Abstract Breast cancer is diagnosed in approximately 1/3000 pregnant women. Chemotherapy may be administered after the first trimester, with improved maternal outcome and relatively few pregnancy and offspring complications. Nonetheless, no information about the effects of different chemotherapy regimens on placenta architecture and vasculature are available. Methods To evaluate histological alterations in placentas of women affected by breast cancer and treated with chemotherapy during pregnancy, we retrospectively analyzed 23 placentas of patients affected by breast cancer and treated with chemotherapy during pregnancy and 23 control placentas of women without breast cancer and with physiological pregnancies of the same gestational age. Results All the patients had breast ductal infiltrating carcinoma, 19 of 23 cases had a G3 cancer. All patients were treated with 2-6 cycles of chemotherapy starting after 16 weeks of gestation, with different protocols. No hypertensive complications and no pre-eclampsia episodes were observed; birth weight was consistent with gestational age in all babies in both group with no uneventful outcomes and no perinatal mortality or fetal malformations. Twenty out of 23 cases (86%) showed hypoxia-induced villous alterations, including increased syncytial knotting (Tenney-Parker changes), perivillar fibrin deposits, distal villous hypoplasia or accelerated maturation and focal villous chorangiosis. These alterations were found in 19 out of 23 controls (83%), with no statistically significant difference between the two groups. Conclusions These results shows that chemotherapy in the second and third trimester of pregnancy may lead to non-specific alterations in placental vasculature and morphology.

Malaria has been reported as a condition caused by infestation with Plasmodium parasite species, is a major public health problem globally especially in developing countries like Nigeria. This study was carried out in Federal Medical Centre Umuahia in Abia State, Nigeria. A study was done to determine the maternal serumlevels of alpha tumour necrotic factor, interleukin 10, interleukin 6and interleukin 4 in malaria infected pregnant women based on their gestational age in Southeast, Nigeria. A total of 150 subjects between the ages of 18-45 years were recruited for the study comprising of fifty (50) subjects each of the 3 trimesters. Commercial ELISA Kit by MELSIN Medical Co Limited was used to measure all the cytokines. The results of Table 1 showed no significant difference of TNF-α (p=0.346), IL-10 (p=0.059), IL-6 (p=0.811) and IL-4 (p=0.257) of malaria infected pregnant women at first trimester and second trimester respectively. The results of Table 2 showed no significant difference of TNF-α (p=0.642), IL-10 (p=0.678), IL-6 (p=0.551) and IL-4 (p=0.280) of malaria infected pregnant women at first trimester and third trimester respectively. The results of Table 2 showed no significant difference of TNF-α (p=0.062), IL-10 (p=0.016), IL-6 (p=0.352) and IL-4 (p=0.914) of malaria infected pregnant women at first trimester and third trimester respectively. The study showed no changes in the cytokines studied among the malaria infected pregnant women based on gestational ages except when IL-10 was compared between the subjects on second trimester and third trimester. This study shows that malaria infection does not changes these cytokines in pregnant women based on gestational ages except the il-10 when compared at second trimester and third trimester but changes when compared at other trimesters.

Pregnancy is a hypercoagulable state associated with an increased risk of venous thrombosis. Calibrated automated thrombogram (CAT) is a test to monitor the thrombin generation (TG), a laboratory marker of thrombosis risk, and increases during normal pregnancy, but it is still unclear whether TG is related to the use of insulin in pregnant women with gestational diabetes mellitus (GDM). We performed thrombin generation by CAT on 135 normal pregnant women, including 43 in first trimester, 32 in second trimester, 60 in third trimester, respectively; 68 pregnant women with GDM were also enrolled, 19 patients with GDM using insulin to control blood glucose and 49 patients control their blood glucose through diet and exercise with noninsulin treatment. The overall CAT parameters were calculated using descriptive statistics method with mean ± standard deviation. Mean endogenous thrombin potential, peak thrombin generation, and StartTail time increased significantly with the pregnancy. There was no significant difference in TG test parameters except StartTail time( P = .003) in insulin-treated GDM group when compared to those without insulin in the GDM group. The normal ranges for CAT parameters in pregnant women were determined. Thrombin generation increased significantly in first trimester and remains stable in second and third trimester. The use of insulin in patient with GDM did not affect thrombin generation test. Our study helps to establish the reference range of thrombin generation in Chinese normal pregnant population and provide more basis to predict the risk of thrombus complicating during pregnancy.

