The purpose of the work is to improve the quality of the process of running-in of car engine parts during running-in using the additive of complex action. The tasks are the theoretical substantiation of the formation of transfer films during engine running-in and the conduct of comparative studies of the developed complex additive to M-8-B motor oil. The paper substantiates that in order to improve the quality and accelerate the running-in of car engine parts during running-in, a complex additive is required, which includes both surfactants and chemically active substances. The preconditions for obtaining transfer films on the friction surfaces of engine connections during running-in are considered. A frictional interaction is described in the case when the soft transfer film is strong and fixed on the main material, and the sliding surface coincides with the boundary between the film and the counter body. The presence in the zone of contact of surfactants with metals leads to the implementation of a special mechanism of frictional interaction, characterized by a colloidal system of particles in the lubricant and structural transformation on the interface. Comparative laboratory studies of the proposed additive were performed. The dependence of the moment of friction force on the time of the tests, the surface roughness of the samples worked in different compositions, as well as the distribution of the microhardness of the samples by depth were investigated. Based on the analysis of additives for running-in of engine parts, the use of complex additives containing surfactants and chemically active substances is determined as promising. Based on these studies, an additive containing ferrite spinel has been proposed. This allows you to control the running-in process through the structurally sensitive properties of the spinel composition. The use of the proposed additive of complex action helps to change the structure of the surface layer of the metal during friction under the action of temperature and load, which leads to improved quality of running-in of car engine parts during running-in.

This article discusses the problem of assessing the effectiveness of the rehabilitation process. The assessment of the effectiveness of the rehabilitation process is recognized by many researchers as one of the most difficult in the theoretical substantiation of the process of rehabilitation and habilitation, in this regard, there are no uniform approaches to assessing the effectiveness of rehabilitation today. The article presents studies by several authors on this issue. Rehabilitation of patients after a stroke most often occurs with the use of rehabilitation complexes. The possibility of applying the assessment of the effectiveness of rehabilitation to such complexes is very relevant, since at the moment there is no unified system for determining the quality of rehabilitation complexes.

he modern interpretation of the principles of therapy of periodontal pathology brings individualized treatment of patients to the first place. The basic principle of local therapy of periodontal diseases, in our opinion, is drug therapeutic treatment. The success of the treatment of periodontal diseases, the duration of remission and the prevention of complications largely depends on the quality of the impact on the oral cavity microflora. Recently, antiseptics of complex action have been successfully used. One of the representatives of the group of antiseptics is the drug «Citeal», which contains active substances: hexamidine, chorhexidine, chlorcresol. The purpose of the work is to increase the effectiveness of treatment of patients with periodontal pathology due to the use of three-component antiseptic agent of complex action — «Citeal». The results of a comprehensive examination of patients after the end of treatment show an improvement in oral hygiene, a decrease in the PMA index in comparison with the data before treatment. The Schiller-Pisarev test also became negative after the treatment. It is recommended to carry out professional hygiene with the use of antiseptic solution «Citeal» in a 1:10 dilution in the form of application to the marginal and alveolar part of the gums and irrigation of periodontal pockets, followed by mandatory rinsing of the mouth with water to remove residues. It was found that an individualized and differentiated approach to the treatment of patients with various forms of gingivitis and generalized periodontitis, who have orthopedic structures, prevents exacerbation of the disease during the year.

The top-crusher with more advanced design and improved performance characteristics has been proposed. The dependence between geometric and mechanical parameters of the rotor is established, providing maximum absolute speed of knife blades for the moment of cutting the stem which creates the best conditions for grinding the stems. It follows from the results of calculations that the proposed chain top-crusher, in comparison to the existing analogues, has: minimum energy consumption (Nprv=13-15 kWt), and, consequently, the lower fuel consumption (in comparison to KIR-1.5 Nprv KIR=18-20 kWt), improved quality of making technological process – by copying the transverse profile of the ridges and the increased reliability of working bodies.

Research goal: the research is aimed at theoretical substantiation of applying the augmented reality technology and its peculiarities at higher technical educational institutions. Research objectives: the research is to solve the problems of determining the role and place of the technology in the educational process and its possible application to physics training. Object of research: teaching physics to students of higher technical educational institutions. Subject of research: the augmented reality technology as a component of the training process at higher educational institutions. Research methods used: theoretical methods include analysis of scientific and methodological literature; empirical methods include studying and observation of the training process. Research results: analysis of scientific publications allows defining the notion of augmented reality; application of augmented reality objects during laboratory practical works on physics is suggested. Main conclusions. introduction of the augmented reality technology in thetraining process at higher technical educational institutions increases learning efficiency, facilitates students’ training and cognitive activities, improves the quality of knowledge acquisition, provokes interest in a subject, promotesdevelopment of research skills and a future specialist’s competent personality.

