaminases ≥ twice upper normal limit with or without proteinuria. We computed the proportion of total participants with non-proteinuric pre-eclampsia at ≤+2 urine dipstick cut-off, determined the factors associated with non-proteinuric preeclampsia using logistic regression; and used Chi-square test to compare the frequency of end-organ dysfunction between non-proteinuric preeclampsia and proteinuric groups.Results: We enrolled 134 of the screened 136 participants. Their mean age was 26.9 (SD±7.1) years and 51.5% were primigravid. The prevalence of non-proteinuric preeclampsia was 24.6% (95% CI: 17.9-32.7). Primigravidity (aOR 2.70 95%CI:1.09-6.72, p=0.032) was the factor independently associated with non-proteinuric pre-eclampsia. There was a non-significant difference in the frequency of end-organ dysfunction in women with non-proteinuric and proteinuric pre-eclampsia, p=0.309.Conclusion: Non-proteinuric preeclampsia is common, especially among primigravidae. End-organ dysfunction occurs similarly in women with non-proteinuric and proteinuric preeclampsia. Obstetrics care providers should emphasize laboratory testing, beyond proteinuria, among all women with hypertensive disorders of pregnancy to optimally diagnose and manage non-proteinuric preeclampsia.

Introduction: A periodical database is important including for skin cancer. Periodical registration is needed to acknowledge changes in pattern and frequencies of skin lesion. Objective: The purpose of this study was to describe the pattern and the frequency of skin lesion in RSUD Kariadi.Method: A cross-sectional study was conducted through analysis of the medical records of patients diagnosed skin lesion in the pathology labolatory of RSUD Kariadi between 2008 and 2009. The variables were secondary data including age, gender, specimen area, dan histopathology diagnosis. Data was choosen by consecutive sampling from 381 medical records of skin tissues examined at laboratorium of pathology anatomy of Dr. Kariadi general hospital during 2008-2009.Result: 381 cases were recorded comprising of 246 (65%) neoplastic and 135 (35%) non neoplastic lesion. 120 patients presented with skin cancer, and 126 with benign skin lesion. Most malignancy was observed among female patients (62.5%) on age catagory of 15-39 (65%). The most common lesion was basal cell carcinoma (48.3%) followed by squamous cell carcinoma (33.3%), malignant melanoma (10%), skin appendix carcinoma (2.5%), other malignancies (4.9%).Conclusion: the most common malignancies in Dr. Kariadi general hospital before 2008 was similar to data from 13 laboratory of pathology anatomy in Indonesia, which is squamous cell carcinoma.

Nevus pigmentosus (NP) merupakan lesi melanositik jinak yang paling umum, puncaknya pada usia 25 sampai 26 tahun. Faktor yang memengaruhinya di antaranya penuaan, pubertas, kehamilan, penggunaan kortikosteroid sistemik, faktor genetik, lingkungan, usia, dan jenis kelamin. Tujuan penelitian ini mengetahui karakteristik pasien NP berdasar atas gambaran histopatologi di Rumah Sakit Al-Islam Bandung. Penelitian ini menggunakan metode deskriptif cross-sectional menggunakan metode pengambilan sampel berupa total sampling. Data yang digunakan berupa data sekunder dari rekam medis periode 2012−2017 dan didapatkan data berjumlah 48 rekam medis. Pengolahan data dilakukan menggunakan program Microsoft Exel tahun 2011. Hasil penelitian menunjukkan frekuensi tertinggi NP terdapat pada usia 25−45 tahun sebanyak 23 kasus (48%), NP lebih sering terjadi pada perempuan dibanding dengan laki-laki, nevus intradermal dengan jumlah 38 kasus (79%), dan regio kepala dengan frekuensi 39 kasus (81%). Perkembangan NP pada usia dewasa dapat disebabkan oleh beberapa kemungkinan di antaranya paparan sinar matahari, sering melakukan aktivitas di luar lingkungan, dan kurang penggunaan sunblock. Efek paparan sinar matahari secara langsung dapat menyebabkan proses melanogensis melalui aktivasi tirosinase akibat teraktivasinya protein kinase C. Simpulan penelitian ini menunjukkan frekuensi tertinggi NP terdapat pada usia 25−45 tahun dengan perbandingan perempuan lebih banyak dibanding laki-laki, serta gambaran histopatologi yang terbanyak adalah nevus intradermal yang berlokasi di regio kepala. THE CHARACTERISTIC OF NEVUS PIGMENTOSUS BASED ON HISTOPATOLOGICAL FEATURES IN AL-ISLAM HOSPITAL BANDUNGNevus pigmentosus (NP) is the most common benign melanocytic lesion and peak at 25 to 26 years of age. The factors that influence NP is included aging, puberty, pregnancy, the used of systemic corticosteroid, genetic factors, environment, age, and gender. The purpose of this study was to describe the characteristics of NP patients based on histopathological features at Al-Islam Hospital Bandung. This study used a cross-sectional descriptive method using a total sampling method to collect the samples. The data used in this study is a secondary data from medical records 2012−2017 and obtained 48 medical records. Data processed by using the Microsoft Excel program 2011. The results showed that the highest frequency of NP occurred at the age of 25−45 years as many as 23 cases (48%), NP is more common in women rather than men, nevus  intradermal with 38 cases (79%), and the head region with a frequency of 39 cases (81%). The progression of NP in adult can be caused by several possibilities including sun exposure, frequent activities outside the environment and lack use of sunblock. The effects of direct sunlight exposure can cause the melanogenesis process through activation of tyrosinase due to activation of protein kinase C. The conclusions in this study showed that the highest frequency of NP is found at the age of 25−45 years old with a ratio of women is more than men, and the highest number of the histopathological features is intradermal nevus located in the head region.

