Agriculture is the origin of existence and survival in any society. However, this sector is always associated with risk and uncertainty, and farmers are faced with many challenges. Therefore, the main aim of this research was to explore the (production) risk factors of irrigated wheat production systems among farmers in Kermanshah province, Iran. The exploratory nature of this study on the one hand and the need for expert judgment on the other hand necessitated a Delphi research design. Thus, this study utilized a three-round Delphi technique. The population comprised of 10 subject-matter experts (SME) and 32 agricultural extension experts in two climates (warm and semi-arid and cold and moderate) in seven counties in Kermanshah province. A purposeful (complete) sample of 42 experts in the agricultural field participated in this study. Data were collected using a questionnaire designed on the basis of the Delphi technique. The findings indicated that, after three rounds of Delphi technique, the panel members reached a consensus on 75 factors which were categorized into biological, natural, managerial, and technological-structural groups for production risk. The results of this research provide useful insights for researchers, extension agents, and farmers.

Leptospirosis is a zoonotic disease of public health importance in India. A country-level evidence gap map was developed to identify gaps on epidemiology of leptospirosis. It is the first such on leptospirosis globally and on any single disease condition in India. The steps for development of evidence gap map were development of a framework to map evidence, retrieval of evidence, data extraction parameters and mapping of available evidence in evidence gap map framework. The prevalence evidence gap map consisted of 157 studies (102 in humans, 55 in animals, and 12 in both). The evidence gap map on risk factors had 120 studies (102 in humans, 11 in animals and 7 in both). There were inter-state differences in availability of research and disparity between animal and human research. Research on high-risk groups was limited and studies did not use the One Health approach to identify epidemiology, which can help understand the issue more comprehensively. The study demonstrates the potential of evidence gap maps to inform research priorities.

AbstractKawasaki disease (KD) is an acute systemic vasculitis that mainly affects infants and young children. The etiology of KD has been discussed for several decades; however, no reproducible risk factors have yet been proven. We used the Japan Environment and Children’s Study data to explore the association between the causal effects of exposure during the fetal and neonatal periods and KD onset. The Japan Environment and Children’s Study, a nationwide birth cohort study, has followed approximately 100,000 children since 2011. We obtained data on exposures and outcomes from the first trimester to 12 months after birth. Finally, we included 90,486 children who were followed for 12 months. Among them, 343 children developed KD. Multivariate logistic regression revealed that insufficient intake of folic acid during pregnancy (odds ratio [OR], 1.37; 95% CI 1.08–1.74), maternal thyroid disease during pregnancy (OR, 2.03; 95% CI 1.04–3.94), and presence of siblings (OR, 1.33; 95% CI 1.06–1.67) were associated with KD onset in infancy. In this study, we identified three exposures as risk factors for KD. Further well-designed studies are needed to confirm a causal relationship between these exposures and KD onset.

AbstractQuantifying natural gas production risk can help guide natural gas exploration and development in Carboniferous gas reservoirs. In this study, the Monte Carlo probability method is used to obtain the probability distribution and growth curve of each production risk factor and production in a Carboniferous gas reservoir in eastern Sichuan. In addition, the fuzzy comprehensive evaluation method is used to conduct the sensitivity analysis of the risk factors, and the natural gas production and realization probability under different risk factors are obtained. The research results show that: (1) the risk factor–production growth curve and probability distribution are calculated by the Monte Carlo probability method. The average annual production under the stable production stage under different realization probabilities is obtained. The maximum probability range of annual production is $$\left( {43.43 - 126.35} \right) \times 10^{8} {\text{m}}^{3} /{\text{year}}$$ 43.43 - 126.35 × 10 8 m 3 / year , and the probability range is 14.59–92.88%. (2) The risk factor sensitivity analysis is significantly affected by the probability interval. In the entire probability interval, the more sensitive risk factors are the average production of the kilometer-deep well (D) and the production rate in the stable production stage (A). During the exploration and development of natural gas, these two risk factors can be adjusted to increase production.

SUMMARYUnprecedented high rates of anthrax outbreaks have been observed recently in cattle and humans in Bangladesh, with 607 human cases in 2010. By enrolling 15 case and 15 control cattle smallholdings in the spatial zone in July–September 2010, we conducted a case-control study, data of which were analysed by matched-pair analysis and multivariable conditional logistic regression. Feeding animals with uprooted and unwashed grass [odds ratio (OR) 41·2, 95% confidence interval (CI) 3·7–458·8, P=0·003], and feeding water hyacinth (Eichhornia crassipes) (OR 22·2, 95% CI 1·2–418·7, P=0·039) were independent risk factors for anthrax in cattle.

Abstract Background American cutaneous leishmaniasis (CL) is a neglected tropical disease typically associated with men working in remote, sylvatic environments. We sought to identify CL risk factors in a highly deforested region where anecdotal reports suggested an atypical proportion of women and children were infected with CL raising concern among authorities that transmission was shifting towards domestic spaces and population centers. Methods We describe the characteristics of CL patients from four participating clinics after digitizing up to 10 years of patient data from each clinic’s CL registries. We assessed risk factors of CL associated with intradomestic, peridomestic, or non-domestic transmission through a matched case-control study with 63 patients who had visited these same clinics for CL (cases) or other medical reasons (controls) between January 2014 and August 2016. The study consisted of an in-home interview of participants by a trained field worker using a standard questionnaire. Risk factors were identified using bivariable and multivariable conditional logistic regression. Results Between 2007 and 2016, a total of 529 confirmed CL positives were recorded in the available CL registries. Children and working aged women made up 58.6% of the cases. Our final model suggests that the odds of sleeping in or very near an agricultural field were five times greater in cases than controls (p = 0.025). Survey data indicate that women, children, and men have similar propensities to both visit and sleep in or near agricultural fields. Conclusions Women and children may be underappreciated as CL risk groups in agriculturally dependent regions. Despite the age-sex breakdown of clinical CL patients and high rates of deforestation occurring in the study area, transmission is mostly occurring outside of the largest population centers. Curbing transmission in non-domestic spaces may be limited to decreasing exposure to sandflies during the evening, nighttime, and early morning hours. Our paper serves as a cautionary tale for those relying solely on the demographic information obtained from clinic-based data to understand basic epidemiological trends of vector-borne infections.
