Gender differences in the dimensions of tricuspid valve orifice and leaflets in the Bangladeshi population - a morphometric study

Background: The present study aims to measure the circumference, length, and breadth of thedifferent leaflets of the tricuspid valve orifice in men and women to enable improved treatmentand management for cardiac patients in future. Materials and methods: In this cross-sectionalstudy, samples of the cadaveric heart (41 male, 19 female, and age ranging between 20 and70 years) were collected within 12–24 hours of death from unclaimed dead bodies that wereautopsied in the morgues of Sir Salimullah Medical College (SSMC) and Dhaka Medical College(DMC), Dhaka, Bangladesh. Unpaired t-test was conducted to determine significant genderdifferences using SPSS 16. Results: There were no significant difference in the circumference,length, and breadth of the anterior, posterior, and septal leaflets of tricuspid valves between menand women. Conclusion :The circumference, length, and breadth of the anterior, posterior,and septal leaflets of tricuspid valves are similar between men and women in the Bangladeshipopulation. Background: It is observed that the morphologic and morphometric variability of the occipital bone structures may coexist in the same individual or among different subjects of the same or different populations and thus, a sound knowledge of the morphometry of this area can provide important benefits in determining safe surgical zones during surgical procedures. Aim: The present study was aimed at measuring the length (right and left) and width (right and left) of the hypoglossal canal among adult dry skulls of two states in southern Nigeria. Materials and Method: This study adopted the cross sectional study design. A total of eighty (80) hypoglossal canal; right and left were selected by simple random sampling and their length and width were measured with the aid of the digital vernier caliper. Results: The hypoglossal canal length on the right side was seen to be higher compared to the left length of the hypoglossal canal while the right hypoglossal canal width was seen to be higher compared to the left hypoglossal canal width and also observed differences between the right and left sides were statistically significant (P=0.01). Conclusion: There was a statistical significant difference with regard to hypoglossal canal length (right and left) and width (right and left) among the studied population. The femur is the weight bearing typical long bone of lower limb which extends from the pelvis to the knee. The anatomical knowledge of different dimensions of femur specially head and neck of the femur is very essential in anthropological and medico-legal practice for sex determination and as well as to radiologists, rheumatologists and orthopedic surgeons for diagnosis and planning of treatment. This is an observational descriptive type of study carried out in the Department of Anatomy, Sir Salimullah Medical College (SSMC), Dhaka from July 2011 to June 2012. The present study was performed on one hundred and ninety-nine (n=199; male-89 & female-110) left sided fully ossified human femur bones collected from the Department of Anatomy and the medical students of 1st & 2nd year MBBS source Sir Salimullah Medical College (SSMC), Dhaka and Dhaka National Medical College (DNMC), Dhaka. Morphometric study was carried out on all samples by direct physical methods. There was statistically significant difference found in the diameter of the head and neck of the femur bones between male and female bones. Abstract Aims & Objectives : Interosseous border of tibia splits distally into anterior and posterior edges. These edges project into anterior and posterior tubercles enclosing a notch called fibular incisura that articulates with the distal fibula forming distal tibio fibular syndesmosis. The aim of the present study was to obtain the morphometric data of fibular incisura that may be helpful for the orthopedic surgeons in ankle reconstructive surgeries and ankle joint replacement. Materials & Method: To describe the morphometry of the fibular incisura, a total of 100 tibia [right 55 and left 45] were obtained from the Department of Anatomy, NRI Medical College, Chinakakani. The width of tibia, width, depth, height of fibular incisura and the length of anterior, posterior incisural tubercles were measured in both right and left tibia using digital vernier calipers. Right and left side para meters were compared using Karl Pearson Coefficient [r value]. Results: Average values for the width, depth and height of the fibular incisura were 16.83mm on the right side and 17.42mm on the left side, 2.85±1.38mm on the right and 2.9±1.57mm on the left side, 38.82±5.8mm on the right side and 38.72±7.68 mm on the left side respectively. While average lengths of anterior and posterior tubercles of the fibular incisura were 13.19±1.96mm, 15.71±2.03mm on the right side and 12.6±1.49mm, 15.66±1.41mm on left side. Thirty-three tibia presented deeply concave [depth>4cm], and sixty-seven had shallow fibular incisura [depth<4cm]. Conclusion: The morphometry and anatomical variability of fibular incisura should be taken to account in assessing radiographs, CT and MRI of talocrural joint.

