Vector-borne diseases in Iran: epidemiology and key challenges

Vector-borne diseases have become a global health concern in recent decades as a result of global warming, globalization, growth in international trade and travel, use of insecticide and drug resistance. This review study addressed the key vector-borne diseases and their current status in Iran to emphasize the requirements for further research on vector-borne diseases. The dispersion patterns of these diseases differ in various regions. Some of them such as Crimean–Congo hemorrhagic fever, and Q fever are distributed all across Iran, whereas some others such as plague, leishmaniasis, tularemia, and malaria are restricted to specific areas. The high prevalence of vectors throughout the country necessitates enhancing the monitoring and surveillance of emerging and reemerging vector-borne diseases and their potential vectors.

Dengue, Chikungunya and Zika are the vector-borne diseases that constitute a potential epidemiological risk due to the recent increase in cases, complications, and severity. The co-circulation of the three diseases is a matter of public health interest due to their transmission by the same vector as well as the increase in the number of cases of severe dengue hemorrhagic fever, post-chikungunya chronic joint disease and microcephaly related to Zika virus. Therefore, it is important to be familiar with the various clinical presentations and laboratory methods to make the differential diagnosis, start appropriate treatment, and prevent the associated complications. Vector-borne emerging and re-emerging diseases pose considerable public health problem worldwide. Some of these diseases are emerging and/or re-emerging at increasing rates and appeared in new regions in the past two decades. Studies emphasized that the interactions among pathogens, hosts, and the environment play a key role for the emergence or re-emergence of these diseases. Furthermore, social and demographic factors such as human population growth, urbanization, globalization, trade exchange and travel and close interactions with livestock have significantly been linked with the emergence and/or re-emergence of vector-borne diseases. Other studies emphasize the ongoing evolution of pathogens, proliferation of reservoir populations, and antimicrobial drug use to be the principal exacerbating forces for emergence and re-emergence of vector-borne infectious diseases. Still other studies equivocally claim that climate change has been associated with appearance and resurgence of vector-borne infectious diseases. Despite the fact that many important emerging and re-emerging vector-borne infectious diseases are becoming better controlled, our success in stopping the many new appearing and resurging vector-borne infectious diseases that may happen in the future seems to be uncertain. Hence, this paper reviews and synthesizes the existing literature to explore global patterns of emerging and re-emerging vector-borne infections and the challenges for their control. It also attempts to give insights to the epidemiological profile of major vector-borne diseases including Zika fever, dengue, West Nile fever, Crimean-Congo hemorrhagic fever, Chikungunya, Yellow fever, and Rift Valley fever.

Vector-borne diseases (VBDs) have re-emerged worldwide due to urbanisation, increase in travel and climate change, becoming a major and serious threat to global public health. In Qatar, the concern has recently risen because of the attribution of the soccer 2022 FIFA World Cup Qatar™, which necessitates fulfilling requirements in terms of prevention and preparedness for disease transmission, including VBDs. This review presents a general overview about current status of vectors and VBDs in Qatar and addresses key challenges and future prospects of control programmes and strategies. It is based on a vector control situation analysis and needs assessment performed during an expert mission in Qatar, November 2017, organised by the WHO Eastern Mediterranean Regional Office and achieved in collaboration with the Ministry of Public Health of Qatar and other local stakeholders. The situation of vectors and VBDs of public health importance in Qatar was analysed based on a systematic literature review by December 31, 2019. The literature reveals that no locally transmitted VBD cases have been recorded in Qatar, but cases were recorded among expatriate workers and travellers who returned from an endemic country. However, data on VBD cases remain scarce except for malaria. The presence of native arthropod vectors is under-recorded to date. A compilation of literature data revealed reports of 30 vector species, including 20 mosquitoes, 2 fleas, 1 louse, 1 fly, and 6 ticks. Overall, Qatar benefits currently from a good surveillance of some VBDs (malaria) and has some capacities in vector control, but no national plan exists, and vector surveillance is in its infancy. In Qatar, clear needs exist in capacity in epidemiology and vector entomology, as well as on the organisational level, and a number of measures are suggested to mitigate and improve VBD risk assessment and management. There is an urgent need to define sustainable solutions for VBD control, management and prevention, and a number of recommendations are suggested.

