The success of complete dentures is highly dependent on the anatomy of the oral cavity. Several conditions can affect the final denture fit. Disorders of the soft tissues, such as denture hyperplasia, denture stomatitis and mucosal pathology may play a role. There are also difficulties related to the shape of the bone, including excessively large undercuts/bony exostoses, tori, enlarged tuberosities and flabby ridges and other factors, such as xerostomia to be considered. The aim of this article is to help diagnose these conditions and to suggest ways of overcoming the individual problems with which patients present before starting denture construction. CPD/Clinical Relevance: The diagnosis of several oral conditions and their appropriate management may help achieve an optimal outcome when constructing complete dentures.

ABSTRACT Objective: To compare the general clinical conditions and oral alterations, and also evaluate the prosthesis, in subjects diagnosed with Alzheimer's disease (AD) or Parkinson's disease (PD), attended at two geriatric centers in the city of Fortaleza - Ceará. Methods: 70 patients were analyzed (between 57 to 91 years) with AD and PD, through evaluation of neurological medical records and oral clinical examination. Variables as sex, age, race, comorbidities, oral manifestations and prosthetic conditions were observed. A total of 20 elderly patients without neurological disease consisted the control group (CG). Fisher's exact test and Kruskal-Wallis were used as statistical analysis. Results: In both groups, blood hypertension was the most frequent comorbidity. As for the oral examination in the group with AD, it was observed that 54.2% had periodontal disease, 34.2% caries. Similar results were found in PD patients (31.4%; 22.8% - respectively). However, no difference was observed between the groups, as the systemic and oral manifestations in both geriatric centers (p> 0.05). Was identified more than half of the subjects (57.7%) using dentures, and 86.5% of these had become maladaptive, with some kind of defect. There was statistical difference in the presence of associated oral pathology, denture stomatitis, between dementia and control group (p = 0.0213). Conclusion: Elderly patients with AD and PD have poor oral health, as well as those without neurodegenerative diseases. However, have more defects and disorders associated with the use of removable dentures.

Candida species including Candida albicans, Candida krusei and Candida glabrata are opportunistic microorganisms that inhabit oral cavity. The objective of this study is to determine the effect of dental caries on Candida spp. biofilm-forming ability on nano- composite with the hypothesis that dental caries enhances the colonization of Candida spp. To assess Candida spp. colonisation in the oral cavity of the paediatric patient, samples were obtained from 30 subjects aged five to six years old from Kuantan, Pahang, Malaysia. The samples were collected from buccal mucosa, palate and tooth surfaces using sterile swabs. 10 mL of patient’s saliva suspension was also collected. Following that, the samples were inoculated on CHROMagar and incubated for 24 h at 37 ºC. Candida biofilm of caries isolate C. albicans (HNFC2), and C. albicans ATCC 32354 were developed on three different types of nano-composites. The study showed that no C. albicans was isolated from the caries-free oral cavity while 76% of children with caries possessed Candida spp. 65% of the yeasts were isolated from the tooth surface. Only 35% of the total isolates were obtained from soft tissues, including palatal and buccal mucosa. C. albicans is the most isolated Candida spp. with 82% and 67% of the yeast were obtained from the tooth surface and buccal mucosa, respectively. Besides, HNFC2 significantly colonised the nano- composites more than the ATCC (P < 0.05). In the comparison of the three types of nano- composites, nano-hybrid-based containing pre-polymerised filler (cB) exhibited the least C. albicans HNFC2 cells colonisation with 7.7 x 10³ cells mL-1. In contrast, the nano-composite that contained bulk-filled nanohybrid (cC) was the most colonised with 14.3 x 10³ cells mL-1. In conclusion, dental caries enhances the colonization of Candida spp. in children's oral cavity, and that caries isolate form more biofilm on nano-composites compared to the lab strain C. albicans.

