Physician burnout has been recognized as a public health crisis. However, there is a paucity of burnout studies in the context of medical internship. We assessed the prevalence and relationship between various training characteristics, personal variables, resilience, and coping with burnout in a cross-sectional study involving 837 interns from ten hospitals across Malaysian healthcare system. The instrument package included demographic questions, the Connor–Davidson Resilience Scale, Brief COPE and the Copenhagen Burnout Inventory. A total of 754 (90.1%) interns completed the inventories. We found a high prevalence of personal-related (73.3%), work-related (69.1%), and patient-related (43.4%) burnout among Malaysian interns. Multivariable analysis showed female gender (odds ratio (OR):1.50; 95% confidence interval (CI): 1.02–2.20), prior work experience (OR: 1.56; 95% CI: 1.05–2.30), and irregular spirituality routines (OR: 1.97; 95% CI: 1.30–2.99) were associated with increased odds of personal-related burnout. Irregular spirituality routines (OR: 2.24; 95% CI: 1.49–3.37) were associated with work-related burnout, while living with other people (OR: 1.77; 95% CI: 1.15–2.73) was associated with patient-related burnout. Lower resilience levels and avoidant copings were associated with personal-, work-, and patient-related burnout. Burnout prevalence among interns is high. The findings support the value of individual-targeted alongside organizational-targeted intervention in burnout reduction. As burnout is prevalent in both years of internship training, ongoing burnout prevention and wellbeing measures are deemed necessary.

Medical training is intensive and predisposes students to psychological distress and burnout. Unaddressed burnout in medical training may persist in the internship phase and impact the quality of patient care. While some associations have been established, the link between some individual factors and training characteristics with distress and burnout in medical training remained unclear. In this study, we aim to examine the prevalence of psychological distress and burnout, and its association with gender, training phase, funding status, cumulative grade points average (CGPA), and coping strategies among medical students. The study applied a multicenter cross-sectional study design and convenience sampling on medical students from two medical schools from Malaysia and India. We used a self-reporting instrument that includes demographic details, the 12-item General Health Questionnaire (GHQ), the Copenhagen Burnout Inventory (CBI), and the Brief Coping Orientation to Problems Experienced (Brief COPE). A total of 748 medical students participated in the study. The prevalence of psychological distress, personal-related, work-related, and patient-related burnout were 33.0%, 56.1%, 35.0%, and 26.2%, respectively. Being male, clinical year, self-funded, and having a CGPA of more than 3.50 predicted psychological distress and burnout with mixed results. Maladaptive coping mechanisms consistently predicted the risk of psychological distress and burnout by more than two times. The findings indicate that primary and secondary mental health interventions have a role in medical training. A systematic intervention should incorporate coping skills training alongside institutional-targeted intervention.

Purpose—This study aimed to estimate the prevalence and risk factors of MSD pain in various anatomical regions among nurses. Method—A cross-sectional study involving a self-administered questionnaire by registered nurses with clinical experience. Data was collected using convenience sampling after obtaining informed consent. The results were drawn from a total of 300 nurses. Results—The nurses presented with occasional mental exhaustion (44.3%) and often physical exhaustion (44.0%). Almost all (97.3%) the nurses complained of having work-related pain during the last 12 months. Body parts with the most pain were the lower back (86.7%), ankles (86.7%), neck (86.0%), shoulders (85.0%), lower legs (84.7%) and upper back (84.3%). The pain frequency was rated as occasional pain for the neck and upper back, pain was often felt for the rest of the parts. Nurses complained of severe pain in the lower back (19.7%), right shoulder (29.7%) and left shoulder (30.3%). The frequency of having musculoskeletal symptoms in any body region was increased with age, lower education level, female gender, high BMI, job tenure and lifestyle. Conclusions—Nurses’ WRMSD complaints should be taken seriously to curb further risk and musculoskeletal hazards.

