Aim To evaluate dynamics of biomarkers for endothelial dysfunction (ED), including endothelin-1 (ET-1) and von Willebrand factor (VWF) in patients with stomach cancer (adenocarcinoma) before and after polychemotherapy (PCT); to compare these results with respective values in healthy volunteers and patients with cardiovascular diseases (CVD); to study correlations of the ED biomarkers with indexes of instrumental evaluation of endothelial dysfunction.Material and methods The study included 75 participants, including 25 healthy volunteers (control group), 25 patients with documented CVDs (arterial hypertension + ischemic heart disease), and 25 patients of the main group with histologically documented stage II-IV stomach cancer (adenocarcinoma) who received different courses of PCT with platinum-based agents (oxaliplatin, cisplatin) and fluoropyrimidines (5 fluorouracil, capecitabin). Laboratory measurement of ED biomarkers, computerized nailfold video capillaroscopy (CNVC), and finger laser photoplethysmography (PPG) (methods for noninvasive evaluation of vascular wall and ED), electrocardiography, 24-h ECG Holter monitoring, and echocardiography (EchoCG) were performed for all patients of the main group prior to PCT and within one months after the last course completion. This evaluation was performed once for healthy volunteers and patients of the CVD group upon inclusion into the study.Results In the main group, ET-1 levels were non-significantly lower than normal and did not change during the courses of antitumor treatment (0.95 [0.6; 1.4] and 0.94 [0.7; 1.4] pg /ml (р<0.9) before and after PCT, respectively). Statistically significant differences were found between the control group and oncological patients after the treatment (р<0.04). Levels of VWF remained within the normal range in all examined participants and did not significantly differ between study groups, including oncological patients before and after the specific treatment (р>0.05 for all comparisons). The correlation analysis detected significant correlations of ET-1 levels with functional disorders of microcirculation, ET-1 with the occlusion index (rs=0.56; p=0.005), ЕТ-1 with percentage of capillary restoration (PCR, rs= –0.72; p=0.018) and with the incidence rate of supraventricular extrasystole (rs=0.48; p=0.032).Conclusion The dynamics of ED biomarkers was studied for the first time in patients with stomach cancer receiving a specific antitumor therapy. Although no significant changes in ЕТ-1 and VWF were observed during the PCT (probably due to exhaustion of the endothelial system and a small patient sample), these indexes can be considered as early vasculotoxicity markers due to the presence of significant correlations with indexes of impaired endothelial function according to the results of instrumental evaluation.

The objective of the work: to study the changes of the functional activity of the endothelium and the plasma-coagulation link of hemostasis in women with chronic inflammatory processes of the internal genital organs (CIPIGO) with and without varicose veins of the small pelvis (VVSP).Materials and methods: 68 women with chronic inflammatory processes of the internal genital organs against the background of varicose veins of the small pelvis (group 1); 52 women with chronic inflammatory processes of the pelvic organs without varicose veins of the small pelvis (group 2); 30 practically healthy women (control group) were examined before and after the treatment. The age of women was between 18 and 45 years old. All the patients of the study groups received the treatment offered by us, which included a diosmine-containing phlebotropic preparation normoven, 500 mg twice a day in continuous mode and a donor of nitric oxide tivortin aspartatum, 5 ml 3 times per os for 14 days each month during six months.Determination of concentration of endothelin-1, prostacyclin was performed by immunoassay using a set of reagents from the firm “Biomediсa” (Austria); the determination of D-dimers in blood plasma was performed by latex-test of the firm “Organon-Tekhnika”.Results of workIn women with CIPIGO with and without VVSP, the increased production of endothelin-1, which has a pro-aggregate and pro-inflammatory effect, was determined. At the same time, the decrease of the synthesis of prostacyclin, which has anti-aggregant properties, was noted. In evaluating the indicators of coagulograms in patients of the studied groups, the propensity for hypercoagulation was revealed, as evidenced by an increase in the level of fibrinogen and a rise of index of the phospholipid-dependent coagulation tests. The decrease of the percentage of fibrinolytic blood activity and the increase in plasma concentration of the molecular marker of thrombophilia D-dimer can be considered as activation of intravascular coagulopathy and the presence of endothelial dysfunction.Conclusions: 1. Our studies found endothelial dysfunction and violations of the plasma-coagulation linkage of hemostasis in both studied groups. 2. After the performed treatment, the improvement of the functional activity of the endothelium and normalization of the main parameters of the plasma-coagulation linkage of hemostasis were observed.