ABSTRACT Background Wearing a dental prosthesis is known to increase oral candidal colonization and predispose the wearer to oral candidosis. Denture wearers frequently use fingers to take the prosthesis out of their mouth. Oral Candida, if present may contaminate wearer's finger. The objective of this study was to investigate the simultaneous candidal colonization of oral cavity and fingertips of complete denture wearers. Materials and methods A total of 25 apparently healthy male subjects who had worn complete dentures for at least 1 year were selected. Information about each patient's denture age, denture hygiene, handling, and wearing habits, and hand washing habits after denture handling were be obtained. Intraoral examination of all the patients was done. For microbiological examination samples were collected from the fingertip and oral rinse of each patient. Candida species were identified with use of germ tube test and commercially available yeast identification system. Data was statistically analyzed. Significance was set at p < 0.05. Results It was found that frequency of hand washing, denture handling and denture stomatitis with respect to fingertip candidal isolation was not statistically significant. But poor denture hygiene and denture stomatitis with respect to oral candidal colonization was statistically significant. Conclusion Denture wearers with oral Candida had a higher prevalence of Candida contamination on their fingers. Patients with removable prostheses should be informed about the importance of proper prosthesis and personal hygiene and the possibility of microbial contamination of the hands and other parts of the body. Peripheral Ameloblastoma (PA) is the rarest variant of ameloblastoma. It differs from the other subtypes of ameloblastoma in its localization: it arises in the soft tissues of the oral cavity coating the tooth bearing bones. Generally, it manifests nonaggressive behavior and it can be treated with complete removal by local conservative excision. In this study we report a case of PA of the maxilla in a 78-year-old female patient and we describe the four different histopathological patterns revealed by histological examination. After local excision and diagnosis, we planned a long term follow-up: in one year no recurrence had been reported. The choice of treatment is illustrated in Discussion.

ABSTRACTRhabdomyosarcoma (RMS), a tumor of skeletal muscle origin, is the most common soft tissue sarcoma encountered in childhood and adolescence. The common sites of occurrence are the head and neck region, genitourinary tract, retroperitonium, and, to a lesser extent, the extremities. In the head and neck region, the most commonly affected sites are the orbit, paranasal sinuses, soft tissues of the cheek, and the neck. RMS is relatively uncommon in the oral cavity, and the involvement of the jaws is extremely rare. Here, we report a case of oral RMS in a 13-year-old child and describe the clinical, radiological, histopathological, and immunohistochemical findings. (Eur J Dent 2011;5:340-343)

Aggregatibacter actinomycetemcomitans is a periodontal pathogen colonizing the oral cavity of a large proportion of the human population. It is equipped with several potent virulence factors that can cause cell death and induce or evade inflammation. Because of the large genetic diversity within the species, both harmless and highly virulent genotypes of the bacterium have emerged. The oral condition and age, as well as the geographic origin of the individual, influence the risk to be colonized by a virulent genotype of the bacterium. In the present review, the virulence and pathogenicity properties of A. actinomycetemcomitans will be addressed.

Background:Curriculum documents identify key concepts within learning prosthetics. Threshold concepts provide an alternative way of viewing the curriculum, focussing on the ways of thinking and practicing within prosthetics. Threshold concepts can be described as an opening to a different way of viewing a concept. This article forms part of a larger study exploring what students and staff experience as difficult in learning about prosthetics. Objectives:To explore possible threshold concepts within prosthetics.Study design:Qualitative, interpretative phenomenological analysis. Methods:Data from 18 students and 8 staff at two universities with undergraduate prosthetics and orthotics programmes were generated through interviews and questionnaires. The data were analysed using an interpretative phenomenological analysis approach. Results:Three possible threshold concepts arose from the data: ‘how we walk’, ‘learning to talk’ and ‘considering the person’.Conclusion:Three potential threshold concepts in prosthetics are suggested with possible implications for prosthetics education. These possible threshold concepts involve changes in both conceptual and ontological knowledge, integrating into the persona of the individual. This integration occurs through the development of memories associated with procedural concepts that combine with disciplinary concepts. Considering the prosthetics curriculum through the lens of threshold concepts enables a focus on how students learn to become prosthetists.Clinical relevanceThis study provides new insights into how prosthetists learn. This has implications for curriculum design in prosthetics education.
