To Study the Clinical Profile and Management of Cellulitis of Lower Limb in Northern India

Cellulitis is a bacterial infection of the dermis and subcutaneous tissues occupying a large proportion of hospital beds. This study was conducted for analysis of patients with cellulitis according to their demographics and clinical presentation and to examine their comorbidities, complications, and its management. This observational cross-sectional study was conducted in the Department of Surgery at Santosh Medical College and Hospitals involving a total of 60 cases having cellulitis and other soft tissue infections of lower limb. Analysis of their demographic profile, management, and complications was done. Cellulitis is seen commonly in males, 46 (76.6%). The mean age of patients affected by cellulitis in the study was 36.4 ± 1.23 years. The most common site affected is leg involving more people in field jobs. The most common risk factor was trauma in 46.6%, and other factors were diabetes mellitus and smoking, while abscess formation was the most common complication observed in 36.6% of cases. A total of 56.6% cases were managed conservatively, while 43.3% cases required surgical intervention. Mean hospital stay in this study was 5.02 ± 0.23 days. It was concluded that cellulitis is subcutaneous, spreading bacterial infection is more common in males, and its incidence is highest in working age group population. Lower limb is commonly involved. Trauma, smoking, and diabetes are significant risk factors for development of cellulitis. Abscess is the most common complication. About 50% patients with cellulitis can be managed conservatively and the rest require surgical intervention.

Background: Diabetes is a major threat to global public health that is rapidly getting worse and its biggest impact is on adults of working age 45-55 in developing countries. Diabetes mellitus causes progressive changes to the kidneys and ultimately results in diabetic nephropathy. Microalbumin measurement is useful in diagnosis at an early stage, before the development of proteinuria.Methods: This is a community based cross sectional study. The study period of 2 months, from September 2019 to October 2019. Adult between 20 to 59 years of age at the time of data collection from house to house visit were included in the study. A total of 138 diabetics were found in the community and accessed for microalbuminuria and other risk factors.Results: Prevalence of microalbuminuria among was 15.1%. Logistic regression model analysis revealed that micro albuminuria was significantly correlated with the diastolic blood pressure (p=0.003) and the duration of diabetes (p=0.001).Conclusions: Due to the high prevalence of diabetes in India we need to screen all diabetics for microalbuminuria. Control of blood pressure should also be done more vigorously in diabetic patients.

BACKGROUND Metopic suture is located anterior to the coronal suture. There are two types: complete and incomplete metopic sutures. Knowledge regarding the metopic sutures in adults is necessary to avoid misinterpretation of radiographs as frontal bone fracture. Knowledge with regard to metopic suture is helpful for anatomists and medico legal experts. This study has been done to find out the incidence of metopic suture, classify the metopic sutures into complete and incomplete type and further classify the incomplete variety based on shape. METHODS This study was conducted on 50 skulls from the Department of Anatomy, Autonomous Government Medical College and Hospital, Ratlam (MP). Skulls with signs of disease and damaged skulls were excluded from the study. RESULTS In the present study, out of 50 dry adult cadaveric skulls, 2 skulls (4 %) showed complete metopism. Incomplete metopic sutures were present in 28 / 50 (56 %) and absent in 20 / 50 (40 %). Out of 28 incomplete metopic sutures, single linear metopic sutures were present in 12 / 28 (42.85 %). Double linear metopic sutures were present in 7 / 28 (25 %). V-shaped metopic sutures were present in 5 / 28 (17.85 %) and U shaped metopic sutures were present in 4 / 28 (14.28 %). CONCLUSIONS Detailed knowledge of the metopic suture is important for neurosurgeons and radiologists in routine practice. Vertical frontal bone fractures may be easily misdiagnosed with persistent metopic sutures. This anatomical knowledge of metopic sutures is very useful for doctors while treating traumatised patients and during surgical intervention including frontal craniotomy. Background: Tuberculosis is a major cause of mobility and mortality in developing countries including India.BCG vaccine has been in use since 1921 and is considered to be one of the safest vaccines with self limited regional Lymphadenopathy being the most common complication. Objectives: The present study aims toevaluate the efficacy and cytomorphological patterns of BCG lymphadenitis. Materials & methods: This cross-sectional study was conducted in the Department of Pathology of Government Medical College / Government General Hospital, Anantapuramu, during the period January 2017 to December 2019. Patients with suspected ipsilaterallymphnodal enlargement after BCG vaccination and age ranging from two months to two years , who were referred for FNAC, were included in this study. FNAs of 17 patients were studied in the present study. FNAC was performed by using a 10 mL syringe with a 23 g needle. Smears were stained with H& E stain (Haematoxylin and Eosin)and ZN ( Ziehl –nelson )stain.The data collected was entered into an Excelsheet and analyzed. Results: Cytology slides prepared were studied, interpreted, and the diagnosis made was analyzed.A total of 17 cases were studied with the mean age of 6.24 months. Majority of the patients were males seen in 64.71% of cases. Ipsilateral Axillary lymphnode was the most common site involved seen in 82.35% cases.Pus was aspirated in 76.47% of cases.On microscopic examination diffuse necrosis with degenerated cellswas the predominant cytological pattern seen in nine (52.94%) cases. ZN stain showed Acid fast bacilli positivity in 15 cases. Conclusion: FNA cytology combined with clinical correlation is useful for diagnosis and management of BCG lymphadenitis.

