Background: Anemia in pregnant women in Brebes Regency, Central Java in 2016 was still high at 60.7%. Brebes Regency is an area of onion farming with a high intensity of pesticide use. Objectives: This study aimed to prove the history of pesticide exposure and lack of nutrient intake (iron and vitamin C) was risk factor for the incidence of anemia in pregnant women in agricultural area. Methods: Case control study with 41 cases (anemia) and 41 controls (non-anemia) as study subjects. The subjects were selected using purposive sampling method. Pesticide exposure history included the involvement of pregnant women in agricultural activities, frequency of exposure and length of exposure was measured by interview using structured questionnaire. Anemia data were obtained by measuring hemoglobin levels. Other independent variables measured were nutritional intake (protein, iron, and vitamin C) using semi-quantitative food frequency questionnaire (SQ-FFQ) method to determine the level of nutrient adequacy in pregnant women. Data were analyzed using chi-square test and logistic regression.Result: Hemoglobin levels in the case ranged from 7.2 g/dl to 10.9 g/dl. The results of multivariate logistic regression showed that pesticide exposure history (OR= 4.9, 95% CI:1.75-13.67), level of iron adequacy (OR = 2.9, 95% CI:1.06– 8.29) and level of vitamin C adequacy (OR = 3.4, 95% CI:1.10– 10.28) as risk factors for the incidence of anemia in pregnant women in agricultural area. Conclusion: The history of pesticide exposure and lack of nutrient intake (iron and vitamin C) was risk factor for the incidence of anemia in pregnant women in agricultural area.

Low birth weight (LBW) is one of the main causes of infant mortality. The problem that arises is whether there is a relationship between risk factors for pesticide exposure during pregnancy and the incidence of LBW in agricultural areas. The purpose of this study was to determine the risk factors for pesticide exposure associated with LBW incidence. This research is a case control study. Subjects were divided into two groups: a case group of 25 farmers with a history of giving birth to LBW and a control group who were neighbors of case subjects without a history of giving birth to LBW, a total of 25 farmers. This research data collection using interviews and observations. Data analysis used univariate and bivariate analysis (chi square and fisher as alternatives). Research results: risk factors for exposure to pesticides that have been shown to be associated with the incidence of LBW in Padangsidimpuan, include: occupation of pregnant women related to pesticides (OR = 6,769) completeness of personal protective equipment (PPE) while doing activities in the fields (OR = 18,857) and storage pesticides (OR = 12,667). The recommended advice is to avoid work that is directly related to pesticides during pregnancy, if you have to participate in agricultural activities, pregnant women should use complete PPE.

Anemia is one of the many problems that occur in pregnant women in developing countries. First trimester pregnant women often experience nausea and vomiting that can affect the pattern of eating that is received. The purpose of this study was to determine whether nutritional intake and nausea and vomiting disorders in first trimester pregnant women were associated with anemia. The sample of this study was all first trimester pregnant women, selected by quota sampling technique. The research design used in this study was cross sectional, which aims to examine the correlation between nutrient intake (iron, protein, and vitamin C) and nausea and vomiting disorders with anemia. The instrument used to determine the intake of iron, protein, vitamin C was food records that were filled by respondents for 3 days. To test the hypothesis of the relationship used the chi square test and Fisher's exact test. From the results of the analysis of the relationship between iron intake, Vitamin C and nausea and vomiting with anemia, p-value = 0.003, p-value = 0.001 and p-value 0.001, it can be concluded that there is a relationship between iron intake, vitamin C and vomiting nausea with the incidence of anemia in first trimester pregnant women (first). While the results of the analysis of the relationship of protein intake with anemia obtained p-value = 0.806, it can be concluded that there is no relationship between protein intake and the incidence of anemia in first trimester pregnant women (first). The mean intake of iron, protein, and vitamin C in first trimester pregnant women both in the anemic and non-anemic groups had a daily intake that was still far from the nutritional adequacy standard set by the government that the RDA of iron was 26 g / dL / day Protein was 76 mg / day and Vit C is 70-85 mg / day.

