Our aims were to investigate vitamin A and E status during lactation and the determinants of breast milk content for the appropriate nutrition of the infant in a study with nursing Brazilian women. We hypothesized that both inadequate intake and the lipoprotein distribution of vitamin A and E during lactation could have an impact on their breast milk levels from early- to mid-lactation. Nineteen adult lactating women participated in this longitudinal observational study, in which dietary records, blood and mature breast milk samples were collected for the analysis of vitamin A and E, and carotenoids in early- (2nd to 4th week) and mid-lactation (12th to 14th week). Nutrient intake was balanced by the Multiple Source Method (MSM), and the intake of vitamin A and E was inadequate in 74 and 100% of the women, respectively. However, these results were not reflected in low serum concentrations of retinol and only 37% of the volunteers were vitamin E deficient according to the blood biomarker. As lactation progressed, vitamin A and E status worsened, and this was clearly observed by the decrease in their content in breast milk. The reduced content of vitamin A and E in the breast milk was not related to their distribution in lipoproteins. Taken together, the contents of vitamin A and E in breast milk seemed to be more sensitive markers of maternal nutrition status than respective blood concentrations, and dietary assessment by the MSM in early lactation was sensitive to indicate later risks of deficiency and should support maternal dietary guidance to improve the infant’s nutrition.

ABSTRAKASI merupakan sumber makanan yang mengandung nutrisi yang lengkap untuk bayi. Rendahnya cakupan ASI di Indonesia menyumbang akibat yang tidak baik bagi kesehatan bayi. Berdasarkan hasil survey pendahuluan, dari 10 ibu menyusui 6 (60%) diantaranya mengatakan bahwa pengeluaran ASI mereka tidak lancar. Salah satu solusi yang tepat untuk meningkatkan suplai ASI adalah dengan mengonsumsi daun ubi jalar yang dipercaya mengandung berbagai macam zat dan vitamin terutama vitamin A. Tujuan penelitian ini adalah mengetahui pengaruh pemberian rebusan daun ubi jalar terhadap kecukupan ASI pada ibu menyusui di Wilayah Kerja Puskesmas Campurejo Tahun 2016. Desain penelitian ini adalah pre eksperimental menggunakan rancangan pretest posttest design. Dalam penelitian ini diperoleh sampel 20 orang menggunakan teknik simple random sampling. Pengumpulan data dilakukan dengan menggunakan lembar kuesioner dan penilaian. uji hipotesis yang digunakan adalah uji peringkat bertanda dari wilcoxon dengan nilai signifikasi α = 0,05. Hasil uji statistik dari Wilcoxon Sign Rank diperoleh nilai p value = 0,000 dengan taraf kesalahan (α =0,05) dapat dikatakan p ≤ α maka dapat disimpulakan bahwa H0 ditolak dan H1 diterima. Hal ini berarti ada pengaruh pemberian rebusan daun ubi jalar terhadap kecukupan ASI pada ibu menyusui diwilayah kerja puskesmas campurejo kota Kediri tahun 2016. Berdasarkan hasil penelitian diharapkan bagi tenaga kesehatan dapat  dijadikan sebagai masukan untuk menggunakan daun ubi jalar untuk meningkatkan kecukupan ASI dalam pengembangan pengobatan tradisional. Kata Kunci : Kecukupan ASI, Daun Ubi jalar, Menyusui   ABSTRACTBreast milk is a food source that contains a complete nutritions for infants. Coverage of breast milk in Indonesia is still very low. Based on the preliminary results of the survey, from 10 breastfeeding mothers, there are 6 (60%) breastfeeding mom of them say that their breast milk isn’t enough for the babies. There’s a rightest solution to increase the breast milk supply, such as; the breastfeeding mom should eat or consume the sweet potato leaves that believed contains various nutritions and vitamins, especially vitamin A. The purpose of this study was to know the effect of giving sweet potato extract to breast milk adequacy on breastfeeding mothers in work area clinic of Campurejo Kediri City in 2016. The research design of this study was pre-experimental that using pre-test and post-test design. In this study obtained 20 samples of people that using simple random sampling technique. The collecting of data is done by using a questionnaires and assessment. The test hypothesis is marked on the Wilcoxon rank test with significance value α = 0.05. The result of statistical test Wilcoxon Sign Rank obtained p value = 0.000 with a standard error (α = 0.05) can be said p ≤ α then it means that H0 is rejected and H1 is accepted. This means that there is an effect of giving sweet potato extract to breast milk adequacy on breastfeeding mothers in work area clinic of Campurejo Kediri City in 2016. Based on the results is the researcher expecting for health workers can be used as input to use sweet potato leaves to improve the adequacy of breast milk in the development of traditional medicine. Keywords: Adequacy of Breast Milk, Sweet Potato Leaves, Breastfeeding.

