Chronic rhinosinusitis is an inflammatory disease of the nasal mucosa and paranasal sinuses which produces several proinflammatory cytokines including; IFN-γ, TGF-β1, IL-1β, IL-3, IL-6, IL-8, TNF-α, IL-5. The use of NaCl  0.9% nasal  wash in  chronic  rhinosinusitis  could reduce  mucin  secretion,  decrease  the  production  of postnasal  drip,  accelerate  mucosal  repair  and  reduce  the  symptoms  of  nasal  obstruction.  From  above, researchers want to know the effect of NaCl 0.9% nasal wash of the levels of cytokines IL-1β and TNF-α in the mucosa  of  the  nose  and  paranasal  sinuses  in  patients  with  chronic  rhinosinusitis.  This  research  is  an experimental study with the technique of pre and post test design to determine the effect of NaCl 0.9% nasal wash of the gene expression of IL-1β and TNF-α of nasal mucosa of patients with chronic rhinosinusitis. The amount  of IL-1β gene copynumber before and  after nasal  wash is  obtained 8.07 ± 0.95  and 8,20 ± 0.93 (p >0.05). The amount of TNF-α gene copynumber before and after nasal wash was 8,83 ±3,83 and 6,72 ±2,55 (p >0.05). IL-1β gene ratio starting and ending intervention in two groups was 52,51 ± 1.21 and 61,99 ± 1.13. TNF-α gene ratio starting and ending intervention in two groups was 9,63 ±2.21 and 334,4 ±1.31. In this study there was no significant reduction in the absolute expression (log copynumber) gene IL-1β and TNF-α of nasal mucosa after being given medical treatment with NaCl 0,9% nasal wash.

The objective of the research is to study of clinical and functional disorders of the nasal mucosa and paranasal sinuses of patients with chronic rhino sinusitis, as well as to make the choice of the optimal treatment method in a sanatorium using natural factors. A survey of 145 patients (94 men and 51 women) with chronic rhinosinusitis at the age of 18 to 65 years was conducted. Of these, male accounted for 64.8%, female – 35.2%. In patients with pathology of the paranasal sinuses, functional disorders of the nasal mucosa and paranasal sinuses were revealed. Particularly, violations of mucociliary clearance, acid-base balance of the nasal mucosa, and a decrease in nasal patency according to the data of active anterior rhinomanometry were defined. The use of natural factors as a treatment: inhalation of natural mineral water Maibalyk and mud applications on the paranasal sinus area significantly improved the course of way of chronic rhinosinusitis and led to normalization of the functional parameters of the nasal mucosa, in contrast to the comparison group (60 people). The comparison group did not use with Maibalyk natural water inhalations and mud applications from Lake Maybalyk, but used only ordinary alkaline inhalations.

Immunomodulatory and Clinical Effects of Long-Term Low-Dose Macrolide Treatment of Chronic Rhinosinusitis with Nasal PolyposisImmunomodulatory treatment of chronic rhinosinusitis with nasal polyposis (CRSwNP) by macrolide antibiotics represents a challenging alternative to conventional therapy and surgery, still being at the very beginning. Immune and inflammatory processes in nasal and paranasal sinus mucosa, crucial in the etiopathogenesis of nasal polyps (NPs) are reflected in levels of various local mediators, found both in mucosa and nasal fluid. In this prospective study, we assessed the immunomodulatory and clinical effects of longterm low-dose oral macrolide treatment in the management of CRSwNP. Twenty-two (n = 22) nonasthmatic, nonallergic patients with CRSwNP were administered clarithromycin (CAM) 500 mg/day single oral dose for eight weeks. We measured the levels of proinflammatory cytokines TNF-α, TNF-β, and IL-1β, Th1 cytokines IL-2, IL-12, and IFN-γ, Th2 cytokines IL-4, IL-5, IL-6, and IL-10, and chemokine IL-8 in the nasal fluid samples, before and after treatment, using a flow cytometric method. We also scored each of the 22 patients before and after therapy according to Tsicopoulos' global nasal symptom score and Malm's endoscopic score. Following treatment, we found significantly reduced levels of IL-8 (p<0.01) and TNF-α (p<0.01) in nasal secretions. Macrolide therapy decreased the size of polyps in 45.45% of the patients. We concluded that long-term low-dose treatment with CAM was effective in the management of CRSwNP. We suggest that macrolides can be an alternative to topical and systemic corticosteroids in the management of CRSwNP.