Objective: study of psychoemotional-volitional sphere and endothelium-dependent factors of cardiovascular risk, the level of performance and recovery time in athletes before and after a 2-month intake of a specialized athlete’s nutrition product (SANP) honey bar-candy “Gold F25 ApiSpeis Light”.Materials and methods: the main group of 58 athletes (under the age of 18) used SANP for 2 months. Control group-1 included 32 schoolchildren and students, not athletes, to assess the age limits of endothelium-dependent vasodilation, blood composition and the presence of risk factors for atherosclerosis in young athletes. The control group-2 included 30 young athletes under 18 years old without the use of SANP to compare the indicators in the main group before and after taking the honey bar. The psycho-emotional sphere was studied using the Spielberger-Hanin scale of anxiety and WAM testing: Well-being, Activity and Mood. In addition, the maximum achieved load, maximum oxygen consumption, time to reach the threshold of anaerobic metabolism, systolic blood pressure, heart rate, neurovegetative stress index and recovery time after exercise were determined. Also, tests with reactive hyperemia (TRH) and hyperventilation (TGV) on the brachial artery were performed to assess endothelium-dependent vasodilation and vasoconstriction, conjunctival biomicroscopy on a slit lamp with calibrometry of arterioles and venules, general and biochemical blood tests.Results: the normalization of situational anxiety, psychoemotional stress and hyperfunction of the sympathoadrenal system was established. There was a decrease in the levels of cortisol, neurovegetative stress index, improvement of well-being, activity and mood against the background of elimination of endothelial dysfunction, improved performance and reduced recovery time after work. The analysis of multiple canonical correlations revealed a close relationship between the psychoemotional state, endothelial dysfunction and microcirculation spasm (R = 0.86; p < 0.0001).Conclusion: it has been proven that a combination of increased situational anxiety, neurovegetative overexertion, hyperfunction of the sympathoadrenal system and endothelial dysfunction can form a spastic type of peripheral circulation and provoke the onset of arterial hypertension. It was found that the course intake of honey ingot has anti-stress, antioxidant, erythropoietic, vasodilating and antispastic effects, improves performance and recovery after stress. Ingot “Gold F25 ApiSpeis Light” is included in the FMBA Formulary as a specialized product for the nutrition of young athletes of the Russian national teams.

Objectives: Studying on the variation in CA 72-4 levels of the gastric cancer’s patients before and after 10 days and 30 days surgery treatment. Materials and methods: The studying group included 42 gastric cancer’s patients who were examinated and treated in cancerology service of Hue University Hospital and gastroenterology service of Hue Central Hospital. The control group included 30 healthy normal examinated at Hue University Hospital. The study groups were clinical, endoscopic anatopathologic examination diagnosed with gastric cancer and quantitative levels of CA 72-4 in three times points: before surgerying, after surgerying 10 days and 30 days postoperatively. Rerults: The concentration of CA 72-4 in gastric cancer’s patients was 10.06 ± 16.49 U/ml. Clearly higher than the control group 1.2 ± 0.4 U/ml(p <0.01). The rate increased levels of CA 72-4 in gastric cancer’s patients before surgerying was 27.5% and the control group was 0%. After 10 days of surgery, CA 72-4 level was 5.56 ± 8.55 U/ml; 82.5% of patients have reduced levels of CA 72-4 and 17.5% no changes; there are 0% increased cases. After 30 days of surgery, CA 72-4 level was 3.79 ± 6,52 U/ml. CA 72-4 level 10 days after surgering have decreased significantly compared to before surgery (p < 0.05) and 30 days after surgery have decreased significantly compared to after 10 days (p < 0.05). 30 days postoperatively, 90% patients had reduced levels of CA 72-4, 10% no changes, no patient had increased levels of CA 72-4 and no patient be relapsed after 30 days of treatment. Conclusions: CA 72-4 concentrations before surgerying increased 27.5%, after surgery 10 days and 30 days reduced step by step, no case have increased CA 72-4 levels, no case relapsed after 30 days.

In this study, we investigated serum epidermal growth factor (EGF) in an oncological population of head- and neck and pulmonary neoplasms and whether serum EGF could serve as a prognostic marker of survival and as a predictive marker for treatment response to platinum-based chemotherapy. A total of 59 oncological patients and a control group of age- and sex-matched healthy volunteers were included in this study. Pre-treatment serum EGF from both groups was determined. Patient’s and tumour characteristics and mortality were recorded during a 5-year follow up period. Baseline serum EGF significantly differed between the oncological patients and the healthy volunteers (p<0.001). Serum EGF was associated with lymph node metastasis (p = 0.004) but not with sex (p = 0.753), age (p = 1.00), TNM stage (p = 0.191) or tumour size (p = 0.077). Neither serum EGF (p = 0.81) nor age (p = 0.55) showed an effect on the patient’s survival. Tumour location was significantly associated with overall 5-year survival (p = 0.003). The predictive capacity of serum EGF of response to chemotherapy was limited (AUC = 0.606), a sensitivity of 80% and a specificity of 56% was observed resulting in a likelihood ratio of a positive and negative test equal to 1.81 and 0.36, respectively. In conclusion, serum EGF levels are 5.5 times higher in an oncological population compared to a control group. Within the oncological population, low serum EGF values are associated with the presence of lymph node metastasis. Further investigation is necessary to determine if the serum EGF levels could serve as a diagnostic biomarker.

