The study objective is to evaluate the effectiveness of the Speroton complex therapy in combined treatment of oligoasthenospermia in patients with varicocele after microsurgical phlebectomy of varicose veins of the spermatic cord.Materials and methods. Microsurgical phlebectomy of the spermatic cord was performed in 40 men (mean age 33.0 ± 2.3 years) with oligoasthenospermia and varicocele. The patients were divided into 2 groups: the main group included 20 men who were treated with the Speroton complex for 3 months in the postoperative period, and the control group included 20 men who did not take Speroton. The spermogram parameters were evaluated before and after the operation.Results. Three months after the start of treatment, a statistically significant improvement in spermogram indicators was noted in all study groups. It was determined that combined therapy that included the Speroton complex administration for 3 months, statistically significantly led to the more considerable increase in sperm parameters than in control group: the sperm concentration increased to 38.6 and 28.9 mln/ml, respectively, the percentage of progressive motile sperm – to 49.2 and 39.4 %, the percentage of motile forms – to 72.2 and 43.8 %, viable forms – to 67.8 and 58.1 %, and the percentage of sperm with DNA fragmentation decreased to 12.1 and 17.2 %, respectively, in the main and control groups.Conclusion. Combined treatment with postoperative Speroton therapy for men of reproductive age with infertility and varicocele determines the more significant improvement in the parameters of sperm than performing only surgical correction of varicocele.

OBJECTIVE To evaluate the effectiveness of combined treatment of pathozoospermia in patients with varicocele by performing micro- surgical phlebectomy of varicose veins of the spermatic cord in combination with adjuvant therapy with the Speroton complex MATERIAL AND METHODS Twenty microsurgical phlebectomies of varicose veins of the spermatic cord were performed in patients with pathozoo- spermia (oligoasthenospermia) against the background of varicocele with the appointment of the Speroton complex in the postoperative period for 3 months with an assessment of spermogram parameters before and after the operation in comparison with a group of 20 men with pathozocelemia without the use of adjuvant therapy. The study of morphological and functional parameters of spermogram of men of reproductive age (18-49 years) without signs of immune infertility (MAR-test IgG <10%) before and after the start of treatment was carried out. The average age of men was 33 ± 2.3 years (M ± m). RESULTS All patients with infertility in combination with varicocele had oligoasthenozoospermia before treatment. Three months after the start of treatment in the study groups, there was a statistically significant improvement in sperm counts due to an increase in the concentration of spermatozoa in 1 ml of ejaculate, an increase in the number of progressively mobile forms and total motility of spermatozoa by 50-68%, an increase in the number of live sperm by 19-20% and a decrease in the num- ber of dead forms of spermatozoa by 30-35%. A statistically significant decrease in the DNA fragmentation index by 34-52% (p <0.05) was found in all study groups. It was found that combination therapy, including microsurgical phlebectomy of the spermatic cord veins using the Speroton complex for 3 months, statistically significantly led to an increase in the concentration of spermatozoa in 1 ml of ejaculate (38.6 [38.2; 69.4]), an increase in progressively mobile forms (49.2 [26.7; 47.2]%) and general motility (72.2 [35.3; 64.7]%) of spermatozoa and viable forms (67.8 [39.3; 81.7] %), a decrease in the DNA fragmenta- tion index compared with the spermogram indicators of patients who underwent only microsurgical phlebectomy (p <0.05 for all study indicators). In the group of patients after combined treatment with the inclusion of adjuvant therapy of the Speroton complex, there was a statistically significant decrease in the sperm DNA fragmentation index to almost normal values (12.1 ± 1.1%) compared with the group of patients who underwent only surgery (17.2 ± 0.8%) (p = 0.01). Combined treatment (micro- surgical phlebectomy of the veins of the spermatic cord in combination with adjuvant therapy Speroton) of pathozoospermia in men of reproductive age with infertility on the background of varicocele can significantly improve the morphofunctional parameters of spermogram after 3 months from the start of treatment compared to men who underwent only surgical correction of varicocele.