Mosquito-borne diseases are a major public health concern. Because of absence of effective therapy and vaccination to treat and prevent such vector-borne diseases, these initial cases have triggered strong concerns within medical communities, turning a bad dream into reality. To date, reducing the mosquito vector population remains the main strategy to control the transmission of mosquito borne diseases. In this connection, community-based perception recognized as an essential tool for long-term vector control management. In this study, a community perception survey was carried out in six selected study areas of Ganjam district of Odisha. A total of 3257 inhabitants from randomly chosen 600 no. of houses from 06 selected study areas were interviewed using a pre-tested structured questionnaire about various mosquito vector-borne diseases. As an outcome, we found that only 36% of people have basic knowledge and idea about vector-borne diseases, out of which only 14% know Anopheles as a causative agent whereas Aedes and Culex were very least concern. Regarding the source of knowledge, electronic media was the principal source of information, while the involvements of health personnel were least mentioned. Results of the current study indicates the necessity to plan health education program to bring important perceptions regarding vector-borne diseases at school and college level because the infrequent source of knowledge is the main cause of an increase in the disease incidence.

Vector-borne diseases are still a health problem in some tropical countries. One vector-borne disease is Dengue Hemorrhagic Fever (DHF). DHF never decreases and even tends to continue to increase, and many cause deaths in children, 90% of them attack children under 15 years. DHF cases in 2018 amounted to 65,602 cases, with 467 deaths (CFR = 0.71%). The behavior of holding water in various places such as tubs and tendons indirectly creates a breeding place for Aedes aegypti mosquitoes. The use of chemicals as insecticides can cause the death of non-target animals, environmental pollution, and the occurrence of vector resistance to insecticides. Therefore it is necessary to do other methods including the use of vegetable insecticides. Sugar apple leaves can be used to kill Aedes aegypti larvae. Sugar apple leaves will obtain throughout the year. The purpose of this study is to know the ability of young and old sugar apple leaves to kill Aedes aegypti larvae. This type of research is a posttest only control group design. The study sample was part of the Aedes aegypti larvae on the final instar III. The experiment to kill Aedes aegypti larvae use two types of sugar apple leaf extract, namely young and old sugar apple leaves with nine treatments four replications. Each treatment consisted of 25 larvae. The research results show There was no significant difference in the number of dead larvae using either extracts from young sugar apple leaves or old sugar apple leaves. LC90 of sugar apple leaf extract was between 0.05632 to 0.08324% and the effective residual age at LC90 (0.06568%) with the death of Aedes aegypti larvae was 92% over 24 hours (1 day).

Different nematodes affect canines, however, Dirofilaria immitis is the most prevalent filariae. D. immitis causes heartworm disease that can be fatal for dogs and potentially transmitted to humans. Climate change, animal migration, among other factors have changed the dynamics of vector-borne diseases, including filariasis. In the last four years, a sudden increase of dogs with microfilaremia was reported by different veterinary centers in Cali, the main city of Southwest Colombia. The objective of this study was to molecularly identify the etiologic agent of this filariasis outbreak reported in this area from 2018-2019, using a novel PCR–RFLP method. Those filariasis cases were initially detected by microscopic and serological tests. A total of 82 canine filariasis cases were molecularly analyzed, identifying 55 (67%) of them as Acantacheilonema reconditum. PCR-sequencing was performed in eight cases confirming this finding. The filariasis cases were statistically associated with male dogs who had clinical signs of anemia, with low levels of hemoglobin and hematocrit (p <0.0001), and high levels of plasma proteins (p <0.001). This emerging canine disease constitutes an important public health concern among clinicians and advises active surveillance to explore its zoonotic potential.

