Pelvic inflammatory diseases occupy a special place in the structure of general morbidity, and are polymicrobial in nature with dominance of opportunistic microorganisms, in particular bacteria of the family Enterobacteriaceae.The aim was to study the composition of the vaginal microbiota in women of reproductive age with pelvic inflammatory diseases, as well as to determine the sensitivity of isolated microorganisms to antibiotics and bacteriophages.The study included 70women of reproductive age, among them 37were diagnosed with colpitis and cervicitis, 33women in the comparison group (women screened for a diagnosis). Isolated microorganisms were identified by abdominoperineal methods, including the disk diffusion method to determine the sensitivity of microorganism cultures of Enterobacteriaceae family to antibiotics, and the method of crosses (evaluation of lytic activity of bacteriophages by the number of crosses) to determine the sensitivity to specific therapeutic bacteriophages.Vaginal biocenosis was characterized by deficit of lactobacilli (<106CFU/ml in 100%), the presence of conditionally pathogenic microflora: bacteria of Enterobacteriaceae family, coccal flora and Candida fungi. From 60.0 to 89.3% of Enterobacteria strains were resistant to aminoglycosides and quinolones, but also had a low level of sensitivity to therapeutic bacteriophages.The obtained data indicate the reduction of colonization resistance of vaginal mucosa in pelvic inflammatory diseases and specify the need to use medicinal drugs only under medical supervision to prevent clinically significant drug resistance.

Hemomicrocirculatory system – is a complex structure that reacts in every pathological process even before the clinical period and takes the first blow. The study of microhemocirculation will provide an opportunity to solve the important for practical medicine questions of pathogenesis of many diseases, as for the prevention and treatment of regional disorders of blood circulation.The objective of the research is to study the state of the hemomicrocirculatory bed (HMCB) of adventitia of varicose veins of the small pelvis (VVSP) in women with chronic inflammatory processes of the organs of the small pelvis (CIPOSP).Materials and methods of research. To evaluate the restructuring of the HMCB of adventitia of VVSP, the operating material of 12 women of reproductive age was used. Mainly, there were pieces of the ovarian vein. The study of the HMCB in the vein wall was performed by the non-injecting method of silver impregnation according to V.V. Kupriyanov. To standardize the results, the condition of the HMCB of adventitia of the venous wall in norm was studied in 5 women of reproductive age, who died as a result of various traumas.Results of the research. After the performed studies, the structural-morphological changes of the HMCB of the adventitia of the small pelvis veins were revealed. The dilation of capillaries, postcapillaries, postcapillary venules was observed. The diameter of the vessels of the HMCB of the ovarian vein adventitia was: venule – 94.21 ± 1.38 μM in comparison with the norm – 48.78 ± 1.60 μM (p<0.001); post-capillary venules – 46.76 ± 1.04 μM in comparison with the norm – 28.29 ± 1.1.01 μM (p<0.001); the capillaries were 11.22 ± 0.14 μM in comparison with the norm – 8.24 ± 0.16 μM (p<0.05), arterioles – 29.02 ± 0.76 μM in comparison with the norm – 25.19 ± 1.15 μM (p<0.01). The architectonics of the arterioles is almost unchanged. Lumen of venules is filled with formed elements. The structure of capillaries is polymorphic. The capillary net was localized and concentrated or was formed as a thick planar net, the capillaries were expanded. There were arterio-venulous anastomoses. Endothelial nuclei are shortened. In some preparations, the diameter of the arterioles corresponded to the diameter of the collection venules.  Conclusions:1. The first discovered by us changes in HMCB of adventitia of varicose veins of the small pelvis in women with CIPOSP can be one of the pathogenetic links of the development and progression of the varicose vein itself, which in turn aggravates the course of chronic inflammation.      2. The timely appointment of drugs that improve microcirculation will enable to prevent the development of dystrophic changes in the vein wall, improve the course of chronic inflammatory processes and reduce or completely eliminate the syndrome of “chronic pelvic pain”.

