Nowadays many methods are offered to reduce pain in labor, both pharmacological (using drugs) and non pharmacological methods. If possible the choice of non-pharmacologic therapy for the management of pain in pregnancy and labor should be considered before using analgesic drugs. One of an effective non-pharmacological method to reduce pain is with massage. The basis of this theory is the gate control theory proposed by Melzak and Wall (Lliadou, 2009).This study aimed to analyze diference of pain relief on active phase of labour with Counter Pressure and abdominal Lifting. This was an experimental with pre test and post test design. The samples were 42 women on their active phase of labor, 21 women for counter Pressure technic and 21 women for Abdominal Lifting technic. Counter Pressure technic decreased pain of labor with a P-value 0.015 (p<0.05) and abdominal lifting technic significantly decreased pain of labour with a P-value 0.001. Abdominal lifting technic (P 0,001) more effective than counter pressure technic (P 0,015) for pain relief in active phase of labour.

Purpose: Conflicting results exist regarding the efficacy of N-acetyl cysteine (NAC) in sepsis treatment. A pivotal factor affecting the therapeutic potency of NAC in sepsis is timing and dosing of its infusion. We aimed to assess the effect of NAC on redox status of patients with sepsis and to compare its efficacy in intermittent and continuous infusion with the objective of developing the infusion regimen and optimizing the timing. Materials and Methods: A prospective, randomized clinical trial was designed to compare the antioxidative effect of NAC in intermittent infusion group (IV: 25 mg/kg bolus and then 25 mg/kg/8 hours 3 times) and continuous infusion group (IV: 25 mg/kg bolus and then 75 mg/kg over 24 hours) in 60 critically ill patients with sepsis (20 patients in each group). Blood samples were collected immediately before and after intervention for total antioxidant capacity (TAC) and malondialdehyde (MDA) assessment. Results: N-acetyl cysteine considerably increased TAC levels in both intermittent (0.68 ± 0.60; P value = .036) and continuous (0.69 ± 0.64; P value = .015) infusion groups when compared to placebo (0.61 ± 0.10); however, the difference in TAC levels between the intermittent and the continuous infusion did not reach statistical significance ( P value = .942). Likewise, NAC treatment decreased MDA levels in both intermittent (19.45 ± 4.18; P value = 0.001) and continuous (22.47 ± 6.68; P value = .002) infusion groups when compared to placebo (31.76 ± 11.06), while the difference in MDA levels between the intermittent and the continuous infusion did not reach statistical significance ( P value = .481). Conclusion: Our data confirmed the antioxidative effect of NAC treatment in patients with sepsis, with no significant difference in intermittent and continuous infusion.

AbstractThis article determines compartment opening of the medial articular space of the knee after pie crust (PC) technique of the medial collateral ligament (MCL) by ultrasound measurements and anatomic dissection. This is a cadaveric study of 12 specimens. Four anatomic references were marked on the skin. Distances between the femur and tibia in the internal compartment at 30 degrees of flexion were obtained with ultrasound measurements in four situations: with and without applying valgus force both prior and after the PC technique. Ultrasound measurements of the medial articular compartment were made twice and mean value was calculated. An anatomical dissection was performed and distances between the puncture marks and the infrapatellar branch of the saphenous nerve was measured. Lilliefors test of normality was applied and variables were expressed as mean and standard deviation (SD). Qualitative variables were expressed by absolute frequencies and percentages. Statistical significance was a two-tailed p-value of < 0.05. Prior to the PC technique, mean (SD) distance between the femur and tibia in the medial compartment were 14.2 (4.0) mm in basal conditions and 17.1 (3.7) mm when applying valgus force (p = 0.003). PC technique increased the mean (SD) distance by 1.9 (1.9) mm under basal conditions (p < 0.01) and 2.9 (1.6) mm when applying valgus force (p < 0.01). The infrapatellar branches of the saphenous nerve were not damaged and the mean (SD) distance between the punctures and the nerve was 9.0 (3.3) mm. The PC is a reproducible, safe, and measurable surgical technique that opens controllably the medial compartment. PC as described avoided damage to the nerve branches.

The activities of children more involve the foot. One of the common problems in the foot is flexible flatfoot. A disturbance in the process of the formation of the arch foot could result in a deformation of the foot and increases the risk of an injury due to postural balance change. Normally, the arch of the foot formed the first five years for the age range of 2-6 years. The right choices of the intrinsic muscle exercises of the foot can prevent deformation and improve postural balance. This study aimed to discuss the effectiveness of foot strengthening exercise to improving postural balance and functional ability of foot on a flexible flatfoot 6-10 years old. This study was pre-experimental research with two groups of pre-post test design. The subjects of this research were 30 students that had been divided into two groups, case, and control. Each group consists of 15 students selected used purposive sampling method based on the criteria of inclusion that had been set. There was a significant difference after foot strengthening exercise between the case and control group, p-value = 0,000. The foot strengthening exercise could improve the postural balance and functional ability of the foot on a flexible flatfoot 6-10 years old.