The objective of the work: to study the changes of the functional activity of the endothelium and the plasma-coagulation link of hemostasis in women with chronic inflammatory processes of the internal genital organs (CIPIGO) with and without varicose veins of the small pelvis (VVSP).Materials and methods: 68 women with chronic inflammatory processes of the internal genital organs against the background of varicose veins of the small pelvis (group 1); 52 women with chronic inflammatory processes of the pelvic organs without varicose veins of the small pelvis (group 2); 30 practically healthy women (control group) were examined before and after the treatment. The age of women was between 18 and 45 years old. All the patients of the study groups received the treatment offered by us, which included a diosmine-containing phlebotropic preparation normoven, 500 mg twice a day in continuous mode and a donor of nitric oxide tivortin aspartatum, 5 ml 3 times per os for 14 days each month during six months.Determination of concentration of endothelin-1, prostacyclin was performed by immunoassay using a set of reagents from the firm “Biomediсa” (Austria); the determination of D-dimers in blood plasma was performed by latex-test of the firm “Organon-Tekhnika”.Results of workIn women with CIPIGO with and without VVSP, the increased production of endothelin-1, which has a pro-aggregate and pro-inflammatory effect, was determined. At the same time, the decrease of the synthesis of prostacyclin, which has anti-aggregant properties, was noted. In evaluating the indicators of coagulograms in patients of the studied groups, the propensity for hypercoagulation was revealed, as evidenced by an increase in the level of fibrinogen and a rise of index of the phospholipid-dependent coagulation tests. The decrease of the percentage of fibrinolytic blood activity and the increase in plasma concentration of the molecular marker of thrombophilia D-dimer can be considered as activation of intravascular coagulopathy and the presence of endothelial dysfunction.Conclusions: 1. Our studies found endothelial dysfunction and violations of the plasma-coagulation linkage of hemostasis in both studied groups. 2. After the performed treatment, the improvement of the functional activity of the endothelium and normalization of the main parameters of the plasma-coagulation linkage of hemostasis were observed.

Catheter-based ultrasound-thrombolysis has been successfully used in a small clinical trial in order to enhance recombinant tissue plasminogen activator (rtPA)-fibrinolysis, for the treatment of spontaneous intracerebral hemorrhages (ICHs). The aim of this study was to investigate the ultra-early effects of ultrasound on hematoma and the surrounding brain tissue in a porcine ICH-model. To achieve this, 21 pigs with a right frontal ICH were randomly assigned to four groups: (1) drainage (n = 3), (2) drainage + rtPA (n = 6), (3) drainage + ultrasound (n = 6), and (4) drainage + ultrasound + rtPA (n = 6). The hematoma volume assessment was performed using cranial MRI before and after the treatments. Subsequently, the brain sections were analyzed using HE-staining and immunohistochemistry. The combined treatment using rtPA and ultrasound led to a significantly higher hematoma reduction (62 ± 5%) compared to the other groups (Group 1: 2 ± 1%; Group 2: 30 ± 12%; Group 3: 18 ± 8% (p < 0.0001)). In all groups, the MRI revealed an increase in diffusion restriction but neither hyper- or hypoperfusion, nor perihematomal edema. HE stains showed perihematomal microhemorrhages were equally distributed in each group, while edema was more pronounced within the control group. Immunohistochemistry did not reveal any ultra-early side effects. The combined therapy of drainage, rtPA and ultrasound is a safe and effective technique for hematoma-reduction and protection of the perihematomal tissue in regard to ultra-early effects.

Objectives: Studying on the variation in CA 72-4 levels of the gastric cancer’s patients before and after 10 days and 30 days surgery treatment. Materials and methods: The studying group included 42 gastric cancer’s patients who were examinated and treated in cancerology service of Hue University Hospital and gastroenterology service of Hue Central Hospital. The control group included 30 healthy normal examinated at Hue University Hospital. The study groups were clinical, endoscopic anatopathologic examination diagnosed with gastric cancer and quantitative levels of CA 72-4 in three times points: before surgerying, after surgerying 10 days and 30 days postoperatively. Rerults: The concentration of CA 72-4 in gastric cancer’s patients was 10.06 ± 16.49 U/ml. Clearly higher than the control group 1.2 ± 0.4 U/ml(p <0.01). The rate increased levels of CA 72-4 in gastric cancer’s patients before surgerying was 27.5% and the control group was 0%. After 10 days of surgery, CA 72-4 level was 5.56 ± 8.55 U/ml; 82.5% of patients have reduced levels of CA 72-4 and 17.5% no changes; there are 0% increased cases. After 30 days of surgery, CA 72-4 level was 3.79 ± 6,52 U/ml. CA 72-4 level 10 days after surgering have decreased significantly compared to before surgery (p < 0.05) and 30 days after surgery have decreased significantly compared to after 10 days (p < 0.05). 30 days postoperatively, 90% patients had reduced levels of CA 72-4, 10% no changes, no patient had increased levels of CA 72-4 and no patient be relapsed after 30 days of treatment. Conclusions: CA 72-4 concentrations before surgerying increased 27.5%, after surgery 10 days and 30 days reduced step by step, no case have increased CA 72-4 levels, no case relapsed after 30 days.

