Abstract Background: Patients' rights (PRs) mean a set of rights that an individual has in the healthcare organizations that ensure that they receive ethical treatment and high quality of care. PRs became an important topic in healthcare practices worldwide and essential for providing high quality care. Aim: This study looked for assessment of knowledge and practices of physicians working in the authors’ Benha University Hospitals, about PRs. Method: A cross-sectional study was conducted using structured questionnaire included 20 questions concerning PRs. Study participants were 230 physicians & 300 patients. Results: 97.0% of the physicians achieved satisfactory score regarding knowledge about PRs ,but only 14.7% of them showed satisfactory score of practices (from patients’ point of view). There is statistically significant difference between score of knowledge and practices of PRs (p<0.001). The score of knowledge was affected by physicians’ age, gender, highest qualification, work position, work experience and average number of working hours, while patients’ opinion in physicians’ practices of PRs was affected by patients’ gender, educational level and department of admission. Patients’ residence & their length of hospital stay (LOS) did not significantly influenced their opinion. Conclusion: There was discrepancy between knowledge and practices of PRs.

Good knowledge, awareness and preparedness regarding coronavirus disease 2019, among community pharmacy practitioners (CPPs), will enable them to provide correct information and support to patients, convey instructions from the public health authorities and protect their own health. Objective: The aim of this study was to determine knowledge, awareness and preparedness regarding coronavirus disease 2019 among CPPs working in Kathmandu, Nepal. Methods: A cross-sectional study was conducted using a convenience sampling method from 10 February to 25 March 2020. Data were analysed descriptively, and one-sample independent t-test and one-way analysis of variance were used to compare scores among different subgroups of respondents ( p < 0.05). Post hoc analyses (following analysis of variance) were done using the Bonferroni test. Results: A total of 81 CPPs participated. Mean ± standard deviation age was 26.70 ± 5.29 years. Majority of the respondents were male (64.2%). Many CPPs (40.7%) had work experience between 1 and 5 years. Almost half (45.7%) had an educational qualification of Diploma in Pharmacy. Significant differences in knowledge ( p = 0.022) and preparedness ( p < 0.001) were found among individuals with differing levels of work experience. Similarly, a significant difference was found between knowledge-preparedness and work experience ( p = 0.022 and p < 0.001), location of pharmacy ( p < 0.001) and educational qualification ( p = 0.031). Awareness ( p < 0.001) and preparedness scores ( p = 0.049) were different among community pharmacies with the different number of medicines available. Conclusion: Knowledge was found to be higher in CPPs with a work experience of more than 5 years. CPPs working at larger community pharmacies were more aware and prepared. CPPs’ knowledge, awareness and preparedness should be evaluated more widely and strengthened.

Background: Modern defence system has a significant role of electrical, electronic and instrument technicians in armed forces. Despite their intensive role, little is known about work-related musculoskeletal disorder in these technicians. Objective: To investigate the prevalence of the work-related musculoskeletal disorder in defence personnel involved in electrical and electronic maintenance and repairs. Methods: A cross-sectional study was carried out for 3 years, in Electrical, telecom and instrument defence technicians (N = 164). Prevalence of work-related musculoskeletal disorder was assessed using the Nordic Musculoskeletal Questionnaire. Effect of age, working hours, work experience, Basal metabolic index and type of job on the prevalence of work-related musculoskeletal disorder was investigated using logistic regression analysis. Results: A total of 119 (72.56%) respondents reported work-related musculoskeletal disorder with 53.04% multiples & 19.51% single work-related musculoskeletal disorder. Simultaneous presence of neck, shoulder and upper back work-related musculoskeletal disorder were observed. Highest rate of work-related musculoskeletal disorder was in neck (44.44%), followed by shoulder (27.16%), ankle/ foot (14.81%), elbow/ forearm (12.96%), low back (11.72%) and wrist/ finger (9.87%). Neck (44%) related work-related musculoskeletal disorder were highest across all job crafts. Work experience (p = 0.012; OR = 0.87; 95% CI – 0.78- 0.97), working hours (p  = 0.031; OR= 1.73, 95% CI – 0.58-5.12) smoking (p = 0.00 ; OR = 5.3,  95 % CI = 4.3 – 8.48) and job crafts like electrician (p = 0.434; OR = 1.89, 95% CI = 1.08-3.31) and telecom mechanics (p = 0.026; OR = 1.26, 95% CI  =0.74 – 2.15) were significantly associated as risk factors of work-related musculoskeletal disorder. Conclusion: High prevalence of work-related musculoskeletal disorder was observed in electrical, telecommunication & instrument mechanics, with higher multiple work-related musculoskeletal disorder. Working hours, work experience, job craft and smoking were highly associated with a work-related musculoskeletal disorder. We suggest suitable ergonomic intervention and awareness program along with smoking control drive to reduce the risk of work-related musculoskeletal disorder.

