Abstract Background: Common thyroid diseases are hyperthyroidism, hypothyroidism, thyroiditis, thyroid tumor and so on. Baidu is currently the most widely used online search tool in China, has developed an internet search trends collection and analysis tool called the Baidu Index. The aim of the present study was to understand the trend and characteristics of public’s online attention to thyroid diseases, and to explore the value of Baidu Index in monitoring online retrieval behavior of thyroid related information.Methods: Taking the period from January 1, 2011 to December 31, 2019 as the time range into consideration, we used the big data analysis tool of Baidu Index and took “thyroid nodules”, “thyroid cancer”, “thyroiditis” “hyperthyroidism” and “hypothyroidism” as the keywords, the data of “search index” and “media index” were recorded on a weekly basis, and all information were aggregated into quarterly and annual to generate the final data which was carried out for secondary analysis. Pearson correlation analysis was used to analyze the correlation between the search index of keywords and the year. One-way Analysis of Variance was used to analyze the differences between search index and media index.Results: Among the five keywords, thyroid nodule search index had the highest growth rate (640%), followed by thyroid cancer (298%). The media’s attention to thyroid diseases had been declining year by year. Unlike the public’s attention, the media index of hyperthyroidism was significantly higher than other keywords.Conclusion: Over the past nine years, the public's attention to thyroid related diseases has been increasing gradually. Baidu Index is an effective tool to track the health information query behavior of Chinese internet users, which can provide a cost-effective supplement to traditional monitoring system.

Background: Acute pancreatitis is usually a mild and self-limiting disease. About 25 % of patients have severe episode with mortality up to 30%. Early identification of these patients has potential advantages of aggressive treatment at intensive care unit or transfer to higher centre. Several scoring systems are available to predict severity of acute pancreatitis but are cumbersome, take 24 to 48 hours and are dependent on tests that are not universally available. Haematocrit has been used as a predictor of severity of acute pancreatitis but some have doubted its role.Objectives: To study the significance of haematocrit in prediction of severity of acute pancreatitis.Methods: Patients admitted with first episode of acute pancreatitis from February 2014 to July 2014 were included. Haematocrit at admission and 24 hours of admission were compared with severity of acute pancreatitis. Mean, analysis of variance, chi square, pearson correlation and receiver operator characteristic curve were used for statistical analysis.Results: Thirty one patients were included in the study with 16 (51.61%) male and 15 (48.4%) female. Haematocrit at 24 hours of admission was higher in severe acute pancreatitis (P value 0.003). Both haematocrit at admission and at 24 hours had positive correlation with severity of acute pancreatitis (r: 0.387; P value 0.031 and r: 0.584; P value 0.001) respectively.Area under receiver operator characteristic curve for haematocrit at admission and 24 hours were 0.713 (P value 0.175, 95% CI 0.536 - 0.889) and 0.917 (P value 0.008, 95% CI 0.813 – 1.00) respectively.Conclusion: Haematocrit is a simple, cost effective and widely available test and can predict severity of acute pancreatitis.Journal of Kathmandu Medical College, Vol. 4(1) 2015, 3-7

Primary thyroid lymphomas are rare. Surgery is seldom indicated. The aim of the study is to find out the main characteristics of primary thyroid lymphomas in our patients, indications for surgery and the possibility of treatment, frequency and characteristics of rare thyroid tumors. Method: retrospective study of 1044 patient operated for malignant thyroid tumor. Results: From 1995 to may 2003, we operated upon 15 patients with primary thyroid lymphomas, 2 men and 13 women mean age of 50.12 years (from 22 to 74 years), also one patient of age 69 with insular thyroid cancer. Reason for surgery was thyroid tumor in all, compressive disturbances in 9, among them 4 with asphyxia. Radical total thyroidectomy was performed in 4 (26.7%), whole in others some residual tumor tissue was could not be removed in spite of thyroidectomy in 3, hemithyroidectomy in 2, tumor debulking in 5 and only open biopsy was performed in one patient. There was no operative mortality, no postoperative hypocalcaemia and no recurrent nerve palsy. Histological type of tumor was Non-Hodgkin lymphoma in 13 patients, Hodgkin disease in 2 female patients of age 22 and 24. Hashimoto thyroiditis was present in 3 patients. After surgery, 13 patients were treated with chemotherapy, one patient died one month after the operation and one patient refused chemotherapy. Follow-up data are available for 9 patients and the mean follow-up period was 20 months (1-48months). Three patients died after a month, 2 and 3 years after surgery. Six patients are without local relapse. In one patient who refused chemotherapy, a year after thyroid surgery, resection of large intestine was performed because of lymphoma of the colon. Conclusion: Malignant thyroid lymphomas are rare. They present with rapidly growing thyroid tumor, compression and asphyxia. Surgery is only temporarily effective and it is necessary to start with chemo-radiotherapy as soon as possible. Rare forms of thyroid cancer have to be histological recognized in order to choose the best way of treatment.

