Caesarean section (c-section) is one of the important risk factor to placenta praevia. There is significant association between placenta praevia and previous c-section pregnancy. It is a leading cause of APH and it affects approximately 0.5% of all labour. This cross sectional type of observational descriptive study based on non- probability technique was done in Faridpur Medical College Hospital from July 2015 - June 2016. A total of 150 pregnant women were studied. Pregnant women with H/O previous c-section once or more beyond 28 weeks gestation were included. Those who were primigravidae or pregnant women without previous c-section were excluded. The data were subjected to chi-square test with SPSS software version 20.0. In our study women with previous c-section were selected; out of which 16 patients (10.67%) were found to have placenta praevia and 134 patients (89.33%) were not found. The mean age of the study subject was 27.25 ± 3.43 years with maximum number of patients having age between 25 to 29 years. Among the study population other placental positions were anterofundal 90(60%) and posterofundal 44 (29.33%). The frequency of placenta praevia out of 16 women who had one c-section was 11 (10%), two c-section was 4 (11.4%) and three c-section was 1 (20%).There was significant association (p-value < 0.05) between number of c-section and placenta praevia. There was found significant relation between placenta praevia and lower segment caesarean section (LSCS) in our study.Faridpur Med. Coll. J. Jul 2017;12(2): 75-77

Background: Women with previous LSCS often have to make a decision about mode of delivery of their second baby. As the rate of caesarean section is continuously increasing, vaginal birth after caesarean section (VBAC) is a good strategy to decrease caesarean rate. The present study was planned to assess the fetomaternal outcome in pregnancies with previous lower segment caesarean section undergoing trial of scar and to identify the factors, which can influence the outcome of trial of scar. Methods: This was a prospective observational study on 100 patients at a tertiary care institute. Pregnant women with previous LSCS were selected randomly for the study on the basis of the inclusion and exclusion criteria. Each labor monitored closely using a partogram. Decision for repeat emergency caesarean was taken by consultant. All women included in the study were followed through delivery and till discharge. Results: Out of 100 pregnant women 49 % cases had successful VBAC, 50% had emergency caesarean and one patient had laparotomy for rupture uterus. In women, who also had a prior vaginal delivery, 72% delivered vaginally, as compared to 40% of the women who did not undergo prior vaginal delivery (p value=0.003). Women who were in spontaneous labor, 59.21% delivered vaginally, whereas women who were induced, 16.6% delivered vaginally. The rate of perinatal complication was more in the patients who required an emergency CS after a failed trial. Conclusions: Our findings may encourage obstetricians to encourage VBAC in the properly screened ANC patients and decrease the rate of recommending caesarean section.

The coverage of Antenatal Care (ANC) visits in Gorontalo District during the last three years is as follows: the first visit of the pregnant women in 2015 reached 90%, the visit in the following year in 2016 decreased to 76%, and the visit in 2017 reached 89,86%. This research aims to find out the factors related to the decrease of Antenatal Care in accordance with the decrease in the Antenatal Care visits in the first contact of pregnant women checking K-1 in the working area of Community Health Center Bongomeme Gorontalo District in 2018. The population of the current research was all pregnant women listed in Kohort pregnant women book in the Community Health Center Bogomeme in 2018 there were 163 pregnant women participated. The research design used was a cross sectional study approach. The obtained data were subsequently examined using chi-square test by referring to p value < 0,005. The research results shown that the parity variable of the pregnancy has p value of 0,038, pregnant women knowledge has p value of 0,012, and pregnant women job has p value of 0,000. Thus, this variable has a parity of pregnancy, knowledge of pregnant women and occupation of pregnant women has a relationship with antenatal visits (K-1), while family support variables with p value of 0.478 means that this variable has no relationship with antenatal visits (K-1). The conclusion was the parity, pregnant women knowledge, and pregnant women occupation have the significant relationship with the antenatal care visits (K-1), while the family support has no significant relationship with antenatal visits (K-1). The pregnant women should maintain the pregnancy spacing, maintain the pregnancy health, and increase their knowledge.