Dengue hemorraghic fever (DHF) is an infectious disease caused by the dengue virus and It is still be a major health problem in Southeast Asia, especially in Indonesia. Kabupaten Jember is still categorized as a dengue endemic area with its disease development expanding from year to year. This study aims to determine the correlation of platelet count and hematocrit levels to the severity of DHF patients at RSD dr. Soebandi Jember, as well as knowing whether the platelet count and hematocrit levels can be used as a predictive value for the severity of DHF patients at RSD dr. Soebandi Jember. This study used an observational analytic study with a cross-sectional study design. The data of this study used secondary data from the medical records of DHF patients at RSD dr. Soebandi Jember. The results of this study with a total of 68 samples showed that there was a significant negative correlation between the platelet count to the severity of DHF patients (p = 0,000, r = -0,487). The hematocrit levels showed an insignificant and negative correlation to the severity of DHF patients (p = 0,658, r = -0,055). The platelet count can be used as a predictive value for the severity of DHF patients. Keywords: DHF, platelet count, hematocrit levels, severity of DHF patients

Introduction: CAD occurs indirectly, usually a person will experience a process of narrowing of the coronary arteries in quite a long period of time. Primary survey results that researchers have done at Siti Rahmah Padang Hospital, recorded from 2017-2018 the number of CAD events as many as 115 cases. Aims : To obtain information about an overview of risk factors for the occurrence of coronary artery disease in the heart policlinic of Siti Rahmah Hospital Padang in 2017-2018. Method: This research covers the field of internal medicine and cardiology, this study was conducted at the Islamic hospital Siti Rahmah Padang in May to November 2019. This research used a descriptive method with a cross sectional approach using secondary data from medical records. The affordable population of this study is CAD patients who seek treatment at Siti Rahmah Hospital in Padang in 2017-2018 with 51 samples using Total Sampling technique. Univariate analysis is presented in the form of a frequency distribution table. Results: Based on the results of the study the most age was at the age of 50-59 years, 21 people (41.2%), the most sex was female, 26 people (51%), hypertension, 27 people (52,9%), experienced DM 26 people (51%) were obese, 16 people (31,4%) and hyperlipidemia, 13 people (25,5%). Conclusion : Most age 50-59 years, most sexes of women, most have hypertension, less than half are obese, most have DM and less than half have hyperlipidemia.

