Повышение экономической эффективности разведения племенной популяции чернопестрого скота в Уральском регионе неизменно сопряжено со сложной селекционной работой по увеличению продуктивной жизни коров. Использование научно обоснованной системы выращивания молодняка и нанотехнологий должно быть направлено на поддержание репродуктивной функции племенного поголовья. При подборе быков необходимо учитывать качество предков быка по женской линии с учетом показателей продуктивности, продолжительности жизни и других функциональных характеристик, определяющих экономическую эффективность производства. Генофонд Уральского типа чернопестрой породы содержит аллели, характерные как для голштинской, так и для чернопестрой породы. С увеличением крови частота встречаемости аллелей улучшаемой породы не уменьшается, точно так же как и не происходит увеличения частоты встречаемости аллелей улучшающей породы.The increase of economic efficiency of breeding populations of blackandwhite cattle in the Ural region to be challenging selection work to increase of productive life of cows. The use of sciencebased system of rearing young and nanotechnology should be aimed at maintaining the reproductive function of the breeding stock. When selecting bulls, it is necessary to study the quality of the ancestors of the bull in the female line, taking into account the indicators of productivity, life expectancy and other functional characteristics that determine the economic efficiency of production. Scientific studies have shown that the correlation between the productivity of genomic evaluation and the improving effect of daughters of the bull ranges from15 to 15. The allele population of cattle blackmotley breed of the Ural type holsteinized contain alleles typical for improving (Holstein) breed and improved (blackandwhite) breed. With the increase in blood, the frequency of occurrence of alleles of the improved breed does not decrease, exactly as, and there is no increase in the frequency of occurrence of alleles of the improved breed.

In a large dairy company in the Ryazan region, studies of the mechanism of endocrine regu-lation of the estrial cycle were carried out, as well as ways of normalization of reproductive function were studied. Experimental data were obtained on the heifers of the Holstein breed of the same age. In a comparative aspect, the economic efficiency of the methods used is assessed. The drugs and methods used differed qualitatively from each other. For the first time, the combined use ofPGF Wakes Forte and Sedimin drugs has been proven to increase the probability of pregnancy in heifers of the same age by an average of 30 %.

Pain is a significant and alarming symptom of cancer seriously affecting the activity and quality of life of patients. Recent research proved that inadequate analgesia shortens life expectancy. Therefore, pain relief is not only a possibility but a professional, ethical and moral commitment to relieve patients from suffering, as well as ensure their adequate quality of life and human dignity. Proper pain relief can be achieved with medical therapy in most of the cases and the pharmacological alternatives are available in Hungary. Yet medical activity regarding pain relief is far from the desired. This paper gives a short summary of the guidelines on medical pain management focusing particularly on the use of opioids. Orv. Boilerplate—the fine-print terms and conditions that we become subject to when we click “I agree” online, rent an apartment, or enter an employment contract, for example—pervades all aspects of our modern lives. On a daily basis, most of us accept boilerplate provisions without realizing that should a dispute arise about a purchased good or service, the nonnegotiable boilerplate terms can deprive us of our right to jury trial and relieve providers of responsibility for harm. Boilerplate is the first comprehensive treatment of the problems posed by the increasing use of these terms, demonstrating how their use has degraded traditional notions of consent, agreement, and contract, and sacrificed core rights whose loss threatens the democratic order. This book examines attempts to justify the use of boilerplate provisions by claiming either that recipients freely consent to them or that economic efficiency demands them, and it finds these justifications wanting. It argues that our courts, legislatures, and regulatory agencies have fallen short in their evaluation and oversight of the use of boilerplate clauses. To improve legal evaluation of boilerplate, the book offers a new analytical framework, one that takes into account the nature of the rights affected, the quality of the recipient's consent, and the extent of the use of these terms. It goes on to offer possibilities for new methods of boilerplate evaluation and control, and concludes by discussing positive steps that NGOs, legislators, regulators, courts, and scholars could take to bring about better practices.

AbstractThe objective of this position paper is to review the current medical evidence andguidelines regarding the treatment of type 2 diabetes (T2DM) and to issue medicalrecommendations strengthening the timely use of insulin in patients with T2DMuncontrolled on noninsulin therapy. When noninsulin therapy fails to achieve or tomaintain HbA1c targets, insulin therapy is required. Timely insulin therapy couldprovide proper metabolic control that might prevent complications, lead toimprovement of life expectancy and quality of life.

Evaluation of the correlation between the economic efficiency level of private companies in Poland and the quality of location in the regions was the aim of the study. Based on the conducted analyses it was established that the level of sales markets absorptiveness shows the strongest correlation with the level of economic efficiency of the companies surveyed. The majority of regions characterised by absorptive sales markets also recorded good results in economic efficiency of companies located there while voivodships with low internal demand were characterised by definitely lower efficiency. Increasingly strong correlation was also recorded between the economic efficiency level of the companies surveyed and the level of knowledge in the regions. This may be the signal that currently the location factors of "soft" nature present increasing influence on the efficiency of companies and it is not impossible that their importance will continue increasing.