Objective: To assess the cost-effectiveness of a single treatment session of acupuncture, when applied in addition to usual care for acute low back pain (ALBP). Methods: Secondary analysis of a multicentre randomised controlled trial in Norwegian general practice. In total, 171 participants with ALBP ⩽14 days were randomised to a control group (CG) receiving usual care or to an acupuncture group (AG) receiving one additional session of Western medical acupuncture alongside usual care. Primary outcome measures for this cost-effectiveness analysis were quality-adjusted life years (QALYs), health care costs and societal costs at days 28 and 365, the incremental cost-effectiveness ratio (ICER) and net monetary benefit (NMB). The NMB was calculated on the basis of the Norwegian cost-effectiveness threshold of NOK 275,000 (USD 35,628) per QALY gained. Missing data were replaced by multiple chained imputation. Results: Eighty-six participants in the CG and 81 in the AG were included in the analysis. We found no QALY gain at day 28. At day 365, the incremental QALY of 0.035 was statistically significant. The differences in health care costs and societal costs were not statistically significant. Three out of four calculations led to negative ICERs (cost saving) and positive NMBs. For the health care perspective at day 365, the ICER was USD –568 per QALY and the NMB was USD 1265, with 95.9% probability of acupuncture being cost-effective. Conclusion: To our knowledge, this is the first cost-effectiveness analysis of acupuncture for ALBP. The findings indicate that acupuncture may be cost-effective from a 1-year perspective, but more studies are needed. Trial registration number: NCT01439412 (ClinicalTrials.gov).

The incidence rate of papillary thyroid cancer (PTC) has increased over the past decades, but the pathogenesis remains unclear. rs2910164, located in pre-miR-146a, has been studied in PTCs with different ethnicity, but the results were inconsistent. Here we evaluate the association between rs2910164 polymorphism and PTC and investigate the effect of this polymorphism on patients’ clinicopathological characteristics. 1238 PTC patients and 1275 controls, all Han population, from Northern China, were included in our study. rs2910164 was genotyped using Matrix-Assisted Laser Desorption/Ionization Time of Flight Mass Spectrometry (MALDI-TOF-MS). Analysis of inheritance model was performed using the SNPStats program. Strength of association was assessed by odds ratio (OR) and 95% confidence interval (CI). Overall, no statistical difference in rs2910164 genotype distribution and allelic frequencies between cases and controls was found, and patients with different genotypes had similar clinicopathological characteristics in terms of stage, location, concurrent of benign thyroid tumor, and thyroiditis, while, as the number of G alleles increased, proportion of patients aged ≥45 years and those without metastasis increased (Ptrend<0.001andPtrend=0.003, resp.). However, no association remained significant after Bonferroni correction under any model of inheritance. Our results suggest no association between rs2910164 polymorphism with PTC and patients’ clinicopathological characteristics.

