Declining Physical Activity and the Socio-Cultural Context of the Geography of Industrial Restructuring: A Novel Conceptual Framework

Background:At the population level, the prevalence of physical activity has declined considerably in many developed countries in recent decades. There is some evidence that areas exhibiting the lowest activity levels are those which have undergone a particularly strong transition away from employment in physically demanding occupations. We propose that processes of deindustrialization may be causally linked to unexplained geographical disparities in levels of physical activity. While the sociocultural correlates of physical activity have been well studied, and prior conceptual frameworks have been developed to explain more general patterns of activity, none have explicitly attempted to identify the components of industrial change that may impact physical activity.Methods:In this work we review the current literature on sociocultural correlates of health behaviors before using a case study centered on the United Kingdom to present a novel framework that links industrial change to declining levels of physical activity.Results:We developed a comprehensive model linking sociocultural correlates of physical activity to processes associated with industrial restructuring and discuss implication for policy and practice.Conclusions:A better understanding of sociocultural processes may help to ameliorate adverse health consequences of employment decline in communities that have experienced substantial losses of manual employment.

At a population level, the method used to determine those meeting physical activity guidelines has important implications, as estimating “sufficient” physical activity might be confounded by weight status. The objective of this study was to test the difference between three methods in estimating the prevalence of “sufficient activity” among Canadian adults with type 2 diabetes in a large population sample (N=1614) while considering the role of weight status as a potential confounder. Our results revealed that estimates of physical activity levels vary by BMI categories, depending on the methods examined. Although physical activity levels were lower in the obese, their energy expenditure estimates were not different from those who were overweight or of a healthy weight. The implications of these findings are that biased estimates of physical activity at a population level may result in inappropriate classification of adults with type 2 diabetes as “sufficiently active” and that the inclusion of body weight in estimating physical activity prevalence should be approached with caution.

Background: Children’s sedentary lifestyles and low physical activity levels may be countered using population-level interventions. This study examines factors influencing the use of a free community-wide physical activity access pass for grade 5 students (G5AP). Methods: A natural experiment with longitudinal data collection. A sample of 881 children completed the 9-month follow-up survey self-reporting where they used the G5AP. Two analyses were conducted: Getis-Ord GI* geographic cluster analysis of the spatial distribution of users, and logistic regression examining the relationship between use and accessibility (informational, economic, and geographic) and mobility options, while accounting for intrapersonal and interpersonal factors. Results: Overall, 44.9% of children used the G5AP with clusters of high use in urban areas and low use in the suburbs. Other factors significantly related to G5AP included gender (girls), informational accessibility (active recruitment), economic accessibility (median household income), geographic accessibility (facilities within 1.6 km of home), and mobility options (access to Boys & Girls Club bus). Conclusions: This study found that a diverse population of children used the G5AP. To continue being successful, community-based physical activity interventions need to ensure that the intervention increases geographic, economic, and informational accessibility and provides mobility options that are available to the target population.

ObjectiveThe proliferation of studies using motivational signs to promote stair use continues unabated, with their oft-cited potential for increasing population-level physical activity participation. This study examined all stair use promotional signage studies since 1980, calculating pre-estimates and post-estimates of stair use. The aim of this project was to conduct a sequential meta-analysis to pool intervention effects, in order to determine when the evidence base was sufficient for population-wide dissemination.DesignUsing comparable data from 50 stair-promoting studies (57 unique estimates) we pooled data to assess the effect sizes of such interventions.ResultsAt baseline, median stair usage across interventions was 8.1%, with an absolute median increase of 2.2% in stair use following signage-based interventions. The overall pooled OR indicated that participants were 52% more likely to use stairs after exposure to promotional signs (adjusted OR 1.52, 95% CI 1.37 to 1.70). Incremental (sequential) meta-analyses using z-score methods identified that sufficient evidence for stair use interventions has existed since 2006, with recent studies providing no further evidence on the effect sizes of such interventions.ConclusionsThis analysis has important policy and practice implications. Researchers continue to publish stair use interventions without connection to policymakers' needs, and few stair use interventions are implemented at a population level. Researchers should move away from repeating short-term, small-scale, stair sign interventions, to investigating their scalability, adoption and fidelity. Only such research translation efforts will provide sufficient evidence of external validity to inform their scaling up to influence population physical activity.