От породы коров и ее особенностей зависят эффективность использования кормов, качество и технологические свойства молока. Материалом исследований служили коровы чернопестрой голштинской и айрширской пород. Для повышения качества сенажа из козлятника восточного использовали микробиологический препарат. В готовом корме с биоконсервантом содержание сухого вещества было выше на 3,66, обменной энергии на 8,04, сырого протеина на 10,86, переваримого протеина на 13,59. Улучшение качества корма оказало положительное влияние на метаболические процессы в рубце подопытных коров. Независимо от технологии приготовления сенажа коровы айрширской породы лучше переваривали, по сравнению с голштинами, сухое вещество на 2,062,91 (Р0,05), органическое на 1,212,04 (Р0,05), сырой протеин на 2,222,49 (Р0,05), БЭВ на 0,921,36, но при этом голштины лучше переваривали сырой жир на 2,732,78 (Р0,05) и сырую клетчатку на 1,551,86. В результате внесения в сенаж биоконсерванта Силостан и улучшения переваримости корма у коров опытных групп значительно улучшился химический состав и технологические свойства молока. Очень важно для сыроделия, что в молоке коров, получавших в рационе сенаж с биоконсервантом, повышается массовая доля казеина на 2,42,7, а доля сывороточных белков и фракции казеина, которые не свертываются под действием сычужного фермента, наоборот, снижается на 1,41,5 и на 0,30,9, соответственно. Уменьшается продолжительность свертывания молока сычужным ферментом, снижаются потери сухого вещества с сывороткой, и увеличивается выход казеинового сгустка. Плотность казеинового сгустка повышается в молоке голштинских коров на 14,3, айрширских на 5,7 и составляет, соответственно, 1,92 и 2,96 г/см2.The efficiency of feed use, the quality, as well as technological properties of milk depend on cow breed and its characteristics. Blackandwhite Holstein and Ayrshire breeds were used as the material for research. Microbiological preparation was used to increase the quality of halyage made of Eastern galega. The content of dry substance in the readymade feed was 3.66 higher, the metabolizable energy 8.04 higher, raw protein 10.86 higher, and digestible protein 13.59. Improvement of feed quality positively influenced the metabolic processes in the rumen of experimental cows. Without regard to the technology of halyage preparation, cows of Ayrshire breed showed betterdigestion of dry substance by 2.06 2.91 (P0.05) than Holstein cows, organic substance by 1.21 2.04 (P0.05), raw protein by 2.22 2.49 (P0.05), nitrogenfree extractive fraction by 0.92 1.36, but Holstein breed showed better digestion of raw fat by 2.73 2.78 (P0.05) and raw fiber by 1.55 1.86. Chemical composition and technological properties of milk improved significantly as a result of addition of Silostan biopreservative to the halyage and improvement of feed digestibility in cows from test groups. The fact that the mass percentage of casein is increased by 2.4 2.7, and the share of serum proteins and ycasein fractions which do not coagulate under the influence of enzyme rennet is inversely decreased by 1.4 1.5 and 0.3 0.9, accordingly, is very important for cheese making. The duration of milk coagulation with rennet is decreased, the loss of dry substance with serum is decreased, and the yield of casein coagulate is increased. Density of casein coagulate is increased in the milk of Holstein cows by 14.3, Ayrshire cows by 5.7, and equals to 1.92 and 2.96 g/cm2, accordingly.

The relevance of the chosen topic is closely related to the development of a system for monitoring the implementation of the May Presidential Decree (2018) to increase life expectancy and improve the quality of life of the population. It has been stated that despite the positive dynamics of life expectancy over the past decade, Russia still has a huge regional differentiation (16,6 years for women, 18,2 years for men in 2016) and an average gender gap - 10,6 years. The choice of factors affecting the gender gap in life expectancy in Russian regions is supported by a conclusion most common in foreign studies that climatic conditions, living conditions, the quality of food and drinking water, alcohol consumption are essential components of public health and life expectancy.The article provides an overview of studies on assessment factors of life expectancy, presents the authors’ approach to identifying gender gaps in life expectancy, living conditions, and lifestyle affecting the indicator of life expectancy. The paper presents the results of a comparative analysis of gender gap in life expectancy in Russia and foreign countries. The paper studies the correlation between living conditions and lifestyle, and life expectancy.Classification of the regions by the life expectancy at birth for men and women is an important component of this study. The authors used Rosstat data for 2016 and the k-means method to select three clusters of Russian regions taking into account gender, living conditions and lifestyle. The paper identifies similar health problems of the regions that are associated with alcohol consumption, poor nutrition and poor quality of drinking water, poor housing and living conditions. The lowest life expectancy rates for men and women are recorded in regions of the 1st cluster (Jewish Autonomous Region, Republic of Tuva, Chukotka Autonomous Area). High mortality rates are a result of external causes. There is a poor quality of drinking water and poor nutrition, excessive alcohol consumption, and inadequate housing conditions.Based on the obtained results were determined possible reserves for reducing the gender and regional differentiation of life expectancy.

