The importance of postgraduate training of doctors on pharmaceutical supply of the population

As the use of medicines in the provision of medical services is an important component, it is advisable to constantly update the knowledge of doctors and monitor regulatory and market changes in the field of pharmaceutical supply. The purpose of this work is to identify educational and practical issues in the pharmaceutical field, knowledge of which contributes to the proper management of the provision of drugs to the medical organizations, motivated choice, appointment, rational therapy of diseased – patients of the doctor. General information on providing relevant knowledge to physicians in the cycles of thematic improvement in health management and economics on a number of issues are presented, including: legal and marketing regulation of health care organizations and the population with medicines; state regulation of drug prices; determining the need for drugs and purchasing for budget funds; regulatory requirements for prescriptions; features of use, storage and accounting in inpatient and outpatient settings of narcotic, psychotropic drugs and precursors, etc. Given that the development of the pharmaceutical healthcare sector is influenced by the market mechanism, it is advisable to familiarize yourself with the marketing management of its subjects. It is shown that knowledge of the main issues of medical care of health care institutions and the population contributes to the quality of professional duties of organizers and specialists on the basis of legal regulation, rational use of drugs, compliance with existing restrictions and standards, and forms the principles of professional interaction and ethical communication between health professionals.

Due to the collapse of the Union, there was a reduction in funding for health care costs, as well as deterioration in the infrastructure and quality of medical services. The transitional economy in the Kyrgyz Republic has identified additional features in the health system. The main ones are the low level of funding, the presence of the shadow market of medical services, inefficient structure and the prevalence of high-cost hospital treatment. The market mechanism is developing, however, without state regulation. The Kyrgyz Republic continues to reform its health-care system. The task was to improve the methods of their financing. New mechanisms for financing medical services have been introduced. Despite the reduction in the number of hospitals, the number of patients treated in hospitals has increased.

In modern conditions, the most important tools of state regulation in carrying out evaluations of the effectiveness of the reforms carried out are forecasting and planning of long-term results taking into account the level of influence of socio-economic and other factors. This is due to the market economy’s inability to regulate itself, especially in crisis situations. Forecasting is closely linked to planning and is the necessary methodological and information basis for the development of plans and programs. Health system reform should be based on addressing systemic health challenges, taking into account socio–economic, demographic and technological trends, and assessment of the effectiveness of reform should capture changes in dynamics. Integrated program and targeted planning provides for the development of medical and social programs, taking into account certain priorities of public health, which makes it possible to concentrate resources on priority areas of health development on the basis of an integrated inter-agency approach to achieving the goals set. With the transition to a market economy, there are signs of indicative planning in health care as a way of consciously, purposefully managing health and economic processes, both in the public, municipal and private health sectors. Indicative planning has proved to be highly effective as a means of public regulation of a market economy. A characteristic feature of indicative planning is the rejection of directionality, command and distribution functions in favor of indicative. Targeted Health Development Programs should use as achievements indicators of resource provision and efficiency of its use, indicators of availability and quality of health care, its safety, timeliness and efficiency, continuity in its provision, satisfaction of consumers, quality of document circulation, etc., which may be influenced by the health management system.

The study allowed us to establish the development of scientific knowledge in public administration, which is to expand its subject-matter scope and methods of studying the processes occurring in the field of health care. In addition, it is proved that the proposals of scientists to expand the forms and content of mechanisms for implementing public administration and public policy in the field of health care are important in the development of methodology and organization of public administration science. The positions of domestic and foreign scientists on the peculiarities of public health management are analyzed. Thus, Bugaytsov S. G. studied the problems of state regulation of the cancer care system, Yarosh N. P. devoted research to regulating the development of social standards in the field of health, Ringach N. O. studied the health care system as a component of national security. Scientific researches of Zhilka K. I. related to the development of cooperation with international organizations in the field of public administration of children's health. Firsova OD improved the mechanisms of geoinformation support of the state health care management in Ukraine. Mokretsov SE studied the problems of public administration of reproductive health in Ukraine in a demographic crisis. Kovalenko T. Yu. developed mechanisms of public administration in the field of sanatorium and resort provision of children in Ukraine. Jafarova D. M. suggested ways to improve the management system of primary health care reform at the local level on the example of the city of Lviv. In general, the scientific achievements of domestic science, allows us to talk about the diversity of the health care system as an object of public administration. As a result, a map of the development of public administration science in the context of a separate object-subject field of the health care system has been formed.

