Abstract: Superficial dermatomycosis is a skin, nail and hair infection caused by fungal pathogen. Based on the pathogen, this infection can be divided into dermatophytosis, pityriasis versicolor and superficial candidiasis. The rapid and proper diagnosis is necessary to determine the initial theraphy and prevent the treatment delay. Superficial dermatomycosis diagnosis can be performed using anamnesis, physical examination or supporting investigation. The routine investigation method commonly use Potassium Hydroxide (KOH) because the KOH method is easy to be performed, rapid, simple and affordable. Chicago Sky Blue (CSB) is a dye to give a better color contrast to the fungi so the fungi would be easier to be detected. Objectives of this research is to observe the sensitivity and specifity difference of Chicago Sky Blue (CSB) dye and Potassium Hydroxide (KOH) methods for Superficial Dermatomycosis. The research was performed using cross sectional design analitical obsevation with 30 research subjects. The subjects consist of 15 superficial dermatomycosis patients and 15 non-superficial dermatomycosis patients. The samples were taken from the patients lesion swabs. The samples were checked using KOH and CSB, then observed by the medical analyst. The superficial dermatomycosis samples consist of mostly dermathophytosis (53.33%), then pityriasis versicolor (26.67%) and superficial candidiasis (20%). The sensitivity and specificity of KOH were 86.67% and 100%, respectively. The sensitivity and specificity of CSB were 93.33% and 100%, respectively. The CSB dye method has a higher sensitivity than KOH. The fungal elements are nicely dyed and more easily detected using CSB dye.  Keywords: KOH, Chicago Sky Blue, sensitivity, specificity, superficial dermatomycosis

Superficial dermatomycosis is a fungal infection of the skin, nail and hair. Based on the pathogen, this infection can be divided into dermatophytosis, phytiriasis versicolor and candidiasis. The rapid diagnosis for this infection is the key point to deliver the right theraphy. The supporting investigation for this infection can be performed directly using KOH. The KOH 20% solution has an indistict color contrast, that cause the fungal element seemed transparent. The observant expertise would be required for this case. The alternative method to enhance sensitivity and specificity of KOH is to add dyes such as Blue Black parker ink. The objectives of this research was to observe the sensitivity and specificity of KOH 20% + blue black parker ink in comparisson with KOH 20% method in superficial dermatomycosis. The research method was cross sectional design analitical obsevation with 30 research subjects. The samples were taken from superficial dermatomycosis lesion swabs. These samples were tested using KOH 20% solution and KOH 20% + blue black parker ink solution in Siti Khodijah Muhammadiyah Hospital, Sepanjang and dr. Makmuri Clinics, Surabaya. The results demonstrated that the sensitivity of KOH 20% solution and KOH 20% + blue black parker ink solution were 86.67% and 93.33%, respectively. The specificity of KOH 20% solution and KOH 20% + blue black parker ink solution were 100% and 100%, respectively. It can be concluded from the research that KOH 20% + blue black parker ink solution can be used for the routine diagnosis for superficial dermatomycosis due to the high sensitivity and specificity.

Parker ink blue black potassium hydroxide (KOH) is a regular stain for the diagnosis of dermatophytoses. KOH giving less color contrast to the fungal element. The Chicago Sky Blue (CSB) solution is a new contrast have various sensitivity and specificity values. This is analytical observational,cross sectional design study conducted at Dermatomycology Division of Outpatient Clinic, Dr. Soetomo General Hospital. 40 samples from lesion and perilesional infected area, diagnosed dermatophytoses with culture positive was include in this study. All two tests were performed examined by KOH and CSB. The slides were examined after incubation in x 10 and x 40 magnifications by experienced observer. The sensitivity, specificity and McNemar test of CSB and KOH was calculated using culture as the standard test. The sensitivities, specificities, PPV, NPV of the Chicago sky blue and KOH respectively are 95% and 100%, 95,24% and 100%, 100% and 100%, with McNemar test p = 1 and Kappa score is 0,950. The Chicago Sky Blue stain provides a good color contrast and IT could be a alternative staining for the dermatophytoses.