The widespread prevalence of comorbid pathology determines the relevance of this problem. Comorbid pathology due to the interaction of diseases, drug pathomorphism, age characteristics of the patient, significantly changes clinical picture and course of the main nosology, affects severity of complications and their nature, significantly affects quality of life and prognosis of patients. Diagnosis and treatment of many diseases is complicated in the conditions of comorbidity. The article presents a clinical case of a comorbid patient with arterial hypertension from the moment of exposure to risk factors and ending with the formation of many concomitant diseases, as an example of trans-nosological comorbidity.

AbstractBackground:Awareness of dementia is examined in different scientific fields as significant for assessment of diagnosis, and for treatment and adaptation to the disease. There are very few longitudinal studies of individual experiences of awareness among people with dementia, related to quality of life.Aim:To examine how younger people (< 65 years) with dementia (YOD) express awareness of the dementia and how, over time, they seem to handle awareness as a strategy to preserve quality of life.Method:A longitudinal qualitative study with individuals with YOD was performed with interviews every six months over five years for a maximum of ten interviews. The interviews were analysed by modified grounded theory.Findings:Awareness is a complex, multidimensional concept. Awareness of dementia is predisposed by personality, life history and established coping styles. The main coping styles – live in the moment, ignore the dementia, and make the best of it – seem to be rather consistent throughout the progression of the disease. Transitions in life situation, such as moving to a nursing home, may change the individual’s awareness of dementia.Conclusion:Unawareness of dementia may have an important adaptive function to preserve quality of life. To increase awareness must be approached with reflexivity and the utmost sensitivity.

Meaningful engagement is an important dimension of quality of life and care for persons living with dementia, including the growing number who reside in assisted living communities. This report presents preliminary findings from an ongoing qualitative study aimed at identifying best care practices to create and maintain meaningful engagement among persons with dementia. Over a 1-year period, we conducted interviews, residents’ record review, and participant observations in four diverse care communities. Our analysis identified four approaches that successfully promote meaningful engagement: (a) knowing the person, (b) connecting with and meeting people where they are, (c) being in the moment, and (d) viewing all encounters as opportunity. Incorporation of these approaches in care routines and adoption by all care partners can promote meaningful engagement, including during crises such as COVID-19.

This review discusses a number of aspects of surgical methods for treating severe chronic heart failure: resynchronizing therapy, mechanical circulatory support systems, and heart transplantation. Surgical methods for the treatment of heart failure are a rapidly developing field of modern cardiology and cardiac surgery. The main surgical method of treatment was and remains orthotopic transplantation of a donor heart. The advent of implantable systems has affected the problem of heart transplantation. Over the past decade, the use of mechanical circulatory support systems has grown significantly. At the moment, there are 3 main directions: creating devices for auxiliary blood circulation, various modes and methods of electrical stimulation of the myocardium, creating devices that mechanically remodel the heart chambers (left ventricle). All of these directions to some extent (depending on the evidence base) have found their place in modern recommendations for the treatment of chronic heart failure. The use of mechanical left ventricular remodeling shows good results in patients suffering from symptomatic heart failure, which leads to a significant and persistent decrease in the volume of the left ventricle and improvement of its function, symptoms and quality of life. Despite the fact that at the moment the geography and prevalence of their use is small, the number of implanted devices will only grow. Thus, given the need for frequent hospitalizations and high treatment costs, it is necessary to improve modern methods of surgical treatment of severe and terminal heart failure, make them more accessible, which will affect the duration and quality of life of these patients.

This article discusses the problem of choosing the correct and effective organizational structure of enterprise management for its correct functioning. The existing types of organizational structures of enterprise management, which are used in practice today, are analyzed, and their positive, negative sides and the type of enterprises for which they can be used are revealed. Along with this, this article discusses and describes methods for choosing an organizational structure for an enterprise. Also, during the study, internal and external factors were identified that affect the choice of an organizational structure. Based on the analysis, a methodology for choosing the most effective type of organizational structure for enterprises was drawn up. The question of choosing an organizational structure is acute for every manager at the very beginning of the operation of an enterprise, because the quality of performance of functions, both of individual divisions and of the entire enterprise as a whole, directly depends on this. Among many types of organizational structures in this study, the types of organizational structures that are most adaptable to changes in external and internal factors are identified. This study provides methods and tools for selecting the appropriate organizational structure for any enterprise. At the moment, some methods, be it goal structuring or computer modeling, are not widely used, which in turn makes it difficult to choose an effective organizational structure for enterprise management. The choice of an effective organizational structure is an extremely urgent problem today for every entrepreneur, who is interested in the stable economic activity of his or her enterprise.