Self-report recall questionnaires used to measure physical activity and dietary intake in children can be labour intensive and monotonous and tend to focus on either dietary intake or physical activity. The web-based software, Synchronised Nutrition and Activity ProgramTM (SNAPTM), was developed to produce a novel, simple, quick and engaging method of assessing energy balance-related behaviours at a population level, combining principles from new and existing 24 h recall methodologies, set within a user-friendly interface. Dietary intake was measured using counts for twenty-one food groups and physical activity levels were measured in min of moderate to vigorous physical activity (MVPA). A combination of the mean difference between methods, type II regression and non-parametric limits of agreement techniques were used to examine the accuracy and precision of SNAPTM. Method comparison analyses demonstrated a good agreement for both dietary intake and physical activity behaviours. For dietary variables, accuracy of SNAPTM (mean difference) was within ± 1 count for the majority of food groups. The proportion of the sample with between-method agreement within ± 1 count ranged from 0·40 to 0·99. For min of MVPA, there was no substantial fixed or proportional bias, and a mean difference between methods (SNAPTM – accelerometry) of − 9 min. SNAPTM provides a quick, accurate, low-burden, cost-effective and engaging method of assessing energy balance behaviours at a population level. Tools such as SNAPTM, which exploit the popularity, privacy and engagement of the computer interface, and linkages with other datasets, could make a substantial contribution to future public health monitoring and research.

Aim: The present study aimed to investigate the cross-sectional association between self-reported use of electronic wearable devices (EWDs) and the levels of physical activity among a representative sample of adults with depression and anxiety in the United States.Methods: For this cross-sectional study, data were pooled from the Health Information National Trends Survey 2019. A sample of 1,139 adults with self-reported depression and anxiety (60.9% women; mean age of 52.5 years) was analyzed. The levels of physical activity and prevalence of EWD utilization were self-reported. The chi-square tests were used to compare individual characteristics through the use of EWDs. Multivariable logistic regression was employed to investigate the association between EWDs and physical activity levels while adjusting for sociodemographic and health-related factors.Results: From the 1,139 adults with self-reported depression and anxiety, 261 (weighted percentage 28.1%) endorsed using EWD in the last year. After adjusting for covariates, the use of EWDs was only significantly associated with a higher odds of reporting intention to lose weight (OR 2.12; 95% CI 1.04, 4.35; p = 0.04). We found no association between the use of EWDs and meeting the national weekly recommendation for physical activity or resistance/strength exercise training.Conclusion: About three in 10 adults suffering from depression and anxiety in the United States reported using EWDs in the last year. The current study findings indicate that among people living with mental illness, EWD use is associated with higher odds of weight loss intent suggesting that EWDs may serve as an opening for the clinical interactions around physical health through identifying patients primed for behavior change. Further large-scale studies using randomized trial designs are needed to examine the causal relationships between EWDs and the physical activity of people with mental health conditions.

In Japan and other developed countries, the lifestyles of many—particularly the younger generation—have been disrupted in recent years. This disruption may manifest as a decrease in physical activity and deterioration in the quality and quantity of sleep. Depressive tendencies are also increasing among university students. This study examined the relationship between mental health, physical activity, physical fitness, and daytime sleepiness. Eighty-five undergraduate students participated in this study (52 men and 33 women, aged 18.9 (±1.4) years). Physical activity levels were measured using an accelerometer (Lifecorder, Kenz, Nagoya, Japan) for two weeks. To evaluate their level of physical fitness, maximal oxygen uptake (V˙O2max) was calculated by an indirect method using a cycle ergometer. Depressive tendencies and daytime sleepiness were evaluated using the Patient Health Questionnaire (PHQ-9). The PHQ-9 score was positively correlated with sleepiness (r = 0.35, p = 0.001) and total steps per day (r = 0.39, p < 0.001). Moreover, the PHQ-9 score was positively correlated with V˙O2max (r = 0.25, p = 0.019). The PHQ-9 score was higher in students with good exercise habits or part-time jobs. An important finding was the positive correlation between depression and variables related to physical activity levels. These results suggest that vigorous physical activity, such as exercise and part-time jobs, might be positively associated with depressive tendencies among university students.

