Background: Management of intra parenchymal hematoma is a dilemma for neuroscientists, still a life-threatening entity which needs multidisciplinary approach.13% of all strokes constitutes hemorrhagic. Mortality and morbidity of patients depends on the status of initial presentation. Prognosis of the surgeries may vary from person to person and according to the comorbidities. The aim of the study was to determine effectiveness of decompressive craniectomy (DC) with hematoma evacuation in reducing the mortality.Methods: Patients with spontaneous intra parenchymal hematomas were selected and those having herniation syndrome were considered for surgery. Patients were evaluated after 30 days and 6 months and mortality was calculated.Results: 126 patients were selected for surgery, in that 102 patients underwent DC with hematoma evacuation and 24 patients who were not willing for surgery were considered as control group. Mortality of patients at 30 days of surgical group was 19.6% and 58.3% respectively, while 6 months mortality was 31.3% and 70.8% for surgical and control group.Conclusions: Surgical intervention can effectively reduce the mortality in spontaneous intra parenchymal hematomas.

The present study was conducted to investigate the efficacy of Ovsynch and Ovsynch plus protocol in postpartum (day 60) Sahiwal cows (n=18). Animals were randomly divided into three equal groups, viz., Ovsynch group Ovsynch plus group and Control group. Animals of group I (n = 6) were treated with traditional Ovsynch protocol. The animals (n = 6) of group II were treated with Ovsynch plus protocol which consisted of an initial intramuscular injection of eCG (Folligon) @ 250 IU on day 60 postpartum followed 3 days later by GPG (Ovsynch) protocol. In group-III Control, no treatment was given to animals (n = 6). Treated animals were inseminated at a fixed time between 14 and 20 hrs after second GnRH injection, irrespective of estrus detection. Blood samples were collected from each animal on days 50 and 60 postpartum to determine the status of cyclicity in animals based on serum concentrations of progesterone (P4). A third blood sample was collected on the day of prostaglandin treatment to determine the response of first GnRH injection. Four animals each were cyclic, and two were acyclic in both treatment groups. Four animals each responded to first GnRH treatment in both treatment groups. Similarly, two animals each got conceived giving conception rate of 50% (2/4) in each treatment. In the control group, one out of 6 animals got conceived yielding 16.66 % conception rate (1/6) during the study period. It may be thus concluded that Ovsynch and Ovsynch plus protocol may be used during the early postpartum period to improve the reproductive efficiency in postpartum Sahiwal cows.

Abstract Background People living with dementia seem to be more likely to experience delirium following hip fracture. The association between mental disorders (MD) and hip fracture remains controversial. We conducted a nationwide study to examine the prevalence of MD in geriatric patients with hip fractures undergoing surgery and conducted a related risk factor analysis. Material and methods This retrospective cohort study used data from Taiwan’s National Health Insurance Research Database between 2000 and 2012 and focused on people who were older than 60 years. Patients with hip fracture undergoing surgical intervention and without hip fracture were matched at a ratio of 1:1 for age, sex, comorbidities, and index year. The incidence and hazard ratios of age, sex, and multiple comorbidities related to MD and its subgroups were calculated using Cox proportional hazards regression models. Results A total of 1408 patients in the hip fracture group and a total of 1408 patients in the control group (no fracture) were included. The overall incidence of MD for the hip fracture and control groups per 100 person-years were 0.8 and 0.5, respectively. Among MD, the incidences of transient MD, depression, and dementia were significantly higher in the hip fracture group than in the control group. Conclusions The prevalence of newly developed MD, especially transient MD, depression, and dementia, was higher in the geriatric patients with hip fracture undergoing surgery than that in the control group. Prompt and aggressive prevention protocols and persistent follow-up of MD development is highly necessary in this aged society.

Background: Immunization is one of the most cost-effective public health interventions to reduce child mortality and morbidity associated with infectious diseases. The objective of this study was to determine the perceptions of caregivers on immunization in Ntungamo district.Methods: Quasi-experimental study was used with health centres assigned to intervention and control groups. Purposive sampling was used to select the two counties where the study was done. Proportional sampling was done to get study samples from each health facility, while systematic sampling was done to get study participants. A total of 787 children from twelve health facilities provided the study sample. A post intervention evaluation was conducted to determine the effect of these interventions. Association of variables was tested using Mann Whitney U-test and Chi-square.Results: On benefits, most caregivers in the intervention group (85.3%) and in the control group (54.3%) regarded immunization as very highly and moderately beneficial to their children respectively. On risks, most caregivers in the intervention group (85.5%) and control group (43.1%) regarded the risk factor associated with immunization as very low and moderate respectively. From hypothesis testing, there was a significant difference on the perceived benefits and risks of immunization between the intervention and control group.Conclusions: Most caregivers in the intervention and control group regarded immunization as very highly beneficial and moderately to their children respectively. Most of the caregivers in the intervention and control group regarded the risk factor associated with immunization as very low and moderate respectively.

