This paper describes indications of goal displacement in regulatory enforcement agencies as reported by enforcement professionals from a range of regulatory domains. The findings suggest that the occurrence of this phenomenon in these agencies may be more prevalent and multifaceted than expected. Among the goal-displacement types reported as most impactful were goal narrowing, induced by calamities, goal diversion through ongoing organizational reform, and goal diversion brought upon by strict regimes of output management. A systematic exploration of these various goal-displacement types as conducted here sheds light onto the intricate nature of goal alignment of these agencies.

Objectives: Discuss the experience and practice of multidisciplinary cooperation of diabetic foot in China and analyze its impact on the quality of care.Methods: This study observed the medical procedure by interviewing 12 key personnel in-depth. We extracted data from medical records and assessed the effect of MDT in three dimensions: quality, efficiency, and cost, to eventually achieve a final conclusion.Results: The studied reform includes the following three aspects: the adjustment of hospital buildings layout and disciplines, one-stop outpatient, and one-stop inpatient service. After the multidisciplinary collaboration, the rate of above-knee amputation is reduced by 3.63%, the disability score per 100 diabetic foot patients decreases by 6.12, the average length of stay decreases significantly, and the cost of hospitalization shows an increasing trend.Conclusions: Multidisciplinary collaboration is performed based on spatial layout adjustment and clinical pathway optimization, which provide more comprehensive and integrated care than a general medical team or a single specialist, thereby reducing the rate of disability, shortening the length of hospitalization. Besides, the new measurable indicator called disability score per 100 diabetic foot patients has been verified to evaluate the living ability of patients after surgery. This paper provides a reference for organizational reform of multidisciplinary diseases to support treatment and management of other multiorgan diseases.

This study aims to explore organizational reform at the Regency/City General Election Commission in West Java, both with regard to the existence of Regency/City KPU as the organizer of the General Election. as well as during the term of office of the commissioner. This study uses qualitative methods, descriptive analysis, case study strategies with steps for selecting topics and cases, reading literature, formulating research focuses and problems, data collection, data refinement, data processing, data analysis, data analysis process, theoretical dialogue, triangulation. findings, and conclusions of research results. data collection by observation, interviews, documentation and focus group discussion (FGD). Data analysis was carried out through data reduction procedures, data display and conclusion drawing/verification. Based on the results of the study, it can be concluded that the existence of the Regency/City General Election Commission is still needed as the organizer of the general election at the regional level; that there must be a change in the organization of the Regency/City General Election Commission in relation to the term of office of the commissioner from five years to 1.5 years for budget efficiency and performance effectiveness.

Abstract Background Organizational reforms of hospitals in Iran are mainly aimed at improving efficiency, reducing government spending on health care, and improving the quality of services. These reforms began with hospital autonomization and have continued with other initiatives such as formation of board of trustees, independent and corporatized hospitals. Objective The purpose of this scoping review was to summarize and compare the results of studies conducted on organizational reform of hospitals in Iran to paint a more clear picture of the status quo by identifying knowledge gaps, inform policymakers, and guide future studies and policies. Method This review’s methodology was inspired by Arksey and O’Malley’s methodological framework to examine the extent, range, and nature of research activity about organizational hospital reforms in Iran. A literature search was performed using PubMed, Scopus, Web of Science, and Google Scholar for English papers as well as SID, IranDoc, Magiran, and the Social Security Research Institute Database for Persian papers from 1991 to April 2020. Results Twenty studies were included in the review. Studies were grouped by the types of organizational reform, study’s objective, setting, methodology, data collection and analysis techniques, and key findings. Thematic construction was used based on the types of organizational reform to present a narrative account of existing literature. Conclusions The autonomy granted to the hospitals was unbalanced and paradoxical in terms of key effective dimensions. Poor governance and regulatory arrangements, low commitment to corporate governance, Inappropriate board composition, weak internal controls, unsustainable financing and inefficient payment mechanisms, poor interaction with stakeholders and ignoring contextual factors have been cited as the main reasons for the failure of organizational reforms in Iran. The limited use of evidence and research was obvious at different stages of policymaking, especially in the policy formulation phase and evaluation of its results.