Background: Acne is a chronic disorder that affects almost 80% of adolescents and young adults, causing psychological and emotional distress. However, the current treatments for acne are either ineffective or have many side effects. This study was designed to confirm and objectively quantify the effect of a new non-drug combined therapy on acne.Methods: This study innovatively utilized ultrasound, which enhanced the absorption of aloe vera gel, and soft mask to make a purely physical method without any drugs. In both the treatment group and control group, the number of papules/pustules and the area of hyperpigmented lesions were counted, and a smart mirror intelligent face system was used before and after the combined therapy. Alterations in the skin functional index were recorded and analyzed statistically.Results: In the treatment group, the combined therapy significantly reduced the number of papules and the area of hyperpigmented lesions and improved skin roughness and local blood circulation. In the control group, there was no obvious improvement over 2 months.Conclusion: This study suggests that the new non-drug combined therapy significantly improved acne, which provided experimental evidence and treatment guidance for patients with mild to severe acne, especially patients with moderate acne. This new therapy may possibly be an appropriate method for patients who seek topical treatments with mild side effects and low antibiotic resistance rates.

Background. There are not enough publications devoted to the study of erythropoietin in human sperm. According to the results of these studies, the erythropoietin takes part in the regulation of spermatogenesis, affecting the synthesis of hormones, in particular steroid ones. Currently, the physiological and pathogenetic effects of erythropoietin on human ejaculate have not been thoroughly studied. In this regard, the study of this protein in the ejaculate in patients with diseases of the male reproductive system, as well as in their absence, is relevant.The study objective is to determine the concentration of erythropoietin in ejaculate samples of healthy and men with oligoastenozoospermia.Materials and methods. Samples of ejaculate of 52 men of reproductive age were examined. The ejaculate was examined using the SQA-V sperm analyzer (MES, Israel). According to the results of the study, two groups were identified: the main group (n = 18) with reduced fertility and the control group (n = 34) with normal spermogram indicators. In seminal plasma samples, the concentration of erythropoietin was determined by solid-phase enzyme immunoassay using the test system “Erythropoietin-IFA-BEST” (A-8776) (Vector-best LLC, Russia).Results. Erythropoietin was detected in all ejaculate samples, the results ranged from 9.37 to 193.95 mME / ml and varied 20.7 times (p = 0.3). The median concentration in the main group was 64.49 mME / ml (41.96; 118.16 mME / ml) and 1.36 times higher than the results of the comparison group, which were 47.16 mME / ml (18.15; 90.94 mME / ml). No statistically significant regularities were found between the concentration of erythropoietin and the indicators of ejaculate fertility (r <|0,3|).Conclusion. In oligoastenozoospermia, there is a tendency to increase the content of erythropoietin in the seminal plasma, which requires further research, taking into account a more detailed stratification of the groups examined for reasons that caused a decrease in the number of spermatozoa.

