Introduction. The problem of maintaining the physical health of teachers in educational organizations is discussed in the article in the context of professional “burnout” and conflict prevention. Research methodology. When studying the content and essence of a comfortable educational environment, we were interested in the works that provided for an understanding of the nature of human interaction and reflected the interdependence and communication style of people in direct contact with each other. Research results. The most important factor of external and internal order, determining the content, direction and intensity of work activity, has always been a special atmosphere in the team, which in scientific and social discourse was defined by various terms such as: “microclimate”, “moral and psychological microclimate”, “sociological microclimate”, “comfortable educational environment”. Discussion. Designing refers to the category of innovative creative activity, since it primarily involves a purposeful transformation of reality, the need for which arises whenever there is an awareness of the necessity to move to a prognostic model of an educational or upbringing system. The main purpose of such “educational changes” consists in the possibility for a person himself/herself to learn and self-change during life, to apply knowledge in solving professional tasks. Conclusion. The term “comfortable educational environment" is used to denote a special atmosphere in the team that determines the content, direction and intensity of work activity and manifests itself in such group effects as team members’ mood and opinion, individual well-being, assessment of the person’s living and working conditions in the team. This special atmosphere is regarded on the whole as a result of people’s joint activity, their interpersonal interaction.

Introduction. The deterioration in the health of children and adolescents in the dynamics of education in general education organizations dictates the need to minimize the risk factors for the development of school-related diseases based on the development of a methodology for calculating the level of hygienic well-being of these organizations, taking into account existing health care opportunities (preserving and improving the health of students). The purpose of the study is to develop a comprehensive indicator for assessing the sanitary and hygienic well-being of educational institutions from the standpoint of health and safety. Material and methods. The methodology for the integrated assessment of the educational environment includes: a) defining a list of the most significant hygienic indicators from the standpoint of health preservation; b) an assessment of the weight of each of them; c) the development of a comprehensive indicator of sanitary and epidemiological well-being, combining individual indicators into one with its translation into a qualitative assessment. Results. Based on the materials of the research, a rationale for a comprehensive assessment of sanitary and hygienic well-being in educational organizations from the standpoint of health care has been proposed, that is, a technique is proposed, the use of which will help determine the full use of opportunities for maintaining and improving health in an educational organization. Discussion. Developed as a result of the conducted research, the algorithm for constructing a comprehensive indicator for assessing sanitary and hygienic well-being allows objectively characterizing the conditions and modes of education in educational institutions from the standpoint of health and safety, determining the risk factors of the educational environment and predicting the likelihood of school-related diseases. The construction of a complex indicator is proposed to be used as an additional assessment of the educational organization’s capacity to preserve the health of students in the dynamics of learning. Conclusion. The proposed model of an integrated assessment of the educational environment, which is based on quantitative (scoring) and qualitative assessments, identifying the resources for maintaining and promoting health in general education organizations, determines: risk factors of the educational environment; definition and prediction of the probability of changes in the students’ health status (positive and negative); justification of the system of management decisions to minimize risk factors and prevent violations in students’ health.

The authors of the article actualize the problem of identifying and justifying the targets for the formation of a psychologically comfortable and psychologically safe educational environment of rural school. The relevance of the problem is determined by modern socio-economic and sociocultural conditions, in which there are many risks and restrictions for psychologically comfortable and safe stay of a person in society. Modern studies of Russian scientists indicate that the psychological well-being of all participants in educational relations, as well as the favorable course of age-related crises and the successful socialization of students in society depend on the processes of improving the educational environment. The article presents the results of the work of the problem group «Targets for the formation of psychologically comfortable and safe environment in rural schools», which was organized during the All-Russian scientific and practical conference with international participation «The formation of psychologically comfortable and safe educational environment in rural schools». The participants of the problem group identified the essential characteristics and features of a psychologically comfortable and psychologically safe educational environment in rural school; we identified and identified as the key targets for the formation of this environment its purpose, the main methodological approach (subject-environment approach), the main functions, the leading idea, principles and goals for the formation of a psychologically comfortable and safe educational environment in a rural school. On the basis of the «Goal Tree» method, a hierarchy of goals for the formation of a psychologically comfortable and safe educational environment is built in three directions: the administrative and management team of rural educational organizations, students of rural educational organizations, teachers of rural educational organizations. The materials presented in the article can become a target reference point in the process of forming environments that meet the characteristics of psychological comfort and psychological safety.