Background. Several animal studies have shown the roles of cytokines in regulating liver regeneration following liver resection (LR), which is a type of surgery designed to remove cancerous tumors from the liver. This study investigated how the expressions and serum levels of some pro- and anti-inflammatory cytokines in patients with hepatocellular carcinoma (HCC) were changed during LR. Methods. Liver tissues from 15 patients with HCC were collected and the levels of interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-α), IL-1α, IL-1 β, IL-10, and transforming growth factor-beta1 (TGF-β1) were assessed using real-time PCR assay at different times before and after LR. The serum values of TNF-α and IL-6 were also measured by ELISA. Results. After 60 and 90 minutes of LR, IL-6 gene expression was significantly increased P < 0.001 − 0.05 . The same trend was also observed in TNF-α expression after 90 minutes of LR P < 0.01 . No significant changes were observed in the expressions of IL-1α, IL-1β, IL-10, and TGF-β1 before and after LR. In addition, LR had significant effects on TNF-α and IL-6 serum levels P < 0.05 − 0.0001 . Conclusion. Our data provided further evidence to reveal that IL-6 and TNF-α cytokines are critical to improve liver regeneration.

Background: Malnutrition and immunology change in stroke would affect the outcome of the stroke patient. The supplement of the extract of snakehead fish as an alternative is expected could reduce the effect pasca stroke.Objective: This study aims to assess the effect of snakehead fish supplementation on nutritional status, levels of albumin, TLC and TNF-α in stroke patients.Method: This type of study is a quasi-experimental, pre-test post-test group design, by providing treatment to the subject of research and treatment effects were measured and analyzed. Subjects were divided into two groups; a first group which receives 3 x 2 capsule of snakehead fish supplementation as well as diet and nutrition education (intervention group) and the second group that received only diet and education (control group).Results: From the results, it was found that nutritional status, which was obtained by measuring UAC in cases and controls are good (SG ≥ 85%) but no significant differences between them (p=0.914). There are significant differences between albumin levels before and after intervention (p=0.023) with an average increase of 0.2 g/dl, but not in the control group (p=0.931). For TLC values increased in the intervention group (990,5 sel/mm3) and a decrease in the control group (645,03 sel/mm3) with a significant difference in both groups (p=0.034). There was a decrease of TNFα in both group; for cases (mean=-2.91) and controls (mean=-2.58) with significant differences between the two groups (p=0.001).Conclusion: The supplementation of an extract of snakehead fish could increase the albumin and TLC level and decrease the TNFα in stroke patients.