Aim To evaluate dynamics of biomarkers for endothelial dysfunction (ED), including endothelin-1 (ET-1) and von Willebrand factor (VWF) in patients with stomach cancer (adenocarcinoma) before and after polychemotherapy (PCT); to compare these results with respective values in healthy volunteers and patients with cardiovascular diseases (CVD); to study correlations of the ED biomarkers with indexes of instrumental evaluation of endothelial dysfunction.Material and methods The study included 75 participants, including 25 healthy volunteers (control group), 25 patients with documented CVDs (arterial hypertension + ischemic heart disease), and 25 patients of the main group with histologically documented stage II-IV stomach cancer (adenocarcinoma) who received different courses of PCT with platinum-based agents (oxaliplatin, cisplatin) and fluoropyrimidines (5 fluorouracil, capecitabin). Laboratory measurement of ED biomarkers, computerized nailfold video capillaroscopy (CNVC), and finger laser photoplethysmography (PPG) (methods for noninvasive evaluation of vascular wall and ED), electrocardiography, 24-h ECG Holter monitoring, and echocardiography (EchoCG) were performed for all patients of the main group prior to PCT and within one months after the last course completion. This evaluation was performed once for healthy volunteers and patients of the CVD group upon inclusion into the study.Results In the main group, ET-1 levels were non-significantly lower than normal and did not change during the courses of antitumor treatment (0.95 [0.6; 1.4] and 0.94 [0.7; 1.4] pg /ml (р<0.9) before and after PCT, respectively). Statistically significant differences were found between the control group and oncological patients after the treatment (р<0.04). Levels of VWF remained within the normal range in all examined participants and did not significantly differ between study groups, including oncological patients before and after the specific treatment (р>0.05 for all comparisons). The correlation analysis detected significant correlations of ET-1 levels with functional disorders of microcirculation, ET-1 with the occlusion index (rs=0.56; p=0.005), ЕТ-1 with percentage of capillary restoration (PCR, rs= –0.72; p=0.018) and with the incidence rate of supraventricular extrasystole (rs=0.48; p=0.032).Conclusion The dynamics of ED biomarkers was studied for the first time in patients with stomach cancer receiving a specific antitumor therapy. Although no significant changes in ЕТ-1 and VWF were observed during the PCT (probably due to exhaustion of the endothelial system and a small patient sample), these indexes can be considered as early vasculotoxicity markers due to the presence of significant correlations with indexes of impaired endothelial function according to the results of instrumental evaluation.

Objective: To evaluate the effect of ketoprofenphonophoresis and femoral nerve block in knee osteoarthritis. Methods: One hundred and fourteen patients with knee osteoarthritis were randomly divided into two groups. The control group consisted of 57 patients who were treated with only ketoprofenphonophoresis. The experimental group consisted of patients were treated with ketoprofenphonophoresis and femoral nerve block. The patients were followed for more than one month. The treatment effect assessed by observing their WOMAC index before and after therapy. Results: All the patients finished the follow-up. Their WOMAC indexes were reduced after therapy and one month after treatment, especially in the experimental group (P < 0.05). The effective efficiency of ketoprofenphonophoresis and femoral nerve block were higher than that in the control group after treatment and after one month of follow-up (P < 0.05). Conclusion: Compared with only ketoprofenphonophoresis, the combined treatment of ketoprofenphonophoresis and femoral nerve block was more appropriate in knee osteoarthritis.

Objective: The study was done to compare the effect of selected yoga techniques, drugs, and combined therapy in depression in geriatrics. Methods: 75 patients between 65 and 75 years of age with depression were included in the study which was assessed by HAM D Score. The study population was randomly allocated to one control group and two study groups. The control group was given Aswangandha and Vacha churna with warm water as anupana after food, selected yoga techniques in the first study group, and combined yoga and the drug were given to the second study group. Outcome variables were assessed for the groups after the 30th day and follow-up after 45 days. Results: The results obtained in the study were analyzed using the ANOVA test. Yoga along with drug experienced a comparatively greater reduction in HAM-D score after treatment and follow-up (p<0.05) than other groups. Conclusion: Holistic approach can bring a better and static result than applying the treatment modalities alone.

Abstract Background: Fluoxetine enhances the levels of brain-derived neurotrophic factor (BDNF); considering its known improving effects on neurogenesis and plasticity, it seems to improve the Anterior Ischemic Optic Neuropathy (AION). This study aimed to evaluate the effect of Fluoxetine on clinical prognosis of patients with AION.Methods: In this double-blind placebo-controlled randomized clinical trial, subjects with AION who were referred to Rasool Akram Hospital were divided into two study groups; the fluoxetine group that received 20 mg Fluoxetine daily(n=50) and the control group (n=50) that received placebo for a period of six months. Patients underwent clinical and paraclinical evaluations before and after the trial. This study was a registered trial with IRCT code IRCT20181109041596N1.Results: One hundred patients were enrolled from August 2019 to December 2020 and assessed in this study. Subjects in Fluoxetine group showed significant improvement in visual acuity in comparison to the placebo group with less score in LogMAR scale (P: 0.008 and 0.002, respectively), improvement in MD parameters of perimetry (P: 0.003 and 0.002, respectively), and decrease in VEP latencies (P (in 1st minute): <0.001 and <0.001, P (in 15st minute): 0.038 and 0.011, respectively). There were no differences in color vision, Rnfl in all dimensions, PSD parameter of perimetry or VEP amplitudes following the trial of Fluoxetine therapy (Ps> 0.05).Conclusion: Fluoxetine showed promising therapeutic value for patients with AION besides its safety as an additive treatment option to corticosteroids.