Summary. The article explores some of the problems of state regulation in the field of health care through the accumulation and application of a wide range of components of the methodological basis of jurisprudence. The basis of this scientific research is a system of principles of scientific knowledge, methodological approaches and methods of scientific research, which in an organic combination are able to ensure the comprehensiveness, objectivity and effectiveness of such research. The article uses a system of principles of scientific knowledge (objectivity, systematicity, professionalism, comprehensiveness, responsibility, publicity, etc.), which determine the ideas and provisions that form the basis of any scientific research, including the study of state law regulation of public health. In addition to the principles of scientific knowledge, a functional scientific approach was used in the work, which allows to reveal the problems of state-legal regulation of health care as an important direction of the functional purpose of the state in the field of public health, to determine the priorities of priority and criteria of effectiveness of state-legal regulation health, methods of its application and generalization of the obtained results, which will allow to conceptualize the idea of the value and priority of the state the general regulation of health care in general and its manifestation in the field of public health in particular. As a cognitive problem state regulation in the health care system was investigated using a system of appropriate methods, including: philosophical and philosophical methods that provide the study of these problems through the plane of the general understanding of the universe and its exceptional value (condition) of life security and human health; general scientific methods, which focus on the objectively conditioned nature of state regulation in the field of public health as a prerequisite for the effective implementation by the state of a function of public health, which is scientific and requires study at the level of various social sciences; special scientific methods that provide research into public health regulation as a phenomenon of public law nature. Within the framework of the conducted scientific research the normative basis of human rights protection in the field of health care was singled out. It has been found out that human rights and citizens' rights in the field of health care in Ukraine are protected by a number of normative legal acts, ratified international instruments, in particular, such as: the Constitution of Ukraine, which recognizes a person, his life and health, honor and dignity, integrity and security of the highest social value, the International Covenant on Economic, Social and Cultural Rights, the Convention on the Rights of Persons with Disabilities, the Convention on the Rights of the Child, the Convention for the Protection of Human Rights and Fundamental Freedoms, European Social Charter. In the article proved that the health of the individual and the population as a whole are of the highest individual and social value, the most important factor of state formation and security of the state, is a basic component of human capital and competitiveness of the country and is closely integrated with the economy, science and defense. The issue of public health is becoming a top priority worldwide, because we are all aware that health is an invaluable asset to every person, an important and necessary condition for a fulfilling life. At the same time, we must not focus solely on the health of the individual, but take into account the health of the individual nation and of society as a whole. In the course of the scientific research the relationship between the current challenges in the field of human health and the need for the states to re-evaluate the policy changes in the direction of solving the problems of reforming the health care system as a value that is of strategic importance for the sustainable development of society and related to urgent tasks of development of the modern state and pledges of favorable social-moral atmosphere and trust to the state. It has been found out that in the process of state-legal regulation in the field of health care it is possible to distinguish the following components: the system of state authorities and local self-government involved in the process of state regulation of relations (institutional component of state-law regulation in the field of health care) and the system of normative legal acts, which constitutes the legal basis of the process of state-legal regulation in the field of health care (the normative component of state-legal regulation in the field of health care). It is proposed to highlight the priority points of state regulation in the field of health care, in particular, the state should clearly define the health of the population as a priority condition for ensuring national security; An important step of the state in the field of health care is to choose the optimal model of the health care system that can take into account and realize the interests of all subjects; Priority direction of the state legal regulation in the field of health care should be a systematic normative basis, which stipulates both the regulation of the existing legal framework in the field of health care and creation of preconditions for ensuring its harmonization with the European legislation; public health regulation should focus on the priority of preserving the health of the population as an important prerequisite for the development and stability of human capital, and therefore the appropriate level of competitiveness of the state; strategic orientation of the state legal regulation in the field of health care is ensuring the effective protection of the environment, which is manifested in the implementation of the state proper ecological function, the combination of effective state regulation of defining areas of environmental safety of the population with the activities of environmental organizations and structures capable of producing new technology. Legal support of human security in the field of health care includes the guarantee, protection and protection of rights and freedoms in the field of health care, which is the main function, as well as the goal and duty of the state. This paper describes certain aspects of the legal regulation of the implementation of the "human right to security in the health sector" and the problems of its enforcement. The research methodology is based on a system of methods of the philosophical, general scientific and special scientific level. The main goal of this scientific article is to define the concept‚principles‚ types and directions of implementation of the “human right to safety” in the concept of “the right to health protection”. The general principles of the implementation of the “human right to security in the health sector” are disclosed. It is emphasized that the legal mechanism for the implementation of the “human right to security in the healthcare sector” is the activity of legal entities, lawmaking and law enforcement agencies, and the existing legal norms governing their activities in the healthcare sector. The investigated human right to safety should be understood as a complex of rights related to the protection of the patient's legitimate interests in the healthcare sector from unlawful encroachments and threats.. The author's understanding of the definition of "patients' right to safety". It is argued that human security in the field of health care belongs to the basic needs of a person - the implementation of this need is determined by the level of development of a country, its economic and cultural components, the level and quality of life of a person living in this country, an effective health care system. It is concluded that the main goal of legal ensuring human security in the healthcare sector is to create the minimum necessary (safe) conditions for the implementation of these rights and obligations when receiving medical services.