Abstract Objectives During pregnancy, systemic physiological alterations lead to some changes in the oral cavity, which could prepare the mouth environment for oral and dental problems. This study was aimed to investigate salivary α-amylase, sialic acid levels, and pH levels in pregnant and nonpregnant females. Materials and Methods In this analytical, case–control study, unstimulated saliva samples were collected with spiting method from 35 pregnant women (case group) and 35 nonpregnant women (control group) and transferred to the laboratory to assess salivary α-amylase, sialic acid, and pH levels. Data were analyzed by SPSS (version: 19) software through statistical methods of independent t-test and analysis of variance. Results The mean sialic acid levels were 2.285 ± 1.230 mg/dL in pregnant and 2.744 ± 1.326 in nonpregnant women without any significant difference (p = 0.138). The mean salivary α-amylase concentrations were 2.461 ± 1.869 U/L and 2.439 ± 2.058 U/L, respectively, in pregnant and nonpregnant women, with no significant difference (p = 0.963).The mean salivary pH in nonpregnant women was significantly more than that in pregnant women (7.845 ± 0.430 and 6.868 ± 0.413, respectively) (p < 0.001). Also, the mean salivary pH levels in pregnant women were 7.474 ± 0.420 in the first trimester, 6.868 ± 0.413 in the second trimester, and 6.568 ± 0.387 in the third trimester, which were significantly different (p < 0.001). Conclusion Salivary sialic acid and α-amylase levels among pregnant women were no different from those of other subjects. During pregnancy, the salivary pH significantly reduced, and the mean salivary pH during pregnancy had a decreasing trend from the first trimester to the third trimester.

Objective: The objective of the study was to measure the proportion of GDM in antenatal outdoor of a hospital, to find out the mean gestational period at which most delivery occurred in GDM and to assess the perinatal outcome.Study design : It was a prospective analytical study conducted in BSMMU, from March,2010 to February,2011.Method: 1489 pregnant women, not known to be diabetic previously, were selected by consecutive sampling in first trimester from Obstetrics outdoor, BSMMU. Their FBS and blood glucose 2hrs after 75gm oral glucose were recorded. We investigated blood glucose in first, second and third trimester in the same pregnant women for screening GDM. Cut off GDM values in fasting stage was e” 6.1mmol/l and 2 hrs after 75gm oral glucose was e” 7.8mmol/l .Result: The proportion of GDM in Obstetrics Outdoor of BSMMU was 6.85%. The mean gestational period at which delivery occurred was lower in GDM ( 36.9 ± 2.2 wks ) than that in non-GDM (39 ± 1.6 wks ) .The most common (31.4%) gestational week during delivery in GDM was 37 completed weeks. Birth weight of 40.2% babies were in the range of 2.5kg to 3.0kg, 31.4% in the range of 3.1 to 3.5 kg and 1% neonate died after birth in GDM.Conclusion: Presently GDM is diagnosed early. The mean gestational period at delivery in GDM is 36.9±2.2 wks. Majority neonatal birth weight is of normal range. Neonatal mortality rate is not increased and is not significantly different from non-GDM women.J Bangladesh Coll Phys Surg 2015; 33(2): 79-85

The PaCOrwas determined by using 'EASY BLOOD GAS .0TO INALMER" and RR was recorded. The PaCO, and RR during different trimesters of pregnant .rumen were compared with that of non-pregnant 11.1101. Statistical analysis was drum with .tudents '1' rest. The PeCOr was significantly lower in first trimester, second trimester and third (rimester of pregnant women than that of non-pregnant women. Similarly, RR was significantly higher in first trimester. second trimester and third trimester of pregnant women than that of nonpregnant women. Again RR was significantly higher in third trimester than in first trimester and second trimester of pregnant women. There were no statistically significant difference of PaCO, among first trimester. second trimester and third trimester of pregnant women. Similarly, there were no statistically significant difference of RR between first trimester and second trimester of pregnant women. It may be concluded from the study that the progressively decreased PaCO• and increased RR throughout Me pregnancy were most likely MP be related to the effect of progesterone-induced hyperventilation. Hypermfilation in pregnancy is due to hypersensitivity of respiratory centre. Due to hyperventilation there is expel out of CO, miming decrease in PaC0r and increase in RR during pregnancy.

ABSTRACTBackground: Pregnant women more often experience urinary tract infections, especially asymptomatic bacteriuria. Asymptomatic bacteriuria in pregnant women increases the risk of preterm labor, low birth weight, and pyelonephritis.Objectives: This study aimed to determine the prevalence of asymptomatic bacteriuria in pregnant women in coastal areas.Method:This study design was analytic observational with cross sectional method. The research was carried out in the coastal area of Kendari city. The population was pregnant women who performed antenatal care in the working area of Puskesmas Mata, Puskesmas Nambo and Puskesmas Abeli in May-October 2016. Sampling was simple random sampling. Data analysis used Chi square test with significance level <0.05.Result:In this study 268 urine samples were obtained from respondents who performed antenatal care in the coastal area of Kendari City. The highest respondents aged 26-30 years old (42.1%), high school education (49.6%), third trimester (55, 6%) and the entrepreneurial husband (41.8%). Examination of bacteriuria showed positive test with nitrite and leukocytes (24.6%) and positive nitrite and/or leukocytes (30.9%). The highest prevalence of bacteriuria occurs in the third trimester (36.9%). Chi-square test showed a significant difference in the prevalence of bacteriuria according to gestational age (p <0.00).Conclusion: The prevalence of asymptomatic bacteriuria in pregnant women in coastal areas was 30.9%. The highest prevalence occurs in third trimester of pregnancy.Key words :asymptomatic bacteriuria, gestational age, pregnant women

