Diabetes melitus adalah suatu kelompok penyakit metabolik dengan karakteristik hiperglikemia yang terjadi karena kelainan sekresi  insulin, kerja insulin atau keduanya. Indonesia sebagai salah satu negara berkembang dengan jumlah penduduk yang tinggi memiliki risiko terjadinya masalah kesehatan yang lebih besar. Salah satu masalah kesehatan adalah meningkatnya epidemi diabetes melitus. Saffron, Crocus sativus, tanaman yang berasal dari genus Iridaceous, adalahtanaman yang dianggap sebagai salah satu tanaman herbal yang penting dalam bidang medis, kosmetik dan industri higienis lainnya.Review ini disusun dengan melakukan penulusuran sumber pustaka melalui database NCBI, Garuda, Sinta, dan Google Scholar. Hasil dari penelusuran 19 artikel yang ditemukan yaitu saffron memiliki kandungan crocin crocetin, procicin, safranalyang dapat menjadi kandidat antioksidan yang memiliki pengaruh signifikan terhadap kadar glukosa dalam darah.  Kata kunci: Crocus sativus, diabetes melitus, glukosa, saffron  POTENCY OF SAFFRON AS ANTIDIABETIC  ABSTRACT Diabetes mellitusis a group of metabolic diseases characterized by hyperglycemia that occurs due to abnormal insulin secretion, insulin action or both. Indonesia as a developing country with a high population has a greater risk of health problems. One health problem is the increasing epidemic of diabetes mellitus. Saffron, Crocus sativus, a plant that belongs to the Iridaceous genus, is a plant that is considered one of the important herbal plants in the medical, cosmetic and other hygienic industries. This review was compiled by searching literature sources through NCBI, Garuda, Sprott, and Google Scholar databases. Diabetes melitus  atau DM merupakan kelompok penyakit metabolic dengan karakteristik hiperglikemia yang terjadi akibat kelainan sekresi insulin, kelainan kerja insulin atau kedua-duanya. Prevalensi diabetes melitus pada penduduk usia>15 tahun sebesar 6,9%, Toleransi Glukosa Terganggu atau TGT sebesar 29,9% dan Gula Darah Puasa Terganggu atau GDPT sebesar 36,6%. Diabetes mellitus merupakan silent killer. Adapun gejala-gejala klasik pada diabetes adalah polifagi, polidipsi dan poliuri. Penanganan pasien diabetes terbagi atas terapi farmakologi dan non farmakologi. Literatur review ini bertujuan mengetahui efek pemberian ekstrak biji jintan hitam terhadap pasien diabetes mellitus tipe 2 sebagai terapi alternatif yang sudah dilakukan oleh peneliti sebelumnya. Metode yang digunakan dalam artikel ini adalah penelusuran artikel melalui database NCBI dan Google Scholar. Tahun penerbitan sumber pustaka adalah dari tahun 2001 sampai 2019 dengan 32 sumber pustaka. Berbagai penelitian eksperimental yang dilakukan menunjukkan bahwa kandungan bioaktif pada biji jintan hitam bermanfaat sebagai anti diabetes. Kandungan biji jintan hitam sebagai antioksidan dapat meningkatkan sekresi insulin dengan meningkatkan metabolism energi di mitokondria, meningkatkan jalur intraseluler pada reseptor dan menurunkan glukoneogenesis dengan menekan sintesis enzim glukoneogenik.   Kata kunci: diabetes melitus, thymoquine, anti diabetes   EFFECT OF BLACK SEED EXTRACT FOR DIABETES MELLITUS TYPE 2 AS ALTERNATIVE THERAPY   ABSTRACT Diabetes mellitus or DM  is an group metabolic disease with the hyperglycemia characteristic because of insulin secretion disorder, insulin activity or both of them. The prevalence of diabetes mellitus at the age > 15 years old, is 6.9 percent, 29.9 percent ImpairedGlucoseTolerance or IGT and 36.6 percent ImpairedFastingGlycaemia or IFG.. Diabetes mellitus is a silent killer. As for classic symptoms of diabetes are polyphagia, polydipsia and polyuria. Intervention for patient diabetes divided by pharmacological and non pharmacological intervention. This literature review aims to find out which effect of Black Seed for patient of diabetes mellitus type 2 as alternative therapythat have been done by previous researchers. The method used in this article is the search of articles through the NCBI and Google Scholar databases. The year of publication of library sources is from 2009 to 2018 with 29 library sources.Various experimental research conducted showed part bioactive component useful as antidiabetic. Bioactive component as antioxidant can increase insulin secretion by increasing energy metabolism at mitochondria and increasing intraceluler pathway on insulin receptor and reduce gluconeogenesis by reduce synthesis gluconeogenic enzyme.   Keywords: diabetes mellitus, thymoquinone, anti diabetic