В статье рассмотрены инструменты государственного регулирования сферы здравоохранения на уровне региона, а также направления его совершенствования, что является в настоящее время весьма актуальным. Авторы, раскрывая важное значение программирования и проектирования государственного регулирования системы здравоохранения, отметили отличия и преимущества проекта перед государственной программой, что объясняет активное использование проектного метода на всех уровнях управления. Среди направлений совершенствования инструментов государственного регулирования сферы здравоохранения в республике предложены такие направления, как активное использование инструментов государственно-частного партнерства, широкая информатизация и цифровизация системы здравоохранения и др. На основе анализа развития системы здравоохранения Республики Башкортостан, показателей обеспеченности населения муниципальных районов и городских округов врачами и средним медицинским персоналом сделаны выводы о дефиците медицинских кадров в регионе и разработаны соответствующие мероприятия. В статье говорится о ключевой проблеме в целом по России, а также в республике, - о неблагополучной демографической ситуации. Представлен анализ демографических показателей за последние годы и предложены эффективные инструменты решения данных проблем.По результатам проведенного исследования авторами определены наиболее эффективные и значимые на региональном уровне мероприятия, позволяющие достичь целей национального проекта «Здравоохранение» и государственной программы «Развитие здравоохранения Республики Башкортостан». The article discusses the instruments of state regulation of the healthcare sector at the regional level, as well as the directions of its improvement, which is currently very relevant.The authors, revealing the importance of programming and designing state regulation of the health care system, noted the differences and advantages of the project over the state program, which explains the active use of the project method at all levels of management.Among the directions for improving the instruments of state regulation of the healthcare sector in the republic, such directions as the active use of public-private partnership instruments, widespread informatization and digitalization of the healthcare system, etc. are proposed.Based on the analysis of the development of the health care system of the Republic of Bashkortostan, indicators of the provision of the population of municipal districts and urban districts with doctors and nurses, conclusions were drawn about the shortage of medical personnel in the region and appropriate measures were developed.The article speaks about the key problem in Russia as a whole, as well as in the republic - the unfavorable demographic situation. The analysis of demographic indicators in recent years is presented and effective tools for solving these problems are proposed.Based on the results of the study, the authors have identified the most effective and significant measures at the regional level, allowing to achieve the goals of the national project “Health” and the state program “Development of health care in the Republic of Bashkortostan.”

The aim: Theoretical substantiation and determination of the main characteristics of the interaction links of medical institutions within one hospital district in the conditions of aggravation of the epidemiological situation in Ukraine on the analysis basis of the legislative base and elaboration of literary sources. Materials and methods: In the work is used a range of methods: content analysis, bibliosemantic, systematic approach, analysis of products of activity. Conclusions: The authors propose a doctrinal definition of the term “hospital district”. The key problems of the domestic healthcare sector in the context of a pandemic have also been identified. The author points out that in order to successfully reform the health care system and the effective interaction of hospitals in one hospital district, it is necessary to pay attention to funding sources and proper legal regulation, as without the latter any initiatives will have no legal force and will be ignored. health care may be ineffective.

