Sixty Sprague-Dawley rats were divided into 5 groups: (a) control group (HS); (b) Immediate rehydration group (IFR); (c) ST36 electroacupuncture (EA) delay rehydration group (EA/DFR): EA at ST36 immediately after blood loss with infusion 3 h later; (d) EA nonmeridian rehydration group (SEA/DFR): EA at nonacupuncture sites with rehydration similar to EA/DFR; (e) ST36 EA group (EA): EA at ST36 immediately after blood loss with no rehydration. Forty-five percent of the entire blood volume was taken out to make lethal hemorrhagic shock models. We recorded the survival rate, intestinal tissue DAO content, and microcirculation. The survival rate of the EA/DFR group and the IFR group was significantly higher than that of the other three groups (P<0.05). Twelve hours after blood loss, intestinal tissue DAO content of the EA/DFR group and the IFR group was significantly higher than that of the SEA/DFR group, EA group, and HS group (P<0.05andP<0.01). The mucosal blood flow of the EA/DFR group and the IFR group was significantly higher than the other groups (P<0.05each). We conclude that EA improves the blood pressure and raises the early survival rate of hemorrhagic shock rats, maintains the intestinal barrier function, and improves the degree of intestinal ischemia.

AbstractWe aim to determine the impact of an artificial liver support system (ALSS) treatment before liver transplantation (LT), and identify the prognostic factors and evaluate the predictive values of the current commonly used ACLF prognostic models for short-term prognosis after LT. Data from 166 patients who underwent LT with acute-on-chronic liver failure (ACLF) were retrospectively collected from January 2011 to December 2018 from the First Affiliated Hospital of Zhejiang University School of Medicine. Patients were divided into two groups depending on whether they received ALSS treatment pre-LT. In the observation group, liver function tests and prognostic scores were significantly lower after ALSS treatment, and the waiting time for a donor liver was significantly longer than that of the control group. Both intraoperative blood loss and period of postoperative ICU care were significantly lower; however, there were no significant differences between groups in terms of total postoperative hospital stays. Postoperative 4-week and 12-week survival rates in the observation group were significantly higher than those of the control group. Similar trends were also observed at 48 and 96 weeks, however, without significant difference. Multivariate Cox regression analysis of the risk factors related to prognosis showed that preoperative ALSS treatment, neutrophil–lymphocyte ratio, and intraoperative blood loss were independent predicting factors for 4-week survival rate after transplantation. ALSS treatment combined with LT in patients with HBV-related ACLF improved short-term survival. ALSS treatment pre-LT is an independent protective factor affecting the 4-week survival rate after LT.

Abstract Objective To investigate the impact of microecological preparation combined with modified low-carbon diet on the glucolipid metabolism and cardiovascular complication in obese patients. Methods From August 2017 to July 2020, 66 obese patients were recruited, and administrated with an modified low-carbon diet with (group A) or without (Group B) microecology preparation and a balanced diet in control group (group C) for 6 months. Meanwhile, 20 volunteers administrated with a balanced diet were recruited as the healthy control group (group D). Results After 6-month intervention, obese subjects in group A and B showed significant improvement of body and liver fat mass, reduction of serum lipid levels, intestinal barrier function markers, insulin resistance index (IRI), high blood pressure (HBP) and carotid intima thickness, as compared with subjects in group C. More importantly, subjects in group A had better improvement of vascular endothelial elasticity and intimal thickness than subjects in group B. However, these intervention had no effect on carotid atherosclerotic plaque. Conclusion Administration of microecological preparation combined with modified low-carbon diet had better improvement of intestinal barrier function, glucose and lipid metabolism, and cardiovascular complications than low-carbon diet in obese patients, but the effect of a simple low-carb diet on carotid atherosclerotic plaque need to be further addressed.

The influence of pretreatment with dibenamine on the development and course of hemorrhagic shock, and the effect of treatment with dibenamine, rapidly acting antiadrenergic drugs, or antihistaminic drugs after hemorrhagic shock had been allowed to become unresponsive to replacement transfusion, were tested in dogs prepared in advance to permit measurement of portal-caval venous pressure gradient. Preliminary dibenamine administration was also tested in rats submitted to hemorrhagic shock. The conclusions were as follows: 1) The protective effect of dibenamine prior to the induction of hemorrhagic shock in the dog consists mainly of a reduction of the bleeding volume. Intrahepatic vasoconstriction is not reduced. A dog which is not under the influence of dibenamine can tolerate a greater degree of blood loss than a dibenaminized dog. After hemorrhagic shock has been allowed to become refractory to replacement transfusion, antiadrenergic and antihistaminic drugs do not reduce intrahepatic vasoconstriction or increase the survival period or the survival rate. 2) Dibenamine given prior to hemorrhage enables the rat to survive a degree of blood loss which is lethal to the untreated rat. This, in part, appears to be due to better blood flow to the respiratory center.