Abstract Background In the current healthcare delivery system funded by National Health Insurance (NHI) in Indonesia, the gatekeeper role of primary care services is critical to ensuring equal healthcare access for the population. To be effective, gatekeeping relies on the performance of general practitioners (GPs). However, the perceptions held by Indonesian GPs about their gatekeeper role are not yet well documented. This study describes the self-perceived knowledge, attitudes and performance of Indonesian GPs with respect to the gatekeeper role and explores associated factors. Methods We conducted a cross-sectional study of all primary care facilities (N = 75) contracted by the regional NHI office in the Banyumas district. The 73 participating GPs completed a written questionnaire that assessed their knowledge, attitudes and performance in relation to the gatekeeper role. Personal and facility characteristics were analysed in a generalised linear model as possible associating factors.Results GPs scored relatively high in the domains of knowledge and performance but scored lower in their attitudes towards the gatekeeper role of primary care. In the full-adjusted model, no factors were significantly associated with the knowledge score. Work experience as GPs, private or civil service employment status and rural or urban location of the primary care facility were linked to attitude scores. Full- or part-time employment and type of facility were factors associated with the performance score. Conclusion GPs in Indonesia are knowledgeable and report that they adequately perform their function as gatekeepers in primary care. However, their attitudes towards the gatekeeper function are less positive. Attitudes and performance with respect to the primary care gatekeeper role are likely influenced more by contextual factors such as location and type of facility than by personal factors. Efforts to address contextual issues could include improvements in practice standards for privately practising physicians and public information campaigns about gatekeeping regulations. Such efforts will be crucial to improving the gatekeeper role of primary care in Indonesia and assuring efficient access to high-quality care for all.

Background. The novel coronavirus disease (COVID-19) pandemic outbreak affects the global social, economic, and political context and becomes a significant threat to healthcare providers who are among the exposed groups to acquire and transmit the disease while caring and treating patients. It is crucial to comply with prevention recommendations so as to stay safe and protected. Therefore, this study aimed to assess COVID-19 preventive practice and associated factors among healthcare workers in Northwest Ethiopia. Methods. An institution-based cross-sectional study was conducted among 630 healthcare workers in Northwest Ethiopia from March to April 2020. A multistage sampling technique was used to select study participants. A pretested and structured self-administered questionnaire was used to collect data. The data were entered using Epi Info 7 and analyzed using STATA 16 statistical software. Both bivariate and multivariable logistic regression analyses were employed to identify associated factors. Adjusted odds ratio (AOR) with 95% confidence interval was used to determine independent predictors of COVID-19 preventive practice. In multivariable analysis, a variable with a P value of less than 0.05 was considered as statically significant. Result. Among 630 healthcare workers participated in the study, the overall good preventive practice towards COVID-19 was found to be 38.73% (95% CI: 34.8, 42.5). Being a male healthcare provider (AOR = 1.48; 95% CI: 1.02, 2.10), having work experience of 6–10 years (AOR = 2.22; 95% CI: 1.23, 4.00), and having poor attitude towards COVID-19 (AOR = 2.22; 95% CI: 1.03, 2.22) were found to be significantly associated with poor COVID-19 preventive practice among healthcare workers. Conclusion. Overall compliance towards COVID-19 preventive practice among healthcare workers was found to be low. Multiple education and training platforms with focus on COVID-19 preventive measures and adequate personal protective equipment and supplies should be provided for healthcare providers.

ObjectivesCardiopulmonary exercise test (CPET) provides accurate evaluation of physical capacity and disease severity in children with congenital heart disease (CHD). However, full participation to obtain optimal measure of VO2max may be difficult. As an alternative, the oxygen uptake efficiency slope (OUES) is a reproducible and reliable parameter measured during CPET, which does not require a maximal exercise to be interpretable. This study aimed to evaluate the OUES of a large cohort of children with CHD, in comparison with healthy controls. We also intended to identify, in this specific population, the clinical and CPET variables associated with the OUES.MethodsThis cross-sectional study was carried out between November 2010 and September 2015 in two tertiary care paediatric and congenital cardiology centres.Results709 children were included (407 CHD and 302 healthy controls). The association of clinical characteristics with weight-normalised OUES (OUESkg) was studied using a multivariable analysis. The mean OUESkg was significantly lower in CHD than in healthy controls (38.6±8.5 and 43.9±8.5; p<0.001, respectively), especially in the most severe CHD. The OUESkg correlated with VO2max (r=0.85, p<0.001), with cut-off values for normal exercise capacity of 38.4 in boys and 31.0 in girls. The decrease of OUESkg was associated with increased age, increased Body Mass Index, number of cardiac catheter or surgical procedures, female gender and decreased forced vital capacity (Z-score).ConclusionThe OUES is significantly impaired in children with CHD and strongly correlates with VO2max. The OUES has the same clinical determinants as VO2max and therefore may be of interest in submaximal exercise.Trial registration numberNCT01202916.

