Communication in a healthcare context

This chapter highlights the centrality of good communication in healthcare generally, and nursing practice in particular. It is proposed that language use needs to be understood as more than just a means of speaking to another person, or a group of people, about some aspect of the care process. Though this is important and essential to effective decision-making and nursing interventions, practitioners need to understand the political dimensions of talk as discourse, of how language plays a performative part in constructing, as well as describing, the social/clinical world. Attention is given to the damaging, as well as productive, attributes of communication—where the euphemistic concept of ‘empowerment’ has to be critically addressed and enacted. The contribution of humanistic psychology to nursing practice is discussed through the core conditions of person-centered working. Verbal and non-verbal skills and behaviours to enact an empathic, genuine, and unconditional foundation for care are outlined. Finally, specialist knowledge and skills, expressed through ‘family intervention’ therapy, locate communication within wider social relations.

OBJECTIVE: To understand the interaction process between the elderly and the family and the nurses during home care. METHODS: Grounded theory qualitative study in a community where 40% of the population is aged 65 or above. The collection of data was made via the non-participating observation of nursing practice during 41 home visits and semi-structured interviews to nurses, the elderly and the family. RESULTS: the following categories emerged - structural organization of at-home care, diagnostic assessment in context and therapeutic intervention in context. CONCLUSION: the central category was "Building the relationship in an at-home context", due to the fact that the relationship between the nurse, the elderly and the family is central across the entire care process. The relation is, simultaneously, the context for all the care and a therapeutic instrument.

Introduction and Objective: Human anatomy is an essential component of the undergraduate nursing curriculum for learning the specific disciplines which deal with clinical practice. Anatomical knowledge provides assurance for the practice of clinical assessment and invasive procedures of legal competence of nurses.  The aim of the study was to analyze the correlation of the content taught in the discipline Human Anatomy with the clinical practice of undergraduate nursing students in the discipline Semiology and Semiotics in Nursing and The Care Process, as well as their assurance to start it.Methods: Quantitative descriptive study with the application of an online questionnaire to 66 undergraduate nursing students at a public education institution in the interior of São Paulo. Data analysis by number of occurrences and Chi-square test.Results: There was partial agreement about the interdisciplinarity between human anatomy and disciplines of clinical nursing practice. The students agreed to be partially assured about the procedures to start the semiological practice of different devices and to perform nursing procedures. The predominance of the superficial approach to content related to the clinical practice of the disciplines Semiology and Semiotics in Nursing and The Care Process was predominant.Conclusions: The teaching of human anatomy, along the lines offered, maintains an unsatisfactory correlation with clinical practice due to the students’ experience, interfering with learning, acting in clinical teaching and professional training.

Psychosocial rehabilitation for patients with mental disorders is one of the priority areas of scientific research in modern psychiatric discourse. The key to solving this problem is a multidisciplinary approach, within which it becomes possible to select congruent rehabilitation and psychoeducational techniques, taking into account the clinical and psychopathological content of this nosological form. In this context endogenous procedural mental disorders deserve special attention, as they are characterized by the highest level of social and labor maladjustment. Aim. To investigate the effectiveness of the developed model of complex psychosocial rehabilitation for patients with schizophrenia. Materials and methods. On the basis of Municipal Non-Commercial Enterprise “Regional Clinical Institution for Provision of Psychiatric Care” of Zaporizhzhia Regional Council, 80 patients with a simple form of schizophrenia were selected at the outpatient stage of treatment for the purpose of further examination. Clinical diagnostics of the studied mental pathology was carried out in accordance with the differential diagnostic criteria of the International Classification of Diseases 10 revision (ICD-10). The inclusion criteria for the study contingent were: informed consent to participate in the study, the absence of severe somatic pathology and information about craniocerebral trauma. In order to analyze the effectiveness of the developed model, the study contingent was randomized into the study group (SG) – 40 patients who received complex therapy within the framework of the developed model of psychosocial rehabilitation, and the comparison group (CG) – 40 patients who received treatment in accordance with the current protocols. Results. Based on the formed principles of psychosocial rehabilitation for patients with a simple form of schizophrenia, a comprehensive model of psycho-rehabilitation support for this patient contingent was generated including four stages: psychopharmacological, family intervention, cognitive-behavioural and ergotherapeutic. Analysing the effectiveness of the developed model, statistically significant differences (P < 0.05) were established between the average indicators in the studied groups according to the criteria “socially useful activity, including work and study” (4.18 ± 0.25 and 3.33 ± 0.27 points in SG and CG, respectively) and “relationships with family and other social relations” (4.35 ± 0.24 and 3.60 ± 0.25 points in SG and CG, respectively) in the absence of statistically significant differences in the criteria for “self-care” and “bothering others and aggressive behaviour”. Conclusions. The effectiveness of the complex model of psychosocial rehabilitation for patients with a simple form of schizophrenia in the areas of socially useful activities (study and work) and social relations was developed and proved, which made it possible to increase the effectiveness of therapy for this contingent of patients.

