Abstract The aim of this study was to investigate the effects of inter-electrode distance (IED), electrode radius (ER) and electrodes configurations on cross-correlation coefficient (CC) between motor unit action potentials (MUAPs) generated in a motor unit (MU) of parallel fibres and in a MU of inclined fibres with respect to the detection system. The fibres inclination angle (FIA) varied from 0° to 180° by a step of 5°. Six spatial filters (the longitudinal single differential (LSD), longitudinal double differential (LDD), bi-transversal double differential (BiTDD), normal double differential (NDD), an inverse binomial filter of order two (IB2) and maximum kurtosis filter (MKF)), three values of IED and three values of ER were considered. A cylindrical multilayer volume conductor constituted by bone, muscle, fat and skin layers was used to simulate the MUAPs. The cross-correlation coefficient analysis showed that with the increase of the FIA, the pairs of MUAPs detected by the IB2 system were more correlated than those detected by the five other systems. For each FIA, the findings also showed that the MUAPs pairs detected by BiTDD, NDD, IB2 and MKF systems were more correlated with smaller IEDs than with larger ones, while inverse results were found with the LSD and LDD systems. In addition, the pairs of MUAPs detected by the LDD, BiTDD, IB2 and MKF systems were more correlated with large ERs than with smaller ones. However, inverse results were found with the LSD and NDD systems.

Research on reliability of heart rate variability (HRV) parameters in athletes has received increasing attention. The aims of this study were to examine the inter-day reliability of short-term (5 min) and ultra-short-term (1 min) heart rate (HR), respiratory rate (RespRate) and HRV parameters, agreement between short-term and ultra-short-term parameters, and association between differences in HR, RespRate and HRV parameters in elite modern pentathletes. Electrocardiographic recordings were performed in stable measurement conditions with a week interval between tests. Relative reliability was evaluated by intra-class correlation coefficients, absolute reliability was evaluated by within-subject coefficient of variation, and agreement was evaluated using Bland–Altman (BA) plot with limits of agreement and defined a priori maximum acceptable difference. Short-term HR, RespRate, log transformed (ln) root mean square of successive normal-to-normal interval differences (lnRMSSD), ln high frequency (lnHF) and SD2/SD1 HRV indices and ultra-short-term HR, RespRate and lnRMSSD presented acceptable, satisfactory inter-day reliability. Although there were no significant differences between short-term and ultra-short-term HR, RespRate and lnRMSSD, no parameter showed acceptable differences with BA plots. Differences in time-domain and non-linear HRV parameters were more correlated with differences in HR than with differences in RespRate. Inverse results were observed for frequency-domain parameters. Short-term HR, RespRate, lnRMSSD, lnHF, and SD2/SD1 and ultra-short-term HR, RespRate and lnRMSSD could be used as reliable parameters in endurance athletes. However, practitioners should interpret changes in HRV parameters with regard to concomitant differences in HR and RespRate and caution should be taken before considering 5 min and 1 min parameters as interchangeable.

Neural networks have become the method of choice in surrogate modeling because of their ability to characterize arbitrary, high-dimensional functions in a data-driven fashion. This paper advocates for the training of surrogates that are 1) consistent with the physical manifold, resulting in physically meaningful predictions, and 2) cyclically consistent with a jointly trained inverse model; i.e., backmapping predictions through the inverse results in the original input parameters. We find that these two consistencies lead to surrogates that are superior in terms of predictive performance, are more resilient to sampling artifacts, and tend to be more data efficient. Using inertial confinement fusion (ICF) as a test-bed problem, we model a one-dimensional semianalytic numerical simulator and demonstrate the effectiveness of our approach.

Background: A virtuous understanding of Electrocardiogram is an indispensable ability and essential skill required in doctors. This study assessed the understanding of the electrocardiogram and acute management of myocardial infarction among emergency doctors, general practitioners, medical officers, house officers and postgraduate students. Methods: A cross sectional survey was conducted for 7 months on n=312 doctors working in tertiary care public and private hospitals and as General practitioners in Karachi, Pakistan. Convenience sampling technique was used. Participants were asked to interpret a 12 lead electrocardiogram attached to questionnaire and write down their findings and diagnoses. They were then asked about its acute management. ANOVA and Chi square was applied for making inferences from the acquired data. p value of less than 0.05 was taken as significant. Approval was taken from the Ethical Review board. Results: Knowledge was assessed on basis of designation, 84.6% of the emergency doctors correctly interpreted the ECG while 75.4% of general physicians read it correctly. House officers and medical officers had the lowest scores in ECG interpretation. Years of experience demonstrated inverse results with >5 years experience at the lowest scores for interpretation of ECG and MI management. Interpretation scores of ECG were significantly high (p value 0.03) in participants who attended workshops compared to those who didn’t. Conclusion: Emergency doctors had better scores when it comes to ECG interpretation and management of MI. Frequent workshops and cardiology rotations should be mandatory for health care professionals to improve essential skill and knowledge of acute management of myocardial infarction.