Abstract High levels of stress are expected when crises affect people’s lives. Therefore, this web-based, cross-sectional study was conducted among university students from Pakistan to investigate the psychological impairment and coping strategies during COVID-19 pandemic. Google forms were used to disseminate the online questionnaire to assess anxiety (Generalized Anxiety disorder-7), depression (Patient Health Questionnaire-9) and the coping strategies (Brief-COPE). A total of 1134 responses (age 21.7±3.5 years) were included. The frequency of students having moderate-severe anxiety and depression (score ≥ 10) were ≍ 34% and 45%, respectively. The respondents’ aged ≥ 31 years had significantly lower depression score than those ≤ 20 years (p = 0.047). Males had significantly less anxiety (6.62 ± 5.70 vs 7.84 ± 5.60, p = 0.001) and depression (8.73 ± 6.84 vs 9.71 ± 7.06, p = 0.031) scores. Those having family member, friend or acquaintances infected with disease had significantly higher anxiety score (8.89 ± 5.74 vs 7.09 ± 5.56, p < 0.001). Regarding coping strategies, majority of respondents were found to have adopted religious/spiritual coping (6.45 ± 1.68) followed by acceptance (5.58 ± 1.65), self-distraction (4.97 ± 1.61) and active coping (4.81 ± 1.57). In conclusion, COVID-19 cause significant impairment on mental health of the students. The most frequent coping strategy adopted by students were religious/spiritual and acceptance coping. During epidemics mental health of students should not be neglected.

Background: Modern defence system has a significant role of electrical, electronic and instrument technicians in armed forces. Despite their intensive role, little is known about work-related musculoskeletal disorder in these technicians. Objective: To investigate the prevalence of the work-related musculoskeletal disorder in defence personnel involved in electrical and electronic maintenance and repairs. Methods: A cross-sectional study was carried out for 3 years, in Electrical, telecom and instrument defence technicians (N = 164). Prevalence of work-related musculoskeletal disorder was assessed using the Nordic Musculoskeletal Questionnaire. Effect of age, working hours, work experience, Basal metabolic index and type of job on the prevalence of work-related musculoskeletal disorder was investigated using logistic regression analysis. Results: A total of 119 (72.56%) respondents reported work-related musculoskeletal disorder with 53.04% multiples & 19.51% single work-related musculoskeletal disorder. Simultaneous presence of neck, shoulder and upper back work-related musculoskeletal disorder were observed. Highest rate of work-related musculoskeletal disorder was in neck (44.44%), followed by shoulder (27.16%), ankle/ foot (14.81%), elbow/ forearm (12.96%), low back (11.72%) and wrist/ finger (9.87%). Neck (44%) related work-related musculoskeletal disorder were highest across all job crafts. Work experience (p = 0.012; OR = 0.87; 95% CI – 0.78- 0.97), working hours (p  = 0.031; OR= 1.73, 95% CI – 0.58-5.12) smoking (p = 0.00 ; OR = 5.3,  95 % CI = 4.3 – 8.48) and job crafts like electrician (p = 0.434; OR = 1.89, 95% CI = 1.08-3.31) and telecom mechanics (p = 0.026; OR = 1.26, 95% CI  =0.74 – 2.15) were significantly associated as risk factors of work-related musculoskeletal disorder. Conclusion: High prevalence of work-related musculoskeletal disorder was observed in electrical, telecommunication & instrument mechanics, with higher multiple work-related musculoskeletal disorder. Working hours, work experience, job craft and smoking were highly associated with a work-related musculoskeletal disorder. We suggest suitable ergonomic intervention and awareness program along with smoking control drive to reduce the risk of work-related musculoskeletal disorder.

