Sanitation of House and School, Personal Hygiene and Infection of Soil Transmitted Helminths among Elementary School Students

<p>In Indonesia, soil transmitted helminths (STH) infection is still a public health problem particularly in the child of elementary school age. The high of that infection was also supported by low environmental sanitation and poor personal hygiene. STH infection is one of 10 great of frequent diseases in Palue Island and in this region still able can be found an adult worm in feces even from regurgitate. This was observational analytic study by cross sectional design. The location of the study was Palue Island, Sikka Regency, East Nusa Tenggara. The repondents of the study was 100 students of eight elementary school. Diagnosis of STH infection was establised based on worm eggs examination in feces used floatation method. Questionnaire, and check list were the instruments for collecting data in this study. There were significant relationship between house’s environmental sanitation (p=0.043), school’s environmental sanitation (p= 0.000), personal hygiene (p = 0.032) with STH infection among the students of elementary school in Palue Island, Sikka Regency, East Nusa Tenggara. Multivariate analysis obtained that personal hygiene was the most influence variable to the STH infection in this region.</p>

BACKGROUND: Helminthiasis is a parasitic disease in human that causes a disturbance in food absorption and lead to malnutrition. The disease slowly impacts on the intelligence. The incidence of helminthiasis in Indonesia remains high, ranging from 2.5% to 62%. A preliminary study found that the proportion of helminthiasis in children is 31.25%. AIM: The aim of the study to analyse the relationship between age, school sanitation and personal hygiene with helminthiasis elementary school children in Juhar Karo Regency in 2019. METHODS: This was an observational study with a cross-sectional design. Subjects were children in grades IV, V and VI with and without helminthiasis. A total of 194 children selected by proportional random sampling were enrolled. Data were collected by interview, observation, and stool examination with the Kato-Katz method. RESULTS: The proportion of helminthiasis in boys was 51.0%, the proportion of poor sanitation in school was 36.6%, and the proportion of poor personal hygiene was 67.5%. CONCLUSIONS: Personal hygiene was significantly associated with the incidence of helminthiasis (RP = 6.052; 95% CI = 3.029-12.902; P-value = 0.001). Improved personal hygiene may prevent the occurrence of helminthiasis. CONCLUSION: The proportion of helminthiasis in elementary school students in this region was 50.0%. Personal hygiene has been shown to be related to helminthiasis. In this study, subjects with poor personal hygiene had a chance of 6.052 times greater to experience helminth infection compared to subjects who had good personal hygiene.

Di Indonesia penyakit cacingan tersebar luas di pedesaan dan di perkotaan dengan prevalensi semua umur 40%-60% dan murid SD 60-80%. Survei Depkes RI di 10 propinsi di Indonesia menemukan prevalensi kecacingan di Kabupaten Pesisir Selatan tahun 2003 (85, 8%) dan tahun 2005 (51,4% ) lebih tinggi dari kabupaten lain. Angka infeksi kecacingan tinggi dipengaruhi oleh kebersihan diri, sanitasi lingkungan dan kebiasaan penduduk. Tujuan penelitian untuk mengetahui hubungan perilaku cuci tangan dengan kejadian kecacingan pada murid SD Kabupaten Pesisir Selatan. Penelitian dengan desain cross sectional ini menggunakan data sekunder hasil survei kecacingan Depkes RI tahun 2005, jumlah sampel 257 orang. Diagnosis penyakit kecacingan ditegakkan berdasarkan pemeriksaan telur cacing pada tinja dengan metode Katto-Katz. Hasil penelitian menunjukan perilaku cuci tangan memakai air dan sabun sebelum makan terbukti berhubungan bermakna dengan kejadian kecacingan (OR=2,35, 95% CI=1,40-3,94), variabel lain yang berhubungan bermakna adalah perilaku BAB tidak dijamban dengan nilai OR = 2,64 (95%CI=1,46-4,77) dan perilaku jajan bukan di warung sekolah (OR =1,96 (95% CI=1,06-3,65). Murid SD dan masyarakat disarankan mencuci tangan sebelum makan dengan air dan sabun. Di samping pengobatan, perlu dilakukan penyuluhan kesehatan dan peningkatan PHBS pada murid dengan pemeriksaan berkala perilaku dan kebersihan diri di sekolah.Kata kunci : Cuci tangan, kecacingan, kebersihan diri, sanitasi lingkunganAbstractIn Indonesia, helminth infection spread out at rural and urban community with prevalence at elementary school of 60% - 80% and for all age of 40%-60%. Helminths prevalence in Pesisir Selatan district is higher than that of other districts which was 85, 8 % on 2003 and 51,4 on 2005. Helmints infection depends on personal hygiene, environmental sanitation and residential habit. The objective of the this study is to know relationship between hand washing behaviour before eating with helminths occurrence among elementary school students in Pesisir Selatan. This research uses cross sectional design, using secondary data of helminths survey of Ministry of Health RI on 2005, with total sample of 257 students. Helminths disease diagnosis was based on laboratory examination of helminth eggs in human feces using Katto-Katz methods. Research’s results showing that hand washing behaviour before eating is related to helminths occurrence (OR= 2,35, 95% CI = 1,40-3,94), other variables with statistically significant relationship to helminths occurrence is defecation behavior not at the toilet (OR = 2,64; (95% CI OR =1,46-4,77) and eating snacks in small shop at school (OR= 1,96 ;95% CI=1,06-3,65). It is recommended to elementary school student and society to familiarize to hand washing before eating with water and soap. In order to prevent and eradicate helminths disease, beside curative effort, there is a need to do health counseling about healthy and clean living (PHBS), undertaking periodical evaluation on behaviour and personal hygiene of student at school.Keywords : Hand washing, helminths, personal hygiene, environmental sanitation