AbstractFor their food needs, the people in Long Midang Village still apply traditional values or local wisdom in the utilization and processing of rice cultivation, especially rainfed lowland rice. This research aims to identify local wisdom that has existed and still being maintained, knowing the factors that cause local wisdom to be maintained or abandoned, and formulating strategies to maintain local wisdom. This study uses a qualitative descriptive analysis method and SWOT analysis tool, data obtained through in-depth interviews. The results showed there are 38 local wisdoms in Long Midang Village, 22 local wisdoms are retained while 16 local wisdoms are removed, the factors that cause local wisdom have been maintained are the availability of organic fertilizers, the use of cheaper traditional tools, land preservation and respect for ancestral heritage. The factors that cause local wisdom have been abandoned are changes in insight/knowledge and more efficient modern technology. Strategies to strengthen local wisdom using aggressive strategies (S-O) namely increasing the use of buffalo in cultivating land as a cost-effective and environmentally friendly local wisdom as well as maintaining mutual cooperation wisdom. Key words: Local Wisdom, Rainfed Rice, SWOTAbstrakDalam memenuhi kebutuhan pangannya, masyarakat di Desa Long Midang masih memegang kuat nilai-nilai tradisi adat atau kearifan lokal yaitu dalam pemanfaatan dan pengolahan budidaya padi khususnya padi sawah tadah hujan. Penelitian ini bertujuan untuk mengidentifikasi kearifan lokal yang pernah ada dan yang masih dipertahankan, mengetahui faktor-faktor yang menyebabkan kearifan lokal terus dipertahankan atau ditinggalkan, dan merumuskan strategi dalam mempertahankan kearifan lokal. Penelitian ini menggunakan metode analisis deskriptif kualitatif dan alat analisis SWOT, data diperoleh melalui wawancara mendalam. Hasil penelitian menunjukkan kearifan lokal di Desa Long Midang berjumlah 38, kearifan lokal yang dipertahankan sebanyak 22 sedangkan kearifan lokal yang ditinggalkan sebanyak 16, faktor-faktor penyebab kearifan lokal di pertahankan adalah tersedianya pupuk organik, penggunaan alat-alat tradisional yang lebih murah, kelestarian lahan dan penghargaan terhadap warisan leluhur. Faktor-faktor penyebab kearifan lokal ditinggalkan adalah perubahan wawasan/pengetahuan dan perkembangan teknologi modern yang lebih efisien. Strategi penguatan kearifan lokal menggunakan strategi agresif (S-O) yaitu meningkatkan penggunaan kerbau dalam pengolahan lahan sebagai salah satu kearifan lokal yang hemat biaya dan ramah lingkungan serta mempertahankan kearifan gotong royong.Kata kunci: Kearifan Lokal, Sawah Tadah Hujan, SWOT

Abstract Introduction It is well established in the literature that frailty is associated with high health costs (Bock 2016, England: BioMed Central). Early identification improves patient outcomes. The 2017 GMS contract for General Practioners’ (NHS England 2017) had tried to implement this. This is a result from the government wanting to improve care for frail patients from the “Five Year Forward Review”. This is first policy worldwide to introduce a policy on frailty screening (Reeves, 2018, BMJ, 362, pp. k3349). Majority of frail patients’ first contact with the NHS is through Primary Care. However, with ever reducing number of GPs and increasing work burden, is it appropriate to ask GPs to undertake this? Methods This review uses the policy analysis tool PESTLE (Political, Economical, Social, Technological, Legal, and Environmental) (Visconti 2016, Corporate Ownership and Control,13), to assess the implications of this new contract obligation on GPs and patients. Results Once frail patients are identified, it is advised that they are reviewed by GPs annually. Whether this actually benefits patients are not clear (Page 2017 British Journal of Clinical Pharmacology, 82(3), pp. 583–623). The evidence for interventions being cost-effective is also inconsistent (Hamilton 2017, BMJ, 358, pp. j4478). Alternative methods include implementing a nurse-led community frailty service, has shown some benefit in Netherlands (Bleijenberg 2017, JAMDA). Clinical Pharmacist can aid with medication reviews and focusing on Geriatrician “outreach” clinics in primary care can improve patient care and outcomes (Goldstein 2014, CJEM, 16(5), pp. 370). Due to the work burden, GPs are often seen to be reactive rather than proactive (Goodwin 2010, The King’s Fund). The shift in focus on frailty can simulate more constructive dialogue between primary care, secondary care, patients and their carers. The BMA has also tried to reassure GPs this is not an added burden, but this is controversial (BMA 2017). Conclusions To successfully implement such a policy, emphasis on clear objective outcomes and strategy is needed. There is a risk of frailty identification becoming a tick box exercise.

