Objectives: The objective of my study was to determine the frequency of familial short height in children aged 3 to 14 years with short height. Study Design: Descriptive study. Setting: Department of Pediatric Endocrinology at The Children Hospital & Institute of Child health Multan (CH& ICH). Period: 1st November 2018 to 31st October 2019. Material & Methods: Eighty four patients were enrolled after taking informed consent from parents/guardians. Heights of all patients, parental heights, mid parental heights were plotted on CDC growth charts. Patients having familial short stature were noted down. Results: Out of 84 patients, 59 (70.2%) were males and 25 (29.85%) were females. Age range was 3-14 years. Mean age of the population was 8.68 ± 3.42 years and mean height and weight were 114.29 ± 24.62cm and 28.95±10.01 kg, respectively. Mean height for mother and father of the patients were 154.39 ±4.56cm and 171.10 ± 3.52cm, respectively. Out of 84 children, 21 (25%) patients had FSS. In our study population FSS was more seen in children < 8 years of age, female gender and families with income of > 20K/months with p value of 0.879, 0.129 and 0.592 respectively. Conclusion: Majority of children presenting with short stature have FSS and CDG and do not have an endocrine disorder. In our study, frequency of familial short stature in children aged 3 to 14 years was 25%. The results should be validated in multicenter studies.

Objectives: LBW has long been labeled as one of the major risk factor for mortality as well as morbidity in neonates. South Asia is said to have highest number of LBWs which estimated to be 1 in 4 newborns that weigh < 2500 grams. This study was planned with an aim to note the burden, types of diseases and outcome in LBW newborns admitted at a tertiary care hospital. Study Design: Descriptive analytical study. Setting: Included all neonates admitted to NICU of Sheikh khalifa Bin Zaid Al Nahyan Teaching Hospital, Rawlakot. Period: 1st July 2018 to 31st December 2018. Material & Methods: The prevalence of LBW amongst all admissions was calculated along with demographic features of all LBW babies like disease, reasons for the admission, duration of hospital stay along with outcome was noted on a predesigned proforma. Results: Out of total of 1410 admission in NICU during the study period, 512 (36.3%) were noted to be LBW. Amongst LBW babies, mean weight was 1.91 kg while 269 (52.5%) were male and 243 (47.5%) female. There were 364 (67.6%) babies born at full term. There were 82 (16.0%) with birth weight of less than 1.5 kg, 166 (32.4%) between 1.5 to 2 kg while 264 (51.6%) were above 2 kg. Amongst all LBW babies, mortality was reported in 185 (36.1%) while 112 (60.5%) died on the 1st day of admission. Respiratory distress syndrome (31.4%), sepsis (20.3%) and neonatal jaundice 58 (11.3%) were the commonest diseases seen. Highest mortality (56.1%) was seen in babies who had birth weight below 1.5 kg (p value = 0.001). Conclusion: LBW is a major cause of hospitalization and mortality. RDS and sepsis were the most frequent diseases noted in LBW babies. Immediate care following birth is vital for babies already at risk of LBW.

This study was aimed to correlate the HRCT score and serum ferritin in diagnosed cases of COVID19 patients with pneumonia.During the pandemic of COVID-19,there was sudden surge in the numbers of HRCT chest scans and along with this amongst the various laboratory investigations serum ferritin was done to observe the role of cytokines which helps in disease progression.The main purpose of this study was to correlate the severity of disease on HRCT chest scan with serum ferritin levels. Aim:To correlate the HRCT score and serum ferritin in diagnosed cases of COVID19 patients with pneumonia. Materials and methods: We included 59 patients (38 men, 21 women age range 18-78years) with documented COVID19 were reviewed. All patients underwent RTPCR tests and had a noncontrast HRCT scan done at presentation. Estimation of serum ferritin was analyzed by using chemiluminescence method.The subjects were divided into three groups:mild,moderate and severe on the basis of HRCT score. Each group's HRCT score correlated with serum ferritin. The details were recorded on a prestructured proforma. The Pearson correlation coefficient test was used for correlations, and p value less than 0.05 was defined as statistically significant.The data was analyzed using Statistical Package for the Social Sciences (SPSS). Results: Out of 59 patients,21 patients were included in the mild group,31 were included in the moderate group and 7 were included in the severe group.CT severity score was found to be positively correlated with ferritin levels (p < 0.001). Strong positive correlations were found between CT scores and serum ferritin in mild (r = 0.84),Moderate (r = 0.92) and severe group (r = 0.082)