The main purpose of the paper is to analyze the process of the health care development as the system-creating factor of human capital. Methodology. This study combines economic and managerial analysis of healthcare and human capital. The background for this analysis is the data of the 2020 Human Capital Index in Ukraine and other developed and developing countries. The method of historical and logical analysis of the literature was used. The functional and structural analysis was used with purpose to research the main tools of human capital’s development. Methods of comparative and statistical analysis and their synthesis were used to study dynamics of human capital. Method of summarization was used to make conclusions and recommendations for improving human capital in developing countries. Works of scientists in the sphere of human capital were used as the informational basis for the conducted study. Findings. In the paper it was analyzed the Human Capital Index in Ukraine and its components from the period of 2010-2020. The problems of the development of Ukrainian healthcare sector as the system-creating element of human capital are viewed. Formation of a new model for organizing medical care based on the development of the four P’s model of medicine (predicting, prevention, personalization, participation) is considered as the main basis for developing effective healthcare sector in Ukraine. The authors of the article see the following key areas of necessary actions in Ukrainian healthcare sector: technological breakthrough, strengthening a healthy lifestyle, ensuring the availability of high-quality medical services, developing human resources, creating a system of effective drug provision, increasing the efficiency of financing. Practical implications. The results of this study form the methodological and practical basis for improvement of the state regulation system of Ukrainian healthcare as a system-creating element of human capital. The results of conducted research could be a framework for formation of effective healthcare system in Ukraine, ensuring its constant development. The main proposals could be used by the Ministry of Healthcare of Ukraine while working out the Concept of national healthcare system development, they should be taken into consideration by state and municipal Health Departments for implement them during the process of Ukrainian healthcare sphere’s reformation.

Artificial intelligence is a technology developed to make machines think like humans. Aristotle's historical artificial intelligence entered the health sector in the 1970s. The first application for artificial intelligence in the internist-1 field in health care, the Casnet expert system, and MYCIN. This technology was later introduced to many areas of health care. The main purpose of this application is for the benefit of doctors and patients. In general, applications, medical decisions, early diagnosis and treatment, drug development, and medical imaging issues deserve attention. Another important issue is the concern about the benefits of artificial intelligence, as well as the possibility of replacing the physicians in charge when medical decisions are left entirely to artificial intelligence. This study aims to provide readers with general information about the areas where artificial intelligence is used in health care. In this study, 14 applications were examined and outstanding results were observed. When doctors say that a comatose patient cannot wake up, artificial intelligence says that a comatose patient can wake up and the patient can wake up. Other advanced artificial intelligence has shown that it can detect cancer more accurately than 58 skin specialists. One study concluded that artificial intelligence with a predictive treatment approach lowered health care costs by 5 to 9% and could lead to an increase in life expectancy of 0.2 to 1.3 years. The purpose of the article is to study the features of regulatory and legal regulation of state personnel policy in the field of health care in Ukraine. The article determines that the formation of the legal framework for the effective functioning and development of state personnel policy of the health care system is a priority for improving the efficiency of the medical sector. The article analyzes scientific developments on improving the staffing of the health care system in Ukraine, in particular: articles of the Constitution of Ukraine, provisions of the Labor Code, articles of the Law of Ukraine "On Employment", articles of the Law of Ukraine "On Collective Bargaining", articles Law of Ukraine "On labor protection", provisions of the Law of Ukraine "On vacations", articles of the Law of Ukraine "On remuneration", provisions of the Law of Ukraine "Fundamentals of legislation of Ukraine on health care", provisions of the Law of Ukraine "On improving accessibility and quality of health care" in the countryside. " It is determined that the main source of labor law and state regulation of labor relations, regardless of the industry, is the Code of Labor Laws. The main elements (features) of the employment contract are studied, namely: individual-personal feature, organizational feature, material feature, regulatory feature, subject feature. The principles of state policy in the field of labor protection are determined. An analysis of the orders of the Ministry of Health for 2020-early 2021 on personnel policy in the field of health care, which confirmed the interest of the direct entity in the form of the Ministry of Health of Ukraine in the formation of regulatory framework for personnel policy of the health care system of Ukraine.