Introduction: The nursing profession is known to be stressful throughout the world. It contributes to various physical and psychological health problems in nurses, which ultimately influencing on their performance and efficiency of work. Objective: To measure the level of job stress among nurses and to find out its relationship with demographic variables and working pattern. Methods: Study was done among 102 nurses in a tertiary care hospital of Surendranagar district from February to March 2020. The level of job stress was measured by a set of 14 questions which are directly related to job stress and response was collected on a five-point Likert Scale. Results: This study revealed, moderate and severe level of job stress among 53% and 45% of nurses respectively. The job stress had no statistically significant association with demographic variables like age and gender. However, job stress had an inverse correlation with the factors like work experience, shift timings and working hours. Conclusion: All the study participants were suffering from mild to severe level of job stress. Younger age group and female nurses were commonly affected with job stress. Work stress was also found more prevalent in nurses with lesser experience, rotational shift and more working hours.

Background: “Burnout” is defined as a state of physical, emotional and mental exhaustion, or a coping mechanism to working conditions that are stressful, demanding and lack of recognition. Effects of burnout are many and can eventually threaten one’s job satisfaction, relationship and also health. High patient load, long working hours, poor logistics and infrastructure support and unreasonable demands from patients make clinicians vulnerable for stress and burnout. Objective was to study the prevalence of burn out in clinicians and factors associated with burnout.Methods: The study was an observational descriptive cross-sectional study conducted among Doctors in clinical departments of a Tertiary Medical care setting in October-November 2015. Sample size was 97. Study was conducted using Maslach Burnout Inventory with additional questions on demographic factors, work experience, hours of work and speciality. The inventory comprised 22 items on a seven-point Likert scale. Frequency of symptoms (ranging from ‘0=never’ to ‘6=every day’). Burnout was measured in three dimensions - emotional exhaustion (EE), depersonalization (DP) and personal accomplishment (PA). The data was collected after taking Institutional ethics committee approval and verbal consent from participants. Data was analyzed using MS Excel 2007.Results: About 15% 0f clinicians showed high emotional exhaustion, 9% high depersonalization, and 18% low Personal accomplishment. More Females showed burnout than males and more clinicians from surgical branches showed burnout when compared to medical branches.Conclusions: Burnout exists among healthcare professionals and measures should be taken to identify causes and take remedial actions.

Introduction: Burnout syndrome is a condition caused by chronic stress at the workplace. Resilience is the ability to cope with the negative situations in life. Purpose: To evaluate the extent of burnout syndrome in nursing teachers at medical high schools and the effect of the level of resilience on burnout syndrome. Material and methods: This cross-sectional study interviewed 101 nursing teachers at four medical schools. Teacher Burnout Scale, The Connor-Davidson Resilience Scale and socio-demographic questionnaire have been used as instruments of the study. Results: The majority of nursing teachers had moderate symptom of burnout syndrome. The average result on the burnout scale was 56.1. Statistical analyzes showed that there is no statistically significant difference in the manifestation of burnout syndrome in teachers in relation to demographic characteristics (sex, marital status, years of work experience and subjects at medical school). By analysing the results of the two scales, it has been established that there is no statistically significant association between the burnout intensity and the degree of resilience in nursing teachers. Conclusion: The results of the study showed moderate level of burnout in nursing teachers. This may help as an encouragement to develop a strategy for preventing emergence of the burnout syndrome. The results will be useful for further research of burnout syndrome in nursing teachers.

