It is estimated that in the year 2020, approximately 25% of the Japanese population will be over 65 years of age. Moreover, suicide is a significant public health problem in Japan, where more than 6,000 elders take their lives each year. The authors compare late-life suicide in urban Kawasaki with suicide among the elderly in rural Higashikubiki over a 12-year period, from 1979 through 1990. The suicide rates in Kawasaki were lower than for Japanese elders as a whole, whereas those in rural Higashikubiki were extraordinarily high. The most frequent method used in both areas was hanging, and none of the victims died of gunshot wounds. In Higashikubiki, almost two thirds of victims lived in a three-generation family and none lived alone. The change of the family system from the traditional extended family to the nuclear family is accelerating, especially in rural areas. The authors suggest that the greatly elevated suicide rates among the elderly in Higashikubiki, and in rural regions of Japan more generally, result from these rapidly occurring changes in traditional social structure.

There has been a rapid ageing of the earth's population and in a few decades, Asia could become the oldest region in the world. In India, due to the reorganization of the family system, the traditional joint family system is on the decline. Due to the emergence of the nuclear family and the high cost of living, family members who previously cared for the elderly need to find employment outside the home. A rapid increase in nuclear families and an exceptional increase in the number of ‘older adults’ in the country have compelled them to live in old age homes. Literature has accentuated the difficulties and apprehensions experienced by older adults during the ageing process and the need for old age homes in order to create an environment that fosters a meaningful existence for them in their twilight years. India's old age homes are trying to uphold the needs, desires and values of older adults. There is a lack of studies that attempt to give older adults a chance to communicate their experiences in a care home. Older adults are an invaluable resource for younger generations and change is needed in society's attitude towards ageing. This review can help psychologists, social workers and caregivers gain insight into the needs of older adults in terms of mental wellbeing, economic and social security and elder abuse and create awareness among the people. Int. A paradigm shift from the joint family system to nuclear family, a fast changes in lifestyles, changing and fluctuating social values and the pressure of working conditions create impediment for the younger generation at present in loving and caring their elderly parents. The age-old traditional institutions in the country are not successful and sometimes failing in accommodating the needs of their elderly, it is the duty of the state to evolve good policies and design necessary programmes for the welfare of the geriatric population to keep the elderly alive and safeguard them with human dignity. It is a constitutional obligation. The present paper analyses in detail about the various policies and programmes of the elderly and how they are implemented from time to time of the various states in India. The findings show clearly about the large scale inadequacy of political economy of elderly welfare in India. The states could not do anything concretely to cater to the needs of the growing geriatric population in India as they are suffering from insufficient availability of resources. If this is the state of elders in India for more than two decades, how the poorer sections of the geriatric population in the society will have dignified living and respect. It reflects clearly at every level on the part of the state, community and family for seeing/finding the lack of genuine commitment in caring the elders and for their welfare in India. The younger generations of today have sidelined to capture and also to possess the wisdom and experiences of the elderly. Instead they are going after the materialistic values. Therefore, they do not give importance in meeting out the needs of their own elders’ care. This has resulted in utterly pushing the geriatric population to the state of helplessness.

The predominant type of living arrangement in pre-independence India was joint family system. It was based on the notion of collectivism and charity. It was a serving place for not only nurturing and preserving social values, but also passing them down through generations. After the advent of industrialization and subsequent emergence of urbanization and modernization, the joint families started disintegrating into nuclear families. The modern nuclear family is perpetually promoting the principle of individualism or independence in contrast to the value of collectivism deep-rooted in joint family. It is, therefore, a value shift in family from collectivism to individualism. It is followed by a loyalty shift in family from lineal ties to conjugal ties. Consequently, nuclear family has become an indispensible social unit in contemporary Indian society. However, its sustainability may perhaps be uncertain in a distant future because of the inclination of present younger generation to complete independence in life. Probably this may result in the emergent of a new type of living arrangement to satisfy the needs of generations of people in a remote future.

Ageing is a natural process which is inevitable. Everyone who is born in this world will eventually age with time. There is a rapid change in the demographic scenario and the growth rate of the older population is increasing gradually in India over the past few decades and it will continue to do so. This can be due to improvement in the field of the medical sector and various other factors as well. As a result, the aged are likely to suffer from problems related to health and health care, family composition, so on and so forth. In India, traditionally the concept of ‘Joint family’ was practised but with the advent of urbanization, modernization, industrialization, and globalization, there have been major structural and functional transformations in the family. The joint family system is disappearing in the course of time as the people now do not have time for their family because of their work as they have to stay away from their family in different places. People have started believing in ‘Nuclear family’ system because the standard and cost of living have gone high and working class people are not being able to look after their large family. This has led to the elderly people shift to the old age homes either forcefully or sometimes by their own choice (which is very rare). The elderly people suffer a lot emotionally. They go through a traumatic phase. At this stage of life, elderly people need a lot of care, love, support, and affection from their family. Deprivation from all these leads to depression, anxiety and sleep disorder and various other health problems like diabetes, hyper-tension, and high/low pressure, etc. including mental problems among the elderly people. There is an immense need for proper human rights and health policies for the well-being and upliftment of the health of older people.