The circumoval precipitin test (COPT), enzyme-linked immunosorbent assay (ELISA) and the immunoblotting anti-adult worm antigen (AWA) and soluble egg antigen (SEA) tests were applied to 17 chronically schistosome-infected patients for the detection of anti-Schistosoma mansoni antibodies before and on four occasions after oxamniquine administration over a period of six months. Compared to a control group, schistosomiasis patients showed high levels of IgG antibodies in AWA and SEA-ELISA. A decrease in IgG levels was observed six months after treatment, although negative reactions were not obtained. Significant decreases in IgG1, IgG3 and, mainly, IgG4, but not anti-SEA IgG2 levels were observed six months after treatment, again without negativity. Analysis of anti-AWA IgG antibodies by immunoblotting before treatment showed a 31 kDa strand in 14 patients (82%) which disappeared in three cases up to six months after treatment; furthermore, anti-SEA IgG antibodies showed the same band in nine patients (53%) before treatment, which disappeared in only four cases up to six months after treatment.

Objective.During pregnancy cytokines and inflammatory mediators stimulate the expression of prostaglandin, the levels of which determine the onset of labor. The aim of this work was to study interleukin IL-1β, IL-6 and IL-8 levels in the vaginal discharge, serum and urine of pregnant women with genitourinary infection before and after specific treatment. One hundred and fifty-one patients were studied during the second or third trimester of their pregnancy.Methods.The selected patients were: healthy or control group (n= 52), those with bacterial vaginosis (n= 47), those with vaginitis (n= 37), those with asymptomatic urinary infection (n= 15) and post-treatment. The level of cytokines was assayed by ELISA test. The Mann–Whitney U-test was used for statistical analysis.Results.The IL-1βlevels in vaginal discharge were: control 103.5 ± 24.2 pg/ml, bacterial vaginosis 1030 ± 59.5, vaginitis 749.14 ± 66.7l (p< 0.0001), post-treatment 101.4 ± 28.7. IL-6 values were similar in both control and infected groups, and there were no patients with chorioamnionitis. In vaginal discharge IL-6: control 14.2 ± 3.9 pg/ml, bacterial vaginosis 13.2 ±3.8, vaginitis 13 ±4.2. IL-8 levels were: control 1643 ± 130.3 pg/ml, bacterial vaginosis 2612.7 ±257.7, vaginitis 3437 ± 460 (p< 0.0001), post-treatment 1693 ±126.6. In urine the results were: control 40.2 ±17 pg/ml, asymptomatic urinary infection 1200.7 ±375 (p< 0.0001). In patients with therapeutic success both IL-1βand IL-8 returned to normal levels.Conclusions.Genitourinary infections induce a significant increase in IL-1βand IL-8 levels in vaginal secretions, and IL-8 in urine as well. Both cytokines could be useful as evolutive markers of infection.

Inflammation and endothelial dysfunction may play an important role in the multifactorial pathogenesis of hypertension. Endocan is also thought to play a role in cell adhesion and inflammatory disorders. The aim of the study was to compare endocan concentrations in patients with primary hypertension and healthy volunteers. There were 104 patients with hypertension (study group) and 21 healthy volunteers (control group). The correlation between endocan, catecholamines, and blood pressure control in patients with primary hypertension and the control group was analyzed. The median endocan concentration in the study group (2.03 ng/mL) was significantly higher than in the control group (1.09 ng/mL, P = .0001). Endocan concentration was correlated positively with renalase ( r = .2, P = .047) and norepinephrine ( r = .25, P = .02). Negative correlation was observed between endocan and body mass index ( r = −.25, P = .016) and leukocyte count ( r = −.36, P = .0004). The present study reports higher plasma endocan concentration in patients with treated, well-controlled primary hypertension compared with healthy volunteers. The higher endocan concentration in the study group may reflect endothelial dysfunction in this population.

