An offshore platform is a workplace with complex facilities and limited space due to the complex installed equipment and components. Therefore, the offshore as enclosed area platform is more likely to have a high risk of COVID-19 outbreak. Furthermore, a company must strictly follow health protocols to prevent workers from being exposed to COVID-19 in the offshore workplace. However, workers are often forced to onboard without proper health protocols because of operational needs and production targets. This paper aimed to explore the essence of the steps in preventing and controlling COVID-19 in the offshore workplace and the challenges. The analysis found that the company must take preventive measures against COVID-19 before workers are on board and in the workplace and control it using the hierarchy of control: engineering control, administrative control, and personal protective equipment (PPE).

Background: In June 2019, 3 people were diagnosed with Ebola virus disease (EVD) in Kasese district, Uganda, all of whom had come from the Democratic Republic of Congo (DRC). Although no secondary transmission of Ebola occurred, an assessment of infection prevention and control (IPC) using the WHO basic IPC facility assessment checklist revealed significant gaps. Robust IPC systems are critical for the prevention of healthcare-associated infections like EVD. A rapid intervention was developed and implemented in Kasese to strengthen IPC capacity in high-risk facilities. Methods: Of 117 healthcare facilities, 50 were considered at high risk of receiving suspected EVD cases from DRC based on population movement assessments. In August 2019, IPC mentors were selected from 25 high-risk facilities and assigned to support their facility and a second high-risk facility. Mentors ensured formation of IPC committees and implemented the national mentorship strategy for IPC preparedness in non-EVD treatment facilities. This effort focused on screening, isolation, and notification of suspect cases: 4 mentorship visits were conducted (1 per week for 1 month). Middle and terminal assessments were conducted using the WHO IPC checklist 2 and 4 weeks after the intervention commenced. Results were evaluated against baseline data. Results: Overall, 39 facilities had data from baseline, middle, and end assessments. Median scores in facility IPC standard precautions increased from baseline 50% (IQR, 39%–62%) to 73% (IQR, 67%–76%) at the terminal assessments. Scores increased for all measured parameters except for water source (access to running water). Greatest improvements were seen in formation of IPC committees (41% to 75%), hand hygiene compliance (47% to 86%), waste management (51% to 83%), and availability of dedicated isolation areas (16% to 42%) for suspect cases. Limited improvement was noted for training on management of suspect isolated cases and availability of personal protective equipment (PPE) (Fig. 1). No differences were noted in scores for facilities with nonresident mentors versus those with resident mentors at baseline (48% vs 50%) and end assessments (72% vs 74%). Conclusions: This intervention improved IPC capacity in health facilities while avoiding the cost and service disruption associated with large-scale classroom-based training of health workers. The greatest improvements were seen in activities relying on behavior change, such as hand hygiene, IPC committee, and waste management. Smaller changes were seen in areas requiring significant investments such as isolation areas, steady water source, and availability of personal protective equipment (PPE). SUMMARY In the context of the COVID-19 pandemic, endoscopy services must adopt preventive measures to maintain proper functioning due to a high risk of disease contagion. Triage protocols before and after the procedure, personal protective equipment, and environmental contamination control are some of the endoscopy society’s recommendations. However, the risk of infection may remain high due to poor control over the source of contamination.Using a combination of standardized supplies and accessories in a hospital, a ventilation mask adapted to be used in endoscopic procedures is proposed to reduce COVID-19 contamination.

Use of appropriate personal protective equipment is essential for healthcare workers when dealing with patients who have tested positive or are suspected of having Covid-19. Personal protective equipment is uncomfortable at best. In hot countries (like India) or in a hot place of work, its wearers are at a high risk of heat-related illnesses. Once in personal protective equipment a healthcare worker can remain in it for at least 6 h at a stretch. In summer when it is hot and humid, personal protective equipment can cause wearer dehydration, heat exhaustion or heat fatigue. In a severe form, this can result in heat stroke and a collapse while on duty. Preventive measures are needed to protect healthcare workers. This review aims to highlight the efficacy and applicability of personal cooling garments.

Penyebab kematian terbesar pada usia sekolah dan  remaja di Indonesia adalah akibat kecelakaan transportasi, disamping penyakit demam berdarah dan tuberkulosis. Berdasarkan data dari Kepolisian, angka kematian karena kecelakaan lalu lintas di Daerah Istimewa Yogyakarta selama empat tahun terakhir terus meningkat. Salah satu upaya yang dapat dilakukan untuk menurunkan angka kematian pada anak adalah dengan melakukan analisis kasus-kasus kecelakaan lalu lintas yang telah terjadi pada anak, sehingga kecelakaan yang sama tidak terulang kembali.Penelitian ini bertujuan untuk menurunkan angka kematian dan angka morbiditas anak korban kecelakaan lalu lintas di Daerah Istimewa Yogyakarta. Analisis deskriptif digunakan untuk mendeskripsikan kecelakaan lalu lintas yang terjadi pada anak, sedangkan Fault Tree Analysis digunakan untuk menentukan faktor-faktor yang mempengaruhi fatalitas dan morbiditas anak-anak korban kecelakaan lalu lintas.Hasil analisis menunjukkan bahwa faktor-faktor yang menyebabkan kematian pada kecelakaan lalu lintas yang terjadi pada anak di Daerah Istimewa Yogyakarta adalah kendaraan lain yang terlibat kecelakaan, hazard, lalu lintas yang bercampur, minimnya sarana prasarana keselamatan, melebihi kecepatan, perilaku tidak tertib dan penggunaan alat keselamatan. Upaya menurunkan angka kematian dan morbiditas pada kecelakaan lalu lintas yang terjadi pada anak di Daerah Istimewa Yogyakarta adalah dengan pendekatan Hazard Control Hierarchy, yaitu dengan Elimination berupa mencegah anak mengendarai motor dan menghilangkan hazard di pinggir jalan, Substitution berupa pemanfaatkan jasa transportasi daring, Engineering Control berupa traffic calming, pemasangan median jalan, dan pembuatan lajur khusus sepeda motor,  Administrative Control antara lain dengan mengoptimalkan Forum Lalu Lintas dan Angkutan Jalan, dan Personal Protective Equipment dengan meningkatkan pemakaian alat keselamatan