<p><strong><em>Background</em></strong><em>: Parkinson's disease (PD) is a chronic neurodegenerative disease that manifests as movement disorders. Based on motor symptoms, PD is classified into subtypes of tremor and postural instability gait disorders (PIGD). The motor symptoms subtype is a predictor of disease progression, therapeutic response, and quality of life for Parkinson's patients. The purpose of this study is to identify some  factors that influence motor symptoms in Parkinson's disease.</em><strong><em>Methods:</em></strong><em> This research use cross sectional design. Samples were selected by consecutive sampling method that met the inclusion and exclusion criteria. Research subjects were 58 people. Statistical analysis using SPSS. p values &lt;0.05 were considered statistically significant.</em><strong><em>Results:</em></strong><em> This research include 58 patients, 55.2% were men with range of age 63.5 ± 8.5 years old. The mean age at onset was 57.9 ± 9.5 years and duration of disease 6.1 ± 4.6 years. Motor symptoms 53.4% dominant tremor. There was a significant relationship between disease stage and motor symptom subtypes (p &lt;0.001). There is no relationship between the patient's age, age at onset and duration of the disease with motor symptom subtypes.</em><strong><em>Conclusions:</em></strong><em> There is a relationship between disease stage and motor symptom. The patient's age, age at onset and duration of the disease are not related to the motor symptoms of Parkinson's patients.</em></p>

Abstract: Pityriasis versikolor is a skin fungal infection that is quite common in Indonesia as a tropical country with a hot and humid climate, especially if the personal hygiene is not good enough. This study was aimed to evaluate the relationship between personal hygiene and the incidence of pityriasis versicolor among male students of the Faculty of Medicine, Sam Ratulangi University. This was a descriptive analytical stuudy with a cross-sectional design. Subjects were 42 medical students of batch 2015-2018. Personal hygiene was determined by using questionnaires and diagnosis of pityriasis versicolor was confirmed based on clinical and Wood lamp examinations. The results showed that pityriasis versicolor was diagnosed in two subjects (4.8%). The Fisher’s exact test obtained a significancy value of 0.003 which indicated that there was a significant relationship between personal hygiene and pityriasis versicolor. Conclusion: There was a significant relationship between personal hygiene and the occurence of pityriasis versicolor. Hasil penelitian memperlihatkan kejadian pitiriasis versikolor pada dua subyek penelitian (4,8%). Uji Fisher’s exact test mendapatkan nilai signifikansi 0,003 yang menunjukkan adanya hubungan bermakna antara higiene personal dan pitiriasis versikolor. Simpulan: Terdapat hubungan bermakna antara higiene personal dan pitiriasis versikolor. Background: Determining severity of mitral regurgitation (MR) is very important, asit is related with prognosis and management of the disease. Currently, there is no goldstandard exists for quantification of MR severity using echocardiography. AmericanSociety of Echocardiography (ASE) guidelines recommend several parameters basedon integrative criteria using color and spectral Doppler and anatomic measurement.Left Ventricular Early Inflow Outflow Index (LVEIO Index) was proposed as one parameterto assess the severity of MR by omitting geometric error from regurgitantvolume method. Thus, LVEIO index offers a simple, easy and accurate measurementin determining severity of MR.Methods: This study was a diagnostic test research with cross-sectional design. Thesubjects were all patients with organic mitral regurgitation who underwent echocardiographyexamination to measure severity of MR using the LVEIO index as well asother parameters measurements based on parameters of ASE guidelines. Diagnostictest was used to determine the sensitivity, specificity, positive predictive value, negativepredictive value and accuracy of LVEIO index.Results: Of 49 patients in this study, 23 of them have severe MR and 26 are non-severeMR patients. The proportion of gender between two groups almost similar with an agerange from 56 to 61 years and the most common etiology finding was degenerative MR.Echocardiography examination showed the ejection fraction in severe MR was 63±4.2%and 64±3.9% in non-severe MR. The value of LAVI, MPAP, E-wave, VC, EROA andLVEIO index increased parallel with severity of MR (LAVI; 67±18.2 vs. 40±14.2 ml/m2,MPAP; 28±12.8 vs. 20±12.6 mmHg, E-wave 1.5±0.3 vs. 1±0.2 m/s, VC 0.8±0.3 vs.0.5±0.1 cm, EROA 0.4±0.1 vs 0,2±0,1cm2, LVEIO 9±2,4 vs. 5±1,8). The sensitivity,specificity, and accuracy of LVEIO index for diagnosis of severe MR were 86%, 84%,and 89%, respectively.Conclusion: LVEIO can be proposed as a relatively simple, easy and accurate methodin determining severity of MR.