Lipid Profile of Healthy Women During Normal PregnancyThe four basic lipid indexes (Chol, Trig, HDL-C and LDL-C) increase during pregnancy, following different rates of increase. Among the four analytes triglycerides show the largest increase and HDL-C the smallest. All analyte values are raised during the 40 weeks of pregnancy, except HDL-C which is stabilized during the second trimester. After delivery the values decrease, except LDL-C which remains steady (for some weeks) before starting to fall following the others. In this study the relations between the four lipid indexes and some predisposing factors (age, gestational age, nationality, body mass index, profession, smoking and diabetes during pregnancy) were investigated. The sample consisted of 413 pregnant women, mainly Greeks and Albanians. After regression analysis it was proved that the only common predisposing factor was the gestational age. Triglycerides and total cholesterol are also influenced by the women's age. The lipid indexes showed no important difference between the pregnant women in the first trimester and the non-pregnant women. On the contrary, there was a statistical difference between the pregnant women in the second and third trimester and between them and the women in the first trimester. The percentages of increase between first and second trimester were: Chol: 38%, Trig: 115%, HDL-C: 30%, LDL-C: 33%. The percentages of increase between first and third trimester were: Chol: 65%, Trig: 208%, HDL-C: 26%, LDLC: 64%.

Background: Assessment of fetal gestational age (GA) is an essential part of obstetric USG. Accurate knowledge of fetal GA is important to facilitate the best possible prenatal care and successful pregnancy outcome.  For the estimation of GA the commonly used parameters in third trimester are:- BPD, HC, AC and FL. As all these parameters are affected by fetal growth disorders and fetal position. TCD can be used as another parameter for the estimation of GA in growth restricted fetuses.Methods: This study was conducted at Mahila Chikitsalaya Sanganeri Gate; SMS Medical College Jaipur from July 2005 to September 2006. A total of 80 pregnant women in third trimester with singleton pregnancy including 40 pregnant women with known cases of IUGR and 40 AGA fetuses were studied for TCD measure.Results: Correlation coefficient between TCD and gestational age was highly significant and no significant difference was found in TCD of AGA and IUGR fetuses.Conclusions: TCD measurement can be used as more reliable parameter for accurate estimation of gestational age in IUGR fetuses in third trimester.

There is a great controversy about the ingestion of caffeine during pregnancy, with some conclusive results suggesting that higher levels of caffeine consumption might have negative effects on pregnant women and babies.A low birth weight has been associated with caffeine consumption during pregnancy. However, not all studies share the same opinion. The mechanism by which caffeine appears to influence fetal growth is not yet defined. Verena (2013) refers in his research that for every 100mg of daily caffeine consumption, the weight of the baby would decrease between 21 to 28 grams and the gestation period was extended in five hours.Considering this, the goal of our study is to investigate the extent to which caffeine consumption in pregnant women can lead to the occurrence of low birth weight.To do this we have conducted a study of descriptive, transversal and quantitative type, consisting of 87 recent mothers, with an average age of 31.2 years, with a weight gain of 10,1Kg during pregnancy, gestational age of 38.8 weeks and birth weight of 3,003Kg, of which 75.86% ingested coffee during pregnancy, an average of 1.3 cups per day.After analysing the data, this study has found that there is no statistically significant difference between coffee consumption during pregnancy and the weight of the newborn.However the small size of the sample is considered the major limitation of the study.Based on the results of other conducted studies, the recommendations related to the maximum amount of caffeine consumption per day should be reviewed.Keywords: Caffeine; Pregnancy; Gestation; Newborn; Underweight newborn

Background: Pregnancy is a major endocrine event in the female lifespan, involving wide-ranged and often dramatic changes in the metabolism of various hormones. Cross sectional, case control, analytical quantitative study was conducted in Sudan, Khartoum state in Yastabsheron obstetric hospital during the period from March to August 2011. Analytical and statistical methods were applied to measure the concentration of A1c% in healthy pregnant women as well as in healthy non-pregnant women to assess the difference in the results.Methods: Blood samples were taken from a total of 90 healthy pregnant women (case group) and 30 healthy non-pregnant women (control group), then samples were analyzed for A1c% by using affinity chromatography technique, and results were recorded in addition to their age, body mass index and the number of pregnancies.Results: showed that, the mean concentration of the A1c% in cases group was (4.407±1.054%) in first trimester, (4.797±0.631) % in second trimester and (4.833±0.626) % in third trimester, and (5.670±0.471%) in control group with a P value of 0.00, indicating the highly significant difference between the two groups. Others finding showed that the mean concentration of A1c% of the first trimester is lower than that of the second and third trimesters, also there was no significant difference between the mean concentration of the second and third trimester. A significant weak positive correlation between A1c% concentration with body mass index and the age of pregnant women.Conclusions: Healthy normal pregnant women have lower A1c% concentrations than non-pregnant women which can be impute to the reduce in plasma glucose values and to the shortened erythrocyte life span that can occur during pregnancy. The body mass index and age affect the concentration of A1c% c, but it is not affected by gravida.