<p><strong>Abstract</strong></p><p><strong>Background</strong>: Ear, nose, and throat (ENT) health service coverage has yet to include all citizens in Central Bali. ENT disorders among elementary school-age children should be of important concern as it might affect their educational process at school.</p><p><strong>Method</strong>: This descriptive research conducted from January to December 2019 employed a cross-sectional design. This study recruited subjects of grade 1 to 6 students from 13 elementary schools in Kubu and Tianyar village, Kubu sub-district, Karangasem district, Bali, who participated during the social service event held by Yayasan Peduli Kemanusiaan supported by Inspirasia Foundation. The total sampling method was used.</p><p><strong>Results</strong>: A number of 1115 elementary school students consisted of 578 (51.8%) male and 537 (48.2%) female students were recruited. The most and least commonly encountered ENT disorder was cerumen impaction (n=837; 75.1%) and rhinotonsilopharyngitis (n=1; 0.1%), respectively.</p><p><strong>Conclusion</strong>: Knowing the ENT disorders among elementary school age children in Central Bali is important as it might serve as a benchmark for conducting a precise intervention to manage ENT disorders since early age.</p><p>Keywords : <em>ENT, elementary school, student</em></p>

Soil Transmitted Helminths (STH) infections is one of health issues in Indonesia that has environment and social basis. It is classified as neglected disease. The Indonesian government already has eradication program, but it is not supported by evaluation and monitoring program. The purpose of this study is to determine the prevalence and relation of each risk factors related to STH infections in elementary school in Ngis village, Karangasem regency, Bali. The study was done by analytical description using cross sectional study. Samples were selected from population based on inclusion and exclusion criteria. Primary data about suspected risk factors were collected using questionnaire. Diagnosis was established using Kato-Katz modification method. Data was analyzed using chi-square with confidence interval 95% or p value ≤0.05 categorized as significant. 138 students enrolled in this study, the median age is 9 (6-13) years. The prevalence of STH infections is 10.1% with 78.6% is single infection of Trichuris trichiura and 21.4% mixed infections. The proportion of STH infections in males is higher than female but it is statistically insignificant. STH infections have significant relationship with some risk factors such as not washing hand after defecation, not washing hand after playing with soil, barefoot, not cutting nails and consuming anti-helminthic routinely. The highest risk factor of STH infections in Ngis village is not having available and proper latrine. (OR=33.9; 95%CI=5.749-199.769). The prevalence of STH infection is quite high with mild to moderate intensity and risk factors namely low hygiene and limited latrines. The implementation of monitoring and evaluation can be an effort to control risk factors and stop the STH transmission chain.

