The aim of the study was to examine the long-term trends in pedestrian mortality for children (aged 0 to 14 years) and young people (aged 15 to 19 years) over four decades in transitional Lithuania. Methods. Road traffic fatality data were obtained from Statistics Lithuania and the Archives of Health Information Centre. Trends were analysed by linear regression using “Independence” as a slopechanging intervention in 1991 and population as a further explanatory factor in structural time series models. Results. The impact of the interventions, along with the reforms and changes related with the Independence, on pedestrian fatality trends in our time series model was found highly statistically significant for children 0 to 14 years (p<0.001) and still significant for young people 15 to 19 years (p<0.05). No significant impact on the trend of road traffic deaths was found for the “control-groups” of non-pedestrian road users in the age group 0 to 14 years and adult pedestrians (over 19 years of age). For the age group 15 to 19 years the effect of reforms was also significant for non-pedestrians (p<0.05). These results indicate that the effect of measures and changes used in the post-independence period was more specific in children that participated in road traffic as pedestrians than in adult pedestrians, or in nonpedestrian road users. Conclusions. Pedestrian deaths in Lithuania fell significantly in the age groups 0-14 and 15-19 years. A declining trend was found in road traffic fatalities and in pedestrian deaths in transitional Lithuania in the post-independence period. Socioeconomic and political transformations, systematic reforms in healthcare along with sustainable preventive measures may have contributed to this decrease. Targeted road safety measures were road traffic regulations, pedestrian education and environmentally based prevention measures. As child pedestrians are the most vulnerable group of road users, continued road safety education and promotion are recommended in order to maintain this trend, and to involve adult pedestrians in this development.

The purpose of this study was to measure the impacts of airport road access design and wayfinding systems on senior driver performances. A car driving simulator was used to model scenarios of differing wayfinding complexity and road design. Three scenario types were designed consisting of 3.8 miles of airport road (i.e. approximately 4 minutes driving to complete each scenario). Experienced car drivers were asked to drive simulated routes. Forty drivers in the age ranges: 50 to 54, 55 to 59 and those aged over 60 were selected to perform the study. Participants drove for approximately 20 minutes to complete the simulated driving. The driver performance was compared between age groups. Results were analysed by Mean, Standard Deviation and ANOVA Test, and discussed with reference to the use of the driving simulator. The ANOVA results showed that in comparison of senior drivers’ age group, there is a low impact between driving behaviour and road safety on airport road access wayfinding design.

AbstractThe steady increase in the number of road users and their growing mobility mean that the issue of road safety is still a topical one. Analyses of factors influencing the number of road traffic accidents contribute to the improvement of road safety. Because changes in traffic volume follow a daily rhythm, hour of the day is an important factor affecting the number of crashes. The present article identifies selected mathematical models which can be used to describe the number of road traffic accidents as a function of the time of their occurrence during the day. The study of the seasonality of the number of accidents in particular hours was assessed. The distributions of the number of accidents in each hour were compared using the Kruskal-Wallis and Kolmogorov-Smirnov tests. Multidimensional scaling was used to present the found similarities and differences. Similar hours were grouped into clusters, which were used in further analysis to construct the ARMAXmodel and the Holt-Winters model. Finally, the predictive capabilities of each model were assessed.

Arriaga’s method of life expectancy decomposition was applied to produce estimates of the contribution of different age groups and underlying causes to changes in life expectancy at birth for the two periods.ResultsAnnualised gains in life expectancy between 2012–2014 and 2015–2017 were markedly smaller than in the earlier period. Almost all age groups saw worsening mortality trends, which deteriorated for most cause of death groups between 2012–2014 and 2015–2017. In particular, the previously observed substantial life expectancy gains due to reductions in mortality from circulatory causes, which most benefited those aged 55–84 years, more than halved. Mortality rates for those aged 30–54 years and 90+ years worsened, due in large part to increases in drug-related deaths, and dementia and Alzheimer’s disease, respectively.ConclusionFuture research should seek to explain the changes in mortality trends for all age groups and causes. More investigation is required to establish to what extent shortcomings in the social security system and public services may be contributing to the adverse trends and preventing mitigation of the impact of other contributing factors, such as influenza outbreaks.

