This study aims to evaluate the Zoning System policy in the admission of new students listed in Permendikbud no. 17 of 2017, the purpose of the zoning system is to equal access to education in each district / city. however, in its implementation, it has not been able to eliminate the mindset of favorite and not favorite schools because the accreditation of several schools has not yet been evenly distributed, besides that, they still have to adapt to the implementation of this policy, one of which is at State Senior High School 2 Bandung which is the first cluster school before the zoning system for the admission of new students. This study uses a William N Dunn model of public policy evaluation with the CIPP approach (Context, Input, Process, Product). The research method uses descriptive research methods with a qualitative approach. Data collection techniques through interviews, observation and documentation study. The results showed that the application of the zoning system in the admission of New Students at State Senior High School 2 Bandung has been going very well but still needs new patterns in learning and the impact of implementing zoning system policies including improving the quality of learning, division of study groups, characteristics of students and the learning process at StateSenior High School 2 Bandung.

Subject Shortcomings in Brazilian infrastructure. Significance The quality of Brazil’s infrastructure is a key business complaint. Infrastructure is viewed as central to boosting the country’s long-term competitiveness, as well as a potential motor of recovery from the economic crisis. However, infrastructure investment remains low. This is due in part to the budgetary restrictions faced by the government, but also to the impact of corruption scandals on leading construction companies. Impacts Better roads, railways and ports will be central to boosting Brazil's exports, notably of primary commodities. Poor infrastructure will continue to affect both business and the daily life of ordinary citizens, particularly in remote regions. The execution of infrastructure investments could produce positive and long-lasting effects on the overall economy.

BackgroundThe impact of mental disorders has been assessed in relation to longevity and quality of life; however, mental disorders also have an impact on productive life-years (PLYs).AimsTo quantify the long-term costs of Australians aged 45–64 having lost PLYs because of mental disorders.MethodThe Survey of Disability, Ageing and Carers 2003, 2009 formed the base population of Health&WealthMOD2030 – a microsimulation model integrating output from the Static Incomes Model, the Australian Population and Policy Simulation Model, the Treasury and the Australian Burden of Disease Study.ResultsFor depression, individuals incurred a loss of AU$1062 million in income in 2015, projected to increase to AU$1539 million in 2030 (45% increase). The government is projected to incur costs comprising a 22% increase in social security payments and a 45% increase in lost taxes as a result of depression through its impact on PLYs.ConclusionsEffectiveness of mental health programmes should be judged not only in terms of healthcare use but also quality of life and economic well-being.Declaration of interestNone.

This paper discusses the importance of the zoning system in the acceptance of SMA / SMK New Students (PPDB) towards the equal distribution of the quality of Human Resources (HR) in East Java Province. The zoning system in accepting high school / vocational students will prevent the accumulation of certain quality human resources in the region. Zoning of high school / vocational student admissions will encourage equal distribution of quality human resources in an area, making it easier to meet the needs of the workforce to support local economic activities. The author uses a descriptive qualitative approach in analyzing the application of zoning systems in PPDB SMA / SMK in East Java. A qualitative case study strategy was used to explain the impact of the policy in reducing disparity in human resource quality between regions in East Java. In this case, the authors use the assumption that providing equal opportunities to obtain quality education in an area will guarantee the availability of quality human resources equally. This paper presents an interesting analysis, that the direct or indirect PPDB zoning system policies at the SMA / SMK level will encourage even distribution of quality human resources in East Java. First, the high school / vocational PPDB zoning policy avoids the accumulation of high-quality prospective students in only one particular school in a particular region, thus raising exclusivity in the field of education. Second, the implementation of the zoning system policy guarantees the availability of quality workforce equally, because students receive teaching by teachers and educational resources with no different quality. The absence of a gap in the quality of graduates will ultimately increase the even distribution of quality human resources in all regions of East Java.

AbstractThe impact of breast cancer-related lymphedema (BCRL) on long-term quality of life is unknown. The aim of this study was to investigate the impact of BCRL on health-related quality of life (HRQoL) up to 10 years after breast cancer treatment. This regional population-based study enrolled patients treated for breast cancer with axillary lymph node dissection between January 1st 2007 and December 31th 2017. Follow up and assessments of the included patients were conducted between January 2019 and May 2020. The study outcome was HRQoL, evaluated with the Lymphedema Functioning, Disability and Health Questionnaire, the Disabilities of the Arm, Shoulder and Hand Questionnaire and the Short Form (36) Health Survey Questionnaire. Multivariate linear logistic regression models adjusted for confounders provided mean score differences (MDs) with 95% confidence intervals in each HRQoL scale and item. This study enrolled 244 patients with BCRL and 823 patients without BCRL. Patients with BCRL had significantly poorer HRQoL than patients without BCRL in 16 out of 18 HRQoL subscales, for example, in physical function (MDs 27, 95%CI: 24; 30), mental health (MDs 24, 95%CI: 21; 27) and social role functioning (MDs 20, 95%CI: 17; 23). Age, BMI, BCRL severity, hand and dominant arm affection had only minor impact on HRQoL (MDs < 5), suggesting a high degree of inter-individual variation in coping with lymphedema. This study showed that BCRL is associated with long-term impairments in HRQoL, especially affecting the physical and psychosocial domains. Surprisingly, BCRL diagnosis rather than clinical severity drove the largest impairments in HRQoL.