ABSTRACT Enterobacteria may gain antibiotic resistance and be potent pathogens wherever they are present, including in fresh fruits and vegetables. This study tested the antibiotic resistance of enterobacteria isolated from 13 types of local and imported fresh fruits and vegetables (n = 105), using the standard Kirby-Bauer disk diffusion method. Phenotypic and genotypic characterizations of AmpC β-lactamases were determined in cefoxitin-resistant isolates. Ten percent of the enterobacteria tested (n = 88) were pansusceptible, 74% were resistant to at least one antibiotic, and 16% were multidrug resistant. Enterobacteria isolates showed the highest antibiotic resistance against ampicillin (66%), cephalothin (57%), amoxicillin–clavulanic acid (33%), cefoxitin (31%), tetracycline (9%), nalidixic acid (7%), trimethoprim (6%), and kanamycin (5%). Three isolates showed intermediate resistance to the clinically important antibiotic imipenem. Escherichia coli isolated from lettuce exhibited multidrug resistance against five antibiotics. Fifteen isolates were confirmed to have AmpC β-lactamase, using the inhibitor-based test and the antagonism test; the latter test confirmed that the enzyme was an inducible type. Four types of ampC β-lactamase genes (CIT, EBC, FOX, and MOX) were detected in eight isolates: four Enterobacter cloacae isolates and one isolate each of Citrobacter freundii, Enterobacter asburiae, Enterobacter hormaechei, and Enterobacter ludwigii. It was concluded that fresh fruits and vegetables might play a role as a source or vehicle for transferring antibiotic-resistant bacteria that might spread to other countries through exportation. The clinically significant AmpC β-lactamase was rarely documented in the literature on bacteria isolated from fruits and vegetables, and to our knowledge, this is the first report on the detection of an inducible type in such commodities.

A Ottonia martiana Miq., arbusto da família Piperaceae, é conhecida na medicina popular brasileira como “Anestésica”. Partes desta planta são utilizadas em alcoolaturas para aliviar dor de dente, devido à sua ação anestésica sobre a mucosa bucal. Estudo fitoquímico realizado com extrato das raízes desta planta indicou a presença de duas isobutilamidas: Isopiperlonguminina e Piperovatina, as quais foramidentificadas por Espectroscopia de RMN de 1H e 13C. Análises por CG-MS nos extratos dos frutos desta espécie em estudo, revelaram a presença de dois isômeros, um do composto estigmasterol e outro do b-sitosterol. No estudo das atividades antimicrobianas, extratos brutos etanólicos das raízes e caules (50 e 100%) desta planta, apresentaram efeito antibacteriano sobre S. aureus, quando testados pelo método por difusão em gel. Também, foi verificado que extratos desta espécie apresentaramatividade antifúngica (crescimentomicelial) sobre Fusariumoxysporum, Colletotrichum acutatum e Rhizoctonia sp., dependente da concentração, do método de extração e do órgão utilizado. Os resultados confirmaram a presença de princípios ativos, os quais poderão vir a ser empregados na terapêutica, inclusive com potencial para controle de microrganismos fitopatogênicos. STUDY PHYTOCHEMICAL AND OF ANTIMICROBIAL ACTIVITY FROM Ottonia martiana Miq., PIPERACEAE Abstract Ottonia martiana Miq., a shrub from the family Piperaceae, is known in Brazilian folk medicine as “Anestésica”. Its parts are used in alcoholic macerates to relieve toothache, due to its anesthetic action on mucous membrane. Phytochemical investigation performed with extract of O. martiana roots indicated the presence of two isobutylamides: Isopiperlonguminine and Piperovatine, which were identified by spectroscopy NMR (1H and 13C). In analysis of the extracts of fruits by GC-MS, isomers of stigmasterol and b-sitosterol are present. In the studies of antimicrobial activity, ethanolic crude extracts of the roots and stems (50 and 100%) from this plant showed antibacterial effects against S. aureus, by paper disk-diffusion method. Also, it was observed that extracts of this species exhibit antifungical activity (mycelial growth) against Fusarium oxysporum, Colletotrichum acutatum and Rhizoctonia sp, dependent on the extract concentration, extraction method and employed organ. The results confirmed the presence of active substances, which could be employed in therapeutics, and with potential to control phytopathogenic microorganisms.

Background: Reproductive and child health clinic attendance is vital in offering family planning health education among women of reproductive age in Tanzania. During these visits, group education sessions on modern contraception is offered to women before receiving the specific services they went for. Those who show interest with modern contraception proceed to individualized sessions on adopting one of the methods. Therefore, being satisfied during the group session is paramount to adopting modern contraception. This study therefore sought to assess the level of client satisfaction with the family planning health education among women of reproductive age from selected clinics in Dar es Salaam. Methodology: This was an observational study conducted in selected Reproductive Health clinics in three municipalities within Dar es Salaam region. Facilities were grouped by districts and ownership from which one third were systematically picked to provide 8 participants each. Interviewer led completion of the study tool was employed to assess satisfaction parameters. Analysis on satisfaction was all or none from a 5-point rating scale and association calculated with significance set at a p-value of less than 5% and 95% confidence interval. Results: There was a general higher satisfaction with the family health education offered during the visit period. Satisfaction with family planning health education was significantly higher among women with higher parity (p=0.009), employed (p=0.00001), and were attended to by a clinician (p=0.0006). A significant disparity within municipalities was also noted (p=0.005). Private facilities, shorter time in facilities, and non-cash payments did not significantly influence satisfaction. Conclusion: Family planning health education satisfaction is high among women of the reproductive age group in Dar es Salaam clinics. We need to explore how this satisfaction is translated to reduce the unmet needs for modern contraception in Tanzania. Furthermore, whether this high satisfaction translates into good knowledge remains unexplored.