Background: Psychological factors such as anxiety have significant contributions in determining dietary intake and nutritional status of elderly. Prevalence of malnutrition in elderly has achieved significant level.Objective: To determine the correlation between anxiety level with dietary intake and nutritional status of elderly in Yogyakarta municipality.Method: This research was observational with cross sectional design. Subjects were elderly in Yogyakarta municipality who fulfilled inclusion criteria. Research subjects consisted of 214 people. Anxiety level was measured with trait-manifest anxiety scale (T-MAS) form, dietary intake with semi-quantitative food frequency questionnaire (SQFFQ) method, and nutritional status was determined based on body mass arm span (BMA). Data were analyzed with Chi-Square test and multiple logistic regression.Results: A percentage of 26.2% subjects had moderate anxiety level. Most dominant factors that influenced anxiety level was sex (OR=3.37). Bivariate analysis showed a significant correlation between anxiety level and nutritional status (p<0.05), subjects with moderate anxiety level were more likely to have higher nutritional status (OR=3.54) and lower nutritional status (OR=2.29). Significant correlation was found between moderate anxiety level with excessive dietary intake (p<0.001; OR=6.22). Significant correlation was also found between dietary intake with nutritional status (p<0.05), subjects with excessive dietary intake were more likely to have higher nutritional status (OR=6.15).Conclusion: Significant correlation was found between anxiety level and nutritional status. Significant correlation was also found between anxiety level and dietary intake, and significant correlation was found between dietary intake and nutritional status.

B a c k g r o u n d : Hysteroscopy is a valuable diagnostic and therapeutic modality in the management of infertility. A i m : To evaluatethe consistency of hysteroscopy based on a histopathological report from endometrial specimens for intrauterine disorders. Materials andMethods: This is a cross-sectional study. The study included 115 infertile patients. All were admitted for investigation of infertile women beforeassisted reproduction in Mehr infertility institute between 2006 and 2007 hysteroscopy, and histological evaluation of endometrial biopsyperformed.We compared the efficacy of hysteroscopy in the diagnosis of benign intrauterine pathology in infertile women in whom the diagnosiswas confirmed by histologic studies. The women had a complete evaluation with preoperative hysteroscopy, and histological analysis of uterinecavity specimens. Sensitivity, specificity, predictive and negative predictive values were calculated for hysteroscopy considering the histologicalstudy as 100%. Results: Sensitivity and specificity of sonography in diagnosing the polyp were stated 81 % and 64% respectively. Sensitivityand specificity of hysteroscopy showed of polyps revealed 85% and 84% respectively. The results indicated that Sensitivity and specificity ofsonography in diagnosing the myoma were 25% and 98% respectively. Sensitivity and specificity of hysteroscopy in diagnosing the myomawere expressed 50% and 93% respectively. C o n c l u s i o n : Hysteroscopy is a safe and rapid direct visualisation of the uterine cavity. We believeit should be replaced by the diagnostic hysteroscopy as a first line infertility investigation.