Introduction. Mesotherapy is a widely used method of preventing external signs of aging in cosmetology. However, drugs that have proven effectiveness are not enough, therefore epiphyseal peptides are of great interest. Purpose: is to study the effect of intradermal administration of peptide epiphysis on the functional para-meters of the skin. Material and methods. There were 38 women in the research devided into 2 comparable groups. The main group (26 persons) was treated by mesotherapy with epiphyseal peptides. Face, neck and decollete zones were treated by the method of “deep nappage” once a week, 5 procedures per course. Isotonic solution of NaCl was similarly injected into the neck and decollete areas of the control group (12 people). The moisture content (Corneometer CM 825, “Courage & Khazaka”, Germany) and the elasticity (Cutometer MPA 580, “Courage & Khazaka”, Germany) of the skin were determined. Functional skin parameters were evaluated before and after treatment (in both groups) and also on the 1st and 6th months after mesotherapy in the main group. Results. Under the influence of epiphysis peptides the moisture content of the skin increases by 10% after treatment (in the control group without changes) and after 1 month, then decreases by 5% in 6 months below the initial values. In the main group R0 and R8 parameters increase immediately after treatment by 21% and 20% (in the control group R0 and R8 decrease by 4-6%), then decrease by 6-7% and by 28%. The R4 parameter increases immediately after treatment by 20-25% (in the control group it decreases by 13%), then decreases by 12% and 24%; F0 and F1 parameteres increase immediately after treatment by 17% and 22% (in the control group they decrease by 4% and 3%), then decrease by 11% and 9%, by 25% and 28%. Conclusion. The revealed dynamics of skin functional indexes may support the stimulating effect of peptide epiphysis on the cellular elements of the dermis (activation of metabolic processes after treatment and improvement of elasticity parameters by 25-28% after 6 months). Mesotherapy with epiphyseal peptides can be recommended as a method of prevention and correction of age-related skin changes.

The objective: peculiarities of pregnancy in women with common worm infestations in Baku. Materials and methods. During the period from 2015 to 2017, 152 women with common worm infestations were examined, in the gestation period from 16 to 40 weeks in the age group from 18 to 38 years. Depending on the nature of the infestation, 2 main groups were formed: the 1st main group included 84 women who had monoinvasia of helminths, and the 2nd main group included 68 women who had mixed helminth infestation. The control group consisted of 42 women who were pregnant without helminthiasis. All women were examined at 16–22, 23–29, 30–36 and 37–40 weeks of pregnancy. Helminthosis diagnostics was performed by detecting helminth eggs in faecal smears, pinworm eggs in scraping from perianal folds, and visual detection of helminth eggs or pinworms in faeces and vomit. Results. Common symptoms of early toxicosis are nausea, hypersalivation, and vomiting. A rare form of morning sickness treat dermatosis pregnant women, pregnant bronchial asthma, tetany, acute yellow atrophy of the liver, osteomalacia. Symptoms of toxicosis were evaluated in 152 pregnant women. The main symptoms of early toxicosis–nausea, hypersalivation and vomiting–were more common in pregnant women with helminthiasis than in women of the control group. In women with helminthiasis, a symptom complex similar to the manifestations of early toxicosis may persist throughout pregnancy, despite ongoing toxicosis therapy. Since persistent nausea and vomiting can be etiologically caused by helminthic-protozoic intoxication, the frequency of nausea and vomiting in different groups in the second and third trimesters was compared. In the second half of pregnancy, women with complaints of nausea and vomiting were present in all study groups, but they were more common in pregnant women with helminthiasis than in women in the control group. Conclusions. The data obtained on the more frequent manifestation of early toxicosis in pregnant women with helminthiasis and its demonstrated resistance throughout pregnancy prove that helminth infestations negatively affect the course of pregnancy. Abstract Background: Fluoxetine enhances the levels of brain-derived neurotrophic factor (BDNF); considering its known improving effects on neurogenesis and plasticity, it seems to improve the Anterior Ischemic Optic Neuropathy (AION). This study aimed to evaluate the effect of Fluoxetine on clinical prognosis of patients with AION.Methods: In this double-blind placebo-controlled randomized clinical trial, subjects with AION who were referred to Rasool Akram Hospital were divided into two study groups; the fluoxetine group that received 20 mg Fluoxetine daily(n=50) and the control group (n=50) that received placebo for a period of six months. Patients underwent clinical and paraclinical evaluations before and after the trial. This study was a registered trial with IRCT code IRCT20181109041596N1.Results: One hundred patients were enrolled from August 2019 to December 2020 and assessed in this study. Subjects in Fluoxetine group showed significant improvement in visual acuity in comparison to the placebo group with less score in LogMAR scale (P: 0.008 and 0.002, respectively), improvement in MD parameters of perimetry (P: 0.003 and 0.002, respectively), and decrease in VEP latencies (P (in 1st minute): <0.001 and <0.001, P (in 15st minute): 0.038 and 0.011, respectively). There were no differences in color vision, Rnfl in all dimensions, PSD parameter of perimetry or VEP amplitudes following the trial of Fluoxetine therapy (Ps> 0.05).Conclusion: Fluoxetine showed promising therapeutic value for patients with AION besides its safety as an additive treatment option to corticosteroids.