The content of the administrative reform plan for the period 2011–2020 of the Government of Vietnam on the basis of the overall reform of the national administration, focuses on four main areas, namely institutional reform and organizational reform. structure, building and developing contingent of civil servants and public finance (Government, 2011). Thus, along with the institutional and organizational reform, the public finance reform is also a central task of the administrative reform in the condition that Vietnam continues to strongly transition from the centralized mechanism. The subsidy to a socialist-oriented market economy, in particular, is even more necessary when Vietnam has become an official member of the World Trade Organization. One of the current concerns for good implementation of public finance reform is the implementation of autonomy and self-responsibility for the use of payroll and administrative management funding for administrative agencies. state and financial autonomy mechanism for non-business units. This article focuses on analyzing the current situation of reforming the financial operation mechanism of public non-business units in Lao Cai province, Vietnam, pointing out the achievements, limitations, causes and some recommendations to end. promote the reform of the financial operation mechanism of public non-business units in Lao Cai province, Vietnam in the context of world economic integration.

Revolution in Military Affairs (RMA) requires fundamental reforms on its 3 main pillars, namely military technology, doctrine, and organizational reform. South Korea is one of the states that has taken steps to initiate the RMA through defense reform policies. South Korea have initiated its own version of RMA by prioritizing the revolution of military technology rather than any other aspects. This article aims to explain why South Korea prioritizes its military modernization through the development of its domestic defense industry. This research applies the concept of military techno-nationalism impulse with a qualitative approach and literature review method to collect data from official documents, websites of the government, journals, and scientific reports. The results of the research indicate that South Korea's choice to prioritize the development of its domestic defense industry is not merely pushed by the urgency to decrease its dependencies toward the United States, but also by the desire to pursue for strategic power to face the North Korean threat as well as other states around the region that full of antagonism atmosphere, to obtain broader economic benefits through the spin-off effect of industrialization, and to gain international prestige as one of the largest arms exporter as well as standing equal with other regional players.

Adapting to the growing frequency of catastrophic wildfires in Greece and mitigating their effects is a complex socio-ecological problem. We used an online survey to query more than 100 engaged stakeholders who can potentially influence possible legislation and fire management organizational reform, emphasizing civil protection agencies and research entities. We focused the questionnaire on the importance of different wildfire effects to understand which were considered negative or unacceptable, indifferent, or positive. For fire prevention, we examined the range of acceptance and views on fuel management and fire use activities that are limited in extent or not allowed in Greece. We also examined the beliefs regarding ignition causes and responsibility, in addition to how different policies might reduce wildfire-related problems. The results revealed an emphasis on reforming wildfire management policies to deal with the way society and agencies function and interact, and mitigate the influence of climate change in wildfire frequency and behavior. In addition, respondents had a negative stance towards allowing wildfires to burn for resource objectives and a strong belief that arsonists are behind most ignitions. They also believe the lack of a national cadaster system is a major source of wildfire-related problems. The results indicate little support for fuel treatments, but increased acceptance for the legalization of fire use during firefighting (backfires). This study summarizes current wildfire perceptions in Greece and identifies opportunities and barriers to changes in wildfire governance to improve risk management programs and guide post-fire management and mitigation.

This study presents analyses of developing process of the regulatory framework and the results of the implementation of the organizational reform of the Russian Empire gendarmerie in the mid-1860s – early 1880s. The process of developing the regulatory framework for the reform was intense and produc-tive, which was not typical of Russian lawmaking. Analyzing the implementation of the reform, the authors examine two contradictory, but interrelated processes: the elimination of post-reform units and the process of formation of various types of units at the local, regional and central levels. A new type of local subdivisions is being formed “security de-partments”, subsequently playing an important role in the functioning of the institute. It is noted that on the territory of the national outskirts, the process of formation of local post-reform bodies had certain peculiarities. In general, the organizational reform was an adequate response to the challenges of the time, had a positive impact on the functioning of the institute and its results were sustainable.