Background:Physical activity (PA) promotion in children and youth is an impetus for prevention and control of NCD morbidity and mortality, but evidence is needed for effective interventions. The aim of the present paper is to summarize the results of the 2013 Nigerian Report Card on Physical Activity for children and youth.Methods:The Technical Report Committee conducted a comprehensive review of available literature in Nigeria. Grades were assigned to 10 PA indicators modeled after the Active Healthy Kids Canada (AHKC) grading system.Results:Specific grades were assigned for several indicators: Overall Physical Activity Levels, C; Organized Sport and Physical Activity Participation, Incomplete; Active Play and Leisure, C-; Active Transportation, B; Sedentary Behaviors, F; Overweight and Obesity, B+. The following indicators were graded as INCOMPLETE: Physical Activity in School setting, Family and Peers, Community and Built Environment, and Government Strategies and Investments.Conclusions:PA levels of Nigerian children and youth are moderate while sedentary behaviors are high. The development of national guidelines for PA and sedentary behaviors can better inform policy and practice on healthy living among Nigerian children and youth.

Background:It is speculated that rural Kenyan children are more physically active than those in developed countries. The purpose of this study was to examine pedometer-measured physical activity levels of western Kenyan youth.Methods:Participants in this study were children in Levels 3 and 5 who attended a private primary school. The sample (n = 72) consisted of 43 girls and 29 boys (average age = 9.8 ± 1.1, range = 8−12 years). Age, gender, tribe, and height and weight measures were collected. Weight status category was determined according to CDC guidelines. Participants wore a sealed Yamax pedometer for 4 weekdays during the measurement period. Data analysis included descriptive statistics and 2-way ANOVA (age × gender).Results:The total sample averaged 14558 ± 3993 daily steps. There was no significant effect for age [F(4,68) = 1.682, P = .102] nor significant age × gender interaction [F(4,68)=1.956, P = .117]. There was a significant effect for gender [F(1,68) = 4.791, P = .033], with boys (16262 ± 4698) significantly more active than girls (13463 ± 3051).Conclusions:The observed daily steps are higher than those observed in the U.S., similar to samples in other developed countries, but lower than Amish youth.

Background:Although more than 1 million US children attend licensed family child care homes, little is known about children’s physical activity in this setting. The purpose of this study is to describe the physical activity of children cared for in child care homes.Methods:The study sample included 31 licensed family child care homes in Washington State. Children aged 3 to 6 wore accelerometers while in child care over 5 days. Minutes per hour spent at 4 activity levels were calculated and averaged for all children in the home. Contextual factors such as provider practices, staff training, and home characteristics were assessed using standardized questions.Results:Accelerometer data from 144 children were included, with 2 to 11 children monitored per home. The mean minutes of sedentary activity per hour (min/h) was 34.3 (SD = 4.6, range 27.7 to 46.6). For moderate-to-vigorous activity (MVPA) it was 8.8 min/h (SD = 2.6, range 3.6 to 14.1) and for vigorous physical (VPA) activity it was 3.1 min/h (SD = 1.4, range: 0.9 to 7.0).Conclusions:The low levels of MVPA and VPA in many homes reinforces the need for additional research to identify policy and practice recommendations that will be most effective in increasing physical activity in this setting.

Abstract Background: Australia like many developed countries is largely an inactive nation. Participation in sport, a form of leisure-time physical activity, can contribute to a range of individual and community health benefits. National sport policy often has a dual focus on population-based participation, and elite performance. Whist there are various sport models depicting the pathway to elite, there has been no strategic population-level sport participation pathway model developed as a guide towards increasing sport participation across the lifespan. The aim of this study is to explore sport participation across the lifespan and develop a neutral (conceptual) model that does not favour community or elite sport and that highlights the critical participation transition points including drop-out. Methods: We conducted a longitudinal analysis of sport participation records for 8 major sports from 2015-2017 for children and young people aged 4-29 years. Individual participation within sports was tracked from 2015 and subsequent years 2016 and 2017. Results: The total number of participants was 579,696. Only half 50.8% played continuously for the three years, 44.7% dropped out, and 4.5 played discontinuously. Drop-out was highest for those aged 4 (57.0%), and lowest for those aged 10-14 years (39.3%). From the retention and drop-out participation patterns across the lifespan, in conjunction with the published literature on sport policy and participation, we developed the Sport Participation Pathway Model (SPPM). The SPPM conceptually depicts current sport participation and in doing so emphasises the significant drop-out of sport across the lifespan. Conclusion: Sport policy should focus on retention strategies. Further, a population level, competitive club-based sport alone is not going to solve the physical inactivity epidemic. Our SPPM, and the new (retention related) data presented in this paper, demonstrates the population trends in participation in sport and the high drop-out of competitive club-based sport. We suggest that for people to eventually become active sport participants, the recruitment narrative could be changed towards, first, engaging people in leisure time physical activities. We feel that this can only be done by developing an integrated policy system across the physical activity-sport spectrum rather than looking at sport participation in a closed (club-based) system.