Abstract Background/Aims  Axial spondyloarthritis (axSpA) is a chronic rheumatic condition, characterised by inflammatory back pain - often associated with impaired function and mobility, sleep disturbance, fatigue, and reduced quality of life. Despite the vast advances in pharmacological treatments for axSpA over the last few decades, physical activity and rehabilitation remain vital for effective disease management. At the Royal National Hospital for Rheumatic Diseases in Bath (RNHRD), the 2-week inpatient axSpA rehabilitation programme has been integral to axSpA care since the 1970’s. Prior research has demonstrated significant short-term improvements in spinal mobility (BASMI), function (BASFI) and disease activity (BASDAI) following course attendance. However, the long-term outcomes are yet to be evaluated in this unique cohort. Methods  Since the early 1990’s, clinical measures of spinal mobility, function and disease activity have been routinely collected at the RNHRD at all clinical appointments through administration of the BASMI, BASFI and BASDAI, respectively. Dates of attending the axSpA course and standard clinical and treatment follow-up data were also collected. Multiple linear regression models were used to investigate the impact of course attendance on final reported BASMI, BASDAI and BASFI scores (final score=most recent). Length of follow-up was defined as time between first and last recorded BASMI. Results  Of the 203 patients within the Bath SPARC200 cohort, 77.8% (158/203) had attended at least one rehabilitation course throughout follow-up. 70.0% (140/203) of patients were male. The mean duration of follow-up was 13.5 years (range 0-35 years); 28.1% (57/203) of individuals with 20+ years of follow-up. Course attendance (yes versus no) significantly reduced final BASMI score by 0.84 (p = 0.001, 95%CI -1.31 to -0.37) and final BASDAI score by 0.74 (p = 0.018, 95%CI -1.34 to -0.13). Although course attendance reduced final BASFI by 0.45 (95%CI -1.17 to 0.28), this relationship did not reach significance (p = 0.225). Whilst minimally clinically important difference (MCID) is, to our knowledge, yet to be defined for BASMI, MCIDs were achieved long-term for both BASDAI and BASFI - defined by van der Heijde and colleagues in 2016 as 0.7 and 0.4 for BASDAI and BASFI, respectively. Conclusion  These results provide novel evidence to support the integral role of education, physical activity and rehabilitation in the management of axSpA. Future work should investigate additional outcomes of critical importance to patients and clinicians, such as fatigue, quality of life and work productivity. Furthermore, a greater understanding of the factors that confound these outcomes may provide insights into those patients who may most benefit from attending a 2-week rehabilitation course. In addition to facilitating identification of those patients who may require additional clinical support. Disclosure  R. Barnett: None. A. McGrogan: None. M. Young: None. C. Cavill: None. M. Abstract Background The persistent quality gap in maternal health services in Nepal has resulted in poor maternal health outcomes. Accordingly, the Government of Nepal (GoN) has placed emphasis on responsive and accountable maternal health services and initiated social accountability interventions as a strategical approach simultaneously. This review critically explores the social accountability interventions in maternal health services in Nepal and its outcomes by analyzing existing evidence to contribute to the informed policy formulation process. Methods A literature review and desk study undertaken between December 2018 and May 2019. An adapted framework of social accountability by Lodenstein et al. was used for critical analysis of the existing literature between January 2000 and May 2019 from Nepal and other low-and-middle-income countries (LMICs) that have similar operational context to Nepal. The literature was searched and extracted from database such as PubMed and ScienceDirect, and web search engines such as Google Scholar using defined keywords. Results The study found various social accountability interventions that have been initiated by GoN and external development partners in maternal health services in Nepal. Evidence from Nepal and other LMICs showed that the social accountability interventions improved the quality of maternal health services by improving health system responsiveness, enhancing community ownership, addressing inequalities and enabling the community to influence the policy decision-making process. Strong gender norms, caste-hierarchy system, socio-political and economic context and weak enforceability mechanism in the health system are found to be the major contextual factors influencing community engagement in social accountability interventions in Nepal. Conclusions Social accountability interventions have potential to improve the quality of maternal health services in Nepal. The critical factor for successful outcomes in maternal health services is quality implementation of interventions. Similarly, continuous effort is needed from policymakers to strengthen monitoring and regulatory mechanism of the health system and decentralization process, to improve access to the information and to establish proper complaints and feedback system from the community to ensure the effectiveness and sustainability of the interventions. Furthermore, more study needs to be conducted to evaluate the impact of the existing social accountability interventions in improving maternal health services in Nepal.