Background: Accurate anthropometric measurements and critical analysis of growth data allow the clinician to promptly recognize children with short stature. The aim of this study was to determine the frequency of etiological factors causing short stature among children referred to the pediatric endocrinology clinic of Assiut University Children’s Hospital, the main tertiary care center in Upper Egypt. Methods: We conducted this descriptive observational study from May 2012 to December 2015, to analyze 637 children (boys 354, girls 283) with short stature. Evaluation included: detailed medical history, physical examination, laboratory tests, bone age and chromosomal analysis. Results: Endocrinological causes accounted for 26% of short stature [of them, 11.8% had growth hormone deficiency (GHD)], 63.6% had normal variants of growth [of them, 42% had familial short stature (FSS), 15.8% had constitutional growth delay (CGD) and 5.5% a combination of both]. Interestingly, celiac disease (CD) constituted 6.6% of children with short stature in our cohort. Conclusions: Although potentially treatable causes such as GHD, hypothyroidism and CD accounted for a considerable percentage of short stature in our study, the majority of short stature in children had normal variations of growth. Growth hormone treatment in children, however, should be promptly initiated with specific clinical indications. CD is a not uncommon cause of short stature.

Objective: To determine the demographic details (gender, age, marital status,level of education, and occupation) of suicide attempters attending the tertiary care hospitalservices at Hyderabad, Pakistan. Study Design: Cross sectional study. Setting: Liaquat universityhospital Hyderabad and Sir Cowasjee Jahangir Institute of psychiatry (CIJP) Hyderabad.Period: Twelve months from the 1st January 2011 to 31st December 2011. Methodology:The aim of the study was. Patients with a history of attempted suicide. Two hundred (200)consecutive cases were recruited and selected according to the inclusion criteria. An informedconsent was obtained from patients for being included in the study. A specially designed semistructuredProforma was used to record demographic details of the patients. Data were enteredinto and analyzed through statistical software SPSS version 20. Results: Total 200 hundredcases were included in the study. Males 78.5 % (N=157) outnumbered female patients 21.5% (N= 43). The age range of study subjects was between 15 -59 years. Mean age of the totalcases was 22.6 years. Majority of the patients presenting with suicide were single comprising61 % (N= 122) of study cases with a P value < 0.001. Total cases of married subjects were 27.5% (N= 55) with a P value < 0.001. While 5.5% subjects were separated and 6 % were divorced.Unskilled labors made the largest group comprising of 24.5 % (N=49) of all the cases. Otheroccupations included farmers (15.5 %), skilled labor (15%), jobless (11.5%), house wives (8.5%), students (7 %), and shopkeepers (6%). Out of the total 200 patients with attempted suicide32% were illiterate, 33.5 % were educated up to primary level, 19.5% were matriculate, 09 %were intermediate passed, and only 6 % were graduate. (Table-II). Conclusion: In Pakistan stillSuicide attacks are reported. Our results suggest that suicidal thoughts can be entirely predictby common reported in male and younger age due to very sensitive to family issues, lowereducation level and major depression due to unemployment or low level earning of unskilledlabor occupation in the Pakistan population.

