AbstractThe seawater fishery in Dusun Sine has the higher potentials, due to its location near the coastal area of Sine Beach. In general, the people in Dusun Sine make a living as sea fishermen. However, the income of the marine fishing community has not maximized because the community still uses the conventional marine product processing production system, so that the marketing of processed marine fish products is also limited. As an effort to increase the selling value of processed marine fish, the International Community Service Group empowers fishermen's wives in Sine Hamlet by conducting several activity programs in the form of socializing the manufacture of modern marine processed products in the form of Sine Foodies, home industry food (PIRT), and the use of e-commerce. This activity was held to use a health protocol and through a presentation method on product marketing licensing in the form of PIRT socialization and marketing of processed marine fish products with e-commerce as well as demonstrations on the process of making processed marine fish products in the form of shredded fish and fish balls using attractive packaging. Hal ini disebabkan letaknya yang berada di dekat wilayah pesisir Pantai Sine. Secara umum, masyarakat di Dusun Sine bermata pencaharian sebagai nelayan laut. Namun, pendapatan masyarakat nelayan laut belum maksimal karena masyarakat masih menggunakan sistem produksi pengolahan hasil laut secara konvensional, sehingga pemasaran produk hasil olahan ikan laut juga terbatas dengan seadanya. Sebagai upaya untuk meningkatkan nilai jual hasil olahan ikan laut, Tim Pengabdian masyarakat Departemen Kimia, Fakultas Sains dan Teknologi, Universitas Airlangga dengan didukung penuh oleh Kelompok mahasiswa KKN Internasional, FST, UNAIR melakukan upaya pemberdayaan para istri nelayan di Dusun Sine dengan melakukan beberapa program kegiatan berupa sosialisasi pembuatan hasil olahan laut secara modern berupa Sine Foodies, pangan industri rumah tangga (PIRT), serta penggunaan e-commerce. Kegiatan ini diselenggarakan menggunakan protokol kesehatan dan melalui metode presentasi mengenai perijinan pemasaran produk berupa sosialisasi PIRT dan pemasaran produk hasil olahan ikan laut dengan e-commerceserta demonstrasi mengenai proses pembuatan produk hasil olahan ikan laut yang berupa abon ikan dan bakso ikan dengan menggunakan kemasan yang menarik. Objective We described our initial experience of a new integrated oncology phamaceutical care practice to enhance the quality of pharmacy service and patient care in Huashan hospital. Data sources: A retrospective study was performed from August 2019 to September 2020. Patients were described as integrated pharmacy service group and routine care group. Medication adherence of patients in integrated pharmacy service group was recorded by the online management system. Patient satisfaction and the cumulative incidence of emergency room (ER) and outpatient visit were evaluated between two groups. Data summary: In total, 323 patients received the integrating oncology pharmacy service. The percentage of the patients missing administration every day was reduced from 29.7% to 0.3% within a 40-day monitoring and intervention period. There was a significant difference on patient satisfaction with pharmacy service in two groups ( P < 0.05). Fewer patients in the integrated pharmacy service group visited clinic and ER compared with routine care group (33.1% vs. 59.2%; P < 0.05). Conclusions As a new practice model, the integrated program is adopted to provide patient care and ongoing monitoring for cancer patients. The practice model delivers high continuity of care for cancer patients and improves communication and collaboration between healthcare professionals and oncology patients. The practice also provides the potential of developing hospital pharmaceutical service and optimizing disease prevention and treatment strategies.

Aim: Initial chest CT findings of patients were compared by grouping them according to the clinical outcome of the infection and those which could predict clinical outcome, prognosis and mortality were investigated. Background: Published studies on chest CT in COVID-19 infection do not go beyond describing the characteristics of the current period. Nevertheless, comparative analysis of chest CT findings on hospital admission among patients in different clinical outcomes is scarce. Objective: 198 consecutive symptomatic patients with COVID-19 infection confirmed by positive polymerase chain reaction (PCR) and who had undergone chest CT were enrolled in this retrospective study. Method: According to their clinical outcomes, we divided them (n:98) into 3 groups. Group 1 (n: 62) involved patients discharged from the service, group 2 (n: 60) included patients hospitalized in the intensive care unit, and group 3 (n: 76) comprised patients who died despite any treatment. Results: Clinical characteristics involving age, dyspnea, hypertension, and chest CT findings of mediastinal lymphadenopathy, pleural effusion, and pericardial effusion, were determined as poor prognosis and mortality predictors, and halo sign in chest CT finding was a good prognosis predictor in multivariate analysis. Conclusion: It was seen that some CT findings were significantly correlated to the patients' endpoints, such as discharge, hospitalization in the intensive care unit, and as a worst consequence, death. These findings support the role of CT imaging for potentially predicting the clinical outcomes of these patients with COVID-19.