Purpose Prediction of future changes and making appropriate strategic decisions require strategic thinking in any organization. It helps managers to create new opportunities. The purpose of this paper is to measure strategic thinking and its affecting factors at Kurdistan University of Medical Sciences. Design/methodology/approach This is a descriptive-analytic as well as a cross-sectional study which was conducted in 2016. Its statistical community included 300 managers and personnel of Kurdistan University of Medical Sciences. The sample size was equal to statistical community. Data were collected using a standard questionnaire. Data were then entered into SPSS20 and were analyzed using statistical tests such as Freedman, Whitney and Kruskal-Wallis. Findings Overall, strategic thinking was evaluated “good” and “somewhat good” among managers (5.0±72.28) and personnel (6.0±25.48), respectively. Moreover, among the elements of strategic thinking, conceptual thinking obtained the highest score among both managers (6.0±06.32) and personnel (5.0±53.52), which was evaluated in a “good” level. There was a significant difference between managers’ and personnel’s strategic thinking score (p=0.001). Different education groups of managers and personnel were different in terms of strategic thinking (p<0.05), but strategic thinking score of managers and personnel were not significant based on the work experience and type of employment. Originality/value Although both managers and personnel received a good score in terms of strategic thinking, improving the level of strategic thinking especially for future trends and opportunities can lead to enhanced strategic thinking among managers and personnel of Kurdistan University of Medical Sciences.

Introduction: Fissure sealant is one of the most important and effective preventing techniques. Having proper information and guidelines is the most important factors for the success of treatment. Therefore, in this study, the knowledge, attitude and practice of general dentists about fissure sealant in Yazd in 2017 were evaluated. Methods: A descriptive cross-sectional study was conducted by a questionnaire in which 140 general dentists were investigated in Yazd. The questionnaire consisted of demographic information, 9 questions about awareness of the issue of Fissure sealant, 4 questions about people's attitude and 7 questions about their practice. After gathering the data, they were analyzed by SPSS 20 version 18 software and ANOVA test. Results: The results of the study showed that the majority of General dentists had a good knowledge (85.72%), their attitude level (77.14%) and performance (85%) were moderate. There was a significant difference in knowledge (P-value = 0.004) and practice (P-value = 0.000) levels in age group and work experience, but there was no significant difference in attitude level (P-value = 0.980). Conclusion: According to the results, the knowledge of dentists in Yazd seems to be good in most cases, but their attitudes and practice are moderate and in some cases poor, despite having good knowledge, the results indicate that they are not up to date with new information and resources. This indicates the need to train new resources in retraining courses with emphasis on functional weaknesses and dentists' awareness.

Background: Working as a nurse means being responsive and highly accountable 24/7 and to be able to offer high-quality care, specifically during pandemics. Studies have shown that the average number of working hours per week is a significant predictor of stress and that the severity of the COVID-19 pandemic has increased the nurses stress levels. Objective: Therefore, we investigated (1) if a change had occurred in the nurses working hours during the COVID-19 pandemic as compared to the hours employed and (2) the influence of the nursing staffs working hours during COVID-19 pandemic on the perceived level of stress. Design: We used an online survey in this Austrian cross-sectional study, distributed using a snowball sampling method. Participants and methods: In the online survey, we asked the nurses many relevant questions, including how many hours they are employed per week and how many hours they had worked on average per week since the outbreak of COVID-19. We used the Perceived Stress scale to measure stress level among these nurses. Data were collected between mid-May and mid-July 2020. Results: Three-quarters of the 2600 participating nurses reported changes in their working hours during the COVID-19 pandemic. The nursing staffs hours of employment were statistically significantly associated with their average number of working hours during the COVID-19 pandemic. About two-thirds of the nurses who were employed either less than 10 hours or for 31-40 hours worked for more than 40 hours. Most of the nurses experienced a moderate level of stress. We identified a statistically significant association between increasing the number of working hours per week and the nurses perceived stress level. In addition, 15% of the nurses who had worked more than 40 hours reported experiencing a high level of stress. In addition, we found that nurses who worked more hours during the pandemic experienced higher stress than nurses who reduced their working hours or kept the same working hours. Conclusions: We found a statistically significant association between an increase in the nursing staffs working hours and their level of stress. We believe that these results reflect the negative consequences of prolonged working hours. For this reason, a (inter-)national discussion is needed on the topic of restricting the working hours of healthcare workers during such pandemics. This discussion can improve the health and safety of the health care workers, the patients, as well as members of the general population.