Acute pancreatitis is sudden inflammatory disease of pancreas, which can vary from a mild form to severe life threatening condition. The management of pancreatitis usually consists of intensive care and multidisciplinary approach, often including surgical intervention or digestive endoscopy. In this article, we present a 68-year-old female with recidivous acute pancreatitis who underwent a series of endoscopic examinations and at the end also an unusual surgical intervention due to numerous complications. At first, it seemed that there was an idiopatic etiology because neither an anamnesis of alcohol consumption nor metabolic risks or CT signs of cholelithiasis were found. The condition was complicated by the development of acute necrotic collection, gastrointestinal bleeding and development of walled-off pancreatic necrosis (WOPN). Later, the biliary etiology was revealed after cholecystolithiasis was found on abdominal ultrasound. The WOPN was endoscopically drained because of the local compression syndrome. After the drainage, we noticed two cases of stent migration and the secondary infection of the WOPN. At the end, the migrated stents caused transient bowel obstruction and were stuck in the distal ileum. After three unsuccessful attempts to endoscopic extraction, the condition was solved by surgical intervention and double enterotomy was performed. The postoperative care was not easy anyway, being complicated by the dehiscence of the surgical wound with the need of opening the wound and use the VAC system to heal it up.

Background: Pregnancy induced hypertension (PIH) is one of the most common and potential life-threatening complications of pregnancy. This study is aimed to investigate and correlate the hemoglobin, haematocrit, white blood cell count, lymphocytes and platelets in PIH patients in their third trimester.Method: Total 126 subjects were studied out of them 63 patients (case) and 63 healthy pregnant women (control) visiting the Obstetrics and Gynaecology department Jhalawar Medical College, Jhalawar were registered in the study and followed during their pregnancy. Two millilitre of blood sample was drawn aseptically using the 5ml syringe from the median ante cubital vein of all the cases and control participants into EDTA-anticoagulated tubes. Haematological parameter calculated by using Sysmex XN-9100™ Automated Haematology System.Results: The mean hemoglobin level of the case group (8.8206±2.53779) was significantly lower than that of the control group (9.7289±2.47033) (p<0.05). The mean platelet count of the case group (131.4937±62.05999) was significantly lower than that of the control group (324.9683±230.78764) (p<0.05). The mean lymphocytes level of case group (1.2510±0.56369) was significantly lower than that of the control group (1.9295±1.4150) (p<0.05). The mean WBC level of case group (36.3467±119.90635) was significantly high than control group (11.5260±4.83059) (p<0.05). The mean haematocrit level of case group (32.6851±7.29789) was significantly high than control group (30.0424±23.38116) (p<0.05).Conclusion: The mean hemoglobin, mean platelets and mean lymphocytes are lower in PIH patients. The mean WBC and haematocrit are higher in PIH patient.

Background: Eclampsia is one of the most common causes of maternal and perinatal mortality and morbidity in India. Amongst the principles of management of eclampsia, the first is the control of convulsions. Magnesium sulphate is the main stay of treatment in eclampsia and imminent eclampsia. Average weight of Indian women is less than the western women due to which lower dose of magnesium sulphate can be used.  The aim of the study was to compare the efficacy of low dose magnesium sulphate regimen with standard Pritchard regimen in control of eclampsia.Methods: The study was carried out at emergency labour room, GMERS Sola Civil Hospital Ahmedabad. 120 patients of eclampsia were divided randomly into study group (n=60) receiving low dose MgSO4 and control group (n=60) receiving Pritchard regimen. The recurrence of convulsion, toxicity of MgSO4 and maternal and fetal outcome was studied.Results: It was observed that with low dose MgSO4 regime, convulsions were controlled in 91.7% of the cases. With standard Pritchard’s regime convulsions were controlled in 95% of patients. The maternal and perinatal mortality and morbidity were comparable in both groups.Conclusions: Low dose magnesium sulphate therapy is as effective as Pritchard’s regime for controlling convulsions in eclampsia and can be safely given in Indian women.