Hospitals are establishing procedure services to address resident training and patient safety. We examined whether a hospitalist procedure service affects a patient’s hospital length of stay (LOS) and the time from admission to paracentesis (A2P). We queried our electronic medical records for all inpatient peritoneal fluid samples from July 1, 2016, to May 31, 2019. LOS and A2P time were compared among patients who had paracentesis by the procedure service, by residents, or by radiology. Of the 1,321 procedures, 509 (38.5%) were performed by the procedure service. In the adjusted analysis, as compared with procedure service, the group that underwent paracentesis by the radiology service had a 27% longer LOS (95% CI, 2%-58%) and 40% longer A2P time (95% CI, 5%-87%). The resident group had shorter A2P (–19%; 95% CI, –33% to 0.2%; P = .05) than the procedure service group but similar LOS. To our knowledge, this is the first study that suggests patient-centered benefits of a hospitalist procedure service.

The rise of hoax news circulating on social media at a certain point can cause confusion and anxiety in society. Therefore, this community service group took the initiative to carry out a media literacy activity for the herbs seller (herbalist) group of Yayasan Perempuan Perkotaan Medan (YP2M). This activity aims to facilitate the group so that they have the skills to respond to the hoax news. The materials provided include what the news is, the news values, the characteristics of hoax news, and the examples of hoax news that are widely spread in society through the social media. This program was held in Gedung Johor, Medan on August 2020 which was attended by around 20 participants. Keywords: media literacy, hoax news, YP2M

IntroductionTo strengthen the care capacity of primary facilities, China has vigorously promoted the construction of a hierarchical medical system and a family doctor care system. In July 2017, a family doctor care plan was launched in an urban district of Chengdu, Sichuan Province, and two family doctor contracting service models were adopted, one provided a basic-service package and the other a paid-service package. In order to evaluate the effect of different models on diabetic patients, this study conducted a real world study based on the district healthcare database.MethodsDiabetic patients who contracted family doctor services January 2018 to January 2019 as reported in the database were enrolled in the paid- or basic-service group. Propensity score matching (PSM) was conducted to balance the distribution of covariances between the groups. The results of the first and last examination of glycosylated hemoglobin, low-density lipoprotein cholesterol (LDL-C), systolic and diastolic blood pressure in the groups were compared by independent sample t-test and chi-square test.ResultsIncluded were 4,871 patients in basic-service and 394 patients in paid-service. In both groups the total control rates of blood pressure, glycosylated hemoglobin and LDL-C at the last physical examination were 43.67, 79.28 and 51.11 percent, respectively, a significant increase from pre-test. The combined control rates of HbAlc, LDL-C and blood pressure in the basic- and paid-service group were 20.76% and 22.37%, respectively. After PSM, there was no significant difference between the groups.ConclusionsUp to now, there is no significant difference between basic-service and paid-service family doctor contracting service models in improving the comprehensive control rate of diabetic patients. The possible reasons may be that the quality and content of paid-service is not as good as expected, the period of implementation is not long enough and the sample size of paid-service patients is limited.

Abstract Background Ward pharmacists are required for the active implementation of therapeutic drug monitoring (TDM). This epidemiological study verified whether Japanese ward pharmacists contribute to improving the TDM implementation proportions of anti-methicillin-resistant Staphylococcus aureus (MRSA) agents using the large health insurance claims database. Methods The patients who received intravenous anti-MRSA agents from April 2012 to March 2017 were enrolled. We defined ward pharmacy service as the “drug management and guidance fee” and/or “inpatient pharmaceutical services premium”. In addition, implementation of TDM was identified by “the specific drug treatment management fee”. We compared the proportions of TDM implementation for vancomycin (VCM), teicoplanin (TEIC), and arbekacin (ABK) in the ward and non-ward pharmacy service groups. To avoid confounding, the propensity score method was employed. Moreover, the clinical variables affecting TDM implementation in each anti-MRSA agent were analyzed by using a multiple logistic regression model. Results The following number of patients were included in the study: VCM (n = 2138), TEIC (n = 596), and ABK (n = 142). After propensity score matching, the proportions of TDM implementation for VCM and TEIC were higher in the ward pharmacy service group than in the non-ward pharmacy service group (VCM: 69.2% vs 60.3%, TEIC: 51.4% vs 34.7%), while no significant difference was observed for ABK (21.2% vs 23.1%). As independent clinical variables affecting TDM implementation for VCM and TEIC, several clinical variables, including ward pharmacy services, were extracted. In contrast, no clinical variables were extracted for ABK. Conclusions We found that the ward pharmacy service is associated with the active implementation of TDM for anti-MRSA agents, such as VCM and TEIC.