Background: Febrile seizures (FS) are the most common seizures in children younger than 5 years. In the last decade, various coding and noncoding sequence variations of voltage-gated sodium channels SCN2A have been identified in patients with seizures, implying their genetic base. We aimed to evaluate the association between SCN2A c. G/A genetic polymorphism among Egyptian children with febrile seizure plus. Methods: The present cross-sectional study was carried out on 100 epileptic infants and children, attendants of the Neurology Unit, pediatric department, Menoufia University Hospitals (Group Ι). The patients were sub-classified into two groups, according to response to anti-epileptic treatment; Group Ι a (drug responder) and Group Ι b (drug-resistant). Evenly divided number of apparently healthy, age and gender-matched children were selected as controls (Group II). A complete history, throughout the systemic examination and radiological & metabolic assessment, whenever needed was provided, all participants were genotyped for SCN2A rs17183814polymorphism by Restriction Fragment Length Polymorphism (PCR-RFLP). Results: Both of A allele and AA, GA genotypes of SCN2A c. 56 G/A were detected more in patients with febrile seizure plus comparison to the control group with a statistically significant difference at frequencies of 17% and 11% and 12% respectively; OR (CI95%): 10.04 (3.49-28.87) and p <0.001. On classifying epileptic patients into 2 subgroups, carriers of SCN2A rs17183814 AA genotype tended to respond poorly to anti-epileptic drugs (AEDs). Moreover, multivariate analysis revealed that rs17183814 A allele and positive family history of epilepsy were considered the highest predicted risk factors for the development of epilepsy p<0.05. Conclusion: SCN2A rs17183814 (A) allele was specifically associated with developing febrile seizure plus and could modulate the patient's response to anti-epileptic medications.

IntroductionAccess to maternal and newborn healthcare has improved in Myanmar. However, regular contact with skilled care providers does not necessarily result in quality care. We assessed adequate contact made by women and newborns with skilled care providers, reception of high-quality care and quality-adjusted contacts during antenatal care (ANC), peripartum care (PPC) and postnatal care (PNC) in Myanmar.MethodsThis cross-sectional study was conducted in a predominantly urban township of Yangon and a predominantly rural township of Ayeyawady in March 2016. We collected data from 1500 women. We measured quality-adjusted contact, which refers to adequate contact with high-quality care, as follows: ≥4 ANC contacts and receiving 11–14 of 14 intervention items; facility-based delivery assisted by skilled care providers, receiving 7 of 7 PPC intervention items; and receiving the first PNC contact ≤24 hours postpartum and ≥2 additional contacts, and receiving 16–17 of 17 intervention items. Using multilevel logistic regression analysis with a random intercept at cluster level, we identified factors associated with adequate contact and high-quality ANC, PPC and PNC.ResultsThe percentage of crude adequate contact was 60.9% for ANC, 61.3% for PPC and 11.5% for PNC. However, the percentage of quality-adjusted contact was 14.6% for ANC, 15.2% for PPC and 3.6% for PNC. Adequate contact was associated with receiving high-quality care at ANC, PPC and PNC. Being a teenager, low educational level, multiparity and low level in the household wealth index were negatively associated with adequate contact with healthcare providers for ANC and PPC. Receiving a maternal and child health handbook was positively associated with adequate contact for ANC and PPC, and with receiving high-quality ANC, PPC and PNC.ConclusionWomen and newborns do not receive quality care during contact with skilled care providers in Myanmar. Continuity and quality of maternal and newborn care programmes must be improved.