Educational system is the most important social institution, which provides the development of a personality and frames his/her worldview and the conception of subjective well-being. Despite the large number of studies on the issue of psychological and pedagogical security of educational environment, the sphere of higher education still remains underexplored. The available empiric material has not received sufficient methodological justification and conceptualization. This article aims to provide the conceptualization of the risk category as applied to the educational environment of a higher education institution, as well as empiric investigation of the risks within the educational environment as viewed by applicants and students.Materials and methods. 213 applicants and 547 students of the Kuban State Technological University took part in the survey. The following investigation methods were used: expert assessment, interview, questioning, content analysis, analysis of means method, cluster analysis and factor analysis.Investigation results. Comparative examination of the manifestation degree of risks within the educational environment as viewed by applicants and students of higher education institution showed significant differences in the assessment of risk by these groups of respondents. From the point of view of applicants, the main risks are connected with social and psychological aspects of the educational environment of a higher education institution, relations within student groups and interaction with teachers, as well as with the issues related to the educational management and leisure arrangement. As viewed by the students, the risks within the educational environment include external and social and psychological risk factors, motivating risks and risks arising from the learning process organization and management and the related issues. Applicants’ notion of the risks within the educational environment of a higher education institution was mainly influenced by information they received from media and by the opinion of their relatives and age mates.Consideration and conclusions. It was proved that the applicants’ idea of the risks within the educational environment of a higher education institution is determined by the opinion of people they know personally, however it changes during the process of studying in higher education institution and acquiring their own experience of being within the environment. The investigation results allow creating a model of psychological and pedagogical security within the educational environment of a higher education institution. This model makes it possible to forecast main risks of the educational environment, their development, and evolvement and find solution to them.

Background:The research approach on Rheumatic diseases (RDs) is challenging and patient involvement as partners in medical research is an emerging force to obtain relevant information and to add unique skills, values and experiences to research. Despite growing interest in non-pharmacologic aspects of clinical research in RDs, the patients’ perspective is currently poorly explored. Objectives:To identify and rank the priorities for clinical research according to patients’ perspective. Methods:A structured face-to-face meeting between physicians and a patient representative was convened to list the non-pharmacologic topics relevant to RD patients. A cross-sectional no-profit on-line anonymous survey was devised to evaluate opinions of RD patients. They were asked to rate the following topics: food/nutrition, air pollution, smoking, work activity, social participation, physical activity, emotional well-being/stress, alternative medicine, and patient-physician relationship. Moreover, patients were asked to explain for which reason a topic was considered important (disease prevention, halting disease progression, symptoms control and disease cure). The survey was disseminated by ALOMAR (Lombard Association for Rheumatic Diseases) between June and October 2019.Results:200 rheumatic patients completed the survey: 130 inflammatory arthritis, 50 connective tissue diseases/vasculitis, and 20 among osteoarthritis, gout, condrocalcinosis, polymyalgia and primary fibromyalgia. Respondents were 178 female with median age of 50 years and median disease duration of 7 years. Among the nine topics identified, the one most rated by patients was the doctor-patient relationship; 188 (94%) of respondents considered this topic very or extremely important (see table below). In descending order, patients rated very or extremely important: psychological well-being/stress 185 (92.5%), physical activity 155 (77.5%), nutrition, eating habits and alcohol 150 (75%), alternative therapies 144 (72%), work activity 144 (72%), environmental pollution 134 (67%), social life 121 (60.5%) and cigarette smoke 119 (59.5%). The topics considered relevant was perceived to be able to influence disease symptoms. Regarding RD prevention, environmental pollution and cigarette smoking were considered the most important topics, while fewer patients believed that research on other topics could help to stop disease progression or to achieve disease healing.RankingTopicNot or quite importantVery or extremely important1Doctor-patient relationship, n (%)12 (6.0)188 (94.0)2Psychological well-being/stress, n (%)15 (7.5)185 (92.5)3Physical activity, n (%)45 (22.5)155 (77.5)4Nutrition/eating habits/alcohol, n (%)50 (25.0)150 (75.0)5Alternative therapies, n (%)56 (28.0)144 (72.0)6Work activity, n (%)56 (28.0)144 (72.0)7Environmental pollution, n (%)66 (33.0)134 (67.0)8Social life, n (%)79 (39.5)121 (60.5)9Cigarette smoke, n (%)81 (40.5)119 (59.5)Conclusion:This survey highlights the relevance of several unmet needs. The holistic approach, in terms of medical consultation and psychological well-being, is considered the most important component able to influence disease perception. By capturing patient opinions on non-pharmacological topics for clinical research, this survey indicates that the active patient involvement is essential to design successful translational studies and improve clinical outcomes.Acknowledgments:We thank the Lombard Association of Rheumatic Diseases (ALOMAR) for its contribution to plan and disseminate the survey and the group that sustain systemic sclerosis (GILS).Disclosure of Interests:Francesca Ingegnoli: None declared, Tommaso Schioppo: None declared, Tania Ubiali: None declared, Silvia Ostuzzi: None declared, Massimiliano Buoli: None declared, Valentina Bollati: None declared, Roberto Caporali Consultant of: AbbVie; Gilead Sciences, Inc.; Lilly; Merck Sharp & Dohme; Celgene; Bristol-Myers Squibb; Pfizer; UCB, Speakers bureau: Abbvie; Bristol-Myers Squibb; Celgene; Lilly; Gilead Sciences, Inc; MSD; Pfizer; Roche; UCB