We present an update on infection prevention and control for COVID-19 in healthcare settings. This update focuses on measures to be applied in settings with increasing community transmission, growing demand for concern about COVID-19 patients, and subsequent staffing issues in the event of shortages of personal protective equipment for healthcare facilities worldwide. The comfort and emotional resilience of health care workers are key components in maintaining essential health care services during the COVID-19 virus (coronavirus) outbreak.

Background: Patient and staff safety at healthcare facilities during outbreaks hinges on a prompt infection prevention and control response. Physicians leading these programmes have encountered numerous obstacles during the pandemic. Aim/objective: The aim of this study was to evaluate infection prevention and control practices and explore the challenges in Pakistan during the coronavirus disease 2019 pandemic. Methods: We conducted a cross-sectional study and administered a survey to physicians leading infection prevention and control programmes at 18 hospitals in Pakistan. Results: All participants implemented universal masking, limited the intake of patients and designated separate triage areas, wards and intensive care units for coronavirus disease 2019 patients at their hospitals. Eleven (61%) physicians reported personal protective equipment shortages. Staff at three (17%) hospitals worked without the appropriate personal protective equipment due to limited supplies. All participants felt overworked and 17 (94%) reported stress. Physicians identified the lack of negative pressure rooms, fear and anxiety among hospital staff, rapidly evolving guidelines, personal protective equipment shortages and opposition from hospital staff regarding the choice of recommended personal protective equipment as major challenges during the pandemic. Discussion: The results of this study highlight the challenges faced by physicians leading infection prevention and control programmes in Pakistan. It is essential to support infection prevention and control personnel and bridge the identified gaps to ensure patient and staff safety at healthcare facilities.

Abstract Background Infection prevention strategies to protect healthcare workers in endoscopy units during the post-peak phase of the COVID-19 pandemic are currently under intense discussion. In this paper, the cost-effectiveness of routine pre-endoscopy testing and high risk personal protective equipment (PPE) is addressed. Method A model based on theoretical assumptions of 10 000 asymptomatic patients presenting to a high volume center was created. Incremental cost-effectiveness ratios (ICERs) and absolute costs per endoscopy were calculated using a Monte Carlo simulation. Results ICER values for universal testing decreased with increasing prevalence rates. For higher prevalence rates (≥ 1 %), ICER values were lowest for routine pre-endoscopy testing coupled with use of high risk PPE, while cost per endoscopy was lowest for routine use of high risk PPE without universal testing. Conclusion In general, routine pre-endoscopy testing combined with high risk PPE becomes more cost-effective with rising prevalence rates of COVID-19.

In carrying out occupational safety and health at PT Pantja Simpati, PT Pantja Simpati has several problems that can cause illness and work accidents such as unavailability of personal protective equipment, non-functioning air vents, employees smoking in the production room, employees drinking in the production section. part. areas, fire fighting equipment lacks lighting, damaged ceilings or roofs, untidy cables, and slippery production floors. Ways to avoid work accidents such as providing PPE and APAR, repairing damaged equipment, and taking firm action against workers. Objectives: Endoscopy confers high risk for acquiring coronavirus disease 2019. Although guidelines recommend that medical staff use personal protective equipment, no infection control equipment have been established for patients. This study aimed to clarify the usefulness of two face masks we had designed for transnasal and transoral endoscopy. Methods: The efficacy of the masks was evaluated by simulating coughing in a mannequin with fluorescent dyes and mapping the droplet trajectory and number. The number of aerosols generated during endoscopy was clinically evaluated in the endoscopy room. Overall, 4356 screening endoscopies were performed with the patients wearing our masks at Medcity21, a health checkup facility, between June and December 2020; the effects of the masks on the patient’s condition were evaluated retrospectively. An 11-item paper-based survey was performed by the endoscopy staff 6 months after the adoption of the mask-based infection control method. Results: Use of both masks reduced the number of droplets released during the simulation. Clinically, the use of both masks did not affect the patients’ conditions during endoscopy and prevented an increase in the aerosols in the endoscopy room. This mask-based infection control method was favorably received, and all staff indicated that understanding the efficacy of our mask-based infection control reduced their anxiety regarding infection. Until December 2020, none of our staff had contracted SARS-CoV-2. Conclusion: Our mask-based infection control method is easy to adopt, inexpensive, and effective; understanding its effectiveness may help ease the fear of infection among endoscopy staff.