Objective: To assessment of retinopathy in type-II diabetics along with microalbuminuria at tertiary care hospital. Material and methods: This study was done at Department of Medicine, D.G Khan Hospital, D.G Khan from April 2020 to October 2020 over the period 6 months. Total 295 patients of type-II diabetes mellitus along with microalbuminuria having age between 45-80 years either male or female were selected. Diabetic retinopathy was evaluated in selected patients. Results: Total 295 patients were selected according to inclusion criteria. The mean age of patients was 58.13 ± 9.412 years. Out of 295 patients, retinopathy was noted in 67 (23%) patients. Out of 147 (49.83%) male patients, retinopathy was noted in 39 (26.5%) patients. Out of 148 (50.17%) female patients, retinopathy was noted in 28 (18.9%) patients. Statistically insignificant association of retinopathy with gender was noted with p value 0.119. Conclusion: Findings of this study showed a higher percentage of retinopathy in patients of type-II diabeties along with microalbuminuria. Most of the victims were males as compared to females and no association of development of retinopathy with gender was detected. Higher number of patients belonged to 4th and 5th decade of life. Background: Epilepsy is fairly a frequent occurrence in the elderly. It is commonly diagnosed after the episode of two or more unprovoked seizures. Unprovoked seizures in elderly are recurrent rather than younger individuals. This study was designed to estimate the concrete burden of frequent causes of epilepsy. Methods: A descriptive cross-sectional study with a total of 153 patients diagnosed case of epilepsy were included in this study at Jinnah Medical College Hospital from February 2018-August 2018. Mean was calculated for age, duration of disease of the patients. Causes of epilepsy, gender, and education was calculated and presented as percentages. Electrolyte readings were taken i.e., Sodium, Calcium and Magnesium levels and imaging was planned to rule out stroke, primary neurodegenerative disorders and tumors. Post stratification Chi square test was applied and p-value less than or equal to 0.05 was considered significant. Results: The mean age of the patients was 63.91±5.68 years and mean duration of the disease was 4.61± 1.07 months. The common causes of epilepsy were found to be cerebrovascular disease 56.9%, cryptogenic 54.2%, neurodegenerative disorder 20.3%, traumatic head injury 11.8%, metabolic abnormalities or electrolyte disturbances 10.5% and brain tumor 7.8%. Conclusion: Elderly patients with first seizure should present to a facility designed in a way that neurologist, cardiologist, rehabilitation and geriatrics work together to identify and treat the condition in a better way. BACKGROUND Seroprevalence of IgG antibodies against SARS-CoV-2 is an important tool to estimate the true extent of infection in a population. However, seroprevalence studies have been scarce in South East Asia including India, which, as of now, carries the third largest burden of confirmed cases in the world. The present study aimed to estimate the seroprevalence of anti-SARS-CoV-2 IgG antibody among hospitalized patients at one of the largest government hospital in India OBJECTIVE The primary objective of this study is to estimate the seroprevalence of SARS-CoV-2 antibody among patients admitted to the Medicine ward and ICU METHODS This cross-sectional study, conducted at a tertiary care hospital in North India, recruited consecutive patients who were negative for SARS-CoV-2 by RT-PCR or CB-NAAT. Anti-SARS-CoV-2 IgG antibody levels targeting recombinant spike receptor-binding domain (RBD) protein of SARS CoV-2 were estimated in serum sample by the ELISA method RESULTS A total of 212 hospitalized patients were recruited in the study with mean age (±SD) of 41.2 (±15.4) years and 55% male population. Positive serology against SARS CoV-2 was detected in 19.8%patients(95% CI 14.7-25.8). Residency in Delhi conferred a higher frequency of seropositivity 26.5% (95% CI 19.3-34.7) as compared to that of other states 8% (95% CI 3.0-16.4) with p-value 0.001. No particular age groups or socio-economic strata showed a higher proportion of seropositivity CONCLUSIONS Around, one-fifth of hospitalized patients, who were not diagnosed with COVID-19 before, demonstrated seropositivity against SARS-CoV-2. While there was no significant difference in the different age groups and socio-economic classes; residence in Delhi was associated with increased risk (relative risk of 3.62, 95% CI 1.59-8.21)

Background: Post-operative outcomes of a major abdominal surgery depend on careful & effective post-operative management. But it is a critical job especially in children. Obtaining adequate analgesia after major surgery is a problematic issue and postoperative pain still imposes a major burden of suffering in surgical patients.Objectives: The principle objectives of the study is to evaluate the effects of intravenous lidocaine infusion in pain management of pediatric population undergone in major abdominal surgery; to reduce post-operative morbidity & enhance better surgical outcome in children.Methodology: This is a randomized control trial carried out from January 2015-June2015,in a tertiary care hospital among 60 cases of 4 to 14 years children with major abdominal surgery without having any pulmonary, cardiac, hepatic or renal insufficiency. Grouping of patients that is lidocaine infusion group (Group A) and control group (Group B) was made among admitted cases for elective abdominal surgery by simple random technique by means of lottery. For assessment of postoperative pain FLACC Scale was used in both groups. Clinical examination findings & specifically designed data collection sheet with a set questionnaire were used as research instruments. Formulated data was analyzed by SPSS version 17, taking p value <0.05 as significant.Results: It is noted that, after 24 hours of operation most of the patients 56.7% of group A had mild pain whereas 90% patients of group B had moderate pain (p<0.001)& during that time there was no patient with severe pain in group A whereas in group B 10% patients were with severe pain. At 48 hours, pain was absent in 13.3% children of group A and 6.7% in group B. In group A most of the children 76.7%had mild pain compared to moderate pain 18 (60%) in group B children at that hours (P<0.001). Again, regarding required amount of analgesics, patients received I/V lidocaine required less amount of analgesics than its counterpart. In present study, complications was noted only 3.3% patien in group A, where as in the opposite group it was found in 23.3% & p was <0.05. In group A, in 50% patients post operative bowel sound was returned within 72 hours, compared to 73.3% patients in group B. The p value was 0.001. About post-operative hospital stay, 83.3% children of the group A were released from hospital after 5th P.O.D whereas, in group B, only 50% children were released after 7th P.O.D of operation. The P value was 0.03 that is also significant.Conclusion: Intravenous lidocaine could improve immediate and late post-operative pain with early recovery after major abdominal surgery in children & it can contribute to rapid postoperative rehabilitation programs.J Shaheed Suhrawardy Med Coll, June 2018, Vol.10(1); 23-27