To determine the most common physical side effects experienced by local chemotherapy patients. Their perceptions of these side effects and informational needs from clinical pharmacists were also evaluated. This was a single center, observational cross-sectional study conducted at department of General Surgery, Guru Gobind Singh Medical College and Hospital, Faridkot, Punjab. A face to face interview was conducted. Information collected included chemotherapy related side effects after last chemotherapy experience, the most worrisome side effects, overlooked by healthcare professionals and the preferred method, amount and source of receiving related information. In this study, hundred patients were enrolled out of them 48 were male and 52 were female. When differential calculations was done, common side effects or adverse effects of chemotherapy in the patients of breast, lung cancer, Ovarian Cancer, Colon cancer, Prostate cancer, Lymphoma Cancer, Cervix cancer where there is much irregular medicine intake 57.4% may be due to common problem of joint pain reported by all the patients under study, with the consecutive problem of nausea and vomiting. The high prevalence of chemotherapy related side effects among local patients is a major concern and findings of their perceptions and informational needs may serve as a valuable guide for clinical pharmacists and physicians to help in side effect management. This study shows the common problems reported by the patients when they are suffering from cancer condition, according to their incidence perceptions as experienced by the patient, this will allow the physician and clinical pharmacist to effectively counsel and manage the common symptoms as reported prior to its occurrence in the patient, so that withdrawal can be checked.

Rotavirus is one of the leading causes of pediatric diarrhea globally. Accurate and rapid diagnosis of Rotavirus diarrhea should reduce unnecessary use of antibiotics and ultimately reduce drug resistance. Study was designed for rapid diagnosis of Rotavirus antigen in stool sample by ICT (Immunochromatographic test) as well as to observe the seasonal variation of rotavirus infection. This cross sectional study was carried out in the department of Microbiology, Dhaka Medical College from January 2011 to December 2011. Eighty stool samples were collected from Dhaka Shishu Hospital and Dhaka Medical College Hospital. All samples were tested for rotavirus antigen by ICT. Among 80 patients, 42 (52.5%) samples were positive for rotavirus antigen. Among these 42 positive samples, 30 (71.43%) were from 0-12 months of age group, 10 (23.81%) from 13 to 24 months of age group and rest 2 (4.76%) from 25 to 36 months of age group. Rotavirus Ag was detected in stool samples from January to April and another peak episode from October to December. A cross sectional study was carried out at the department of Forensic Medicine in Dhaka Medical College during the period of January 2008 to December 2009. Data were collected from 3rd copy of the post mortem reports which were preserved in the department of Forensic Medicine with the verbal consent of the doctors who performed autopsy report. During this period total 5114 autopsies were conducted. Out of this 970 cases (19%) were suicidal in nature. It was noticed that all suicidal deaths occurred from 10 years to all age group respectively, but top amongst age group of suicidal deaths occurred in between 21 to 30 years of people. Suicidal deaths are more common in female than male. Suicidal deaths due to hanging is highest, next common causes of death due to organophosphorus compund poisoning. Suicidal deaths by hanging is more in female than male but in poisoning cases male are more lvictimised than female. Objectives of our study are to see the occurrence and methods of suicidal death. Pleural effusion is present when there is >15ml of fluid is accumulated in the pleural space. It can be divided into two types; exudative and transudative pleural effusion. Tuberculosis and parapneumonic effusion are the common cause of exudative pleural effusion whereas heart failure accounts for most of the cases of transudative pleural effusion. This study was a hospital based cross sectional study performed at Nepal Medical College during the period of January 2016-December 2016. A total of 50 patients who fulfilled the inclusion criteria were enrolled. Pleural effusion was confirmed by clinical examination and radiology. After confirmation of pleural effusion, pleural fluid was aspirated and was analysed for protein, LDH, cholesterol. The Heffner criteria was compared with Light criteria to classify exudative or transudative pleural effusion. Among 50 patients, 30 were male and 20 were female. The mean age of patient was 45.4±21.85 years. The sensitivity and specificity of using Light criteria to detect the two type of pleural effusion was 100% and 90.9%, whereas using Heffner criteria was 94.87%, 100% respectively(P<0.01). There are variety of causes for development of pleural effusion and no one criteria is definite to differentiate between exudative or transudative effusion. In this study Light criteria was more sensitive whereas Heffner criteria was more specific to classify exudative pleural effusion. Hence a combination of criteria might be useful in case where there is difficulty to identify the cause of pleural effusion.