SummaryDiabetes mellitus is one of the most common chronic diseases which continue to increase in number and significance. It presents the third most prevalent condition among medically compromised patients referring for dental treatment. Diabetes mellitus has been defined as a group of metabolic diseases characterized by hyperglycemia resulting from defects in insulin secretion, insulin action, or both. Hyperglycemia leads to widespread multisystem damage which has an effect on oral tissue. The present article summarizes current knowledge regarding the association between diabetes mellitus and oral and dental health.

Abstract: Diabetes mellitus (DM) is a group of metabolic diseases with characteristic hyperglycemia that occurs due to insulin secretion, insulin action or both. This disease affects many societies and continuously growing in Indonesia. Periodontal disease is an oral health problem which has a relatively high prevalence in the community where periodontal disease in all age groups in Indonesia.The purpose of this study was to determine the periodontal disease in patients with diabetesmellitus in RSUP Prof. dr. R. D. Kandou Manado. This descriptive study has 68 sample taken with total sampling technique. The sample is examined using evaluation criteria gingival index and CPITN index. The result show that subjects with periodontitis with a score of 4 is the highest as many as 18 people (44%) and subject with a score of 2 is that at least as many as 8 people (19.5%). And subjects with bad gingivitis is the highest as many as 10 people (52.6%) and subject with the good gingivitis is the at least as many as 5 people (26.3%). Based on the result of this study, periodontal disease in patients with diabetes mellitus in RSUP Prof. Dr. R. D. Kandou most that periodontitis with the number of 41 people (68.3%) compared to gingivitis which is just as many as 19 people (31.7%)Keywords: diabetes mellitus, periodontitis, gingivitis, periodontalAbstrak:Diabetes Melitus (DM) merupakan suatu kelompok penyakit metabolik dengan karakteristik hiperglikemia yang terjadi karena sekresi insulin, kerja insulin atau kedua-duanya. Penyakit periodontal merupakan masalah kesehatan gigi dan mulut yang memiliki prevalensi cukup tinggi di masyarakat dimana penyakit periodontal pada semua kelompok umur di Indonesia.Tujuan penelitian ini adalah untuk mengetahui penyakit periodontal pada penderita diabetes mellitus di RSUP Prof. dr. R. D. Kandou Manado. Jenis penelitian ini adalah penelitian deskriptif dengan jumlah sampel 60 orang yang diambil dengan teknik total sampling. Sampel diperiksa dengan menggunakan kriteria penilaian indeks gingiva dan indeks CPITN. Hasil menunjukkan bahwa yang mengalami periodontitis dengan skor 4 adalah yang paling tinggi yaitu sebanyak 18 orang (44%), dan subjek yang mengalami skor 2 adalah yang yang paling sedikit yaitu sebanyak 8 orang (19.5%). Sedangkan yang mengalami gingivitis yang paling tinggi yaitu gingivitis buruk sebanyak 10 orang (52.6%) dan yang paling sedikit adalah yang mengalami gingivitis ringan yaitu sebanyak 5 orang (26.3%). Berdasarkan hasil penelitian ini dapat disimpulkan bahwa penyakit periodontal yang paling banyak ditemui pada penderita diabetes melitus di RSUP Prof. Dr. R. D. Kandou Manado adalah penyakit periodontitis yaitu sebanyak 41 orang (68.3%) dan yang paling sedikit adalah gingivitis yaitu sebanyak 19 orang (31.7%)Kata kunci: diabetes melitus, periodontitis, gingivitis, periodontal