Background: Gestational diabetes mellitus (GDM) is a common cause of hyperglycaemia in pregnancy accounting for about 90% of all diabetic pregnancies. Women with GDM are at increased risk of maternal and fetal morbidity and mortality which are preventable through early diagnosis and treatment. Objective: The aim was to determine the prevalence of GDM, compare the maternal and neonatal complications among GDM and non-GDM pregnant women, and the risk factors associated with GDM. Methodology: A prospective cohort study was carried out among 105 pregnant women attending the antenatal clinic of RSUTH between February and August 2017. They were interviewed using a pre-structured questionnaire that covered variables related to socio-demographic factors and family, medical, and social history. Fasting blood sugar (FBS) was done after an overnight fast. Women who had FBS less than 7 mmol/L had 75 g Oral Glucose Tolerant Test (OGTT) done. Those diagnosed with gestational diabetes mellitus were the exposed group while those negative for GDM were the non-exposed group. Both groups were followed up to delivery, and maternal and fetal outcomes were noted. Statistical analysis was carried out using SPSS version 20 and significance set at p < 0.05. Results: The prevalence of GDM was 10.5%. Positive history of GDM in previous pregnancy was the only independent risk factor (p=0.04, Adj OR: 26.89, 95% CI 2.86 to 252.61). GDM mothers had a significantly higher risk of developing pre-eclampsia (RR=7.48; 95% CI =3.36 to 16.63). Neonates of GDM mothers were at increased risk of fetal macrosomia (RR =9.00; 95% CI=1.36 to 59.4) and neonatal admissions (RR=8.00; C.I =1.19 to 53.67). Conclusion: The study revealed that the prevalence of GDM was high and that those with GDM were at increased risk of developing fetal and maternal complications. A history of GDM in previous pregnancy was an essential risk factor for subsequent GDM.

Background: The prevalence of growth disorders among school-aged children in Indonesia is high (30.7%). Pesticides have been massively used in Indonesian agricultural areas. Objective: To determine if exposure to pesticides is associated with stunting among children in agricultural areas. Methods: This case-control study included 160 children (48 cases and 112 controls) aged 8–12 years. Exposure to pesticides was measured based on the history of the exposure since perinatal period, infancy, and childhood of the participants. Stunting was determined as a height for age z-score (HAZ) < -2 SD. Other variables measured were levels of thyroid stimulating hormone (TSH), insulin-like growth factor-1 (IGF-1), hemoglobin, zinc, albumin, nutrient adequacy level (energy and protein), and history of infection, low-birth weight (LBW), and mother's height. Results: There were no significant difference between the cases and controls in terms of in the baseline characteristics, except for the median IGF-1 level; it was significantly (p<0.001) lower in the cases (66.73 ng/mL) than the controls (112.57 ng/mL). High level of pesticide exposure (p=0.029) and low IGF-1 levels (p<0.001) were significantly associated with stunting. After adjusting for confounding variables, these variables were found to be independent risk factors for stunting in children (aOR 3.90, 95% CI 1.15 to 13.26; and aOR 8.35, 95% CI 3.65 to 19.14, respectively). Conclusion: Pesticide exposure could be a risk factor for the occurrence of growth disorders in children living in agricultural areas. Necessary actions should be taken to protect children living in agricultural areas from exposure to pesticides.

Introduction: Diabetic ketoacidosis (DKA) is a complication of diabetes mellitus which has a high risk of mortality. Mortality in DKA patients in developed countries is less than 5%, some other sources mention 5-10%, 2-10%, or 9-10%. Mortality events at clinics with simple facilities and elderly patients can reach 25-50%. The mortality rate of DKA patients is generally higher in infection conditions, especially in developing countries and in septic patients. Several factors such as age, sex, and high blood glucose can increase mortality risk of DKA patients. Other risk factors such as history of discontinued insulin therapy, impaired bicarbonate levels, pH, and increased leukocytes of DKA patients due to infection, abnormal albumin levels, electrolyte disturbances, and Serum Creatinine (SK) were thought to affect mortality of DKA patients. The purpose of this study was to determine the risk factors associated with mortality of DKA patients in Dr. Soetomo General Hospital Surabaya. Methods: The method used in this study was observational analytic involving 63 adult patients diagnosed with DKA with analysis using Chi-Square test. Results: From 63 patients included in this study, 37 patients diagnosed with DKA died and 26 patients lived. In a multivariate analysis, DKA severity with p = 0.001 (p < 0.005) was identified as having a relationship with mortality of DKA patients Dr. Soetomo General Hospital Surabaya. Conclusion: Severity is the only risk factor associated with mortality of DKA patients in Dr. Soetomo General Hospital Surabaya.