Abstract Introduction In 30% of patients with syncope, the underlying cause remains unexplained after clinical investigations. Unexplained syncope tends to recur, significantly impacting patients' quality of life of patients and mortality. Thus, there is a need for timely and more accurate diagnosis to initiate treatment. Dedicated care pathways are recommended by ESC guidelines. Purpose Patients with recurrent syncope were followed over time and patient outcomes with ILR were compared to patients with the same syncope burden, age, gender and mortality risk score who did not receive an ILR. Method A representative database of 4.9 million patients insured by German company statuary health insurances (BKK) was analysed over a time period of 10 years, 2007–17. Patients with recurrent syncope (two times ICD-10 GM diagnosis codes R55), age between 45–84 and no diagnosis code for the syncope were included in the analysis and followed for at least 2 years. Patients with ILR were matched to patients without ILR based on age, gender and Charlson Comorbidity index (CCI) using mahalanobis distances. The index event was the device implant in the ILR group and the second syncope event in the control group. Life expectancy, syncope hospitalisations, fall related injuries, health care costs, diagnoses and treatment rates were compared between the groups. Results A total of 412 patients with ILR for recurrent unexplained syncope were matched to the control group. Overall mean age was 68, mean was CCI 2.7, 42% were females. The risk of death was 2.35 times higher in the control group during follow up as shown in Figure 1 (p-value logrank test <0.0001). Cardiovascular related diagnosis and treatment rates were higher in the ILR group with 69% of patients having a cardiology diagnosis compared to 41% in the control group. Over a quarter (27%) of ILR patients received an implantable cardiac device compared to 5% in the control group. Ablation rates were 7% in the ILR group compared to 0% in the control group. Median health care costs were € 3,847 higher in the ILR group including the costs of the ILR implant, follow up and higher rates of cardiac treatments. These extra costs appear moderate given the substantially higher mortality risk in the control group. Conclusion This study of patients with recurrent unexplained syncope shows a remarkable difference in life expectancy in patients with ILR compared to a matched control group. Two large claim data analysis have recently shown higher rates of cardiovascular death as well as all-cause mortality in patients with unexplained syncope. A more vigilant cardiac workup might be needed to identify a possible underlying cardiac condition. Higher rates of cardiac device therapy in the ILR group were likely to play an important role for their better life expectancy. Cardiac therapies such as pacemakers, defibrillators and ablation have also been shown to significantly improve patients' quality of life. AbstractA number of patient-reported outcomes (PROs) predict increased mortality after primary cancer treatment. Studies, though, are sometimes affected by methodological limitations. They often use control variables that poorly predict life expectancy, examine only one or two PROs thus not controlling potential confounding by unmeasured PROs, and observe PROs at only a single point in time. To predict all-cause mortality, this study used control variables affording good estimates of life expectancy, conducted multivariate analyses of multiple PROs to identify independent predictors, and monitored PROs two years after diagnosis. We recruited a consecutive sample of 824 patients with uveal melanoma between April 2008 and December 2014. PROs were variables shown to predict mortality in previous studies; anxiety, depression, visual and ocular symptoms, visual function impairment, worry about cancer recurrence, and physical, emotional, social and functional quality of life (QoL), measured 6, 12 and 24 months after diagnosis. We conducted Cox regression analyses with a census date of December 2018. Covariates were age, gender, marital and employment status, self-reported co-morbidities, tumor diameter and thickness, treatment modality and chromosome 3 mutation status, the latter a genetic mutation strongly associated with mortality. Single predictor analyses (with covariates), showed 6-month depression and poorer functional QoL predicting mortality, as did 6–12 month increases in anxiety and 6–12 month decreases in physical and functional QoL. Multivariate analyses using all PROs showed independent prediction by 6-month depression and decreasing QoL over 6–12 months and 12–24 months. Elevated depression scores six months post-diagnosis constituted an increased mortality risk. Early intervention for depressive symptoms may reduce mortality.

At present, there is a weakening of the reproductive function of wages. This is especially noticeable on the example of a number of regions, which are characterized by a low level of average monthly wages compared to the all-Russian value and low purchasing power of wages, which extremely negatively affects the level and quality of working life of workers. Regional imbalances in the implementation of the reproductive function can be eliminated by developing the investment climate, modernizing existing industries, creating new effective jobs with decent wages, which implies the development of the intellectual, qualification and professional level of workers. Human development in the region is therefore a priority.