Prameha is disease of Mutravaha Srotasa having Kapha dominancy which can be correlated with diabetes mellitus. The term diabetes mellitus describes a metabolic disorder of multiple etiologies characterized by chronic hyperglycemia with disturbances of carbohydrate, fat and protein metabolism resulting from defects in insulin secretion, insulin action, or both. Madhumeha is considered as a subtype under the Vatika type of Prameha and it is characterized by passage of urine with sweet taste like honey along with sweetness of whole body. With appropriate use of Ayurvedic preventive measures such as Dincharya, Ritucharya, Aharvidhi and therapeutic measures Madhumeha (DM) can be prevented.

The prevalence of type 2 diabetes mellitus is rising globally and this disease is proposed to be the next pandemic after COVID-19. Although the cause of type 2 diabetes mellitus is unknown, it is believed to involve a complex array of genetic defects that affect metabolic pathways which eventually lead to hyperglycaemia. This hyperglycaemia arises from an inability of the insulin-sensitive cells to sufficiently respond to the secreted insulin, which eventually results in the inadequate secretion of insulin from pancreatic β-cells. Several treatments, utilising a variety of mechanisms, are available for type 2 diabetes mellitus. However, more medications are needed to assist with the optimal management of the different stages of the disease in patients of varying ages with the diverse combinations of other medications co-administered. Throughout modern history, some lead constituents from ancient medicinal plants have been investigated extensively and helped in developing synthetic antidiabetic drugs, such as metformin. Teucrium polium L. (Tp) is a herb that has a folk reputation for its antidiabetic potential. Previous studies indicate that Tp extracts significantly decrease blood glucose levels r and induce insulin secretion from pancreatic β-cells in vitro. Nonetheless, the constituent/s responsible for this action have not yet been elucidated. The effects appear to be, at least in part, attributable to the presence of selected flavonoids (apigenin, quercetin, and rutin). This review aims to examine the reported glucose-lowering effect of the herb, with a keen focus on insulin secretion, specifically related to type 2 diabetes mellitus. An analysis of the contribution of the key constituent flavonoids of Tp extracts will also be discussed.

ABSTRAK Diabetes Melitus merupakan penyakit yang paling menonjol yang disebabkan oleh gagalnya pengaturan gula darah. Lidah buaya berkhasiat untuk menurunkan kadar gula dalam darah bagi penderita diabetes dan dapat mengontrol tekanan darah. Tujuan penelitian untuk mengetahui pengaruh jus lidah buaya (AloeBarbadensis Miller) terhadap penurunan glukosa darah puasa GDP) dan 2 Jam PP (Post Prendial) pada penderita DM (Diabetes Melitus). Desain penelitian merupakan Quasi Eksperimental dengan pendekatan one group pretest – postest design. Sampel terbagi menjadi 14 responden. Data dianalisis dengan paired t-test. Hasil menunjukan rata-rata penurunan glukosa darah puasa pada intervensi (28,42 gr/dl) dan glukosa darah 2 jam pp pada intervensi (40,57 gr/dl). Untuk analisis bivariat terdapat perbedaan antara glukosa puasa dan 2 jam pp dengan glukosa darah puasa GDP) dan 2 Jam PP (Post Prendial) pada penderita DM (Diabetes Melitus). Kesimpulan didapatkanlidah buaya dapat menurunkan kadar glukosa darah.Berdasarkan hasil penelitian jus lidah buaya dapat menjadi salah satu alternatif keperawatan non farmakologi dalam penyakit diabetes melitus. Kata Kunci : Lidah Buaya ; Diabetes Mellitus THE EFFECT OF VEGETABLE VOCATIONAL JUICE ON FAST BLOOD GLUCOSE LEVELS AND 2 HOURS OF PP (Post Prandial) IN DIABETES MELLITUS  ABSTRACT Diabetes mellitus is the most prominent disease caused by the failure of blood sugar regulation. Aloe vera is efficacious can to  reduce blood sugar levels for diabetics and can control blood pressure. The purpose of this study was to determine the effect of Aloe Barbadensis Miller on the reduction of fasting blood glucose GDP and 2 hours of PP (post prendial) in patients with diabetes mellitus. The research design is Experimental Quasi with one group pretest - postest design approach. The sample is divided into 14 respondents. Data were analyzed by paired t-test. The results showed an average decrease in fasting blood glucose at intervention (28.42 gr / dl) and 2 hours pp blood glucose at intervention (40.57 gr / dl). For bivariate analysis there was a difference between fasting glucose and 2 hours pp with fasting blood glucose GDP) and 2 hours PP (Post Prendial) in patients with diabetes mellitus. The conclusion is that aloe vera can reduce blood glucose levels. Based on the results of research on aloe vera juice can be an alternative non-pharmacological nursing in diabetes mellitus. Keywords: Aloe Vera ; Diabetes Mellitus