Background: Tendoachilles is the chief plantar flexor of the ankle joint. The present study was conducted to find out the possible variations of tendoachilles between the right and the left leg of both male and female.Materials and methods: A cross sectional observational study was conducted in the department of Anatomy, Dhaka Medical College, Dhaka, from July 2013 to June 2014. The data were collected from both the right and the left tendoachilles of 60 human cadavers taken from Anatomy Departments of different Government and Nongovernment Medical Colleges in Dhaka city. Among the 60 cadavers, 30 were male and 30 were female. After dissection, the length of tendoachilles and the breadths at different levels of tendoachilles were measured and recorded.Results: The mean length of tendoachilles was significantly greater in the right than in the left leg of both male and female. The tendoachilles was longer in male than in female in both the right and the left leg (P<0.01). There was significant positive correlation between the height of cadaver and the length of tendoachilles of both the right leg and the left leg in case of male. Positive correlation was also observed between the height of cadaver and the length of tendoachilles of both the legs in case of female, but that was not statistically significant. The mean breadths of tendoachilles were significantly greater in the right leg than in the left leg at its junction with gastrocnemius and at the level of its junction with calcaneus in both male and female and also in male than in female in both legs. Significant difference was observed between the breadth of right and left tendoachilles at the level of 4 cm above its junction with calcaneus in male but not in female (P>0.05).Conclusion: Significantly greater values were observed in the length and in most of the breadths of tendoachilles in the right leg of both sexes and in male in both legs. There was significant positive correlation between the height of cadaver and the length of tendoachilles of both legs in case of male, but, not in female.Bangladesh Journal of Medical Science Vol.17(3) 2018 p.388-394

Introduction: Medical education is at crossroads with various approaches in use throughout the world. Many medical schools in developing countries are on the verge of transition from traditional didactic lecture based approach to a problem-based approach. This study was conducted to evaluate the efficiency and efficacy of horizontal integration in comparison to isolated didactic lectures.Methods: This study was conducted at an Indian Medical college where integration is not routinely practiced. Lectures of anatomy and physiology were integrated and students’ understanding was assessed using pre-test and post-test in the form of 10 multiple-choice questions each. Opinion of students was taken using a questionnaire. Total students participating in the study were 118.Results: The mean score in pretest was found to be 5.32±1.88 and mean post-test score was 5.43±1.89. Students t test revealed a P value of 0.65 reflecting no statistically significant difference in the results. In the subjective assessment 76% of students didn't have any past experience of integration of 2 difference subjects, 87% students said that integration of anatomy and physiology lectures facilitated the understanding of the subject, 75% students wanted more topics to be taught in an integrated manner in future whereas 23% students say that only few relevant topics should be integrated. 47% of students recommended the integration of more subjects and the subject chosen was biochemistry. Most students said that such integration provided a better and holistic understanding of topics being covered. Students identified no disadvantages of integration.Conclusions: The objective result showed no significant improvements in scores of pre-test and post-test. On the other hand the subjective response of the students pointed towards an integrated approach being superior to stand alone didactic lectures in providing better understanding.

Background: By assessing the size of the foramen magnum, it is possible to distinguish two genders, as well as the intergenerational interdependence of the affected individuals. The present study aimed at assessing the value of diameters of foramen magnum for gender identification among the Iranian population.Methods: This analytical comparative cross-sectional study was performed on 200 Iranian cadavers (100 men and 100 women) referred to Kahrizak Autopsy Hall in Tehran between 2017 and 2018. In each case, the foramen magnum was measured by observation and its parameters were measured in a longitudinal anteroposterior and transverse diameter, using a calibrated caliper.Results: The Mean±SD anterior-posterior diameter of the foramen magnum in men and women were 35.59±0.49 mm and 33.90±1.07 mm, respectively, which was significantly higher in men (P<0.001). Similarly, the Mean±SD transverse diameter of the foramen magnum in males and females was 29.49±0.56 mm and 28.5±0.50 mm, respectively, which was significantly higher in men (P<0.001). Based on the area analysis under the Receiver Operating Characteristic (ROC) curve, the anterior-posterior and transverse diameters of the foramen magnum had a high value in the differentiation of the male and female genders (AUC=0.953 and 0.896, respectively, P<0.001). The Mean±SD foramen magnum index in men and women was 84.15±3.02 and 82.87±1.95, respectively, which had a significant difference between the two genders.Conclusion: Given that the anterior-posterior and transverse diameters of the foramen magnum in Iranian men are far more than Iranian women, these diameters can be used to distinguish between the two genders in the precise legal field.