Abstract Background: The effects of dietary supplementation with guava leaf extracts (GE) on growth performance, diarrhea and intestinal barrier function, as well as associated with its modulation of serum and fecal metabolic changes in weaned piglets challenged by enterotoxigenic Escherichia coli (ETEC) were investigated.Method: Fifty weaned piglets (Duroc × Yorkshire × Landrace) from 5 pens (2 piglets per pen) were randomly divided into five groups: blank control group (BC), negative control group (NC), or those supplemented with 50 mg kg-1 (S50), 100 mg kg-1 (S100), or 200 (S200) mg kg-1 diet GE, respectively. On day 4, all piglets (except for BC) were orally challenged with about 1.0 × 109 colony-forming units (CFU) enterotoxigenic ETEC. After 28-day trial, growth performance, diarrhea incidence, intestinal barrier function and metabolomics of serum and fecal were investigated.Results: We demonstrated that dietary supplementation with GE (50-200 mg kg-1) reduced diarrhea incidence of piglets and increased expression of intestinal tight junction proteins (ZO-1, Occludin, Claudin-1) (P < 0.05) and sodium hydrogen exchanger 3 (NHE3) (P < 0.05). Moreover, dietary supplementation with GE (50-200 mg kg-1) upregulated level of tetrahydrofolic acid (THF) and reversed higher level of nicotinamide-adenine dinucleotide phosphate (NADP) caused by ETEC in serum compared with NC group (P < 0.05), and enhanced antioxidant ability of piglets. In addition, dietary addition with GE (100 mg kg-1) reversed the lower level of L-pipecolic acid caused by ETEC in feces compared with NC group (P < 0.05), and decreased oxidative stress response of piglets. Further, there were no differences (P > 0.05) in the final weight, average daily feed intake (ADFI) and F/G among dietary groups during the overall period, and piglets in S50 group has the higher average daily gain (ADG). Conclusion: Dietary supplementation with 50-200 mg kg-1 GE reduced diarrhea incidence of weaned piglets challenged by ETEC and exhibited positive effect on improving intestinal barrier function. Meanwhile, dietary addition with GE organized and redistributed energy resources through similar or dissimilar metabolic pathways, and finally enhanced antioxidant ability of piglets challenged by ETEC.

The golden trevally fishes (Gnathanodon specious) (2.19 ± 0.23 g) were cultured in glass tanks with density of 20 fishes/tank and they were fed supplemental diets of different MOS concentrations (0; 0.2; 0.4 and 0.6%) for 90 days. Collected data included growth rate, survival rate and some hematological characteristics of this fish. The results demonstrated that MOS supplementation did not affect growth performance, erythrocyte density and blood cell size, however the survival rate was significantly increased. On the other hand, the total number of white blood cells (BC) on the 60th day in the fish fed with MOS supplements (5.78–6.96 × 104TB/mm3) was higher than that in the control group (only 5.43 × 104TB/mm3) with the largest total leukocytes (6.96 ± 0.50 × 104TB /mm3) at 0.2% MOS (p < 0.05).

Hemorrhagic shock causes severe depression of macrophage functions and is associated with increased susceptibility to sepsis. Because hemorrhagic shock and resuscitation encompasses several pathophysiological conditions, such as hypotension, low-flow conditions, hypoxia, and reperfusion injury, it remains unknown whether severe hypotension in the absence of blood loss has any adverse effects on macrophage functions. To study this, systemic arterial hypotension was induced in C3H/HeN mice for 15 min by intravenous infusion of sodium nitroprusside or ATP-MgCl2. Peritoneal macrophages (PM) was harvested 20 h later with lavage. Antigen presentation was measured by coculturing PM with the D10.G4.1 Th cell clone. Tumor necrosis factor (TNF), interleukin (IL)-6, IL-1, and prostaglandin (PG) E2 levels in supernatants of PM stimulated with lipopolysaccharide were measured with bioassays or radioimmunoassay. Systemic arterial hypotension resulted in a significant decrease of PM capacity to present antigen. Although the release of TNF, IL-6, and IL-1 by PM was unaltered after hypotension, PGE2 release by PM was significantly elevated compared with the control group. These data indicate that chemically induced systemic arterial hypotension without blood loss leads to a depression of antigen presentation, which may be caused by elevated release of the immunosuppressive eicosanoid PGE2.