Abstract Maternal nutrition status plays an important role in the development of fetal alcohol spectrum disorders (FASD), but its direct evidence is lacking. This study compared a standard chow with a semi-purified energy dense (E-dense) diet on birth and metabolic outcomes in rats after ethanol (EtOH) consumption during pregnancy. Pregnant Sprague-Dawley rats were randomized into four groups: chow (n=6), chow+EtOH (20% v/v) (n=7), E-dense (n=6), and E-dense+EtOH (n=8). Birth outcomes including litter size, body and organ weights were collected. Metabolic parameters were measured in dams and pups at postnatal day (PD) 7. Maternal EtOH consumption decreased body weights (p <0.0001) and litter sizes (p <0.05) in chow-fed dams. At PD7, pups born to dams fed E-dense diet had higher body (p <0.002) and liver weights (p <0.0001). These pups also had higher plasma total cholesterol (p <0.0001), triacyclglycerol (p <0.003) and alanine aminotransferase (p <0.03) compared to those from chow-fed dams. Dams fed E-dense diet had higher plasma total- (p <0.0001) and HDL-cholesterol (p <0.0001) and lower glucose (p <0.0001). EtOH increased total cholesterol (p <0.03) and glucose (p <0.05) only in dams fed the E-dense diet. Maternal exposure to E-dense diet attenuated prenatal EtOH-induced weight loss and produced different metabolic outcomes in both dams and pups. While the long-lasting effects of these outcomes are unknown, this study highlights the importance of maternal diet quality for maternal health and infant growth, and suggests that maternal nutrition intervention may be a potential target for alleviating FASD.

Background: The nutritional status of vitamin A in lactating mothers and infants is still not optimistic. Due to the dietary habits and dietary restrictions of postpartum customs in China, vitamin A supplementation has been advocated as a potential strategy to improve vitamin A status of lactating mothers with inadequate dietary vitamin A intake. Existing clinical trials are limited to single or double high-dose maternal administrations. However, in China, vitamin A supplements are readily available in the form of daily oral low-dose supplements, and the effect of these is unknown. This study aimed to evaluate the effects of daily oral low-dose vitamin A supplementation on the retinol levels in the serum and breast milk of lactating mothers and the health status of infants in China. Methods: Lactating mothers who met the inclusion criteria and planned to continue exclusive breastfeeding were randomly assigned to receive either daily oral vitamin A and D drops (one soft capsule of 1800 IU vitamin A and 600 IU vitamin D2), or a matching placebo for 2 months. Before and after the intervention, dietary intake was investigated by instant photography, and the retinol concentration in maternal serum and breast milk was determined by ultra-high performance liquid chromatography-tandem mass spectrometry. During the trial, the health status of infants was diagnosed by a paediatrician or reported by lactating mothers. A total of 245 participants completed the study, with 117 in the supplementation group and 128 in the control group. Results: After the 2-month intervention, maternal serum retinol concentrations increased in the supplementation group with no change in the control group. Although breast milk retinol concentrations decreased significantly in both groups, the decrease in the supplementation group was significantly lower than that in the control group. However, maternal vitamin A supplementation was not associated with a lower risk of infant febrile illness, respiratory tract infection, diarrhoea, and eczema. Conclusions: Daily oral low-dose vitamin A supplementation is helpful in improving maternal vitamin A status, despite having no effect on infant health status through breast milk.

Background: Osteopontin (OPN) is a glycosylated phosphoprotein found in human tissues and body fluids. OPN in breast milk is thought to play a major role in growth and immune system development in early infancy. Here, we investigated maternal factors that may affect concentrations of OPN in breast milk, and the possible associated consequences for the health of neonates. Methods: General characteristics, health status, dietary patterns, and anthropometric measurements of 85 mothers and their babies were recorded antenatally and during postnatal follow-up. Results: The mean concentration of OPN in breast milk was 137.1 ± 56.8 mg/L. Maternal factors including smoking, BMI, birth route, pregnancy weight gain, and energy intake during lactation were associated with OPN levels (p < 0.05). Significant correlations were determined between body weight, length, and head circumference, respectively, and OPN levels after one (r = 0.442, p = < 0.001; r = −0.284, p = < 0.001; r = −0.392, p = < 0.001) and three months (r = 0.501, p = < 0.001; r = −0.450, p = < 0.001; r = −0.498, p = < 0.001) of lactation. A negative relation between fever-related infant hospitalizations from 0–3 months and breast milk OPN levels (r = −0.599, p < 0.001) was identified. Conclusions: OPN concentrations in breast milk differ depending on maternal factors, and these differences can affect the growth and immune system functions of infants. OPN supplementation in infant formula feed may have benefits and should be further investigated.