Background: Functional tricuspid valve regurgitation secondary to left-sided valve disease is common. DeVega repair is simple, but residual regurgitation with subsequent impairment of the right ventricular function is a concern. This study aims to compare tricuspid valve repair using DeVega vs. ring annuloplasty and their impact on the right ventricle in the early postoperative period and after six months. Methods: This is a prospective cohort study of 51 patients with rheumatic heart disease who underwent tricuspid valve repair for secondary severe tricuspid regurgitation. Patients were divided into two groups: group A; DeVega repair (n=34) and group B; ring annuloplasty repair (n=17). Patients were assessed clinically and by echocardiography before discharge and after six months for the degree of tricuspid regurgitation, right ventricular diameter and tricuspid annular plane systolic excursion (TAPSE). Results: Preoperative echocardiographic assessment showed no difference in left ventricular end-systolic diameter, end-diastolic diameter, ejection fraction and right ventricular diameter, however; group A had significantly better preoperative right ventricular function measured by TAPSE (1.96 ± 0.27 vs1.75 ± 0.31 cm; p=0.02). Group B had significantly longer cardiopulmonary bypass time (127.65 ± 13.56 vs. 111.74 ± 18.74 minutes; p= 0.003) and ischemic time (99.06 ± 11.80 vs. 87.15 ± 16.01 minutes; p= 0.009). Pre-discharge, there was no statistically significant difference in the degree of tricuspid regurgitation, but the right ventricular diameter was significantly lower in group B (2.66 ± 0.41 and 2.40 ± 0.48 cm; p=0.049). After six months of follow up, the degree of tricuspid regurgitation (p= 0.029) and the right ventricular diameter were significantly lower in the ring annuloplasty group (2.56 ± 0.39 and 2.29 ± 0.44 cm; p=0.029). Although there was a statistically significant difference in preoperative TAPSE, this difference disappeared after six months. Conclusion: Both DeVega and ring annuloplasty techniques were effective in the early postoperative period, ring annuloplasty was associated with lesser residual regurgitation and better right ventricular remodeling in severe functional tricuspid regurgitation than DeVega procedure after 6-months of follow up.

The abuse of some pesticides especially to suicide is one of the current problems of pesticides. Aluminum phosphide induced poisoning usually happens to suicide and sometimes it is due to accidental occupational exposure and in a few cases it has some criminal intensions. This study is conducted to evaluate patients poisoned with aluminum phosphide. In the present study the medical records of cases of poisoning with rice tablets (aluminum phosphide) hospitalized in Ahvaz Razi hospital is studied. Accordingly, a checklist is prepared that included demographic information of patients (age, gender) and information on patient records (information on poisoning) are completed using the patients’ medical records. The analysis of data is done by SPSS V22. 18 patients poisoned with rice tablet (aluminum phosphide) are studied. Results of the study show that 11 patients are male and seven are female. The mean patient age is 27.06 ±8.04 years that is 28 ±9 and 25 ±6.02 in men and women respectively. Statistical tests show no statistically significant difference in mean age in both genders (P> 0.05). Among patients, 11 subjects took aluminum phosphide to attempt suicide and 3 cases took it unintentionally and of course the reason is not mentioned in four cases. Among the patients who tried to commit suicide by taking aluminum phosphide, 6 cases are male and 5 cases are female that no statistically significant difference is observed between the genders in this respect (P> 0.05). In addition to the study of the complications caused by this poisoning and its mortality, it is recommended to responsible authorities to provide the necessary educations and treatments to prevent this type of poisoning.

Background: Adverse maternal and perinatal outcomes are related to pregnancies spaced too closely together. Objective of present study was to compare the expulsion rate and complications between post placental IUCD insertion between caesarean section and vaginal delivery.Methods: This study was a prospective comparative study conducted in the department of Obstetrics and Gynecology, at Agartala Government Medical College over 1.5 Years (January 2016-June2017) All cases at term pregnancy delivering by caesarean section and vaginal delivery were divided into two different groups. Sample size of 105 in each group. Subjects recruited from-obstetrics OPD and casualty of Agartala Government Medical College (AGMC) and GB Pant Hospital expulsion rate and complications. Comparative evaluation of Expulsion rate and complications following post placental IUCD insertion between caesarean section and vaginal delivery at the end of six months, one year and one and half year.Results: There was no significant difference in either complications between the two groups (P value-.913) or outcomes (p value-.035). Expulsion rate 18.2% following vaginal delivery compared to those with intracaesarean insertion i.e 3.8%.Conclusions: The complications associated with postplacental Intrauterine device insertion is insignificant, still the awareness, acceptance and continuation are very low. Therefore Information, education Communication activity by the field workers must be enhanced to overcome this knowledge gap.

Abstract In recent years, a novel technique has been employed to maintain a distance between the prostate and the rectum by transperineally injecting a hydrogel spacer (HS). However, the effect of HS on the prostate positional displacement is poorly understood, despite its stability with HS in place. In this study, we investigated the effect of HS insertion on the interfraction prostate motion during the course of proton therapy (PT) for Japanese prostate cancer patients. The study population consisted of 22 cases of intermediate-risk prostate cancer with 11 cases with HS insertion and 11 cases without HS insertion. The irradiation position and preparation were similar for both groups. To test for reproducibility, regular confirmation computed tomography (RCCT) was done four times during the treatment period, and five times overall [including treatment planning CT (TPCT)] in each patient. Considering the prostate position of the TPCT as the reference, the change in the center of gravity of the prostate relative to the bony anatomy in the RCCTs of each patient was determined in the left–right (LR), superior–inferior (SI) and anterior–posterior (AP) directions. As a result, no significant difference was observed across the groups in the LR and SI directions. Conversely, a significant difference was observed in the AP direction (P &lt; 0.05). The proportion of the 3D vector length ≤5 mm was 95% in the inserted group, but 55% in the non-inserted group. Therefore, HS is not only effective in reducing rectal dose, but may also contribute to the positional reproducibility of the prostate.