Background: Family medicine is a field in which complete and detailed set of healthcare services are provided to the patients and their families. In developed countries, freshly graduated students choose family medicine as a priority for their career. However, in Pakistan it is not the case. The present study is undertaken to determine the perceptions of final year medical students’ about Family Medicine as a viable career. Methods: A total number of 504 students participated in the study. This was a cross-sectional study. The study participants were in their final year of medical college. Data was collected using a preapproved questionnaire. Data was entered and analyzed via SPSS version 17 and Chi-Square test was used post-stratification. Results: Only 14.3% (n=72) medical students had heard about Family Medicine. Only 18% (n=92) would select family medicine as a profession. The most frequent rationale for choosing the field of Family Medicine was the variety of patients seen in general practice (55.4% n=51). Conclusion: There is a dire need to focus on increasing awareness about the field of family medicine among medical students. The students should be counseled on the advantages along with the disadvantages of choosing this field as a medical profession.

Abstract Background In March 2017, Togo was declared the first country in sub-Saharan Africa to eliminate lymphatic filariasis as a public health problem, but post-validation surveillance has been lacking. In some areas of the country, migrant groups from neighboring countries that are still endemic for LF pose a risk of reintroduction of LF to Togo. The objective of this study was to identify the risk posed by migrant groups by measuring their prevalence of LF infection and investigating any positive case using Togo’s case investigation algorithm to prevent resurgence of LF and sustain Togo’s elimination success. Method A cross-sectional study was conducted in 2018 in the northernmost region of the country. Three migrant populations were identified: (i) nomadic Peuhls, (ii) Togolese members of local communities who migrate annually to neighboring countries for seasonal labor, and (iii) refugees from Ghana who came to Togo because of a communal conflict in Ghana. A questionnaire was designed to collect data on demographics and history of LF and MDA; all participants were tested for circulating filariasis antigen (CFA) using the filariasis test strip (FTS). Any CFA-positive case was confirmed with nocturnal microfilaremia. Results Refugees, seasonal economic migrants and nomadic Peuhls represented 42.1%, 31.4% and 26.5% of the study participants, respectively. The overall prevalence of CFA was 4.2% (58/1391) with the highest prevalence in the nomadic Peuhl group (11.9%), but only one of them (0.07%) was confirmed positive with nocturnal microfilaremia. Using the case investigation algorithm, no other positive case was identified in the positive case’s surroundings. Conclusion This study demonstrates that nomadic Peuhls, with a CFA prevalence of 11.9%, pose a potential risk for reintroduction of LF into Togo while Ghanaian refugees and seasonal economic migrants do not appear to pose a significant risk. Periodic monitoring of migrants, especially the nomadic Peuhl population, is a potential post-validation surveillance approach that could be used to promptly detect any LF cluster that may arise.