Background: Despite several reports on the clinical manifestations of sulfur mustard (SM) intoxication, there is no study on serum concentrations of thyroid hormones long-term after SM exposure. In this study, the changes in thyroid functioning parameters 20 yr after SM exposure were evaluated. Methods: This study is a part of a larger historical cohort study conducted in 2007 following 20 years of the exposure to SM, called Sardasht–Iran cohort study (SICS). We (SICS) comprised an SM–exposed group from Sardasht City, West Azerbaijan Province, Iran (n=169 as hospitalized group and n=203 as non-hospitalized exposed group); and control participants were selected from Rabat, a town near Sardasht (n=126). Peripheral blood samples were taken in fasting state and then the sera were separated. T4, T3, TSH, antithyroglobulin (anti–Tg), and antithyroid peroxidase (anti–TPO) concentrations in the sera were measured by the ELISA method. Results: The mean of T3 concentration was significantly higher in the exposed than control group (0.88 ± 0.26 nmol/L vs 0.8 ± 0.25 nmol/L, P<0.001). The levels of TSH, T4, and T3up were not significantly different between the exposed and control groups. Thyroglobulin level was significantly higher in the exposed non-hospitalized group (56.07 ± 140.22 µg/L vs 17.66 ± 41.49 µg/L, P=0.004), but the level of anti–Tg and anti–TPO showed no significant differences between the two groups. Conclusion: More studies are needed on the alterations in thyroid hormones, their gene expressions, and mechanisms involved in SM exposure to clarify the causes of these alterations.

They were added to page 89 on 17th May 2018.Anthelmintic resistance (AR) to commonly used dewormers is one of the major world-wide constrain in livestock production. The present study was investigated the status of AR in BAU dairy farm, Mymensingh and Talukder dairy farm, Sirajganj. Faecal egg count reduction test (FECRT) was applied to assess AR in cattle of two dairy farms during January to June 2017. The anthelmintics tested were Albendazole (ABZ), a benzimidazole anthelmintic (Almex®, Square Ltd.) and Ivermectin (IVM) (Vermic®, Techno drugs Ltd.), administered at the doses recommended by the manufacturers. In each farm, cattle were divided into treatment and control (not treated) group based on faecal egg counts (FEC), that is at least 200 eggs/g. At 14 days after treatment, faecal samples were collected for post-treatment FEC, which is compared between treatment and control group. Resistance was defined if there was <95% reduction, with lower 95% confidence limit (CL) <90% in the FEC. AR was present in both the dairy farms involved in this study. The FECRT using ABZ revealed 79.7% (95% CL 87.9, 65.8) reduction and 95.8% (95% CL 98.7, 87.1) reduction of FEC in BAU and Talukder dairy farms, respectively. Also, FECRT using IVM revealed 77.9% (95% CL 97.7, 85.5) and 94.2% (95% CL 97.7, 85.5) reduction of FEC in BAU and Talukder dairy farms, respectively. Our study suggest that AR is present in both selected dairy farms and further extensive studies are required to determine the extent of AR in different cattle farms of Bangladesh.Res. Agric., Livest. AbstractHuman hydatidosis is an important public health issue in India and many other developing countries. There is lack of data related to occurrence of this disease in certain risk groups. The present study was therefore designed to assess the seroprevalence of human hydatidosis among occupational risk groups in Punjab (India). One hundred and forty nine human serum samples from dog handlers, veterinarians/para veterinarians, dairy farmers, patients with liver disorders, HIV positives and control group (visiting hospital for other problems) were collected and analyzed using a commercially available enzyme linked immunosorbent assay (ELISA). The results revealed that 23 (15.43 %) subjects were seropositive for human hydatidosis. High seropositivity were recorded in dog handlers (30 %), followed by the other risk group populations. Relative risk of being infected was found to be 4.80 times high in dog handlers when compared to control group population. Future studies must be planned to determine the significance of these findings and the status of human hydatidosis in India.

Aging is a stage of life that increases the risk of physical diseases, some of which are chronic and can affect different dimensions of life’s quality and the status of psychological factors in these patients. In addition to medical supports, it is essential to provide sport complexes in accordance with the physical and mental condition of these patients. The aim of this study was to evaluate the effect of a period of aquatic therapy exercise on the quality of life and depression in aged males suffering from chronic physical pains.In a semi-experimental study, 30 aged males who voluntarily referred to Niyayesh health center of Shiraz were selected. They were randomly divided in to two groups; experimental (n=15) and control (n=15). Subjects of aquatic therapy exercise were involved in an 8-week-activity in water; 3 sessions per week each lasting approximately 50 to 70 minutes. At the same time, the control group was only followed-up and was not engaged in any exercise during this period. The variables of the quality of life and depression among the patients were measured by standard questionnaires in the beginning and end of the eight-week-aquatic therapy exercise. Results of this study showed that aquatic therapy exercise can be used as an effective and helpful method to promote public health in aged males suffering from chronic physical pains because it leads to the improvement of multi-dimensional variables of quality of life and depression in the patients.