AbstractThe microbiome serves important functions in human health, and postmortem, the microbial signatures of colonized organ tissue could be useful in helping to predict the manner of death in cases where this information is not known. We surveyed the microbiota (16S rRNA V4 amplicon sequencing) of 265 organ tissue samples including liver, blood, brain, heart, prostate, spleen and uterus from cadavers in Italy, Finland and the United States with confirmed manners of death comprising either accidental death, natural death, homicide, and suicide. Geographic locality (i.e. nationality) had a strong effect on observed microbial composition. Differing PERMANOVA results between unweighted and weighted UniFrac (nearly inverse results) suggest that specific bacteria may be associated with ethnicity and age, but that these differences are negligible when taking into account the relative abundance of bacterial taxa; weighted UniFrac measures suggest that although taxonomic composition may not vary significantly between different manners of death, PMI, or BMI categories, the relative abundance of specific taxa vary significantly. Various tissues exhibit differential associations with bacteria, and prostate and uterus were substantially different compared to other organs. For example, in Italian cadavers, the bacteria MLE1-12 permeated nearly all tissues, except the prostate and uterus. We identified specific bacterial ASVs as biomarkers of either natural or accidental death and suicide, but not for homicide. While the manner of death may have an impact on microbial associations, further investigation under more controlled conditions will be needed to validate whether these associations are predictive in forensic determinations.ImportanceThe utilization of microbial data in the context of forensic investigations holds great promise for the field of forensic science. Identification of taxa that are associated with postmortem interval (PMI), specific manners of death (MOD), or other traits such as age, sex, ethnicity, and nationality may allow investigators to refine the circumstantial details surrounding the death of an individual. In this study we find nationality (geographic location of cadaver) to be a dominant predictor of cadaver microbiome composition. We also identify a number of cadaver-specific traits to be associated with microbial alpha- and beta diversity, as well as bacterial taxa that are differentially associated with these traits.

Objective: The aim of the present study was to design an efficient delivery system with an anticipated swelling and drug release properties for a prolonged drug release as well as to target colon for various hydrophilic drugs. Methods: For this purpose, the pH-responsive hydrogel comprising a combination of Eudragit and acrylic acid was formed. The hydrogels were characterized for spectral (FTIR), thermal (TGA/DSC), structural (XRD), and morphological (SEM) investigations. Oral tolerability was assessed in rabbits for biocompatibility and oral use of the prepared hydrogels. Results: The results showed that an increased incorporation of Eudragit and cross-linking agent retorted the swelling, drug loading, and drug release properties at both acid (pH 1.2) and basic pH (pH 6.8 and 7.4) , while acrylic acid presented the inverse results. The oral tolerability and toxicity studies depicted that the developed hydrogels were safe up to 3800 mg/kg body weight and caused no hematological or histopathological changes when compared with the control group. Conclusion: Therefore, the newly developed formulations presented adequate swelling, drug loading, release behavior, and biocompatibility properties and thus can be used as a promising tool for the colonic delivery of various hydrophilic drugs.

Abstract Bladder cancer (BC) is one of the commonest malignancies in the urinary system. Recent evidences have shown that Phospholysine phosphohistidine inorganic pyrophosphate phosphatase (LHPP) serves as a tumor suppressor in hepatocellular carcinoma and cervical cancer. However, little is known about its function in BC. Here, we aimed to investigate the role of LHPP in BC. We found that LHPP was down-regulated in BC tissues and cells. Knockdown of LHPP promoted the proliferation and growth of BC cells T24 and 5637. Inverse results were observed in SW780 and BIU87 cells with ectopic LHPP expression. LHPP also repressed the glycolysis of BC cells. At the molecular level, LHPP silencing led to enhanced phosphorylation of both AKT and p65, as well as up-regulation of their downstream targets Bcl-2 and Cyclin D1. Inhibition of AKT by MK2206 blunted the increased phosphorylation of p65 caused by LHPP knockdown, suggesting that LHPP silencing activated p65 through AKT. Importantly, p65 inhibitor (caffeic acid phenethyl ester) exhibited larger suppressive effect on the proliferation of LHPP knockdown BC cells as compared with Ctrl cell. Our study demonstrates that LHPP suppresses BC cell growth via inactivating AKT/p65 signaling pathway.

Despite research related to flexible or continuum curvilinear robots, there lacks a common simulation tool for continuum robots, which are unlike rigid robots. Thus, in this paper, a robotics toolbox is utilized to model a wire-driven flexible manipulator as one of the continuum robots. Constant curvature property can enable the robotics toolbox to represent the flexible manipulator and validate its kinematics. Moreover, because the closed-form inverse kinematics methods developed previously for real-time control conceded limitations in modeling some continuum robots, we hereby develop an inverse kinematics method for the wire-driven flexible manipulator which can provide fast and reliable inverse results. Experimental results showed that geometrical information offered a stable starting point for the proposed inverse kinematics algorithm. Moreover, the first and second derivatives of a fitness function further contributed to a fast-converging solution within a few microseconds. Lastly, for the potential feasibility of an active compliance controller without physical force/torque sensors, a reaction torque observer was investigated for a flexible manipulator with direct drive mechanisms.
