El propósito de este estudio fue describir la asociación entre el estado nutricional, la presencia de síndrome metabólico (SM), y el estado inflamatorio, en pacientes con Enfermedad de Chagas (ECh), atendidos en la consulta externa del Instituto de Medicina Tropical en Caracas. El estudio fue de tipo transversal y correlacional, en el cual se seleccionaron 34 pacientes a los cuales se les realizó un diagnóstico parasitológico, inmunológico y molecular de la ECh. Se evaluaron variables antropométricas, clínicas y bioquímicas, así como el SM el cual fue determinado por los criterios del III Panel estadounidense para el Tratamiento de Adultos del Programa Nacional de Educación sobre el Colesterol (ATP-III, por sus siglas en inglés). Se encontró que la mayoría de los pacientes presentaron sobrepeso u obesidad (73,5%), un porcentaje de grasa corporal (% GC) alto o muy alto (82,3%), y obesidad abdominal (61,8 %). La frecuencia de SM fue de 29,4% y más del 90% mostraron valores elevados de Proteína C Reactiva ultrasensible (PCRus). Valores más elevados del IMC se asociaron con un estadio más avanzado de la ECh. Los sujetos con presencia de ADN de Tripanosoma cruzi (T. cruzi) circulante en sangre, presentaron mayor % GC, y en su mayoría, fueron diagnosticados con SM. En conclusión, los pacientes evaluados mostraron un exceso de adiposidad, que puede favorecer el estado inflamatorio, el desarrollo de SM y la progresión de la ECh. The purpose of the study was to describe the association between nutritional and inflammatory status and the presence of metabolic syndrome (MS) on patients with Chagas disease (CD) treated at the Outpatient Services of the Tropical Medicine Institute in Caracas, Venezuela. The study was cross-sectional and correlational. Thirty-four (34) patients were selected and a molecular, immunological, and parasitological diagnostic test was ran for Chagas disease. Anthropometric, clinic, and biochemical variables were evaluated, and the MS was determined using National Cholesterol Education Program Expert/Adult Treatment Panel III (ATP-III) criteria. The results showed a high percentage of patients overweight or presenting obesity (73.5%), a high and very high percentage of body fat (82.3%), and abdominal obesity (61.8%). The prevalence of MS was 29,4% and more than 90% of patients showed elevated values of high sensitivity C-reactive protein (hsCRP). Higher body-mass index values were associated with advanced stages of the CD. Subjects in the presence of T. cruzi DNA in the blood showed a greater percentage of body fat and, most of them, were diagnosed with MS. In conclusion, the evaluated patients showed an excess of adiposity which may favor an inflammatory status, the development of the MS, and the progress of the CD.

RESUMOObjetivo: identificar o Nível de Atividade Física e Percentual de Gordura Corporal de escolares. Método: estudo quantitativo, transversal, por amostragem composta de 640 escolares (Idade: 10±1 anos; Massa Corporal, 37,25±13,90kg; Estatura, 1,45±0,08m; IMC, 17,50±5,04kg/m²). O NAF por meio PAQ-C, %GC pelo método de dobras cutâneas e o IMC. Resultados: há diferenças significativas (p < 0,01, d = 0,37) no escore do PAQ-C entre os sexos, com superioridade nos valores masculinos; foram encontradas diferenças significativas na distribuição dos níveis de atividade física divididos pelo sexo (p < 0,0001, d = 6,7) e na distribuição do %CG pelo sexo (p < 0,0001, d=0,28). Conclusão: foram identificados índices de obesidade dentre os escolares (43% e 53,10%, populações feminina e masculina, respectivamente) ao mesmo tempo em que essa população apresenta que é ativa ou moderadamente ativa fisicamente (67,07% e 67,56%, masculino e feminino, respectivamente); contudo, não foi identificada associação entre Nível de Atividade Física e Percentual de Gordura Corporal. Estes resultados corroboram os achados gerais da literatura que demonstram que o NAF não é fator determinante para o valor Percentual de Gordura Corporal. Conclusion: obesity rates among schoolchildren (43% and 53.10%, female and male, respectively) were identified at the same time as this population shows that they are active or moderately physically active (67.07% and 67.56 %, male and female, respectively); however, no association was identified between Physical Activity Level and Body Fat Percentage. These results corroborate the general findings of the literature that demonstrate that PAL is not a determinant factor for the Percentage of Body Fat. Aim: To explore the association of circulating miRNAs with adiposity, metabolic status and inflammatory biomarkers in patients with metabolic syndrome (MetS). Patients & methods: Serum levels of 372 miRNAs were measured in patients with (n = 6) and without MetS (n = 6) by quantitative PCR array, and dysregulated miRNAs were validated in a larger cohort (MetS, n = 89; non-MetS, n = 144). Results: In the screening study, seven miRNAs were dysregulated in patients with MetS, and miR-421 remained increased in the validation study. miR-421 was associated with a high risk of MetS and insulin resistance and hypertension and correlated with glycated hemoglobin, triacylglycerols, high-sensitivity C-reactive protein, IL-6, resistin and adiponectin (p < 0.05). Conclusion: Circulating miR-421 is a potential biomarker for insulin resistance, metabolic dysregulation and inflammatory status in patients with MetS.