Background. The article is devoted to the problem of traffic accidents that occurred against the background of alcohol intoxication. With the development of technology and science over the past decade, the number of victims with severe multisystem damage has increased worldwide. The most common causes of injuries are traffic accidents. According to the WHO, road traffic injuries are one of the leading causes of death among people of working age. The UN Decade of Road Safety (2011–2020) has indicated that road safety is the most painful and difficult issue to address. The purpose of the study was to examine the impact of alcohol intoxication on the circumstances of injuries based on the participation in traffic in victims with multisystem injuries due to traffic accidents. Materials and methods. A scientific retrospective analysis of 312 cases of multisystem injuries in road accident victims who were treated in the Polytrauma Department of the Kyiv City Clinical Ambulance Hospital in 2018–2019 years. Results. The injuries due to road accidents occur in 46 % of people who are intoxicated. There is a probable dependence of road accident victims on the signs of alcohol intoxication and participation in the traffic. In a state of alcohol intoxication, drivers are injured in 46.43 % of cases, pedestrians in 52.53 %, passengers in 31.43 %. The comparative analysis of the risk of death in victims of accidents with and without alcohol intoxication and based on the participation in traffic proved that the risk of death of drivers while intoxicated is more than 2-fold higher than in drivers without alcohol intoxication and is 0.49. Conclusions. Pedestrians have the greatest clinically effective risks of fatalities in the traumatic process, regardless of the presence or absence of alcohol intoxication, such a risk is critical or catastrophic. Alcohol intoxication increases the risk of a fatal traumatic process in active road users (drivers and pedestrians) and does not impact this result in passive road users (passengers)

Abstract Background Seasonality in tuberculosis (TB) has been found in different parts of the world, showing a peak in spring/summer and a trough in autumn/winter. The evidence is less clear which factors drive seasonality. It was our aim to identify and evaluate seasonality in the notifications of TB in Germany, additionally investigating the possible variance of seasonality by disease site, sex and age group. Methods We conducted an integer-valued time series analysis using national surveillance data. We analysed the reported monthly numbers of started treatments between 2004 and 2014 for all notified TB cases and stratified by disease site, sex and age group. Results We detected seasonality in the extra-pulmonary TB cases (N = 11,219), with peaks in late spring/summer and troughs in fall/winter. For all TB notifications together (N = 51,090) and for pulmonary TB only (N = 39,714) we did not find a distinct seasonality. Additional stratified analyses did not reveal any clear differences between age groups, the sexes, or between active and passive case finding. Conclusion We found seasonality in extra-pulmonary TB only, indicating that seasonality of disease onset might be specific to the disease site. This could point towards differences in disease progression between the different clinical disease manifestations. Sex appears not to be an important driver of seasonality, whereas the role of age remains unclear as this could not be sufficiently investigated.

Background: In 2014, a recommended one-dose of inactivated hepatitis A vaccine was included in the Brazilian National Immunization Program targeting children 12–24 months. This decision addressed the low to intermediate endemicity status of hepatitis A across Brazil and the high rate of infection in children and adolescents between 5 and 19 years old. The aim of the study was to conduct a time-series analysis on hepatitis A incidence across age groups and to assess the hepatitis A distribution throughout Brazilian geographic regions. Methods: An interrupted time-series analysis was performed to assess hepatitis A incidence rates before (2010–2013) and after (2015–2018) hepatitis A vaccine program implementation. The time-series analysis was stratified by age groups while a secondary analysis examined geographic distribution of hepatitis A cases. Results: Overall incidence of hepatitis A decreased from 3.19/100.000 in the pre-vaccine period to 0.87/100.000 (p = 0.022) post-vaccine introduction. Incidence rate reduction was higher among children aged 1-4 years old, with an annual reduction of 67.6% in the post-vaccination period against a 7.7% annual reduction in the pre-vaccination period (p < 0.001). Between 2015 and 2018, the vaccination program prevented 14,468 hepatitis A cases. Conclusion: Our study highlighted the positive impact of a recommended one-dose inactivated hepatitis A vaccine for 1–4-years-old in controlling hepatitis A at national level.

