This study examines disparity in call duty and hazard allowance and its implication on health worker's commitment and industrial dispute in tertiary hospital in South-South, Nigeria. A survey method that allows for the use of structured questionnaire was adopted. The study used a combination of purposive and random sampling techniques in determining a sample of 1191 participants from 4 tertiary hospitals in South-South Nigeria. Data gathered from the field was coded and analyzed using Linear Regression at 0.05 level of significance. Results revealed that call duty discrepancies and hazard allowances significantly affect health workers' commitment and industrial disputes in tertiary hospitals. The study recommended, among others, that National Salaries, Incomes and Wages Commission should carry out a comprehensive review of all health worker's wages and allowances in Nigeria with the view of harmonizing them. The commission should also evolve a periodic pay reform for health workers in Nigeria.

BPJS complains that the puskesmas still carry out unnecessary referral actions (non-specialized diseases) to hospitals. The purpose of this study is to analyze the factors that influence the referral of PBI JKN patient patients to advanced health facilities. This is quantitative research with survey method through cross sectional approach. The research was conducted at Pancur Batu Puskesmas Deli Serdang District. The study population was 18,459 people and the sample was 186 people with purposive sampling techniques. Data analysis used univariate analysis, bivariate analysis with chi-square, and multivariate analysis using multiple logistic regression tests. The results showed that the factors that influenced were knowledge, availability of infrastructure and information about referrals, p < 0.05. Variables that do not affect are distance, motivation, attitudes of health workers, p > 0.05. The variable that has the greatest influence in this study is the variable availability of infrastructure with the value of Exp (B) / OR = 6.810 means that respondents who stated that the availability of infrastructure in hospitals was adequate, had the chance to be referred to 6.8 times higher than respondents who stated the availability of infrastructure facilities in hospitals is inadequate. It is recommended that the Pancur Batu Puskesmas repair some Puskesmas infrastructure facilities that are considered incomplete and less satisfying for some patients so that patients do not ask for referrals for diseases that can be treated at the Puskesmas.

Abstract Background Extracorporeal life support (ECLS) is an established tool to stabilize severely ill patients with therapy-refractory hemodynamic or respiratory failure. Recently, we established a mobile ECLS retrieval service at our institution. However, data on the outcome of patients receiving ECLS at outside hospitals for transportation into tertiary hospitals is still sparse. Methods We have analyzed all patients receiving ECLS in outside hospitals (Transport group, TG) prior to transportation to our institution and compared the outcome to our in-house ECLS experience (Home Group, HG). Results Between 2012 and 2018, we performed 978 ECLS implantations, 243 of which were performed on-site in tertiary hospitals for ECLS supported transportation. Significantly more veno-venous systems were implanted in TG (n = 129 (53%) vs. n = 327 (45%), p = 0.012). Indication for ECLS support differed between the groups, with more pneumonia; acute respiratory distress syndromes in the TG group and of course, more postcardiotomy patients in HG. Mean age was 47 (± 20) (HG) vs. 48 (± 18) (TG) years, p = 0.477 with no change over time. No differences were seen in ECLS support time (8.03 days ±8.19 days HG vs 7.81 days ±6.71 days TG, p = 0.675). 30-day mortality (n = 379 (52%) (HG) vs. n = 119 (49%) (TG) p = 0.265) and death on ECLS support (n = 322 (44%) (HG) vs. n = 97 (40%) TG, p = 0.162) were comparable between the two groups, despite a more severe SAVE score in the v-a TG (HG: − 1.56 (± 4.73) vs. TG -3.93 (± 4.22) p < 0.001). Mortality rates did not change significantly over the years. Multivariate risk analysis revealed Influenza, Peak Insp. Pressure at implantation, pO2/FiO2 ratio and ECLS Score (SAVE/RESP) as well as ECLS support time to be independent risk factors for mortality. Conclusion Mobile ECLS support is a tremendous challenge. However, it is justified to offer 24 h/7d ECLS standby for secondary and primary hospitals as a tertiary hospital. Increasing indications and total numbers for ECLS support raise the need for further studies to evaluate outcome in these patients.

Parental factors play vital role in academic attainment of their offspring. Motivation and counseling stimulate someone to take a desired course of action. This paper attempts to explore the perception of children towards their patents in support, involvement, counseling, and motivation on learning mathematics. This study employed survey research design of the quantitative approach. The total population of the study was90 students (15 students from each six schools) enrolled in Grade ix of Sindhuli district in 2019. The data were collected through self-structured questionnaire using three points-Likert scale items. Mean and standard deviation of each item were calculated and Chi-square test at 5% level of significance was used to test the hypothesis. The finding of this study shows that the parents of Sindhuli district are very conscious about the study of their children whatever their educational level. This study suggests that parent involvement is an inseparable component to promote mathematical achievement of their offspring.

The purpose of this study is to analyse the environmental supportive administrative services provided by principals for effective classroom management in public and private secondary schools in Anambra State. One research question and one null hypothesis guided the study. It was a descriptive study conducted in Anambra state covering the six education zones in the state. A total of 15,990 respondents constituted the population for the study. A sample of 852 teachers was used. The instrument for data collection is a structured questionnaire developed by the researchers. The questionnaire was duly validated by experts who are lecturers. The reliability of the instrument was also established. Cronbach alpha statistic was used to determine reliability which gave the reliability coefficient of 0.81. The instrument was administered on the respondents directly by the researchers who assisted by six research assistants. Out of 852 copies of the questionnaire distributed, 850 copies were correctly filled, retrieved and subjected to statistical analysis. The data were analyzed using mean ratings to answer the research question and t- test to test the hypothesis at 0.05 level of significance. Findings indicated that, in public secondary schools, adequate workshops are not provided as part of environmental supportive administrative services for effective classroom management. Accordingly, it was among others recommended that government should organize workshops for secondary schools on environmental supportive administrative services.

Ocular injury occurs commonly and it may lead to visual impairment if it is not properly managed. We report the case of a 35-year old man with a retained infected ocular foreign body in his right eye following an injury. The foreign body was not discovered despite consulting non-ophthalmologists at a Maternity Centre and an Optical Shop over a period of ten days.  At presentation at the Specialist Hospital, he was unable to see with swelling of the eyelid, red eye and discharge. On examination, the visual acuity in the affected eye was NLP and an infected stick that penetrated the eyeball was discovered in the superonasal aspect of sclera covered by the swollen upper lid. This resulted in endophthalmitis, cataract and blindness of that eye. Ocular injuries must be promptly referred to the ophthalmologist for appropriate care and to prevent complications. The case clearly showed a diagnostic and therapeutic challenge for paramedics who encounter eye injuries outside the tertiary hospital setting. Therefore, this report raises public health concern intended to increase awareness on the management of eye injuries. The incorporation of Primary Eye Health into Primary Health Care along with training and re-training of Community Health Extension Workers, General Practitioners and other rural health workers cannot be overemphasized.