Abstract Background Appropriate and well-resourced medical internship training is important to ensure psychological health and well-being of doctors in training and also to recruit and retain these doctors. However, most reviews focused on clinical competency of medical interns instead of the non-clinical aspects of training. In this scoping review, we aim to review what tools exist to measure medical internship experience and summarize the major domains assessed. Method The authors searched MEDLINE, Embase, PsycINFO, ERIC, and the Cochrane Library for peer-reviewed studies that provided quantitative data on medical intern’s (house officer, foundation year doctor, etc.) internship experience and published between 2000 and 2019. Three reviewers screened studies for eligibility with inclusion criteria. Data including tools used, key themes examined, and psychometric properties within the study population were charted, collated, and summarized. Tools that were used in multiple studies, and tools with internal validity or reliability assessed directed in their intern population were reported. Results The authors identified 92 studies that were included in the analysis. The majority of studies were conducted in the US (n = 30, 32.6%) and the UK (n = 20, 21.7%), and only 14 studies (15.2%) were conducted in low- and middle-income countries. Major themes examined for internship experience included well-being, educational environment, and work condition and environment. For measuring well-being, standardized tools like the Maslach Burnout Inventory (for measuring burnout), Patient Health Questionnaire-9 (depression), General Health Questionnaire-12 or 30 (psychological distress) and Perceived Stress Scale (stress) were used multiple times. For educational environment and work condition and environment, there is a lack of widely used tools for interns that have undergone psychometric testing in this population other than the Postgraduate Hospital Educational Environment Measure, which has been used in four different countries. Conclusions There are a large number of tools designed for measuring medical internship experience. International comparability of results from future studies would benefit if tools that have been more widely used are employed in studies on medical interns with further testing of their psychometric properties in different contexts.

Abstract Purpose To describe how health systems may respond to sudden changes in operations by leveraging existing resources and to share one organization’s experience responding to the coronavirus disease (COVID-19) pandemic. Summary In a health system based in Illinois and Wisconsin, pharmacy services are provided by a single, integrated department responsible for all aspects of pharmaceutical care within the organization. Hospital, retail, ambulatory care, and population health services are all managed under one leadership team. All pertinent ancillary services are also managed within the department, including informatics, supply chain, and drug policy. During the COVID-19 pandemic, the pharmacy services leadership has successfully managed volume and capacity challenges by redirecting resources to where they are needed. A disaster response framework based on Federal Emergency Management Agency guidance was put in place, and change management principles were used to rapidly operationalize change. Components of the nimble response have included quickly increasing capacity, thoughtful and timely communication to all team members, strategic decision making with available data, creating an agile pool of labor, and maintaining an efficient system supply chain. Well-being and resilience are emphasized alongside reflection on lessons learned. Some changes made in the urgent response to the pandemic are being considered for long-term implementation. Conclusion Organizations have the potential to respond to almost any situation if they are integrated and teams work together to build flexibility. The keys to success are thoughtful maximization of existing resources and strong communication.