Acute coronary syndrome (ACS) is the most common heart disease and has been a leading cause of mortality in Indonesia’s and developed countries population aged over 45 years endemic. The diagnosis of ACS is made by fulfilling 2 of 3 WHO criteria: typicalishemic chest pain, electrocardiogram (ECG) changes specific for ACS and the raise of cardiac biochemical markers. Cardiac troponin T(cTnT) or I (cTnI) are two novel biomarkers with high diagnostic sensitivity and specificity for early diagnosis of ACS especially acutemyocardial infarction (AMI). Troponin I and T are proteins of cardiac myofibrils, released into the bloodstream in the death damages ofcardiomyocyte caused by AMI or injury.The level of cTnI will not increased in patients with decreased renal functions, which is distinctfrom cTnT. The aims of study are to define the sensitivity and specificity of cTnI and cTnT to be cardiac biochemical markers for AMIpatients. From 41 subjects; 29 AMI and 12 subjects non AMI patients in Cardiac Intensive Care Units (CICU) and Emergency Room(ER) of Rumah Sakit Dr. Hasan Sadikin (RSHS) Bandung from September to October 2007 was evaluated. Design of the study was cross sectional and quantitative observational study. The cTnI and cTnT assay using the quantitative immunochromatography method.Sensitivity, spesificity, Positive Predictive Value (PPV) and Negative Predictive Value (NPV) in subjects who met 2 WHO criteria for AMI,Troponin I was 74%, 86%, 96% and 40%. Sensitivity, spesificity, PPV and NPV In subjects who met 3 WHO criteria for AMI, Troponin Iwas 89%, 57%, 62% and 87%. Sensitivity, specificity, PPV and NPV cTnI was 90%, 100% 100 and 80% for diagnosis of AMI. In subjectswho met 2 WHO criteria for AMI, Troponin T Sensitivity, specificity, PPV and NPV has 88%, 71%, 94% and 56%. In subjects who met3 WHO criteria for AMI sensitivity, specificity, PPV and NPV Troponin T was 94%, 35%, 53% and 89%. And Sensitivity, specificity, PPVand NPV Troponin T was 97%, 67%, 88%, and 89% for diagnosis of AMI. Troponin T is more sensitive than troponin I, but troponin Ihas greater specificity than troponin T in AMI. Troponin I is more specific because no influence from renal dysfunction.

BACKGROUND: Dengue virus infection is an infectious disease caused by the dengue virus and transmitted by the Aedes aegypti mosquito. Dengue virus (DEN-V) consists of 4 serotypes, namely DEN-1, DEN-2, DEN-3, and DEN-4. The most feared result of DHF is death. Death in children is caused by hypovolemic shock due to plasma leakage from intravascular to extravascular space due to endothelial dysfunction. AIM: This study aims to analyse difference in sVE-Cadherin levels in Dengue Hemorrhagic Fever (DHF) with and without shock. MATERIAL AND METHODS: The method of taking samples is consecutive sampling, namely the research subjects obtained based on the order of entry in the hospital with a comparative cross-sectional design. From the results of the calculation using the sample formula, the sample size for each group is set at 32 people. So that the total sample size used for both groups is 64 people. The serum sVE-Cadherin levels using the ELISA method. The statistical test used is the independent t-test. The value of p < 0.05 was said to be statistically significant. RESULTS: The result showed that there was no difference in mean sVE-Cadherin levels between DHF patients with shock and without shock (p > 0.05). CONCLUSION: This study concluded that there was no difference in mean of sVE-Cadherin level in DHF patients with shock and without shock.

Objective: To determine the sensitivity and specificity of mesothelin compared with Ca-125 as a tumor marker in predicting ovarian malignancy. Method: The cross sectional study design with diagnostic tests was conducted in 30 samples of patients undergoing elective laparotomy due to ovarian tumor. We compared the sensitivity and specificity between mesothelin and Ca-125, then the data were analyzed using SPSS software version 22.0. Result: According to the ROC curve analysis, optimal sensitivity and specificity value of mesothelin was 63.2% and 54.5% at a cut-off point of 0.45 pg/ml; or 42.1% and 72.7% at a cut-off point of 0.55 pg/ml. While the value of both optimal sensitivity and specificity of Ca-125 was 73.7% and 63.6% at a cut-off point of 46.63 U/ml. Conclusion: Mesothelin and Ca-125 are not different significantly for the AUC value of 50%. Due to higher sensitivity and specificity of Ca-125 than mesothelin, Ca-125 is still used as tumor marker for screening the ovarian malignancy.
