Background: Fires and burns incidence are on constant rise. Efforts had been made since very ancient times to curb such debilitating injuries resulting from domestic violence, vitriol incidences, accidents and mishaps. Treating the burns always had been an intricate matter of concern. Since primitive times the treatment modalities had been changed over and over in the search of best available options. Aim: To compare the efficacy of empirical treatment with that of advanced medical care among the victims of burns. Methods: The study comprised of 300 cases of burns presented from January to December 2020 in Accidents and Emergency Department of the Jannah Hospital Lahore for empirical treatment and those managed in the Burn Center of Jannah Hospital Lahore for advanced medical care of burns. Those who have been referred from periphery and could not get advanced medical care or had their wound healed when brought to above set-up were considered as treated as empirically managed burn victims. Results: The comparative analysis of empirical therapy and advanced medical care revealed significant difference when observed among the literacy level and medium of burns with 0.000 value of p. While no significance difference was observed when analysis was carried out for age and gender with a p value of 0.187 and 0.496 respectively. Background: Empirically chosen antibiotics based on the local resistance pattern of uropathogens remain the principle treatment of urinary tract infections (UTI).Methods: Antibiogram of most frequent uropathogen was determined. Based on the antibiogram result, authors compared effectiveness of drugs recommended for UTI by National centre for disease control (NCDC), India, and assessed age and gender based variability in the effectiveness of these drugs. Results: 1278 urine samples were accounted, of which 405 samples showed significant growth. E. coli was the most common uropathogen (n=146, 36%) followed by enterococcus species (31%) and Klebsiella pneumoniae (10%). Using McNemar’s test authors found that nitrofurantoin (90% sensitivity) was statistically the most effective drug among drugs recommended by NCDC for uncomplicated cystitis. Furthermore, authors used Fisher’s exact test on adults and paediatrics and found that significant difference in effectiveness was observed for nitrofurantoin (p-value <0.001) and cotrimoxazole (p-value 0.034). Using logistic regression, authors found that with age, effectiveness of ciprofloxacin and cotrimoxazole deteriorate significantly (p-value 0.021 and 0.002 respectively). Additionally, authors observed that cotrimoxazole has significantly better efficacy in males compared to females (p-value 0.022).Conclusions: In accordance with present study, nitrofurantoin can be used as first line treatment for uncomplicated cystitis. Age and gender should be considered while prescribing empirical treatment for UTI. Periodic surveillance should be carried out to identify the on-going pattern of antibiogram to update the guideline for empirical therapy.

AbstractThis paper examines the relationship between employee engagement and job satisfaction. People spend most of their time at work, and their motivation is considered to be an important factor for job performance. Enthusiastic employees, who focus their efforts on achieving their companies′ goals are a key competitive advantage in the modern world. The effect of employee engagement on business performance has been studied by various experts. They found out the similar conclusion: “the more enthusiastic the workers are, the better operating results they achieve for the company”. An occasional sample of 594 respondents who are employed in the public and non-public sector in Slovenia was used for the purpose of this study. The main goal of the research is to determine whether (and how) the employee engagement influences job satisfaction. A written survey was conducted from 4 January 2016 to 14 March 2016. IBM SPSS 20 was used for the statistical analysis. The results confirm that the relationship between employee engagement and job satisfaction is positive and statistically significant (5 % significance level), based on the linear regression F (1, 583) =296.14, p-value = 0.000, R-square = 0.337. The results also show that there is no statistically significant difference between employee engagement and gender and there is no statistically significant difference between job satisfaction and gender.

Background: The ostiomeatal complex (OMC) is not a separate anatomical structure although it is a functional unit of structures, including the middle meatus, uncinate process, infundibulum, maxillary sinus ostium, ethmoidal bulla, anterior ethmoid sinus ostium, and frontal recess. Concha bullosa is the pneumatization of the concha, which is one of the most common anatomical variations in the middle turbinate. Methods: This study was conducted using the cone-beam computed tomography (CBCT) images of 172 patients in the archives of the Department of Oral and Maxillofacial Radiology, Dentistry School, Ahvaz Jundishapur. Patient information including age and gender, presence or absence of concha bullosa, the involved side (left or right), and its type (i.e., extensive, lamellar, and bulbous) were collected in the information form. Finally, the chi-square test (with SPSS, version 22) was used to analyze the data, and P value less than 0.05 was considered statistically significant. Results: Patients with and without concha bullosa were 39.1 and 41.7 years, respectively, but it was no significant difference in terms of age (P = 0.321). Out of 52 patients with concha bullosa, 19 (36.5%) cases were males and 33 (63.5%) of them were females. The prevalence of concha bullosa was higher for the bilateral side (20 patients, 38.5%, P = 0.000). The prevalence of bulbulsand lamellar-shape was nearly the same (32.7% and 30.8%, respectively). Eventually, the extensive shape with 36.5% was more frequent for the shape of concha bullosa (P = 0.000). Conclusions: The prevalence of concha bullosa was high. There was no significant difference in terms of age (P = 0.321) and gender (P = 0.058) of patients with concha bullosa. The extensive type and the bilateral appearance of concha bullosa were more significant (P = 0.000).