The microbiota is increasingly recognized for its ability to influence host health and individual fitness through multiple pathways, such as nutrient synthesis, immune system development, and even behavioral processes. Most of these studies though focus on the direct effects microbiota has on its host, but they do not consider possible interactions with other individuals. However, host microbiota can change not only host behavior but also the behavior of other individuals or species toward the host. For example, microbes can have an effect on animal chemistry, influencing animal behaviors mediated by chemical communication, such as mosquito attraction. We know that host skin microbes play a major role in odor production and thus can affect the behavior of mosquitoes leading to differences in attraction to their hosts. Ultimately, the vector feeding preference of mosquitoes conditions the risk of vertebrates of coming into contact with a vector-borne pathogen, affecting its transmission, and thus epidemiology of vector-borne diseases. In this mini review, I provide an overview of the current status of research on the interaction between mosquito behavior and host skin microbiota, both in humans and other vertebrates. I consider as well the factors that influence vertebrate skin microbiota composition, such as sex, genetic makeup, and infection status, and discuss the implications for pathogen transmission.

Background: One of the factors that lead to high incidence of DHF is climate change.Objective: To analyze the effect of climate factors (temperature, humidity, wind speed, and rainfall) associated with DHF incidence in Banjarmasin City, 2012-2016.  Methods: We used the national data on annual reported incidence from Health Office of Banjarmasin City and climate variations from Meteorology Climatology and Geophysics Agency 2nd Class Climatology Station Syamsudin Noor Banjarmasin, January 2012-December 2016.  The analysis techniques using path analysis to explained the mechanism of causal relationships between variables.Results: The result showed the overall incidence of DHF in Banjarmasin City during 2012- 2016 was 243 cases, of DHF cases were fluctuates by the monthly trend, where the highest number of DHF cases in January to March, climate variation which occurred in Banjarmasin City period 2012-2016 included temperatures ranged from 25.8-28.7°C, humidity ranged from 65-88%, wind speed ranged from 4-6 knots and rainfall ranged from 0.0-546.7 mm, and the path analysis showed that rainfall variable (X4) was the only variable which positively effected to DHF incidence variable (Y) equal to 0.613 unit (Y = 0.613 X4) (p value = 0.002).Conclusion: Climate information can used as a precautionary signal through early warming of the readiness in facing the outbreaks of vector borne diseases so that further efforts in environmental management by manipulation method and environmental modification.

Background: Countries in the World Health Organization (WHO) Eastern Mediterranean Region (EMR) are predisposed to highly contagious, severe and fatal, emerging infectious diseases (EIDs), and re-emerging infectious diseases (RIDs). This paper reviews the epidemiological situation of EIDs and RIDs of global concern in the EMR between 2001 and 2018. Methods: To do a narrative review, a complete list of studies in the field was we prepared following a systematic search approach. Studies that were purposively reviewed were identified to summarize the epidemiological situation of each targeted disease. A comprehensive search of all published studies on EIDs and RIDs between 2001 and 2018 was carried out through search engines including Medline, Web of Science, Scopus, Google Scholar, and ScienceDirect. Results: Leishmaniasis, hepatitis A virus (HAV) and hepatitis E virus (HEV) are reported from all countries in the region. Chikungunya, Crimean Congo hemorrhagic fever (CCHF), dengue fever, and H5N1 have been increasing in number, frequency, and expanding in their geographic distribution. Middle East respiratory syndrome (MERS), which was reported in this region in 2012 is still a public health concern. There are challenges to control cholera, diphtheria, leishmaniasis, measles, and poliomyelitis in some of the countries. Moreover, Alkhurma hemorrhagic fever (AHF), and Rift Valley fever (RVF) are limited to some countries in the region. Also, there is little information about the real situation of the plague, Q fever, and tularemia. Conclusion: EIDs and RIDs are prevalent in most countries in the region and could further spread within the region. It is crucial to improve regional capacities and capabilities in preventing and responding to disease outbreaks with adequate resources and expertise.