Background: Among older adults, pain intensity and pain interference are more common in women than men and associated with obesity and inflammatory markers. Objective: We examined whether the obesity and pain relationship is mediated by the high-sensitivity C-reactive protein (hsCRP), a nonspecific marker of systemic inflammation, and whether this relationship differs by sex. Methods: Items from Medical Outcomes Study Short Form-36 were used to measure pain intensity and pain interference in daily life. Ordinal logistic regression was used to assess the cross-sectional association among body mass index (BMI), hsCRP levels, pain intensity and pain interference using gender-stratified models adjusted for demographic variables. Results: Participants included 667 community-residing adults over the age of 70 years, free of dementia, enrolled in the Einstein Aging Study (EAS). In women (n = 410), pain intensity was associated with obesity [BMI ≥30 vs. normal, odds ratio (OR) = 2.29, 95% confidence interval (CI) 1.43-3.68] and higher hsCRP (OR = 1.28, 95% CI 1.08-1.51). In a model with obesity and hsCRP, both remained significant, but the association between hsCRP and pain intensity was somewhat attenuated. Obesity (OR = 3.04, 95% CI 1.81-5.11) and higher hsCRP levels (OR = 1.30, 95% CI 1.08-1.56) were also independently associated with greater pain interference in women. After adjustment for pain intensity and BMI, hsCRP was no longer associated with pain interference in women. Greater pain intensity and being overweight or obese continued to be significantly associated with pain interference in women. In men (n = 257), obesity and hsCRP were not associated with pain intensity or pain interference. Conclusions: In women, the relationship between obesity and higher levels of pain intensity or interference may be accounted for by factors related to hsCRP.

High sensitivity C-reactive protein (hsCRP) is a marker of metabolic syndrome (MS) and cardiovascular (CV) disease. Lipoprotein-associated phospholipase A2 (Lp-PLA2) also predicts CV disease. There are no reports comparing these markers as predictors of MS.Methods. Cross-sectional study comparing Lp-PLA2 and hsCRP as predictors of MS in asymptomatic subjects was carried out; 152 subjects without known atherosclerosis participated. Data were collected on demographics, cardiovascular risk factors, anthropometric and biochemical measurements, and hsCRP and Lp-PLA2 activity levels. A logistic regression analysis was performed with each biomarker and receiver operating characteristic (ROC) curves were constructed for MS.Results. Mean age was 46 ± 11 years, and 38% of the subjects had MS. Mean Lp-PLA2 activity was 185 ± 48 nmol/mL/min, and mean hsCRP was 2.1 ± 2.2 mg/L. Subjects with MS had significantly higher levels of Lp-PLA2 (P=0.03) and hsCRP (P<0.0001) than those without MS. ROC curves showed that both markers predicted MS.Conclusion. Lp-PLA2 and hsCRP are elevated in subjects with MS. Both biomarkers were independent and significant predictors for MS, emphasizing the role of inflammation in MS. Further research is necessary to determine if inflammation predicts a higher risk for CV events in MS subjects.