Background: Adequate neonatal transport is a key component of care of the sick newborns who require referral to tertiary care center. Poor transportation is one of the iatrogenic factors associated with greater neonatal mortality. Neonatal transport is the greatest challenge faced today in our country. The purpose of this study was to find out characteristics of transport of referred neonates and to idention the factors that contribute to mortality.Methodology: This cross sectional study was conducted in Dhaka Shishu (Children) Hospital from June 2013 to November 2013. Both term and preterm neonates who were referred within first seven days of life were included and those with gross congenital abnormalities and left against medical advice were excluded from the study. After enrollment, data were collected using a structured questionnaire including birth details, interventions before transportation, reasons for referral, and details of transportation. Outcome & duration of hospital stay were also recorded. Neonates who were expired considered as group I and who were survived considered as group II. The study variables were analyzed for their association with immediate outcome by applying chi square test and t test. P value <0.05 was considered significant.Results: This study found that out of 332 neonates 181 were expired with 54.5% mortality rate. One eighty one neonates who were expired, considered as group I and one fifty one neonate were survived, considered as group II. The mortality was significantly high in male neonates [RR 0.80 (0.66-0.97)] and neonates those delivered at home [RR 1.34(1.10-1.64)] (p<0.05). Perinatal asphyxia, pre-term low birth weight, neonatal sepsis were the main causes of referral. It was found that transportation without any referral note [RR 1.40 (1.14- 1.71)], no advice regarding maintenance of airway[RR 1.50(1.17- 1.92)]and keeping warm [RR 1.51(1.17-1.950], resuscitation on admission [RR 1.63(1.23-2.17)] and transportation required > 3hours [RR 1.36(1.09-1.69)] were associated with significantly higher mortality among referred transported neonates(p<0.05).Conclusions: This study found that male neonates, home delivery, transportation without any referral note, no advice regarding maintenance of airway and keeping warm, resuscitation needed on admission and prolonged transportation time were significantly associated with mortality of referred transported neonates.Bangladesh J Child Health 2017; VOL 41 (3) :159-164

OBJECTIVES: To determine the frequency of complications following cataract surgery in diabetic patients admitted in the ophthalmology unit. METHODOLOGY: A prospective descriptive interventional case series study was conducted after approval of the ethical committee, from June 2017-June 2020 at the Ophthalmology department MTI-MMC. A total of 129 patients from either gender were enrolled in study. All the study patients went through detailed history and complete ocular examination. After necessary investigations, surgical procedure was carried out. Results were analyzed through the SPSS-24 version. RESULTS: Out of the total 129 eyes of the diabetic patients, fifty-nine (45.7%) were males and seventy (54.3%) were females with a ratio of 1:1.2. Uveitis leads the chart in complications found in twenty (15.50%) eyes while PODR being the least common found in only ten (7.75%) eyes. Worse visual acuity was observed in fourteen (10.85%) eyes. Striate keratopathy and posterior capsule opacification were found in sixteen (12.40%) and fifteen (11.62%) eyes respectively. Among the patients, 15.7% were having more than one complication during follow-up visits and eighty-eight (68.2%) eyes were found to have none complication. The age group 51-60 years observed frequent complications as compared to other groups. Similarly female gender (38.57%) has frequent complications as compared to males (2.7%). CONCLUSION: The study concludes Uveitis as the most common complication observed in 15.50% 0f the eyes while worse visual acuity (10.85%) and progression of diabetic retinopathy (7.75%) being the least common. Striate keratopathy was found in 12.40% while posterior capsule opacification in 11.62% of the eyes.