The results of the survey 596 women of reproductive age living in the service area of the Moscow Regional Perinatal Center are analyzed in the article. Among the women surveyed 91,2% had a positive attitude toward childbearing and they considered an important condition to create a happy family the presence of children. At that 58,4% of them would like to have in their family of two children and 25,7% - three or more. Only every second woman believes that she has a desired number of children in the family. Has been established that pregnancies in 10,3% cases ended artificial abortion and in 1,9% spontaneous abortion. Reliable differences have been revealed in the frequency of pregnancies depending on the level of education. It was established that with increasing ordinal number of pregnancy reliably reduced the proportion of births a living child. Each tenth of the women surveyed has reported about complications after abortion. 8,9% of respondents have been treated for infertility, 13,0% from the miscarriage. 56,4% of women used protection from pregnancy. With this purpose most often a condom was used and interrupted intercourse.

Background: Nosocomial infections are associated with increased morbidity, mortality, and medical burdens. Pseudomonas aeruginosa and Acinetobacter baumannii are not-fermentative gram-negative bacteria that considered as the most important nosocomial infection. In the current study, we have aimed to evaluate the sensitivity of Acinetobacter baumannii and Pseudomonas aeruginosa microorganisms to the colistin antibiotic. Methods: In this descriptive cross-sectional study, patients admitted to the ICU ward of Firoozgar Hospital from July 2018 to March 2019 were evaluated, and 169 Patients infected with Acinetobacter baumannii, and Pseudomonas aeruginosa were included. Acinetobacter baumannii and Pseudomonas aeruginosa were isolated, and antibiotic sensitivity was determined by the disk diffusion method according to Clinical & Laboratory Standards Institute (CLSI) criteria. E test was also used to determine MIC-50 and MIC-90 of colistin. Results: Acinetobacter baumannii was around 8 times more frequent than Pseudomonas aeruginosa. Colistin resistance was detected in only 4(2.4%). The mean age of patients infected by Acinetobacter baumannii was significantly higher than those infected with Pseudomonas aeruginosa. Moreover, the mean time of the hospitalization period did not show any significant differences in the different groups. Conclusion: Our findings indicated that the majority of isolated Pseudomonas aeruginosa and Acinetobacter baumannii were sensitive to Colistin. Therefore, it could be effectively used for patients with a confirmed diagnosis of Pseudomonas aeruginosa and Acinetobacter baumannii.

A pregnant cow of Kuri breed, estimated age of six years old with second calving was transported on land from Maiduguri, Borno state Nigeria. The journey took four days to the Akinyele International Cattle Market. The pregnant cow calved immediately on arrival at the market. A week after calving, the nursing cow was offered for sale and slaughter. The nursing cow and the calf were purchased and put under intensive medical and management care of an experienced cattle stockman in the market. The nursing cow became diarrheic on the second week of arrival at the market. Clinical examination by visual observation revealed projectile watery faeces, stained hindquarter and the pair rectal sterile swab samples were collected. Though the temperature and pulse rate were within the normal range. Standard bacteriological faecal culture of the rectal swab incubated overnight at 370C in selenith F- broth, Nutrient and blood agars at 370C for 24 hours were done, revealing heavy mixed growth of Escherichia coli, Klebsiella species and Bacillus species. Identification of colonies were based on morphology, gram staining, cultural and biochemical characteristics. In-vitro Antibiotic sensitivity testing on discrete colonies of bacteria faeces were done, using agar- Disk diffusion method. The E. coli, Klebsiella and Bacillus species were sensitive to ciprofloxacin® (10μg) and pefloxacin® (30μg). Sulfanol® (sulphadimidin) a broad spectrum antibiotic was administered immediately intramuscularly at 333mg in 3ml per 10kg for four days. Defaecation of the nursing cow stopped for two days after treatment. On the third day of the treatment, the faeces became pasty but the cow died overnight despite the good response to therapy.