Bifidobacterium longum (B. longum) ES1 is a probiotic strain capable of modulating microbiome composition, anti-inflammatory activity and intestinal barrier function. We investigated the use of B. Longum ES1 in the treatment of patients with diarrhoea-predominant irritable bowel syndrome (IBS-D). Sixteen patients were treated for 8 or 12 weeks with B. Longum ES1 (1 × 109 CFU/day). Serum zonulin and cytokines were measured at baseline (T0) and at the end of therapy (T1). Clinical response to therapy was assessed by IBS Severity Scoring System. Interleukin (IL)-6, IL-8, IL-12p70 and tumor necrosis factor (TNF) α levels decreased from T0 to T1, irrespective of treatment duration (p < 0.05), while zonulin levels diminished only in patients treated for 12 weeks (p = 0.036). Clinical response was observed in 5/16 patients (31%): 4/8 (50%) treated for 12 weeks and 1/8 (13%) treated for 8 weeks. Abdominal pain improved only in patients treated for 12 weeks (5/8 vs. 0/8, p = 0.025), while stool consistency improved regardless of therapy duration (p < 0.001). In conclusion, the results of this pilot study showed, in IBS-D patients treated for 12 weeks with B. longum ES1, a reduction in the levels of pro-inflammatory cytokines, and intestinal permeability as well as an improvement in gastrointestinal symptoms, but further studies including a placebo-control group are necessary to prove a causal link.

Carbon tetrachloride (CCL4) is often employed in the production of chlorofluorocarbons, petroleum refining, oil and rubber processing, and laboratory applications. Oral, subcutaneous, and inhalation exposure to CCL4 in animal studies have been shown to be capable of leading to various types of cancer (benign and malignant, liver, breast, and adrenal gland tumors). The present study also evaluated the protective role of infliximab (INF) against the deleterious effects of CCL4 on the intestinal system. Twenty-four male Sprague-Dawley rats were randomly assigned into three groups, control ( n = 8), CCL4 ( n = 8), and CCL4 + INF ( n = 8). The control group received 1 mL isotonic saline solution only via intraperitoneal (i.p.) injection. The CCL4 group received a single i.p. dose of 2 mL/kg CCL4. The CCL4 + INF group received a single i.p. dose of 7 mg/kg INF followed 24 h later by a single dose of 2 mL/kg CCL4. All rats were euthanized 2 days following drug administration. CCL4 group samples also exhibited diffuse loss of enterocytes, vascular congestion, neutrophil infiltration, an extension of the subepithelial space and significant epithelial lifting along the length of the villi with a few denuded villous tips. In addition, CCL4 treatment increased intestinal malondialdehyde (MDA) level and caspase-3 positivity. On the other hand, INF decreased MDA levels, caspase-3 positivity, and loss of villous. Our findings suggest that CCL4 appears to exert a highly deleterious effect on the intestinal mucosa. On the other hand, INF is effective in preventing this CCL4-induced intestinal injury by reducing oxidative stress and apoptosis.

Fluid resuscitation could hardly be performed immediately after fatal hemorrhagic shock in outpatients. We investigated whether electroacupuncture (EA) at Zusanli (ST36) could prevent fatal hemorrhagic shock induced heart failure with delayed fluid resuscitation and whether the protective role of EA is related to the autonomic nervous system. Sixty Sprague Dawley rats were randomly divided into five groups (n=12 each): group of sham hemorrhagic shock (SHAM), group of EA, group of sham EA (SEA), group of delayed fluid resuscitation with EA (EA + DR), and group of delayed fluid resuscitation with SEA (SEA + DR). After blood loss for 6 hours, caspase-3 activity and positive rate of TUNEL in EA + DR group were significantly lower than in other hemorrhagic shock groups (e.g., versus SEA + DR: 0.156±0.039 versus 0.301±0.042; P<0.05). Immediately EA treatment after the blood loss enhanced the protective effect of delayed resuscitation on the cardiac tissue of hemorrhagic shock rats. Considering the significant changes of epinephrine (137.8±6.9 ng/L versus 98.6±7.4 ng/L; P<0.05) and acetylcholine (405±8.6 pmol/L versus 341±10.1 pmol/L; P<0.05) after EA treatment (SEA + DR versus EA + DR), this cardiac protective effect may be related to regulation of the autonomic nervous system.

Reports of the efficacy of l-noradrenaline in the treatment of clinical shock stimulated an investigation of its effect in controlled hemorrhagic hypotension. Seventy-three 350-g. male Sprague–Dawley rats were subjected to a standardized hemorrhagic shock procedure. Of 15 control animals that received no treatment, only one survived for 48 hours; none survived of the five controls that received a constant intravenous infusion, after the shock procedure, of 5% glucose in distilled water until death or for 36 hours. The treated animals received, after the shock procedure, an infusion of l-noradrenaline (0.5–2.0 μg./min.) in 5% glucose in distilled water. The survival rates for the treated animals were: treatment for 1 hour, 1/8; treatment for 4 hours, 4/15; treatment until death or for 36 hours, 8/15. Fifteen animals received, in addition to noradrenaline for 36 hours, hydrocortisone administered intravenously (0.7 μg./min.) or intramuscularly (2.5 mg. every 6 hours); seven of these animals survived.Analysis of variance showed that there was no difference in the shock procedure undergone by the controls and by the treated survivors. The Chi square test on the survival rates revealed that the infusion of noradrenaline for 1 hour or 4 hours did not improve survival, but infusion for 36 hours produced a very significant increase in survival time and in total survival rate. The addition of hydrocortisone neither enhanced nor impaired this improvement.