Abstract Background: Khat chewing is a routine practice in Yemen. Associations between Khat chewing and various adverse oral-dental health conditions have been recorded, however, adverse skin health evidence is still lacking. Objective: To investigate the difference between oral and skin symptoms and signs while chewing Khat and after quitting chewing among Yemeni khat abusers. Method: A descriptive cross-sectional study was conducted on a sample of 300 Yemeni Khat ex-chewer participants in china. Khat chewing status data, a range of oral and skin symptoms and signs, and other pertinent variables were collected using self-administered questionnaires by WeChat. The mean differences between oral and skin symptoms and signs while chewing Khat and after quitting chewing were investigated using the Paired Samples t-test. Result: Of the 300 Yemeni who participated in the study, 255(85%) were men and 45(15%) were women. The oral symptoms and signs mean while chewing was 0.26, 0.022 (standard deviation [SD], whereas that after quitting chewing Khat was 0.10 (mean), 0.269 (standard deviation [SD], t (12.00), P=0.00. The skin symptoms and signs mean while chewing was 0.13, 0.28 (standard deviation [SD], whereas that after quitting chewing Khat was M=0.06, 0.21(standard deviation [SD], t (9.407), P=0.00. Conclusions: There is a statistically significant difference between the mean oral and skin symptoms and signs while chewing and after quitting, whereas long-term Khat chewing negatively affects oral and skin health.

OBJECTIVE: Assess the differentiated aspiration process of palivizumab, from planning to administration, and from the patient's profile to obtained savings as a result of the optimization of doses.METHODS: Cross-sectional study carried out by means of secondary data obtained from the medical records of 858 children. Variables of the study were as follows: information on the profile of cared children, amount of acquired and administered doses, vaccination procedure, and optimized doses. Data were extracted from care management reports.RESULTS: The innovative technique and the vaccination planning allowed for 98 children to be favored by leftovers of the medication, implying savings of R$ 1.144.347,52 in 2012 and 2013.CONCLUSION: The employment of the differentiated aspiration process favored the availability of the medication to 54 children other than those previously programmed, and the application of 78 leftovers. The establishment of a previous management planning process of programmed contents to be administered to children, in compliance with their profile, especially their body weight, favored the inclusion of other admitted newborns. The systematization process of the team work toward the administration of palivizumab implied the reduction of public expenditures.

Objetivo: analisar o perfil de usuários e os motivos da consulta de enfermagem em estomaterapia. Método: trata-se de um estudo quantitativo, descritivo, exploratório e transversal. Coletaram-se os dados em prontuários de 252 usuários nos quais foram consultados em 1116 consultas de enfermagem. Analisaram-se os dados pelo programa SPSS versão 20, empregando-se a estatística descritiva. Resultados: revela-se, quanto ao sexo, que não houve diferença significativa; quanto à faixa etária, 53,6% dos participantes tinham 65 anos ou mais de idade; a causa do estoma foi a doença neoplásica maligna em 65,5% da amostra; o motivo da consulta, em 56%, foi a troca de bolsa de ostomia e 50,4% dos pacientes realizaram somente uma consulta no período analisado. Conclusão: mostrou-se, que o enfermeiro estomaterapeuta precisa refletir no planejamento e nas estratégias de cuidados prestados, contemplando a educação do autocuidado do usuário e educação permanente da equipe, enfatizando a sensibilidade do acolhimento, visto que a maioria dos pacientes tem neoplasia maligna, situação de grande impacto na vida do usuário e de sua família. Results: it is revealed, as for sex, that there was no significant difference; as for the age group, 53.6% of the participants were 65 years old or older; the cause of the stoma was malignant neoplastic disease in 65.5% of the sample; the reason for the consultation, in 56%, was to change the ostomy bag and 50.4% of the patients had only one consultation during the analyzed period. Conclusion: it was shown that the stoma nurse needs to reflect on the planning and care strategies provided, contemplating the education of the user's self-care and permanent education of the team, emphasizing the sensitivity of the welcoming, since the majority of patients have malignancy, situation of great impact on the life of the user and their family. Resultados: se revela, en cuanto al sexo, que no hubo diferencia significativa; en cuanto al grupo de edad, el 53,6% de los participantes tenían 65 años o más; la causa del estoma fue la enfermedad neoplásica maligna en el 65.5% de la muestra; el motivo de la consulta, en el 56%, fue cambiar la bolsa de ostomía y el 50,4% de los pacientes tuvieron una sola consulta durante el período analizado. Conclusión: se demostró que el enfermero de estomas necesita reflexionar sobre la planificación y las estrategias de atención provistas, contemplando la educación del autocuidado del usuario y la educación permanente del equipo, enfatizando la sensibilidad de la recepción, ya que la mayoría de los pacientes tienen neoplasia maligna, situación de gran impacto en la vida del usuario y su familia.