Objective: To compare the level of high sensitivity C-Reactive Protein (hs-CRP) on patients with premature rupture of membranes (PROM) at term pregnancy. Methods: The study was cross-sectional design. The data collection included age, parity, gestational age, birth weight, and APGAR score on 28 pregnant women at term pregnancy consisting of 14 subjects of Premature Rupture of Membrane (PROM) less than 12 hours and the remaining one was equal or more than 12 hours. We took blood samples from the subjects for the examination of hs-CRP level. Data were analyzed using non-parametric statistical test and processed using Statistical Package for Social Sciences (SPSS) version 20. Results: The mean hs-CRP level on PROM <12-hour and 12-hour group was 12.9 and 17.8 mg/l. There was not significant difference on hs-CRP level between both of group at term pregnancy (p=0.734). Conclusion: The level of hs-CRP does not have association with the incidence of PROM. IntroductionInflammatory status could play a role in alterations of blood pressure (BP) circadian rhythm. The aim of our study is to compare levels of usual inflammatory markers in patients with and without circadian BP abnormalities.Material and methodsThis is a cross-sectional design study with retrospective data analysis which included patients from an Internal Medicine Department with normal and high BP levels older than 18 years who were separated into two groups according to the circadian profile of BP (dipper and non-dipper) based on the results of 24-hour ambulatory BP monitoring. Patients were assessed for demographic characteristics and cardiovascular risk factors. We considered as inflammatory markers the platelet count (PTC), erythrocyte sedimentation rate (ESR), ultra-sensitive C-reactive protein, ferritin, fibrinogen, and uric acid.ResultsThe study included 551 patients (mean age of 54 years, 47% women). The non-dipper group had a higher percentage of individuals with higher ESR (OR = 1.77, 95% CI: 1.23–1.55, p = 0.001), uric acid (OR = 1.50, 95% CI: 1.04–2.16, p = 0.028) and fibrinogen (OR = 1.72, 95% CI: 1.18–2.51, p = 0.001) and a higher percentage of patients with higher PTC (OR = 0.54, 95% CI: 0.37–0.78, p = 0.005). These results were independent of age, waist circumference, presence of arterial hypertension, diabetes or hyperlipidemia, and use of antihypertensive drugs including renin angiotensin aldosterone system blockers.ConclusionsPatients with impaired circadian BP rhythm were associated with an unfavorable inflammatory status independently of BP levels. This fact could play a role in the prognostic differences observed between dipper and non-dipper patients.

<p>Proses penuaan mempengaruhi peningkatan distribusi lemak abdominal dengan indikator lingkar pinggang (LP) dan rasio lingkar pinggang panggul (RLPP). Penimbunan lemak abdominal menyebabkan disfungsi jaringan adiposa sehingga mempengaruhi biomarker proinflamasi yaitu kadar serum <em>high-sensitivity C-reactive Protein</em> (hs-CRP). Penelitian bertujuan untuk mengetahui hubungan LP dan RLPP dengan kadar hs-CRP lansia wanita. Rancangan penelitian <em>cross sectional</em> pada 53 subjek dipilih secara <em>consecutive sampling</em>. Antropometri yang diukur adalah LP dan RLPP. Pengukuran kadar serum hs-CRP dianalisis dengan metode <em>enyme-linked immunosorbent assay</em> (ELISA). Wawancara yang dilakukan yaitu data diri, asupan, aktivitas fisik, dan riwayat konsumsi obat. Data asupan diperoleh dengan metode <em>food recall</em> 3x24 jam. Aktivitas fisik diperoleh menggunakan <em>International Physical Activity Questionnaire</em> (IPAQ). Analisis data digunakan uji korelasi <em>Spearman</em>. Hasil penelitian menunjukkan bahwa persentase LP pada lansia wanita yang berisiko sebesar 90,6%, RLPP yang berisiko sebesar 98,1%, dan kadar hs-CRP tinggi sebesar 30,2%. Terdapat hubungan positif antara LP dengan kadar serum hs-CRP (r=0,417 ; p=0,002). Dalam penelitian ini RLPP, aktivitas fisik, asupan energi, karbohidrat, protein, lemak, serat, vitamin A, vitamin C, vitamin D,vitamin E, dan selenium tidak berkorelasi dengan kadar hs-CRP. Simpulan penelitian ini adalah LP berkorelasi positif dengan kadar serum hs-CRP, namun RLPP tidak berkorelasi dengan kadar serum hs-CRP</p>