Somach cancer is the third most common cause of cancer-related deaths worldwide. The objective of the paper was to analyze the incidence and mortality trends of stomach cancer in Central Serbia in the period between 1999–2017. Materials and Methods: trends and annual percentage change (APC) of the incidence and mortality rate with corresponding 95% confidence intervals (CI) were calculated by joinpoint regression analyses. The optimal number of Joinpoints was identified using the Monte Carlo permutation method. The trend was considered to be significantly increasing (positive change) or decreasing (negative change) when the p-value was below 0.05 (p < 0.05). Results: the total number of new cases was 16,914 (10,873 males and 6041 females) and the total number of mortality cases was 14,790 (9348 in and 5442 in females). Almost one third (30.8%) of new cases were registered in the 60–69-year age group, and new cases were significantly more frequent in males than in females (30.8% vs. 29.02%, p < 0.001). Joinpoint regression analysis showed a significant decrease of incidence trend in females during the 2000–2015 period with APC of −2.13% (95% CI: −3.8 to −0.5, p < 0.001). An insignificant decrease in incidence trend was in males with APC of −0.72% (95% CI: −2.3 to 0.9, p = 0.30). According to the joinpoint analysis, a significant decrease of mortality trends both in males during 2000–2015 with APC of −2.21% (95% CI: −1.6 to −7.5, p ≤ 0.001 and in females, during the same period, with APC of −1.75% (95% CI: −2.9 to −0.6, p < 0.001) was registered. From 2015 to 2017, a significant increase of mortality was registered with APC of 44.5% (95% CI: from 24.2 to −68.1, p ≤ 0.001) in females and in males with APC of 53.15% (95% CI: 13.5 to −106.6, p ≤ 0.001). Conclusion: a significant decrease of stomach cancer incidence trend in females and insignificant decrease of incidence trend in males were determined in Central Serbia. Based on presented results, the mortality trend decreased significantly both in males and in females during 2000–2015, and from 2015 to 2017 we recorded a significant increase in mortality in both sexes. We found significantly more new cases in women than in men in the age group of 40–49, and the mortality of stomach cancer was significantly more frequent among females compared to males in the age groups 30–39, as well as in the 50–59 age group. There is a need for improving recording and registration of new cases of stomach cancer, especially in females. Urgent primary and secondary preventive measures are needed—introducing stomach cancer screening and early detection of premalignant changes. The Safe System (SS) approach to road safety emphasizes safety-by-design through ensuring safe vehicles, road networks, and road users. With a strong motivation from the World Health Organization (WHO), this approach is increasingly adopted worldwide. Considerations in SS, however, are made for the medium-to-long term. Our interest in this work is to complement the approach with a short-to-medium term dynamic assessment of road safety. Toward this end, we introduce a novel, cost-effective Internet of Things (IoT) architecture that facilitates the realization of a robust and dynamic computational core in assessing the safety of a road network and its elements. In doing so, we introduce a new, meaningful, and scalable metric for assessing road safety. We also showcase the use of machine learning in the design of the metric computation core through a novel application of Hidden Markov Models (HMMs). Finally, the impact of the proposed architecture is demonstrated through an application to safety-based route planning.

Every year we lose many young road users in road traffic accidents. Based on traffic accident data issued by the Indonesian National Police in 2017, the number of casualties was highest in the age group 15-19, with 3,496 minor injuries, 400 seriously injured and 535 deaths. This condition is very alarming considering that student as the nation's next generation lose their future due to the accidents. This figure does not include other traffic violations, not having a driver license, not wearing a helmet, driving opposite the direction, those given ticket and verbal reprimand. To reduce traffic accident for young road user, road safety campaigns were organized in many schools in Jakarta. This activity aims to socialize the road safety program to increase road safety awareness among young road users/students including the dissemination of Law No. 22 of 2009 concerning Road Traffic and Transportation. Another purpose of this program is to accompany school administrators to set up a School Safe Zone (ZoSS), a location on particular roads in the school environment that are time-based speed zone to set the speed of the vehicle. The purpose of this paper is to promote the road safety campaigns strategies by considering various campaign tools.
