Background Exclusive breastfeeding by healthy mothers to their healthy, term babies who underwent vaginal birth, should be readily accomplished. However, exclusive breastfeeding by Indonesian mothers has declined. Objective To assess the monthly success rate prevalences for exclusive breastfeeding for the first 6 months of life, as well as factors that affect exclusive breastfeeding. Methods A prospective cohort study was conducted in 243 healthy mothers with healthy term babies who underwent normal births at Cipto Mangunkusumo Hospital (CMH), Jakarta, Indonesia. Guided interviews were conducted monthly for six months. Bivariate and multivariate analyses were performed on the factors potentially affecting breastfeeding with equal subject numbers. Results Exclusive breastfeeding prevalences were 64.8% (first month), 53.7% (second month), 43% (third month), 30.7% (fourth month), 23.5% (fifth month), and 22.3% (sixth month). Multivariate analysis revealed that the mother’s confidence in breast milk production, as well as husband or family support, affected the success of exclusive breastfeeding for each month. Maternal not working/studying outside the home affected the success of exclusive breastfeeding in the third (RR 3.38; 95%CI 1.21 to 9.43) and fourth months (RR 6.56; 95%CI 1.39 to 30.99).Conclusion Exclusive breastfeeding prevalences in Cipto Mangunkusumo Hospital decrease in a monthly fashion up to the sixth month. Several factors affecte the success rate for each month in the six month period, including maternal confidence in breast milk production and family support.

The overall objective of this study was to determine factors influencing the knowledge and practice of Exclusive Breast Feeding1 (EBF) among lactating mothers with infants aged zero2 to six months at Ahero Sub District Hospital in Nyando District, Kenya. A cross- sectional design was conducted to 117 breastfeeding mothers at Ahero Sub-District Hospital at Maternal Child Health clinic; these were selected consecutively until the desired sample was attained. The independent variables were maternal education, marital status, both maternal and child age, maternal employment status, health factors (place of delivery and maternal chronic illness), and cultural beliefs while dependent variables included EBF practice and mothers’ knowledge of EBF3 (awareness of EBF and its benefits), pre lacteal feeds and exclusive breastfeeding consistency. Exclusiveness was measured in time initiated (30 minutes initiation after birth) Data were collected by trained enumerators who administered a structured questionnaire. Analysis was done using Statistical Package for Social Scientists (SPSS) by running frequencies and cross tabulation. Chi square test was used to check for strength of association between variables at 95% confidence level. The study found that there was low EBF practice (33%) in the area and this posed a great concern. It was established that marital status, employment, maternal education, place of delivery and both infant and mothers’ age were closely associated with knowledge and practice of EBF. Marital status and employment were strong predictors of both knowledge and practice which showed statistical significance of P= 0.02 and P=0.00, respectively. It was evident that family support is a key factor in the success of EBF with special focus on partner involvement. Actions to empower women and promotion of EBF campaign are recommended to achieve the fourth Millennium Development Goal (MDG) and probably reduce infant mortality in the area. Further studies are required to assess the impact of a strong focused educational program (focused on building family support) on improving EBF rates.

The success of exclusive breastfeeding is a high level of maternal education, a fairly high level of maternal knowledge, the existence of support and understanding of husbands to breastfeed exclusively, the role of potential groups to provide information to pregnant and breastfeeding mothers, the existence of counseling, supportive attitudes of officers and want to respond to every problem that is being faced. From the results of the study it is recommended to maintain and even increase and promote the success of exclusive breastfeeding. ASI is a fat emulsion in a solution of protein, lactose and organic salts that are secreted by the two glands of the mother's breast as the main and best food for infants because in the milk it contains immune, anti-infectious and nutritional nutrients needed to grow and develop and control excess weight gain. body or obesity. Exclusive breast milk is breast milk that is given to infants without additional food at least until the age of 4-6 months