Objective The aim of this study was to detect the arginase Ι (ARG I) enzyme in asthma patients, clarify its role, in addition to examining the relationship of this enzyme with bronchial asthma. Methods: Blood samples were collected from 100 patients from the Department of Medicine in Rizgary Hospital in Erbil City, in addition to intact 100 volunteers; the introduced questionnaire was filled out on the basis of type-induced asthma, duration of the disease, age of the patients, gender, family history, and allergy condition, Serum was separated to perform Enzyme-linked immunosorbent assay (ELISA) in Medical Research Center to examine the association of ARGΙ with bronchial asthma. By ARG activity we can measure the conversion of arginine to ornithine and urea. By using a quantitative colorimetric assay at 490 nm, employing a QuantiChrom arginase assay kit (Bioassay Systems). Results: Our results depicted the association between ARGΙ and bronchial asthma: based on their age, significant elevation of serum arginase level was observed in the patients with ≥81 years old, which mean value (100.16±19.77c), p-value (0.000); also the duration of asthma ≥20 years (82.48±38.81c) , p-value (0.01) were remarkably affected; this sign was found in those with types of induced asthma and with allergy condition.  But the non- significant difference in the frequency of abnormal serum arginase level was observed in those patients that have a family history of asthma disease and gender of the patients. This finding demonstrated a remarkable association of ARG Ι in the development of asthma at p ˂ 0.05.

Abstract Objective The aim of this study was to evaluate the serum RE1 silencing transcription factor (REST) level in Alzheimer’s disease (AD), mild cognitive impairment (MCI), and elderly controls by using surface plasmon resonance (SPR) technology. Materials and Methods In this case–control study of 133 subjects, 49 patients with AD, 49 patients with MCI, and 35 elderly controls were recruited. The REST protein concentrations were evaluated by SPR. The resonance unit for each sample was recorded and the concentration of serum REST of study group was derived from the standard curve. All the experiments were done in triplicates. Statistical analysis was done and p-value of < 0.05 was considered as statistically significant. Results A significant difference was observed in the Montreal Cognitive Assessment score, Hindi Mental State Examination scale (HMSE) score education, disease duration, and gender among the groups. A significant (p>0.0001) difference in the duration of disease between AD and MCI was observed. It was observed that the mean concentration of serum REST was not significantly (p = 0.266) different among the groups. Conclusion This study first time evaluated the serum levels of REST in AD, MCI and age-matched elderly controls. The rest levels were similar in all groups; however, it can provide a new direction to future blood-based biomarker studies of REST.

Abstract Introduction To evaluate the dosimetric effect of photon energies on fixed field intensity-modulated radiotherapy (IMRT) and dual arc (DA) planning and to compare the dosimetric differences between conventional IMRT and DA radiotherapy planning. Materials and Methods IMRT and DA plans were generated for 15 patients having cervical cancer using different photon energies. IMRT and DA plans were generated using seven fields and double arcs, respectively. Dosimetric comparison was done in terms of planning target volume (PTV) coverage, sparing of organ at risk (OAR), homogeneity index (HI), conformity index (CI), and monitor units (MUs). Photo-neutron (energy ≤10MV) contribution was not considered for this study. Near region (NR) and far region (FR) were contoured to evaluate the dose deposited in nontarget area. Results No significant difference was observed (p > 0.05) in PTV coverage for conventional IMRT and DA; however, 6 MV yielded significantly better coverage over 15 MV (p < 0.05) for both the treatment modalities. Mean bladder dose was significantly more for conventional IMRT compared with DA. For rectal mean dose, p-value was nonsignificant for IMRT in comparison to DA, while significant difference was observed for change in photon energies for both treatment modalities respectively, except for 10 MV versus 15 MV DA plans. Significant improvements in HI (except 6 MV vs. 10 MV DA), CI (except 6 MV vs. 10 MV IMRT and DA), MUs, NR, and FR were noted. Conclusion DA generates more conformal, homogenous plans, requires less numbers of MUs, and deposits fewer doses to NR and FR regions of nontarget tissues in comparison to conventional IMRT. Although increase in photon energy for IMRT and DA plans reduces numbers of MUs and dose deposited to NR and FR regions, yet the choice for treatment of carcinoma cervix remains 6 MV due to production of photo-neutrons at higher energies.

Recurrent abortion consider as one of the most common problem in world. There are different causes associated with recurrent abortion, these may be occur due to complication of microorganism infection or associated with immunological disturbance like abortion caused by Anti-phospholipid syndrome, or associated with SLE. The anti-b2glucoprotien antibodies have a role in abortion. In this study, we wish to determine whether there is a relationship between the levels of these anti-b2glucoprotien antibodies and abortion. Thirty females patients with spontaneous abortion (10 of them with Antiphospholipid syndrome,10 of them with systemic lupus erthymatosus and final 10 with toxoplasmosis with ages ranged between (20-46) where taken from (Al-Hussein Medical City/Kerbala). Control group consisted of 20 healthy people who were free from history of abortion who matched in age and gender with patients. B2gp(IgMandIgG) EASIA Kit, generic assay) and was studied using the enzyme-linked immunosorbent assay (ELISA) method. T-test and ANOVA and Pearson correlation used to analyze results by using SPSS version 24. P-value ≤ 0.05 was considered significant. The patients with age range blow 30 years show high percent(63.3%) among the other groups .most of the patients have high percent of 3 times abortion were reported in all different groups .The mean value of anti-B2gP-I IgG (45.354) show highly a significant difference results in aborted patients with APS and significant difference in patients with SLE when compared with controls groups. the Anti-B2gPI IgM not give statistically significant difference when camper to health cases. we conclude that the Anti-B2GPI IgG levels were increased significantly in aborted females with APS and SLE .