Summary Background The 2019 coronavirus diseases (COVID-19) led out the mental health crisis. Aim To determine the psychological status and post-traumatic stress symptoms (PTSD) among general population (except confirmed and suspected cases, and close contacts) and their association with the coping strategy types during the COVID-19 outbreak. Design A cross-sectional study. Methods Participants were recruited from the community through snowball sampling with anonymous online questionnaires, using 28-item General Health Questionnaire, 22-item Impact of Events Scale-Revised and 28-item Brief Coping Inventory to measure their psychiatric disorders, PTSD level and coping strategies. Results Of the total 1109 participants, 42.65% and 67.09% self-reported psychiatric disorders and high PTSD level, respectively. Age, occupation and education level were significantly association with psychological status. The status of psychiatric disorders was also significantly related to high PTSD level. Using both emotion and problem coping was better for psychiatric status [adjusted odds ratio (aOR) = 0.72, 95% confidence interval (CI): 0.54–0.98], and problem-focused coping was significantly associated with high PTSD level (aOR = 2.09, 95% CI: 1.25–3.51). Conclusion Negative psychological outcomes were common among the general people during the COVID-19 outbreak, and the findings may provide references for intervention guidelines of mental health for the community population.

To assess the prevalence of upper extremity work-related musculoskeletal disorders (WMSDs) among surgical device mechanics compared to a control group, a total of 70 employees were included and assigned to three occupational groups (grinders, packers, and control). Personal factors, work exposure, manual skill, and complaints were assessed by two self-administered questionnaires and an industry test. WMSDs were diagnosed in a standardised clinical examination. The two-one-sided t-tests (TOST) procedure was used to test the clinical equivalence of the respective grinding and packaging groups vs. the control group in terms of the Disabilities of the Arm, Shoulder and Hand (DASH) score. Thirty-nine study participants (56%) experienced at least one WMSD at the elbow, forearm, and/or wrist, mainly with signs of epicondylitis and nerve entrapment at the medial elbow. The risk of grinders developing upper extremity WMSD was about 2.5-times higher and packers had an 8.6-fold higher risk of a clinically relevant DASH > 29 compared to the control group. However, these differences were not statistically significant. The groups were also proven to be clinically equivalent in terms of DASH score. Surgical device mechanics do not seem to have worse DASH values or be at higher risk of upper limb WMSDs compared to a control group. This is the first study to analyse and compare different workplaces in this industry that are also common in other industries.

BACKGROUND: Physicians are at risk of having high levels of stress which affect their performance. Finding the stressors and the coping skills to manage stress could be used to develop program to decrease stressful situation. No study has been done on Physicians' stress and coping in Iran. The main objective of this study is to find out the main stressors and coping strategies among Iranian Physicians working in hospital in Tehran-Iran.METHODS: A cross-sectional study was performed on 780 Physicians,using a questionnaire consisted of two sections ;The first section were the stressors which included 67 questions and The second section were The Brief COPE with 28-items for assessing a broad range of coping behaviors among respondents.RESULTS: A total of 1100 questionnaires were distributed to all the available Physicians in the hospitals selected. 780 Physicians returned complete questionnaires with observed response rate of 75%. The majority of respondents (56.9%) were women. The first 3 sources of stress in workplace (Job stressors) are physical environment problem (75%), too much volume of work and poorly paid. The main sources of stress outside the work place (non-job stressors) ranked by Physicians were; financial problem (9.09), not enough time to spend with family (8.87), conflicts with household tasks (7.36).The top five coping strategies used by Iranian Physicians were Behavioral Disengagement, Planning, Instrumental support, Acceptance, and turning to religion.CONCLUSIONS: This study revealed that both workplace and non-job sources of stress can affect the Physicians performance and there is an association between gender and coping skills.