Pendahuluan: Prevalensi Diabetes Melitus (DM) di Indonesia tinggi dan bahkan mengalami fluktuasi dari 422 juta jiwa menjadi 463 juta jiwa dalam 5 tahun. Pada tatalaksana penyakit DM, salah satunya ialah terapi farmakologi menggunakan Obat Antihiperglikemik (OAH). Saat ini selain OAH, masyarakat juga menggunakan herbal untuk mengatasi DM. Senyawa aktif herbal yang telah terbukti berkontribusi dalam pengobatan beragam dan multikomponen, namun tidak semua komponen tersebut diketahui memberikan efek terapeutik pada DM. Maka, tujuan dari tinjauan pustaka ini untuk mengetahui interaksi yang terjadi antara obat konvensional dan herbal untuk DM. Metode: Pencarian literatur dilakukan pada bulan Mei 2020. Situs yang digunakan adalah Google Scholar, Elsevier dan NCBI.  Kata kunci yang digunakan untuk menemukan sumber jurnal ialah “interaksi obat DM dan herbal”, “interaction of diabetic drugs and herbs”, “herbal untuk diabetes”, dan “herbs for diabetes”. Jurnal yang digunakan sebagai sumber berjumlah 15 jurnal dengan fokus penelitian secara in vivo  dan klinis. Hasil: Multikomponen pada herbal tersebut jika digunakan bersamaan dengan OAH berpotensi untuk menimbulkan interaksi, beberapa studi melaporkan bahwa OAH dan herbal untuk antidiabetes memiliki interaksi sinergis atau antagonis. Berdasarkan pustaka beberapa penelitian interaksi obat dan herbal bisa terjadi, seperti Metformin, Glibenklamid dan Pioglitazon dengan Aloe vera berinteraksi sinergis. Kesimpulan: Potensi interaksi antagonis dan sinergis muncul pada beberapa hasil penelitian interaksi herbal dengan OAH.

The goal of this review is to develop the hypothesis, and review the evidence, that protein restriction, through synergistic effects on multiple organ systems predisposes to loss of normal regulation of fuel homeostasis that plays the central role in the development of type 2 (non-insulin-dependent) diabetes mellitus. The ability of insulin to regulate glucose production and disposal varies between individuals. These differences, together with the various compensatory mechanisms that are invoked to attempt to normalize fuel homeostasis, are of fundamental importance in the development and clinical course of type 2 diabetes mellitus. Protein deprivation impacts on both insulin secretion and insulin action. These effects may persist even when a diet containing adequate protein is presented subsequently. Data are presented that suggest that protein restriction results in an impaired ability of pancreatic β-cells to compensate adequately for the defect in insulin action in insulin-resistant individuals. This persistent impairment of insulin secretion resulting from protein restriction predisposes to loss of glucoregulatory control and impaired insulin action after the subsequent imposition of a diabetogenic challenge. This inability to maintain the degree of compensatory hyperinsulinaemia necessary to prevent loss of glucose tolerance may have relevance to the increased incidence of diabetes on changing from a nutritionally-poor diet to a Western diet, and to the hypothesis that some cases of type 2 diabetes in adulthood may be related to poor early nutrition.