The multidimensional social problems that everyone seeks to remedy them are very complex and no actor can confront them on their own. So the different parties have to work together by creating relations of partnerships.The State has long been the main actor in the control and regulation of social relations. However, in recent years there has been a rapid decline in their role given the enormous charge and lack of resources. Hence the need for the intervention of other parties.Some review of literature explores the conceptualization of social partnership in order to meet the needs of the organization or solve organizational problems. More and more large companies and multinationals get started on a voluntary approach to social responsibility and have begun to move closer to certain social enterprises by concluding partnership agreements.The purpose of this study was to study the social transformation that follows the creation of a relation of a social partnership between a social enterprise and a company that has involved a strategy of social responsibility. Then, we will present the environment to finally study the impact of social partnership on the environment; on economic, cultural and political dimensions.

Intuition is a quality that nurses have traditionally valued, but nowadays evidence-based research climate is often denigrated. Because it is difficult to investigate and quantify, intuition has tended to be seen as unreliable, unscientific and unsuitable for nursing practice. However, intuition, like caring, cannot be subjected to measurement and should not be overlooked as an important tool. Intuition is identified as a useful tool that needs to be recognized in nursing. At the same time, intuition is described as an important type of nursing knowledge and a valid way of knowing in clinical nursing practice. It is knowing something or deciding to do something without having a logical explanation. Intuition in practice has been linked to enhanced clinical judgment, effective decision making and crisis aversion. The inability to provide rationale for an action or decision makes intuition challenging for nurses to describe, explain or openly acknowledge. In the nursing literature, intuition is recognized as an important component of decision-making theories. In the literate outlines the two predominant theoretical approaches to decision making: the systematic positivistic approach and the intuitive humanistic approach. Both approaches are valuable and emphasise the need to investigate quantitatively the intuitive humanistic approach in decision making. To understand the intuitive approach to decision making, nurses need to first identify intuitive feelings and intuition use in their clinical practice. Qualitative studies demonstrate intuition use in expert decision making and label it as a source of knowledge coming from feelings, sensations and connections. Intuition is a component of decision-making models applied in nursing practice. Empirical research which requires nurses to recognize intuition and utilize it effectively in nursing practice.

O artigo tem como escopo refletir, do ponto de vista teórico, sobre a formação humana a partir da vida e obra do pensador italiano, Antonio Gramsci (1891-1937). Nesse sentido, objetiva abordar algumas categorias filosóficas de sua práxis político-pedagógica, entre elas, escola unitária, hegemonia e onilateralidade, como expressões de uma concepção teórica e política de um projeto socialista de sociedade e de ser humano. Nessa perspectiva, a escola unitária, ao integrar de forma dialética trabalho manual e intelectual, a dimensão técnica e política, autonomia intelectual e moral, conforma-se em princípio educativo do processo de formação humana. E, por fim, inaugura um autêntico humanismo em que o ser humano é compreendido como o conjunto das relações sociais, um ser concreto e histórico, um devir humano-social.Palavras-chave: Gramsci. Abstract. Alkhairaat and NU are two different organizations and structures. NU is active in various fields with not small political dimensions, while Alkhairaat focuses on education and da'wah. But in Manado it is interesting, the Islamic community considers that Alkhairaat is an embryo of NU. This is certainly related to the historical-sociological factors of these two institutions born in the local context. This study wants to answer two questions; First, what is the background of the relationship between Alkhairaat and NU in Manado in 1955-1998 and what is the dynamics and pattern characteristics of the Alkhairaat relationship with NU in Manado during 1955-1998. Temporally, this research was restricted from 1955 because this year Alkhairaat built its social relations with NU, which at that time was the party participating in the first election in Indonesia. The final limitation of the study was set in 1998 because this year there was a change in the body of NU by declaring the PKB as a party formed by and for NU citizens, but open to anyone including non-Muslims. Namun di Manado menarik, masyarakat Islam menganggap bahwa Alkhairaat adalah embrio dari NU. Ini tentu terkait dengan faktor historis-sosiologis kedua lembaga ini lahir dalam konteks lokal. Penelitian ini ingin menjawab dua pertanyaan; Pertama, Bagaimana latarbelakang terjalinnya hubungan antara Alkhairaat dan NU di Manado pada tahun 1955-1998 dan Bagaimana dinamika dan karakteristik pola relasi Alkhairaat dengan NU di Manado selama tahun 1955-1998. Secara temporal, penelitian ini dibatasi dari tahun 1955  karena pada tahun inilah Alkhairaat membangun relasi sosialnya dengan NU yang saat itu menjadi partai peserta dalam Pemilu pertama di Indonesia. Adapun batasan akhir penelitian ditetapkan tahun 1998 karena pada tahun ini terjadi perubahan di tubuh NU dengan mendeklarasikan PKB sebagai partai yang dibentuk oleh dan untuk warga NU, namun terbuka bagi siapapun termasuk non-Muslim. Research with the title "Therapeutic Communication in Nursing Care in Class III Inpatient at the Prof. DR. W.Z. Johannes Kupang Hospital "is based on the importance of the therapeutic communication of nurses and patients at Prof. DR. W.Z. Johannes Kupang Hospital. This study focuses on how therapeutic communication in nursing care in class III inpatient at the Prof. DR. W.Z. Johannes Kupang hospital with the aim to analyze therapeutic communication in nursing care in inpatient room class III of Prof. DR. W.Z. Johannes Kupang hospital. Case study method is used in this research. The results of this study indicate that in the nursing care process that takes place in inpatient rooms of class III at Prof. DR. W.Z. Johannes Kupang hospital, the four stages of therapeutic communication can be found in the three parts of the therapeutic communication process, namely when the patient enters, when giving nursing care, and when the patient is discharged. The pre-interaction stage and the introductory stage appear in the communication process when the patient enters. The work phase which is the core of the therapeutic communication process appears in the communication process at the time of nursing care. The last stage is the termination stage which is divided into temporary termination stages, which is seen in the communication process of nurses and patients each time the nurse finishes providing routine nursing care every day. The final termination stage that appears in the nurse's communication with the patient is when the patient is declared allowed to go home or when the patient dies. Participants involved in the therapeutic communication process are not only nurses and patients but is also supported by doctors and patients' families. In the end these stages are interrelated with one another and create a fairly good communication process in nursing care in class III inpatients at the Prof.. DR. W.Z. Johannes Kupang hospital.

