The Dietary Reference Intakes (DRIs) are a set of recommendations for healthy persons. For the most part, recommendations are determined experimentally under controlled conditions of light activity. During increased physical activity, it is expected that micronutrient requirements would increase relative to the inactive state. Micronutrients of interest to athletes are those associated with oxygen handling and delivery, such as iron, and vitamin D, a newly emerging function of which is to maintain muscle strength. The DRI report on electrolytes (including water) is the most recent set of recommendations. In addition to recommendations for intakes to meet needs, many micronutrients have an upper level that indicates caution in consuming a large amount. We illustrate the process of setting DRI values for the micronutrients (including electrolytes and water), and provide a summary of instances where physical activity needs were considered when DRI values were derived. Understanding the origin of DRI values for micronutrients will assist in understanding how to use the values in assessment and planning.Key words: iron, vitamin D, electrolytes, physical activity, nutrient reference standards, adverse effects.

Knowledge of the effect of modifiable lifestyle factors in the pediatric multiple sclerosis (MS) population is limited. We therefore conducted a scoping review, following the framework provided by Arksey and O’Malley. Four databases were searched for pediatric MS and modifiable lifestyle factors using index terms and keywords, from inception to May 2018. All quantitative and qualitative primary articles were included and limited to English and full text. Of the 7202 articles identified and screened, 25 full-text articles were relevant to our objective and were included. These articles focused on diet obesity, physical activity, and sleep. In cross-sectional analyses, these lifestyle factors were associated with increased risk of pediatric onset MS (POMS), and increased disease activity. Diet, particularly vitamin D and vegetable intake, was associated with reduced relapse rate. Obesity was linked to increased risk of POMS, and physical activity was associated with reduced relapse rate and sleep/rest fatigue. Thus, available studies of lifestyle related outcomes in pediatric MS suggest specific lifestyle related factors, including obesity, higher vitamin D levels, and higher physical activity may associate with lower disease burden in POMS. Studies reviewed are limited by their observational designs. Future studies with longitudinal and experimental designs may further clarify the role of modifiable lifestyle factors in this population.

Objectives To evaluate the serum 25-hydroxyvitamin D [25(OH)D] concentration in Brazilian osteoporotic patients and the modifiable factors of vitamin D status in this population. Subjects and methods In a cross-sectional study, 363 community-dwelling patients who sought specialized medical care were evaluated between autumn and spring in São Paulo, Brazil. Serum levels of 25(OH)D and parathormone (PTH), biochemical and anthropometric measurements, and bone density scans were obtained. The group was assessed using two questionnaires: one questionnaire covered lifestyle and dietary habits, skin phototype, sun exposure, medical conditions, and levels of vitamin D supplementation (cholecalciferol); the other questionnaire assessed health-related quality-of-life. Logistic regression and a decision tree were used to assess the association between the variables and the adequacy of vitamin D status. Results The mean age of the overall sample was 67.9 ± 8.6 years, and the mean 25(OH)D concentration was 24.8 ng/mL. The prevalence of inadequate vitamin D status was high (73.3%), although 81.5% of the subjects were receiving cholecalciferol (mean dose of 8,169 IU/week). 25(OH)D was positively correlated with femoral neck bone mineral density and negatively correlated with PTH. In the multivariate analysis, the dose of cholecalciferol, engagement in physical activity and the month of the year (September) were associated with improvement in vitamin D status. Conclusions In this osteoporotic population, vitamin D supplementation of 7,000 IU/week is not enough to reach the desired 25(OH)D concentration (≥ 30 ng/mL). Engagement in physical activity and the month of the year are modifiable factors of the vitamin D status in this population.

<p>Proses penuaan mempengaruhi peningkatan distribusi lemak abdominal dengan indikator lingkar pinggang (LP) dan rasio lingkar pinggang panggul (RLPP). Penimbunan lemak abdominal menyebabkan disfungsi jaringan adiposa sehingga mempengaruhi biomarker proinflamasi yaitu kadar serum <em>high-sensitivity C-reactive Protein</em> (hs-CRP). Penelitian bertujuan untuk mengetahui hubungan LP dan RLPP dengan kadar hs-CRP lansia wanita. Rancangan penelitian <em>cross sectional</em> pada 53 subjek dipilih secara <em>consecutive sampling</em>. Antropometri yang diukur adalah LP dan RLPP. Pengukuran kadar serum hs-CRP dianalisis dengan metode <em>enyme-linked immunosorbent assay</em> (ELISA). Wawancara yang dilakukan yaitu data diri, asupan, aktivitas fisik, dan riwayat konsumsi obat. Data asupan diperoleh dengan metode <em>food recall</em> 3x24 jam. Aktivitas fisik diperoleh menggunakan <em>International Physical Activity Questionnaire</em> (IPAQ). Analisis data digunakan uji korelasi <em>Spearman</em>. Hasil penelitian menunjukkan bahwa persentase LP pada lansia wanita yang berisiko sebesar 90,6%, RLPP yang berisiko sebesar 98,1%, dan kadar hs-CRP tinggi sebesar 30,2%. Terdapat hubungan positif antara LP dengan kadar serum hs-CRP (r=0,417 ; p=0,002). Dalam penelitian ini RLPP, aktivitas fisik, asupan energi, karbohidrat, protein, lemak, serat, vitamin A, vitamin C, vitamin D,vitamin E, dan selenium tidak berkorelasi dengan kadar hs-CRP. Simpulan penelitian ini adalah LP berkorelasi positif dengan kadar serum hs-CRP, namun RLPP tidak berkorelasi dengan kadar serum hs-CRP</p>