Artikel ini membincangkan kajian ringkas berkaitan analisis kos kitaran hayat terhadap langkah-langkah pembaikan pengaratan bagi jambatan dan struktur marin konkrit yang terdedah kepada karbonasi atau serangan natrium klorida daripada air laut atau sumber-sumber lain. Perisian kos kitaran hayat, Bridge LCC 2.0 digunakan untuk menjalankan analisi kitaran hayat untuk tiga kes kajian melibatkan kaedah nilai bersih kini. Keputusan kajian menunjukkan analisis kos kitaran hayat berkeupayaan untuk membantu jurutera dan agensi pengangkutan dalam menilai keputusan penyelenggaraan yang efektif berkaitan dengan masalah pengaratan. Ia boleh digunakan sebagai alat analisis ekonomi kejuruteraan yang membantu mantaksir kos-kos perbezaan dan membuat pilihan terhadap langkah pembaikan pengaratan yang berkesan. Analisis kos kitaran hayat bagi langkah pembaikan dipengaruhi oleh banyak pemboleh ubah seperti kos permulaan, kos penyelenggaraan, tahun kekerapan, dan jangka masa analisis. Amalan terbaik untuk analisis kos kitaran hayat bukan sahaja mengambil kira perbelanjaan oleh agensi, tetapi perlu mempertimbangkan kos-kos oleh pengguna dan analisis sensitiviti di sepanjang jangka hayat sesuatu langkah pembaikan. Kata kunci: Analisis kos kitaran hayat, jambatan konkrit, pengaratan, langkah, pembaikan, pemulihan struktur, keberkesanan kos, kaedah nilai bersih kini (NPV) This paper discusses a short study on life cycle cost analysis (LCCA) on corrosion remedial measures for concrete bridges and marine structures, which are subjected to carbonation or ingress of sodium chloride from sea water and other sources. Life cycle costing software, Bridge LCC 2.0, was used to perform life cycle cost analyses on three case studies, based on net present value method. The analysis of the results showed that LCCA is capable of assisting engineers or transportation agencies to evaluate optimum maintenance decisions in corrosion–related problems. It can be used as an engineering economic analysis tool that helps in qualifying the differential costs and choosing the most cost–effective corrosion remedial measures. Life cycle costs for the remedial measures are influenced by many costing variables such as initial costs, periodic maintenance costs, frequency years and analysis period. The best practice of LCCA should not only consider agency expenditures but also user costs and sensitivity analysis throughout the service life of a remedial measure. Abstract Context: It remains inconclusive whether mutations in thyroid hormone receptor (TR) genes naturally occur in thyroid cancer and whether these genes could be suppressors of this cancer. Objectives: Our objectives were to examine further mutations of TRα and TRβ genes in thyroid cancer and also to examine their methylation as an epigenetic silencing mechanism in thyroid cancer. Experimental Design: Instead of using a cDNA sequencing approach used in previous studies, we used genomic DNA to sequence directly the coding regions of the TRα and TRβ genes to search mutations in various differentiated thyroid tumors and used methylation-specific PCR to analyze promoter methylation of these genes. Allelic zygosity status at TRβ was also analyzed. Results: We found no TRα gene mutation in 17 papillary thyroid cancers (PTCs) and 11 follicular thyroid cancers (FTCs), and no TRβ gene mutation in 16 PTCs and 12 FTCs. We also found no methylation of the TRα gene in 33 PTCs, 31 FTCs, 20 follicular thyroid adenomas (FTAs), and 10 thyroid tumor cell lines. In contrast, we found hypermethylation of the TRβ gene in 10 of 29 (34%) PTCs, 22 of 27 (81%) FTCs, five of 20 (25%) follicular thyroid adenomas, and three of 10 (30%) thyroid tumor cell lines, with the highest prevalence in FTC. We additionally examined loss of heterozygosity at TRβ and found it in three of nine (33%) PTCs and three of nine (33%) FTCs. Conclusions: Mutation is not common in TR genes, whereas hypermethylation of the TRβ gene as an alternative gene silencing mechanism is highly prevalent in thyroid cancer, particularly FTC, consistent with a possible tumor suppressor role of this gene for FTC.

ABSTRACTObjective: Disaster mental health (DMH) is vital to comprehensive disaster preparedness for communities. A train-the-trainer (TTT) model is frequently used in public health to disseminate knowledge and skills to communities, although few studies have examined its success. We report on the development and implementation of a DMH TTT program and examine variables that predict dissemination.Methods: This secondary analysis examines 140 community-based mental health providers' participation in a TTT DMH program in 2005–2006. Instructors' dissemination of the training was followed for 12 months. Bivariate and multivariate analyses were conducted to predict dissemination of the training program.Results: Sixty percent of the trainees in the DMH TTT program conducted training programs in the 12-month period following being trained. The likelihood of conducting training programs was predicted by a self-report measure of perceptions of transfer of training. The number of individuals subsequently trained (559) was predicted by prior DMH training and sex. No other variables predicted dissemination of DMH training.Conclusions: The TTT model was moderately successful in disseminating DMH training. Intervention at the organizational and individual level, as well as training modifications, may increase cost-effective dissemination of DMH training.(Disaster Med Public Health Preparedness. 2010;4:339-343)