Abstract.IMPACT OF THE CARE PARTNERHIP ON HOSPITALIZED CHILDREN AND THEIR FAMILIESBackground - For children, hospital admission often means to be removed from their home environment, their daily activities, and their friends. The care partnership is considered to be essential in the child’s hospitalization process, resulting in benefits not only for children and their families but also for healthcare professionals.Objective - To identify the impact of the nursing care partnership on hospitalized children and their families.Methods - A systematic literature review was conducted. After application of the inclusion/exclusion criteria, a search performed in the online databases EBSCOhost, b-on, and SciELO revealed 5 primary studies published between 2005 and 2014 that addressed the research question.Results- The results showed that both children and their families benefit from the care partnership, to the extent that the opportunity to be involved in care delivery reduces their anxiety levels, increases their feeling of independence, promotes a good communication with the nursing team, brings about feelings of peace and reassurance, and facilitates the adaptation to the child’s new health condition.Conclusions- The nursing team is responsible for including family members in the care process, encouraging their presence and active participation and allowing them to provide emotional support and safety to the hospitalized child. Their presence throughout the hospitalization period is essential to minimize its impact on children and their families.Keywords: Care partnership, integration into care, participation in care, parents, family, pediatrics, child.Resumo.Antecedentes- O período de hospitalização representa muitas vezes para a criança o afastamento do seu ambiente familiar, das suas actividades diárias e dos seus amigos, pelo que a parceria de cuidados é reconhecida como essencial no seu processo de hospitalização, trazendo benefícios quer para a criança e sua família, quer para a equipa de profissionais de saúde.Objectivo- Conhecer o impacto da parceria de cuidados de enfermagem para as crianças hospitalizadas e suas famílias.Metodologia- Trata-se de uma Revisão Sistemática da Literatura. Após os critérios de incluso e exclusão foram seleccionados 5 artigos primários, publicados entre 2005-2014 e pesquisados nas plataformas eletrónicas: EBSCOhost, b-on e SciELO, que deram resposta à questão de revisão.Resultados- Os resultados obtidos demonstraram que a parceria de cuidados é benéfica tanto para a criança como para a sua família, na medida em que a oportunidade de participação nos cuidados, diminuios níveis de ansiedade, aumenta o sentimento de independência, promove uma comunicação adequada com a equipa de enfermagem, sentimentos de paz e encorajamento e facilita a adaptação à nova condição de saúde da criança.Conclusões- Cabe à equipa de enfermagem incluir a família no processo de cuidados, permitindo a sua presença e o seu envolvimento ativo, dando a possibilidade aos familiares de prestar apoio emocional e segurança à criança hospitalizada. O acompanhamento ao longo de todo o processo de internamento é crucial para minimizar o impacto da hospitalização na criança e na sua família.Palavras-chave: Parceria de cuidados, integração nos cuidados, participação nos cuidados, pais, família, pediatria e criança.