Abstract: In elderly, the biological function of the systems in human body will decrease. Physical exercise is a systematical and programmed exercise to increase the body function and skills. Overtraining will influence the inflammatory response marked by the release of proinflammatory cytokines inter alia TNF-α. This was an experimental study with a post test design. Respondents were elderly in Panti Werdha Bethania Lembean trained for physical exercise by using dumbbells of 1-2 kg for 10-15 minutes, 3 times a week. Of 25 respondents, before the physical exercise there were 4 respondents with TNF-α above normal level (>100 pg/ml) and 21 respondents within normal level (10-100 pg/ml). After the 5-week physical exercise, there were 21 respondents with decreased TNF-α level and 4 respondents with increased TNF-α level. Moreover, there were no elderly with TNF-α level <10 pg/ml before and after physical exercise. Conclusion: Among elderly in Panti Werdha Bethania Lembean the averages of TNF-α level before and after physical training were within normal limit. However, there was a decrease of the average level of TNF-α after physical training.Keywords: physical training, elderly, TNF-αAbstrak: Pada lanjut usia (lansia), setiap individu akan mengalami penurunan fungsi biologis dari berbagai sistem dalam tubuh. Latihan fisik merupakan suatu aktivitas yang dilakukan secara sistematis dan terencana dalam meningkatkan fungsional tubuh dan keterampilan. Latihan olahraga berat akan merangsang respon inflamasi yang ditandai dengan pelepasan sitokin pro-inflamasi antara lain TNF-α. Penelitian ini bersifat eksperimental dengan rancangan post test design. Penelitian dilakukan terhadap lansia di Panti Werdha Bethania Lembean selama 5 minggu. Latihan beban 1-2 kg selama 10-15 menit dilakukan 3 kali seminggu. Dari 25 responden, sebelum latihan beban terdapat 4 orang dengan kadar TNF-α di atas batas normal (>100 pg/ml) dan 21 orang dalam batas normal (10-100 pg/ml). Setelah melakukan latihan beban terdapat 21 responden dengan penurunan kadar TNF-α dan 4 responden dengan kenaikan kadar TNF-α. Sebelum dan sesudah latihan beban tidak terdapat responden dengan kadar TNF-α <10 pg/ml. Simpulan: Pada lansia Panti Werdha Bethania Lembean rerata kadar TNF-α sebelum dan sesudah latihan beban masih dalam batas normal. Walaupun demikian, terdapat penurunan rerata kadar TNF-α sesudah latihan beban.Kata kunci: latihan beban, lansia, TNF-α

Abstract. In a recent paper, Ferrer and Pardo (2014) tested several distribution-based methods designed to assess when test scores obtained before and after an intervention reflect a statistically reliable change. However, we still do not know how these methods perform from the point of view of false negatives. For this purpose, we have simulated change scenarios (different effect sizes in a pre-post-test design) with distributions of different shapes and with different sample sizes. For each simulated scenario, we generated 1,000 samples. In each sample, we recorded the false-negative rate of the five distribution-based methods with the best performance from the point of view of the false positives. Our results have revealed unacceptable rates of false negatives even with effects of very large size, starting from 31.8% in an optimistic scenario (effect size of 2.0 and a normal distribution) to 99.9% in the worst scenario (effect size of 0.2 and a highly skewed distribution). Therefore, our results suggest that the widely used distribution-based methods must be applied with caution in a clinical context, because they need huge effect sizes to detect a true change. However, we made some considerations regarding the effect size and the cut-off points commonly used which allow us to be more precise in our estimates.

Hepatitis has become a health problem in the world. The hepatitis virus infected many people. According to the teacher of MTsN 02 Bondowoso more than 20 students have hepatitis A viral infection. The purpose of this research was to know the differences of students' knowledge about hepatitis A before and after counseling in MTsN 02 Bondowoso 2015. This study used pre-experimental (pre-post test design). This study used stratified random sampling technique, 127 students from 270 sample involved this research,and 143 students was excluded. We used questionnaires to collect data. The results showed that the mean value of the students 'knowledge about hepatitis A before counseling in MTsN 02 Bondowoso 2015 was 83.96 with the lowest value of 37.5 and the highest value was 100. The mean value of the students' knowledge about hepatitis A after counseling in MTsN 02 Bondowoso 2015 was 93.21 with the lowest value waf 62.5 and the highest value was 100. Paired t test showed that t (-9.07) > t table (1.98), the null hypothesis (H0) was rejected. There was a difference between students' knowledge about hepatitis A before and after counseling in MTsN 02 Bondowoso 2015. This study showed that routine counseling by healthcare provider was important to prevent hepatitis A infection.; Keywords: counseling, knowledge of students, hepatitis
