Introduction: Globally 17.9 million people die each year from CVDs (Cardio Vascular Diseases) and 85% of all CVD deaths are due to heart attacks and strokes. According to the World Health Organization (WHO), 15 million people worldwide suffer from stroke every year and after ischemic heart disease, stroke is the second most leading cause of death. Objective: To study alcohol consumption as a risk factor in patients with acute strokes. Method: Case control study was conducted among 148 newly diagnosed cases of CT scan / MRI-confirmed stroke and 148 non-stroke controls attending the tertiary hospital of central Gujarat. Sample size calculation was done, assuming the power ratio of the number of controls same as numbers of cases i.e.,1:1, specifying values for two-sided confidence level=95% (error=5%), power of study=80%, odd ratio=2.15, percent of cases with exposure=82% and percent of controls with exposure=68%. Patients who are willing to participate in the study were included. The data were analyzed using the EPI INFO TM 7.0.8.0 software. Results: Out of the total 148 cases, 100 (67.56 per cent) were male and 48 (32.43 per cent) were female. The odds of getting stroke among alcoholic was two times more as compared to non-alcoholic patients. Heavy alcohol drinkers have a significant association. Conclusion: According to the study results male married patients who drank heavily had a higher risk of stroke than light/moderate alcoholics.

Aim: This study examined the role of lymphocyte-to-monocyte ratio (LMR), an inflammatory biomarker, in predicting the severity of calcific aortic valve stenosis (CAVS) in a Chinese case–control study. Results: The LMR significantly decreased in the patients with CAVS compared with healthy controls. An inverse correlation was observed between the severity of stenosis and LMR in the patients. Additionally, the LMR was identified in the multivariate analysis as an independent predictor of severe CAVS. Conclusion: This study provides evidence of an inverse correlation between the severity of CAVS and LMR. LMR could potentially be applied as an independent predictor of severe CAVS and could be incorporated into a novel predictive model.

Abstract Introduction Nephrolithiasis is pathological calcification in the excretory passages of the body and is prevalent among 7.6% of Indians. We aimed to study the various risk factors associated with renal stones from India. Method It was a hospital-based case-control study conducted over 18 months in a tertiary hospital in Delhi. Cases were defined as patients with renal stones diagnosed on the basis of history and radiological examination. Controls were similar to cases in all respects except for the diagnosis and selected from the hospital. A total of 18 risk factors, including age, gender, heavy metals, stress, metabolic factors, alcohol intake, dietary habits, co-morbidities, etc. were assessed. Logistic regression analysis was performed to calculate the strength of the risk associations. Results In the analysis of 60 cases and controls, we found 6 times, 5.5 times, and 2.4 times increased odds of renal stones in patients with increased arsenic, cadmium, and lead concentrations in blood, respectively. Similarly, there are 3 times increased odds of renal stones in patients suffering from stress. Conclusions Exposure to smoke, occupation dust, and contaminated water may lead to an increased ingestion/inhalation of heavy metals like cadmium, arsenic, and predisposing people to an increased risk of renal stones.

Abstract Background The importance of influenza is increasing mainly because of the appearance of novel pandemic strains such as swine and avian. Each year, influenza has spread around the world causing about 250,000–500,000 deaths and more than 5 million cases of severe illness. The objective is as follows: evaluating the outcomes of patients with influenza A (H1N1) virus in relation to certain TNF-308, IL6, and IL8 polymorphisms and identifying the associated factors with the severe outcome. Subject and methods This is a case–control study. The cases were patients confirmed by real-time polymerase chain reaction (RT-PCR) to be influenza A (H1N1) virus infected. The controls were healthy individuals. Medical history and outcome of the disease was registered. In all study participants, polymorphisms of TNF rs1800629, IL6 rs18138879, and IL8 rs4073; odds ratio (OR); and the 95% confidence interval (95% CI) were calculated. Results Infection with influenza A (H1N1) virus was associated more with the following genotypes: TNF-308 AA (OR = 4.041; 95% CI = 1.215–13.4) and IL8 AA (OR = 3.273; 95% CI = 1.372–7.805). According to our study results, HCV (OR = 3.2, 95% CI 1.2–8.5), renal disease (OR = 3.4, 95% CI 0.9–13.6), cancer (OR = 3.1, 95% CI 0.3–31.1), TB (OR = 8.4, 95% CI 1.8–39.7), ICU (OR = 2.9, 95%1.2–7.1), and mortality (OR = 7.9, 95% CI 0.9–67.4) are considered as risk factors for influenza A (H1N1)-infected patients. Conclusions Our findings concluded that TNF-308 (AA) and IL8 (AA) polymorphisms may increase the susceptibility to be infected with H1N1influenza virus.

Objective To evaluate whether the presence of maternal blood pressure reduces the risks of morbidity, perinatal mortality and morbidity at 24 months of age in very low birth weight infants (VLBWIs) compared with a control group. Methods A retrospective, observational, case-control study. Total 49 VLBWIs were allocated to the study group, called the maternal arterial hypertension group (AHG), and matched with 44 in the control group (CG). The infants were assessed during hospitalization and at 12 and 24 months corrected age at a specialized clinic. For the assessment of growth, the World Health Organization (WHO) Anthro software (Geneva, 2006) was used, and for the psychomotor assessment, the Denver II test was used. Results In relation to the antenatal variables, the infants of the AHG had more centralized circulation assessed by Doppler, received more corticosteroids and magnesium sulfate, and were born by cesarean section more frequently. In terms of the postnatal and in-hospital outcomes, the AHG had a higher gestational age at birth (30.7 versus 29.6 weeks) and a lower frequency of 5-minute Apgar scores of less than 7 (26.5% versus 52.3%). The CG had a higher rate of pulmonary dysplasia (30.2% versus 8.3%). There were no differences in terms of hospital mortality, complications, somatic growth and functional problems at 24 months of corrected age. Conclusion The presence of maternal hypertension, especially preeclampsia, was not a protective factor against morbidity, mortality and evolution in VLBWIs aged up to 24 months. Therefore, the clinical practice should be focused on prolonging the pregnancy for as long as possible in these conditions as well.

Abstract Background: Chronic kidney disease (CKD) increases the susceptibility to the infection of herpes zoster (HZ). Less is known about the risk factors of HZ in CKD patients. Methods and Participants: This is a case-control study. CKD patients diagnosed with HZ infection between January 2015 and October 2020 in a tertiary hospital were identified. One age- and gender- matched control was paired for each case, matched to the date of initial HZ diagnose. The uni- and multivariate analysis were used to evaluate the risk factors for development of HZ in CKD patients. Results: Forty-six HZ patients and controls were identified. In general, about 80% (72 out of 92) patients were classified at end-stage renal disease (ESRD, CKD Ⅳ to Ⅴ). Multivariate analyses revealed that immunosuppressive agents (odds ratio: 12.50, 95% CI: 1.53-102.26, P=0.021) and dialysis (odds ratio: 3.33, 95% CI: 1.13-9.78, P=0.029) were independent risk factors of HZ in patient with CKD. Conclusion: Immunosuppressive medication and dialysis were associated with HZ infection in CKD. Further guideline may highlight the necessity of zoster vaccine for patients with CKD, who undertake immunosuppressive or dialysis treatment.