Abstract: Anxiety and other psychiatric disorders such as depression, become as the most suffered mental illness. Each year, an average of 38 million people suffer from anxiety and depression. Anxiety can occur in children when they are facing stressful situations threatening their lives, inter alia violence against children. Children often feel anxious, especially when they lost of love, attention, and support from their parents. This study was aimed to obtain a description of the level of anxiety in children who had experienced violence in elementary school at Malalayang district in Manado. This was a descriptive quantitative study with a cross-sectional design using purposive sampling method. Respondents were fourth grade and sixth grade elementary school children aged 9-12 years. The instrument used was a questionnaire of Revised Children's Manifest Anxiety Scale (RCMAS). The results showed that of 315 children who had experienced violence, there were 246 children (78.09%) had normal anxiety, 41 children (13.02%) had mild anxiety, 21 children (6.67%) had moderate anxiety, and 7 children (2.22%) had severe anxiety. Conclusion: Most of the respondents who experienced anxiety as a result of violence were in the normal anxiety level.Keywords: anxiety, child abuse, children of elementary school age, RCMAS Abstrak: Kecemasan dengan gangguan psikis lainnya yaitu depresi, menjadi penyakit kejiwaan yang paling banyak diderita. Setiap tahunnya rata-rata sebanyak 38 juta orang mengalami kecemasan dan depresi. Kecemasan dapat dialami oleh anak-anak ketika menghadapi situasi stres yang mengancam hidupnya, salah satunya ialah kekerasan pada anak. Anak-anak sering merasa cemas terutama bila kehilangan kasih sayang, perhatian dan dukungan dari orang tuanya. Penelitian ini bertujuan untuk mendapatkan gambaran tingkat kecemasan pada anak yang mengalami kekerasan di Sekolah Dasar (SD) di Kecamatan Malalayang Kota Manado. Jenis penelitian ialah deskriptif-kuantitatif dengan desain potong lintang dan menggunakan purposive sampling. Responden penelitian yaitu anak SD kelas IV-VI usia 9-12 tahun. Instrumen yang digunakan ialah kuesioner Revised Children’s Manifest Anxiety Scale (RCMAS). Hasil penelitian memperlihatkan dari 315 anak yang mengalami kekerasan, terdapat 246 anak (78,09%) mengalami kecemasan normal, 41 anak (13,02%) mengalami kecemasan ringan, 21 anak (6,67%) mengalami kecemasan sedang, dan 7 anak (2,22%) mengalami kecemasan berat. Soil-Transmitted Helminths (STH) Infection is a worm infection which is transmitted through soil. STH is still a public health problems around the world, including Indonesia. The tropical climates of Indonesia is a suitable environment for viability of STH. The prevalence of STH in Bali since 2003-2007 reached 40.94%-92.4%. The altitude of an area may affect the rate of STH infection. The rate of STH infection is higher on population in highland area compared to lowland area. This study aimed to identify risk factors of STH infection in elementary school students in highland and lowland area in Gianyar Regency. This study used cross sectional analytic design and was conducted in 2016. The total samples of this study were 104 students which were consisted of 53 students in highland area and 51 students in lowland area. The samples were collected using cluster random sampling technique. The specimens were examined using Kato-katz method. The results showed that the prevalence of STH infection in elementary school students in highland area (28.31%) was higher than in lowland area (1.96%). In lowland area, the risk factor could not be clearly identified because there was only one student with a positive STH infection. Meanwhile, the identified dominants risk factors which influence STH infection in highland area were habit of washing hands using soap (AOR=24.99), fingernails cleanliness (AOR= 13.19) and parents income (AOR=18.83).

Abstract: Stunting is a condition where height is not in accordance with age. It is due to chronic malnutrition which causes nonoptimal brain development that can affect children’s cognitive development, performance at school, and learning ability, as well as consequently influences learning achievement at school. This study was aimed to identify the correlation between stunting and learning achievement of elementary school students at Tikala Manado. This was an analytical study with a cross sectional design. Respondents were elementary school students at Tikala Manado aged 7-13 years that had their average grades. Data were analyzed by using unpaired T-test. The results showed that there were 232 students as respondents. Stunting was found in 103 students (44%) and not stunting in 129 students (56%). The average grade of stunting students was 67.16 and of not stunting students was 68.53. Statistical analysis showed that there was no significant difference between the average student grade in stunting students and not stunting students (P=0.215; α=0.05). Conclusion: There was no significant correlation between stunting and learning achievementKeywords: stunting, learning achievement Abstrak: Stunting merupakan keadaan dimana tinggi badan tidak sesusai dengan usia. Hal ini dapat menyebabkan perkembangan otak tidak optimal sehingga berpengaruh pada perkem-bangan kognitif dan performance anak di sekolah, serta kemampuan belajar, yang selanjutnya berpengaruh pada prestasi belajar anak di sekolah. Penelitian ini bertujuan untuk mengetahui hubungan stunting dan prestasi belajar pada siswa Sekolah Dasar di Kecamatan Tikala Manado. Jenis penelitian ialah analitik dengan desain potong lintang. Responden ialah siswa sekolah dasar di Kecamatan Tikala Manado yang berusia 7-13 tahun dan rerata nilai rapor siswa. Analisis uji statistik yang digunakan ialah uji T-test tidak berpasangan. Hasil penelitian mendapatkan 232 siswa sebagai responden. Stunting didapatkan pada 103 siswa (44%) dan yang tidak stunting 129 siswa (56%). Rerata nilai rapor pada siswa stunting 67,16 dan yang tidak stunting 68,53. Hasil uji analisis menunjukkan tidak terdapat perbedaan rerata prestasi belajar antara siswa stunting dan tidak stunting (P=0,215; α=0,05). Simpulan: Tidak terdapat hubungan bermakna antara stunting dan prestasi belajar.Kata kunci: stunting, prestasi belajar

Background: The prevalence of Soil Transmitted Helminth (STH) infections in Indonesia is still high, especially in elementary school children in North Sumatra Province.  STH infection is thought to cause harm to nutrient requirements due to lack of calories, causing health and nutritional conditions to decline. Purposes: To determine the relationship of STH infection with nutritional status in 105926 State Elementary School students. Method: Analytic observational with cross sectional design.  Sampling uses a total sampling method, with a total sample of 87 people.  Nutritional status was determined by the CDC-NCHS 2000 growth curve and the determination of the STH infection used the Kato-Katz examination method.  Results: 26 samples (29,9%) were positive and 61 samples (70,1%) were negative for STH infections.  Positive samples infected with STH have nutritional status that is dominated by moderate and good nutritional status. Conclusion: There is no correlation between STH infection and nutritional status in 105296 State Elementary School students in Percut Sei Tuan District, Deli Serdang Regency, North Sumatra.