AbstractMaternal vitamin intake during pregnancy is crucial for pregnancy outcomes and the child's subsequent health. However, there are few valid instruments for assessing vitamin intake that address the effects of nausea and vomiting during pregnancy (NVP). This study aimed to investigate the validity of a FFQ concerning vitamin intake during early and late pregnancy with and without NVP. The participants comprised 200 Japanese pregnant women who completed the FFQ and from whom blood samples were taken in early and late pregnancy. Energy-adjusted dietary vitamin intakes (vitamin C, folate, vitamin B6, vitamin B12, vitamin A, vitamin E and vitamin D) from FFQ were compared with their blood concentrations. A subgroup of women with NVP was investigated. In early pregnancy, significant correlations between FFQ and biomarkers were observed for vitamin C (r0·27), folate (r0·18) and vitamin D (r0·26) in women with NVP and for vitamin A (r0·18), vitamin B12(r0·24) and vitamin D (r0·23) in women without NVP. No significant correlations were observed in either group for vitamins B6or E. In late pregnancy, similar significant associations were observed for vitamin C (r0·27), folate (r0·22), vitamin B6(r0·18), vitamin B12(r0·27) and vitamin A (r0·15); coefficients were higher among women without NVP. Our study demonstrates that the FFQ is a useful tool for assessing intake of several important vitamins in early and late pregnancy regardless of NVP status.

Four cases of extensive seborrheic dermatitis or Leiner's disease are presented which dramatically improved after the nursing mother was given injections of biotin. The relationship between seborrheic dermatitis in infants and biotin deficiency is discussed and the literature reviewed. From this review it appears that breast milk is deficient in biotin in comparison to cow's milk. The deficiency is further aggravated by poor maternal nutrition. Infection and diarrhea in the infant may also contribute to low blood levels of biotin. On the basis of this limited experience, injections of biotin to the nursing mother appear to be a useful treatment for the breast-fed infant with extensive seborrheic dermatitis.

Background: Underweight is a public health problem caused by many factors. The prevalence of underweight in East Java in 2016 was 17.3%, wich was higher than the program’s targetted (12.9%). Breast milk is the best food to support the growth and development of the baby. Coverage of Exclusive Breast Milk in East Java in 2015 decreased (68.8%), compared to 2014 (72.89%). Early supplementary feeding in infants under 6 months may affect nutritional status. Objectives: The purpose of this study was to analyze the relationship of Exclusive Breast milk with the incidence of underweight in infants aged 0-23 months in East Java Province in 2016. Method: This study is a secondary data analysis of Nutrition Status Monitoring in East Java Province. The study population was all babies in East Java. Total sample is 4738. The data were analyzed using Chi-square test and Logistic Regression, with 95% CI (α = 0.05). Results: The results showed there was a significant relationship between exclusive breastfeeding only from birth until before the last 24 hours in infants aged 0-23 months with the incidence of underweight, with (p=0.000010;OR=1.654;95%CI=1.319–2.052), there was a significant  relationship of first to provide a food other than breast milk in infants aged 0-23 months  with the occurrence of underweight, (p = 0.000;OR=0.272; 95%CI=0.217–0.341).Conclusion: In conclusion, breastfeeding only in infants from birth until before the last 24 hours and first provide food other than breast milk in infants aged 0-23 months associated with underweight. Because breast milk is sufficient nutritional needs of infants aged 0-6 months.ABSTRAK Latar Belakang: Masalah gizi kurang merupakan masalah kesehatan masyarakat yang disebabkan oleh banyak faktor. Prevalensi underweight di Jawa Timur tahun 2016 sebesar 17,3%, lebih tinggi dari target program Dinas Kesehatan Provinsi Jawa Timur sebesar 12,9%. ASI merupakan makanan terbaik untuk mendukung pertumbuhan dan perkembangan bayi. Cakupan ASI Eksklusif di Jawa Timur tahun 2015 menurun (68,8%), dibandingkan tahun 2014 (72,89%). Pemberian makanan pendamping ASI dini pada bayi di bawah 6 bulan dapat mempengaruhi status gizi. Tajuan: Tujuan penelitian untuk menganalisis hubungan ASI Ekslusif dengan kejadian underweight pada bayi usia 0-23 bulan di Provinsi Jawa Timur tahun 2016.Metode: Penelitian ini merupakan analisis data sekunder survei Pemantauan Status Gizi Provinsi Jawa Timur. Populasi penelitian adalah semua bayi berusia 0-59 bulan yang ada di Jawa Timur. Total sampel sebanyak 4738 bayi.  Analisis data dilakukan dengan menggunakan uji Chi-square dan Regresi Logistik.Hasil: Hasil penelitian menunjukkan ada hubungan antara pemberian ASI saja sejak lahir sampai sebelum 24 jam terakhir pada bayi usia 0-23 bulan dengan kejadian underweight (p=0,000010;OR=1,654;95%CI=1,319–2,052), ada hubungan pertama kali memberikan makanan selain ASI pada bayi usia 0-23 bulan dengan kejadian underweight,  (p = 0,000;OR=0,272; 95%CI=0,217–0,341).Kesimpulan: Simpulan yaitu pemberian ASI saja pada bayi sejak lahir sampai sebelum 24 jam terakhir dan pertama kali memberikan makanan selain ASI pada bayi usia 0-23 bulan berhubungan dengan kejadian underweight. Pemberian ASI saja sudah mencukupi kebutuhan nutrisi bayi usia 0-6 bulan.
